Chikungunya Virus-Encoded nsP2, E2 and E1 Strongly Antagonize the Interferon-β Signaling Pathway

  • Chikungunya virus (CHIKV) is a single-stranded positive-sense RNA virus, belonging to the genus Alphavirus of the Togaviridae family. It causes multiple symptoms, including headache, fever, severe joint and muscle pain, and arthralgia. Since CHIKV was first isolated in Tanzania in 1952, there have been multiple outbreaks of chikungunya fever. However, its pathogenesis and mechanisms of viral immune evasion have been poorly understood. In addition, the exact roles of individual CHIKV genes on the host innate immune response remain largely unknown. To investigate if CHIKV-encoded genes modulate the type I interferon (IFN) response, each and every CHIKV gene was screened for its effects on the induction of the IFN-β promoter. Here we report that CHIKV nsP2, E2 and E1 strongly suppressed activation of the IFN-β promoter induced by the MDA5/RIG-I receptor signaling pathway, suggesting that nsP2, E2, and E1 are the major antagonists against induction of IFN-β. Delineation of underlying mechanisms of CHIKV-mediated inhibition of the IFN-β pathway may help develop virus-specific therapeutics and vaccines.

  • Purpose : The purpose of this study was to investigate the clinical application of Traditional Korean Medicine(TKM) treatment to climacteric symptoms after hysterectomy. Methods : The patient was a 51-years-old woman who suffered from hot flush, arthralgia, fatigue et al after hysterectomy. The patient was treated by TKM such as herb medication and acupuncture therapy. And her health status was evaluated by Kupperman's index and Medical Outcome Study Short Form 36( SF-36). Results : Climacteric symptoms after hysterectomy was reduced( Kupperman's index from 40 to 20). It got better that Physical Functioning(PF), General health(GH), Bodily pain(BP), Role limitation-Emotion(RE) and Mental Health(MH). So, it means that her health status was improved. Conclusion : TKM is expected to have positive effects on climacteric symptoms after hysterectomy, and can be improve woman's health status after hysterectomy.

  • Myxoma is the most common primary tumor of the heart. The common symptoms of myxoma can be similar to those of infective endocarditis, which include fever, weight loss, fatigue and arthralgia, but it is very rare for a myxoma to become infected. We report on a case of a 76-year-old male patient with fever and loss of conscious-ness, and he underwent an emergency operation after suffering shock state due to the infected myxoma.

The Meaning of Quality of Life for Bone Marrow Transplant Survivors (골수이식생존자의 삶의 질 의미)

  • The purpose of this study was to explore the concept of quality of life for bone marrow transplant (BMT) survivors and to gain understanding of nursing interventions that may improve QOL in this population. The data was gathered from 32 BMT survivors using seven open-ended questions. The items were based on previous research of Ferrell et al., (1992). Content analysis was performed on written responses to seven questions regarding BMT and QOL. The results were as follows : 1. The meaning of QOL for BMT survivors were "being healt", "being able to take a role", "having relationships", "self-accomplishment", "peace of mind", "spiritual well-being", "economic stability" and "being alive". 2. The impact of BMT on physical well-being were "skin impairment", "digestive problems", "infections ", "fatigue/weakness", "arthralgia", "eye dryness". "weight gain", "amenorrhea" and "hand tremor". 3. The impact of BMT on psychological well-being were "fear of recurrence", "sence of peace" and "hope". 4. The impact of BMT on socioeconomic status were "financial burden", "limitation of social activities" and "sence of withdrawal". 5. The impact of BMT on spiritual well-being were "dependency on Supreme Being", "spiritual arousal " and "spiritual maturity".   • The purpose of this study was to assess the pain-relieving effect of iontophoretically applied prednisolone on the patients with temporomandibular joint (TMJ) pain. 30 TMJ pain patients participated in this study. Inclusion criteria for subject selection were 1) tenderness of TMJ on palpation and 2) visual analog scales (VAS) of above 20 mm. The patients with polyarthralgia, rheumatic arthralgia, and systemic diseases were excluded for this experiment. The patients were randomly assigned to one of three groups; (1) Group 1 for administration of 2% prednisolone, (2) Group 2 for administration of 1% prednisolone, and (3) Group 3 for administration of saline. Phoresor PM600(Motion Control Utah, U.S.A) was used for administration of drugs by iontophoresis. Baseline and post-medication pain levels were recorded by VAS, pressure pain threshold (PPT), and palpation index (PI). Post-medication data were compared with baseline data for each group. The results were as follows : 1. All the groups showed significant decrease of VAS scores after treatment. 2. PPT was significantly increase only in Group 1 and 2 not in Group 3. 3. PI was significantly decreased only in Group 1 and 2 not in group 3.

The Anti-inflammatory and Analgesic Activities of Gumiganghwaltang (구미강활탕의 소염.진통작용)

  • This study was investigated on anti-inflammatory and analgesic activity on Gu-Mi-Gang-Hwal-Tang which has been used for the treatment of common cold and headache, arthralgia, fever in oriental medicine. The anti-inflammatory activities of Gu-Mi-Gang-Hwal-Tang water extract (GMGHT) on the carrageenin induced edema, acetic acid induced edema, and corton oil induced granuloma pouch in rats were examined. The analgesic effect of the GMGHT was investigated utilizing acetic acid induced writhing syndrome in mice. The antipyretic activity of the GMGHT was also investigated. The oral administration of GMGHT has been shown the anti-inflammatory activities in 1% carrageenin and acetic acid induced edema in rats. The GMGHT has been shown significant inhibitory effects on glanuloma and exudate formation in rats, especially the GMGHT (450 mg/kg) has the efficacy more than the GMGHT (150 mg/kg). The writhing syndromes induced by acetic acid in mice were inhibited, especially analgesic activity of the GMGHT (450 mg/kg) is similar to that of piroxicam and antipyretic activities were recognized in rats. In the present study, the GMGHT is indicated to have the anti-inflammatory, analgesic and antipyretic activities.

  • The purpose of this study was to investigate relationship between the eating behavior, physical and mental health. For this purpose, the data was collected by using questionnaires and intervews distibuted to 293 the elderly residing in Seoul. This study was designed to observe the eating behavior(eating habit, healthy food preference, smoking, drinking), physical health(weight, height, desease and heath of the current and past, self-awareness of the health) and mental health(weight, height, desease and heath of the current and the past, self-awareness of the health) and mental health(meeting, excursion, exercise, service, activi ty, depression). The major results are: 1. The heathy elderly had the better eating habit and the non healthy tried to stop smoking and drinking for their own health. 2. There were many elderly with neuralgia and arthralgia, especially women were worse. The sleep disorder related to many disease. When physical health was bad, so was mental health. 3. More than 50 percentage of subjects had light depression, especially women and the elderly at 60-75age were more serious. The depressive elderly had no meetings, excursion, exercise, service activity and showed a hight tendency for under-weight or obesity. In conclusion, the relationship between the eating behavior and physical and mental health wa very significant, so it was necessary to provide comfortable living condition to the elderly.

A Case of Successful Treatment of Refractory Synovitis Acne Pustulosis Hyperostosis Osteitis (SAPHO) Syndrome with Adalimumab

  • Synovitis acne pustulosis hyperostosis osteitis (SAPHO) syndrome is a rare disease that involves the skin, bones and joints. It is thought to be caused by infection with low-toxicity bacteria and to be the result of reactive infectious osteitis. However, this hypothesis has not yet been clearly established. New SAPHO syndrome treatment methods are needed because the disease does not respond to treatment in many cases. In this paper, a case is reported of SAPHO syndrome with pain in the acromioclavicular joint and with squamous and pustular macules on the palms and soles. First, the patient was treated with aceclofenac, prednisolon and sulfasalazine for two weeks. However, the symptoms were not relieved, so methotrexate and pamidronate were added to the treatment. Since no improvement was seen after four weeks of treatment, adalimumab was prescribed. The skin lesions were relieved two weeks later, and the bone pain and arthralgia, four weeks later. No recurrence or adverse effects were observed at the 22-week follow-up.

  • Based on 11 Classics of Materia Medica designated by Ministry of health and welfare bulletin 1995-15 as legal basis in scope and preparation of herbal prescription, we reviewed indication, property and processing of Rhus Verniciflua Stokes which appear extensively in literature since earlier Joseon Dynasty. Following is the conclusion of this review after comparing with 'Korean Pharmacopoeia: commentary on herbal product standard'. 1. The properties of Rhus Verniciflua Stokes in 'Korean Pharmacopoeia :commentary on herbal product standard' should be changed as 'Tonifying the middle, restoring the muscle, fulfilling marrow, breaking old static blood, tonifying and activating after removing mass, unblocking the meridian, killing worms' and indication should be changed as 'Old static blood, deep rooted mass, wind-cold-dampness arthralgia, cough, 9 types of chest pain, abdominal or flank accumulation of stagnated qi[氣], amenorrhea, hernia mass, small bowel or bladder colic pain, abdominal pain due to worm accumulation'. 2. The processing of Rhus Verniciflua Stokes in 'Korean Pharmacopoeia: commentary on herbal product standard' should include 'natural drying or steaming drying followed by grinding and stir-baking until ripened or smoking appears'.

Free radical scavenging activity of hyangsapyungwisan extract for herbal-acupuncture and protective effects against oxidative damage of HUVECs (향사평위산(香砂平胃散) 약침액(藥鍼液)의 자유기 소거활성 및 혈관내피세포의 산화적 손상에 대한 보호효과)

  • Objectives : Hyangsapyungwisan (HPS) has been used for treatment of cardiovascular diseases including of arthralgia, myalgia in traditional Korean medicine. However, the medical actions of HPS have not been clearly investigated. The aim of this study was to elucidate the antiradical and antioxidant activity of the extract for herb-acupuncture (HPS-HA) obtained from HPS. Methods & Results : HPS-HA exhibited a stronger inhibition rate (55.5%) on lipid peroxidation of rat liver homogenate induced by $FeCl_2$-ascorbic acid. In addition, HPS-HA markedly interfered with hydroxylradical generation through iron ions chelating action. While pro-oxidant effect of HPS-HA was nearly undetectable at concentrations of 0.5-10㎎/mL. Moerover, HPS-HA revealed a potent antiradical activities on 2,2-diphenyl-1-picryl-hydrazyl (DPPH) radicals, superoxide anions, nitric oxide and peroxynitrite. Furthermore, HPS-HA inhibited copper- and AAPH-mediated oxidation of human low-density lipoprotein (LDL), and also suppressed the relative electrophoretic mobility of LDL. HPS-HA showed strong protective activity against oxidative damage of HUVECs induced by SIN-1. Conclusions : These results suggest that HPS-HA may be effective in protecting against oxidative diseases.
