관절 조영술을 이용한 족관절 염좌의 진단과 치료 (Diagnosis and Treatment of the Ankle Sprain using Arthrography)

  • Purpose: To evaluate the effectiveness of ankle arthrography in diagnosis and treatment of the ankle sprain. Materials and Methods: Arthrography was performed to eighteen patients who were diagnosed as ankle sprain clinically from September, 1990 to April, 2003. Splint immobilization for three days and return to daily life were for the eleven patients who showed normal limits of extension of joint and no dye leakage on arthrography, cast for 3 weeks and compression brace wearing were for 4 patients who showed anterior talofibular ligament tear on arthrography. 3 patients diagnosed as anterior talofibular and calcaneofibular ligament tears were treated with cast for 6 weeks and then brace for 3 weeks. Results: The range of motion of the injured joint was recovered normally at the time of 3 month of postoperative follow up examination. But two patients complained a mild pain after exercise but it did not affect ordinary activities. Eleven patients who were normal on arthrography returned to daily activities in a week. Conclusion: It is reasonable to determine the extent of ankle sprain and treatment method for it using arthrography.

Multidetector CT arthrography를 이용한 견관절 병변의 진단 - MRI, MR arthrography와의 비교 - (Multidetector CT (MDCT) Arthrography in the Evaluation of Shoulder Pathology: Comparison with MR Arthrography and MR Imaging with Arthroscopic Correlation)

  • Purpose: The purpose of the present study was to evaluate the diagnostic efficacy of CT arthrography (CTA) in the assessment of various shoulder pathologies, compared with MR arthrography (MRA) and MRI with arthroscopic correlation. Materials and Methods: CTA in 84 patients, MRA in 70 patients, and MRI in 27 patients were obtained. A radiologist interpreted each image for 5 pathologies: Bankart, SLAP, Hill-Sachs lesion, full-thickness, and partial-thickness rotator cuff tear. Detailed arthroscopic reports were compared with CTA, MRA, and MRI. The sensitivity, specificity, predictive values, and accuracy were calculated. The agreement between each diagnostic modality and arthroscopy was calculated. Diagnostic efficacy was assessed by the areas under the receiver operating characteristic (ROC) curves. Results: The diagnostic values of all three imaging groups were comparable to each other for Bankart, SLAP, Hills-Sachs, and full-thickness cuff tear lesions, but those of CTA were lower than MRI and MRA for partial-thickness cuff tears. The areas under the ROC curves for CTA, MRA, and MRI were not significantly different for all pathologies, except for partial-thickness cuff tears. Conclusion: CTA was equally competent to MRA or MRI in demonstrating Bankart, Hill-Sachs lesions, SLAP, and full thickness rotator cuff tears but not as efficient in diagnosing partial thickness rotator cuff tears.

회전근 개 부분 파열에서 자기공명 관절조영술과 관절경 소견의 비교 연구 (Comparative Study of MR-arthrography and Arthroscopy in Partial Thickness Rotator Cuff Tears)

  • 목적: 회전근 개 부분 파열에 대한 정확한 진단을 위해서는 영상검사가 필수적이다. 이 논문의 목적은 자기공명 관절조영술과 관절경 소견을 비교하여 자기공명 관절조영술의 회전근 개 부분 파열에 대한 진단능력을 평가하는 데에 있다. 대상 및 방법: 신체 검사상 회전근 개 질환으로 의심되어 자기공명 관절조영술 및 관절경 수술이 시행된 177예의 환자를 대상으로 하였다. 파열의 자기공명 관절조영술 소견과 관절경 소견을 비교하여 회전근 개 파열의 위치에 따른 자기공명 관절조영술의 진단 능력을 평가하였다. 관절면 회전근개 부분 파열의 크기에 대한 일치도를 구하였다. 결과: 파열의 위치에 따라 관절면, 점액낭측 및 양면 파열의 3 군으로 분류하였으며 각각 56예, 26예, 17예였다. 자기공명 관절조영술은 관절면 회전근 개 부분 파열에 대한 민감도 82%, 특이도 88%를 보였으며 점액낭측 파열인 경우에는 민감도 11%, 특이도100%를 보였다. 관절면 회전근개 부분 파열의 크기에 대한 자기공명 관절조영술 소견과 관절경 소견의 일치도는 72%였다. 결론: 자기공명 관절조영술은 관절면 회전근 개 부분 파열의 진단에는 유용하나 점액낭측 파열의 진단에는 한계가 있었다.

경두개방사선사진에서의 하악과두 위치와 관절원판 위치간의 상호관계 (INTERRELATIONSHIP BETWEEN MANDIBULAR CONDYLAR HEAD POSITION IN TRANSCRANIAL VIEW AND ARTICULAR DISC POSITION)

  • This study was designed to evaluate the interrelationship between the condylar head position in transcranial view and the articular disc position in the arthrography. The condylar positions were assessed by subjective method and linear measurement method on the transcranial view. The subjects for this study consisted of 24 symptomatic joints with normal disc position, 37 joints with anterior disc displacement with reduction and 44 joints with anterior disc displacement without reduction that were classified by arthrotomography under the fluoroscopic guidance. The interrelationship between the condylar head position in transcranial view and the articular disc position in the arthrography was evaluated by Chi square test. The obtained results were as follows : 1. There was no significant interrelationship between the position of condylar head in closed mouth state on transcraniaJ view and articular disc position in the arthrography (p>0.05). 2. There was no significant interrelationship between the changes of interarticular distance in 1 inch opening state and articular disc position in the arthrography (p>0.05). 3. There was no significant interrelationship between the position of condylar head related to the apex of articular eminence in 1 inch opening state and articular disc position in the arthrography(p>0.05). 4. There was significant interrelationship between the changes of interarticular distance that is assessed by linear measurement method in maximum opening state and articular disc position in the arthrography(p<0.05), but there was no significant interrelationship when the condylar head position was assessed by subjective method(p>0.05). 5. There was significant interrelationship between the degree of condylar translation in maximum opening state and articular disc position in the arthrography(p<0.05). 6. The correlation coefficient between two methods to assess the position of condylar head were 0.7989: the condylar head position in articular fossa in closed state, 0.6847: interarticular space in 1 inch opening state, 0.8965: the degree of condylar translation in 1 inch opening state, 0.5944: the changes of interarticular space in maximum opening state, 0.9215: the degree of condylar translation in maximum opening state.

Comparison between Conventional MR Arthrograhphy and Abduction and External Rotation MR Arthrography in Revealing Tears of the Antero-Inferior Glenoid Labrum

  • Objective: To compare, in terms of their demonstration of tears of the anterior glenoid labrum, oblique axial MR arthrography obtained with the patient's shoulder in the abduction and external rotation (ABER) position, with conventional axial MR arthrography obtained with the patient's arm in the neutral position. Materials and Methods: MR arthrography of the shoulder, including additional oblique axial sequences with the patient in the ABER position, was performed in 30 patients with a clinical history of recurrent anterior shoulder dislocation. The degree of anterior glenoid labral tear or defect was evaluated in both the conventional axial and the ABER position by two radiologists. Decisions were reached by consensus, and a three-point scale was used: grade 1=normal; grade 2=probable tear, diagnosed when subtle increased signal intensity in the labrum was apparent; grade 3=definite tear/defect, when a contrast material-filled gap between the labrum and the glenoid rim or deficient labrum was present. The scores for each imaging sequence were averaged and to compare conventional axial and ABER position scans, Student's t test was performed. Results: In 21 (70%) of 30 patients, the same degree of anterior instability was revealed by both imaging sequences. Eight (27%) had a lower grade in the axial position than in the ABER position, while one (3%) had a higher grade in the axial position. Three whose axial scan was grade 1 showed only equivocal evidence of tearing, but their ABER-position scan, in which a contrast material-filled gap between the labrum and the glenoid rim was present, was grade 3. The average grade was 2.5 (SD=0.73) for axial scans and 2.8 (SD=0.46) for the ABER position. The difference between axial and ABER-position scans was statistically significant (p<0.05). Conclusion: MR arthrography with the patient's shoulder in the ABER position is more efficient than conventional axial scanning in revealing the degree of tear or defect of the anterior glenoid labrum. When equivocal features are seen at conventional axial MR arthrography, oblique axial imaging in the ABER position is helpful.
