코일 색전술로 치료된 다발성 폐동정맥기형 1 예 (A Case of Multiple Pulmonary Arteriovenous Malformation Treated with Coil Embolization)

  • Pulmonary arteriovenous malformation(PAVM) is an uncommon congenital anomaly. As pulmonary arteriovenous malformation is a direct communication between the branches of pulmonary artery and vein which originated from the malformation of capillary development, major disturbances in gas exchange can result. This malformation results in the several symptoms such as dyspnea, hemopyssis, cyanosis, and severe neurologic complaints. However, the most of patients are usually asymptomatic. Selective pulmonary angiography is well known the helpful diagnostic method. Recently, therapeutic embolization has been advocated as the treatment of choice for pulmonary arteriovenous malformations. We report a case of multiple pulmonary arteriovenous malformation, which was detected on the simple chest X-ray and successfully treated with coil embolization in a 19-year-old asymptomatic woman.

흉벽에 발생한 동정맥기형 -1례 보고- (A Case Report of Arteriovenous Malformation on the Chest Wall)

  • Arteriovenous malformations are vascular anomalies containing a communication between artery and vein without an intervening capillary bed and also are the most dangerous of vascular malformations being hemodynamically active. Treatment must be careful usually limited and considered in the phase of activity of hemodynamics. The patient was 29-year-old female and had no specific signs and symptoms except buldging, pal- pable mass on the right posterolateral chest wall from several years ago and it was gradually growing from that time. The operation was done with ligation of the right 9th, 1 Oth intercostal arteries and dissection from other normal tissues and then excised the arteriovenous malformation mass and its feeding vessels. The pathologic result was arteriovenous malformation.

Brown-Sequard 증후군과 Horner씨 증후군으로 발현된 동정맥 기형 파열로 인한 경추 경막외 혈종 - 증례보고 - (A Cervical Epidural Hematoma Caused by Ruptured Arteriovenous Malformation Presenting as a Brown-Sequard and Horner's Syndrome - A Case Report -)

  • We report a rare case of Brown-Sequard syndrome associated with Horner's syndrome in cervical epidural hematoma caused by a ruptured arteriovenous malformation. A 54-year-old man developed sudden sharp neck pain, radiating to the interscapular area. Within hours, left side hemiplegia and decreased tactile sense and loss of contralateral pain sense ensued. Emergency cervical magnetic resonance image showed an epidural hematoma over the cervico-thoracic junction. The hematoma was located in the left posterolateral area of the cervical spinal canal. Emergent decompressive laminectomy and an evacuation of the hematoma were performed. A tangled soft tissue mass found in the hematoma was proven to be an arteriovenous malformation. To the authors, knowledge, this might be the first case of a Brown-Sequard syndrome associated with Horner's syndrome caused by ruptured cervical epidural arteriovenous malformation.

  • The present case illustrates an arteriovenous malformation of the cheek in a 25-year-old male. The clinical presentation. radiographic findings, differential diagnosis, treatment and histopathologic description are presented. Diagnosis of the lesion was confirmed by angiography, and the lesion was treated by angiographically controlled vascular embolization followed by complete surgical excision.

  • The cerebral arteriovenous malformation(AVM) rarely coexists with primary intracranial tumor. The authors experienced a patient with intracerebral hematoma due to AVM rupture in whom intracranial meningioma and intracranial aneurysms coexisted. The meningioma was located at convexity of right frontal lobe, and arteriovenous malformation at temporo-occipital lobe of same hemisphere with feeding from right middle cerebral artery, and three intracranial aneurysms exist at the cavernous portion of right internal carotid artery, AVM feeding artery, and intranidal of the AVM. The authors report a rare case of coexisted intracranial AVM, meningioma and aneurysms with review of literatures.

  • The authors report a unique case of unilateral Moyamoya disease with a rare combination of arteriovenous malformation (AVM) who presented with intracerebral hemorrhage (ICH). A 50-year-old man suffered from sudden onset of mental deterioration and right hemiparesis. Brain computed tomography (CT) showed intracerebral hemorrhage on left thalamus. Brain CT angiography and cerebral digital subtraction angiography (DSA) revealed AVM combined with unilateral moyamoya disease involving left middle cerebral artery (MCA) and choroid plexus in left lateral ventricle. Intraventricular hemorrhage and hydrocephalus were managed conservatively. A rare case of unilateral Moyamoya disease accompanied by a cerebral arteriovenous malformation is described and discussed with review of pertinent literature.

  • Head and neck arteriovenous malformation usually forms huge mass, cause profuse bleeding or potenially compromise the airway. This bleeding is vulnerable to be uncontrollable and life­threatening. Sometimes it has a high mortality. Although surgical resection is possible in some cases, the morbidity such as a defects of soft tissue is very high and its reconstruction is very difficult. The authors report an 11 year old female patient in whom occlusion of arteriovenous malformation with glue after transcutaneous embolization made a satisfactory results. At the beginning, she was transferred for massive oral bleeding. The bleeding was persistent and it was not possible to remove the packing in spite of many times of embolizations through feeding arteries. The massive bleeding trom the left upper alveolar mucosa compromised the airway and tracheotomy was done. Whenever the hypovolemic shock was occurred in a short time, blood transfusion and cardiopulmonary resucitation were done. To embolize the vascular mass of arteriovenous malformation, as a final trial before operation, the spinal needle was administered through the left upper gingiva under the fluoroscopy. The glue was injected on the target. The bleeding was stopped and we have noticed the absence of nidus on follow-up angiography after 3 weeks. We experienced that some cases of arteriovenous malformation in head & neck revealing the bleeding could be treated with transcutaneous embolization instead of surgical resection.

  • Prompt medical attention is crucial for congenital intracranial vascular malformations in children and newborns due to potential severe outcomes. Imaging is pivotal for accurate identification, given the diverse risks and treatment strategies. This article aims to enhance the identification and understanding of congenital intracranial vascular abnormalities including arteriovenous malformation, arteriovenous fistula, cavernous malformation, capillary telangiectasia, developmental venous anomaly, and sinus pericranii in pediatric patients.
