Background: The purpose of this study was to investigate the effects of ipsilateral and contralateral load changes during single-leg standing on the leg muscle activities of healthy people. Design: Randomized controlled trial. Methods: For all the subjects, a load was randomly applied to the ipsilateral or the contralateral side. While the load was applied, the subject raised a hand and then performed single-leg standing for 10 seconds using the dominant side. Results: During single-leg standing, the muscle activity of the gluteus medius, peroneus longus on the supporting side increased statistically significantly when an upper limb load was applied contralaterally, but no statistically significant differences were detected in the muscle activities of the tibialis anterior and the gastrocnemius using a test of within-subjects effects. Conclusion: It can be seen that muscle activities increase during exercise when the amount and frequency of a load are increased and when the same load is applied to different sides of the body. Such muscle activity increases may be applied to change the intensity of exercise when one is in a static posture, such as during single-leg standing.
The purpose of this study was to investigate the effect of the screen size of smart devices on the bending angle of the cervical spine. The subjects of this study were 30 healthy adults(15 men and 15 women) who used smartphones and tablet PC(personal computer). The changes in the bending angle of the upper and lower cervical spine were measured in the subjects after they had used a smartphone and a tablet PC for 300 seconds each. To make sure that all subjects began in the same starting position, an angle-measuring instrument was used to set the angles of the ankle, knee, hip, and arm joints to 90 degree. The subjects were asked to keep the trunk straight. They were asked to hold a smartphone in their hand and to bend their neck so that they could look down at the screen. Once they began using the smartphone in this manner, they were free to change their posture. We used a paired t-test to compare the bending angle of the cervical spine on subjects who used smartphones and tablet PC in the long-term and short-term there production error of cervical and the significance level was cervical. The results showed that, when using a smartphone and a tablet PC for 300 seconds, there was no significant difference in the bending angle of the upper cervical spine(p>.05), although there was a significant difference in the bending angle of the lower cervical spine(p<.05).
An excavator is a construction machine that can perform various tasks such as trenching, piping, excavating, slope cutting, grading, and rock demolishing. In the 2010s, unmanned construction equipment using ICT technology was continuously developed. In this paper, the path design process was studied to implement the output data of the decision stage, and the path design algorithm was developed. For example, the output data of the decision stage were terrain data around the excavator, excavator mechanism information, excavator hydraulic information, the position and posture of the bucket at key points, the speed of the desired bucket path, and the required excavation volume. The result of the path design was the movement of the hydraulic cylinder, boom arm, bucket, and bucket edge. The core functions of the path design algorithm are the function of avoiding impact during the excavation process, the function to calculate the excavation depth that satisfies the required excavation volume, and the function that allows the bucket to pass through the main points of the excavation process while maintaining the speed of the desired path. In particular, in the process of developing the last function, the node tracking method expressed in the path design table was newly developed. The path design algorithm was verified as this path design satisfied the JCMAS H02 requirement.
Objective: Nonspecific low back pain (NSLBP) is experienced worldwide by many age groups. Yoga is recommended as an exercise to reduce back pain and stress because it is a breathing exercise, posture, and meditation as key elements. The aim of this study is to compare the effects of yoga and stabilization exercise on pain intensity, function, and depression. Design: An open-label, parallel arm, randomized controlled trial Methods: Twenty-four participants were allocated to the experimental and the control group in a ratio of 1:1. Yoga (experimental group) and stabilization exercise (control group) were received twice a week for 6 weeks Participants were assessed at baseline and post-intervention for pain intensity (numeric pain rating scale), function (Aberdeen low back pain scale, flexibility,and strength), and depression (Beck depression inventory). Results: When the experimental group (Yoga) and control group (stabilization exercise) were performed twice a week for 6 weeks, numeric pain rating scale, Aberdeen low back pain scale, and flexibility in post-intervention showed significant improvement in both groups (P<0.05), However, in all variables, the experimental group showed a positive benefit compared to the control group (P<0.05). Conclusions: The results of this study show that yoga has more positive benefits compared to stabilization exercise in pain intensity, function, and depression in individuals with NSLBP.
This study developed a functional prototype jacket designed to reduce loads on the upper extremities of workers performing repetitive motions in the same posture for extended periods of time. Dynamic taping lines were applied to the upper extremities, and three dimensional (3D) supporters were inserted in the abdomen and back waist areas corresponding to the core muscles. Clothing pressure on the upper-extremity dynamic taping lines was set to two levels (proto P1 and proto P2), and the 3D supporters were designed in three types (proto FW, proto FW/BW, proto FW/BW/BBX). According to the subjective pressure perceived on each part of the upper extremities, the level proto P1 pressure was preferred. The proto FW/BW/BBX 3D supporter was rated as excellent, and the perceived pressure was ranked as satisfactory. The prototype jacket performed upper-extremity load reduction when the upper-extremity clothing-pressure level was set to 1.8 kPa, 2.1 kPa, and 2.4 kPa on the upper arm, forearm, and wrist regions, respectively, and when 3D supporters were installed in the abdomen and back of the waist with the addition of a back band.
Purpose: This study aimed: to confirm the balance ability of patients with spinal cord injury in the sitting state through a functional reach test using an elastic aid; and to propose a balance improvement plan. Methods: The study evaluated seven patients with spinal cord injury who could maintain a sitting posture through minimum assistance. A functional reach test was performed before and after wearing an elastic aid while sitting on a chair, and the effects before and after use of the elastic aid were compared and analyzed through a motion analyzer. Results: In the functional reach test, the forward movement distance of the hand was 97.45 mm before wearing the elastic aid, but significantly increased to 131 mm after wearing the aid (p<0.05). Corresponding forward movement distances for the shoulder were 81.26 mm and 113 mm (p<0.05 for the increase). There was no statistically significant change in lateral functional arm extension. Conclusion: It was confirmed, through a functional reach test, that trunk stability in patients with spinal cord injury increased with use of an elastic aid. In future, more efficient rehabilitation treatment programs will be possible if trunk stability in patients with spinal cord injury is improved by using elastic aids, and if various exercise treatments are also included in the rehabilitation programs.
PET-CT (Positron Emission Tomography-Computed Tomography)의 융합영상(Fusion image)에서 긴 영상 획득 시간에 의한 환자의 움직임은 융합영상에서 부적합한 결과로 나타나 영상의 질과 진단에 큰 영향을 준다. 의료장비 회사에서 제공하는 팔 지지대는 환자 자세에서의 편의성과 안전성을 고려하지 않아 PET/CT를 검사하는 동안 팔과 머리의 움직임 때문에 뇌기저부의 영상 결손이 생기고 머리의 좌우, 상하 움직임으로 인하여 재촬영이 빈번하다. 이에 우리는 두경부와 팔의 움직임을 최소화할 수 있는 환자보정기구를 개발하여 PET/CT 촬영 시 환자에게 편의성과 안전성을 제공하고 환자의 움직임으로 인한 재촬영을 줄이고자 한다. 2012년 6월부터 7월까지 PET Torso 검사를 위해 핵의학과에 내원하는 환자들 중 움직임에 불편함이 전혀 없는 환자 20명과 어깨 통증으로 팔을 올리기가 불편한 환자 20명을 대상으로 Patient Holding System (PHS)와 팔의 경사도($25^{\circ}$, $27^{\circ}$, $29^{\circ}$, $31^{\circ}$, $33^{\circ}$, $35^{\circ}$)를 변화시켜 가장 편안한 자세와 경사도를 선정 하였다. 또한 지지대 재질의 투과성에 대하여 유관업체의 검증을 하였고, 환자의 머리와 팔을 밴드(벨크로 형태)로 고정하여 이에 대한 편의성을 질의하였다. 그리고 움직임에 의한 재촬영 빈도를 알아보기 위해 2012년 1월부터 12월까지 PET Torso를 시행한 환자들 중 환자보정기구 사용 전/후 재촬영 여부를 분석하였다. 움직임에 불편이 전혀 없는 환자에서는 18명이 PHS와 팔의 각도가 $29^{\circ}$에서 가장 편안함을 느낀다고 답하였으며, 2명은 각각 $27^{\circ}$, $31^{\circ}$이라고 답하였다. 어깨 통증을 느끼는 환자에서는 15명이 $31^{\circ}$에서 가장 편안함을 느낀다고 답하였고, $33^{\circ}$에서 2명, $35^{\circ}$에서 3명이었다. 이를 위해 손잡이 부분이 상, 하 움직임이 가능하도록 제작하였다. 환자보정기구 재질의 투과성은 검증된 상태이며, 환자보정기구 자체의 움직임을 방지하기 위하여 PHS와 보정기구를 밴드(벨크로 형태)로 고정시켰다. 두경부의 움직임을 최소화할 수 있는 홈을 만들고 머리를 고정할 수 있게 고정밴드를 부착 하였고, 손목 및 전완과 상완의 움직임을 제한할 수 있는 고정밴드를 부착하여 팔의 흔들림으로 인한 움직임을 최소화 하였다. PET Torso 촬영 시 움직임에 의한 재촬영 빈도는 환자고정기구 사용 전 10.56% (191건/1,808명), 사용 후 2.77% (48 건/1,732명)로 7.79%의 발생 빈도를 줄일 수 있었다. 최근 의료 환경의 변화와 혁신으로 고가의 영상검사가 늘어남에 따라 고객이 요구하는 차별화된 서비스 제공은 필수적이다. 따라서 PET/CT를 검사하는 동안 환자의 편안함과 안전성을 확보하기 위해 인체공학적인 환자보정기구를 제공하여야 한다. 따라서 본 연구를 통해 PET Torso 검사 시 환자의 체형과 상황에 따라 인체공학적 경사도를 상, 하 조절할 수 있는 환자보정기구를 제작하였으며, 팔의 움직임에 의한 뇌기저부의 영상결손을 줄이고 머리의 움직임으로 인한 재촬영을 줄일 수 있었다.
본 연구는 물리치료사의 직무에 따른 Work-Related Musculoskeletal Disorder(WMSD)s에 대한 증상을 파악하기 위하여 통증치료군(56명), 성인 운동치료군(53명), 소아 운동치료군(22명)을 대상으로 근골격계 증상조사표와 Rapid Upper Limb Assessment(RULA)를 사용하여 근골격계 증상 및 작업의 유해도를 알아보기 위해 실시하였다. 그 결과 통증치료군 69.6%, 성인 운동치료군 84.9%, 소아 운동치료군 81.8%, 평균 77.9%의 자각 증상 호소율을 보였다. RULA를 이용한 인간공학적 위험 평가에서 Final Wrist & Arm Score와 Final Score에서 성인 운동치료군이 단위 시간당 작업 부하량이 가장 많은 것으로 나타났으며 Action Level은 통증치료군이 평균 $3.0{\pm}0.9$, 성인 운동치료군은 $3.3{\pm}0.6$, 소아 운동치료군은 $3.2{\pm}0.8$로 성인 운동치료군에서 가장 높았다. 근골격계 증상에 영향을 미치는 요인 중 키, 가사 노동, 육체적 부담정도, 과거 사고가 통계적으로 유의하였다. 이에 따라 물리치료사의 WMSDs를 방지하기 위한 장비 및 교육시스템이 보급되어야 할 것으로 사료된다.
본 연구는 파악력의 강도가 두 가지 팔 자세에서 어깨근육 근활성도에 어떠한 영향을 미치는지 알아보기 위하여 실시되었다. 건강한 성인 22명(남 11명, 여 11명)을 대상으로 실시하였고, 4가지 파악력(최대 파악력의 0%, 30%, 50%, 70%)을 팔의 중립 자세와 팔 $90^{\circ}$ 거상 자세에서 적용 했을 때 상승모근, 하승모근, 극하근, 전삼각근, 후삼각근의 근활성도를 표면 근전도를 이용하여 측정하였다. 팔 중립 자세에서는 파악력의 강도에 따라 극하근, 전삼각근, 후삼각근이 유의한 차이를 보였고(p<0.05), 팔 $90^{\circ}$ 거상 자세에서는 전삼각근, 후삼각근이 유의한 차이를 보였다(p<0.05). 본 연구의 결과, 2가지 팔의 자세에서 각각 파악력의 강도에 따라 어깨근육 근활성도가 변화됨을 확인하였다. 손 쥐기 과제를 중립자세에서 실시할 경우 어깨관절 안정근인 극하근이 활성화되고, 팔 $90^{\circ}$ 거상 자세에서는 삼각근을 억제된다는 점을 고려하여, 손을 사용하는 작업 시 인간공학적 접근으로 어깨 손상의 예방과 손상 후 재활 훈련 시 효율적인 전략으로 사용할 수 있을 것으로 여겨진다.
목 적: 정위적체부방사선치료(Stereotactic body radiotherapy) 시 환자셋업의 재현성 및 장시간 안정된 자세의 유지를 위한 고정용구는 정확한 치료를 위해 무엇보다 중요하다. 이에 본 연구는 상품화된 고정용구 두 가지와 자체 제작한 고정용구를 비교 평가하여 정위적체부방사선치료 시 최적의 고정용구를 적용하는데 그 목적이 있다. 대상 및 방법: 일반인 5명을 대상으로 각기 다른 세 가지 고정용구(A: Wing-board, B: BodyFix system, C: Arm up holder with vac-lock)를 각각 적용하였으며 대상자의 가장 안정적인 호흡주기를 선택하여, 고글 모니터를 착용 후 일정한 호흡을 30분간 유지토록 하여 호흡신호를 획득하였다. 호흡신호의 분석은 본원에서 자체 개발한 프로그램을 통해 호흡신호의 최고값(peak value)과 최저값(valley value)의 표준편차(standard deviation) 및 분산값(variation value)을 시간대별로 분리하여 획득하고 이를 이용해서 상대적 비교지수를 구하여 각 고정 용구를 비교평가 하였다. 결 과: 각 고정용구의 호흡시간대별 편차의 변화를 고려한 안정도를 평가하였으며, 고정 용구 별 비교지수는 각 실험자별로 다음과 같다. A: 11.20, B: 4.87, C: 1.63 / A: 3.94, B: 0.67, C: 0.13 / A: 2.41, B: 0.29, C: 0.04 / A: 0.16, B: 0.19, C: 0.007 / A: 35.70, B: 2.37, C: 1.86으로 나타났으며, 실험자 5명 모두 고정용구 C를 사용하였을 때 가장 안정된 값을 나타냈다. 반면 A를 사용했을 때 4명, B에서 1명이 상대적으로 가장 불안정한 호흡결과를 나타냈다. 결 론: 자체개발한 정위적체부방사선치료 고정용구(arm up holder with vac-lock)는 다른 두 고정용구에 비해 호흡을 안정적으로 유지시킴으로서 조사분할내 종양움직임(intra-fraction organ motion)을 감소시켜 치료효과를 높일 수 있을 것으로 생각된다. 특히 셋업을 장시간 유지시켜야 하는 정위적체부방사선치료의 특성상 시간의 경과에 따라 불안한 호흡주기를 나타낸 다른 두 고정용구에 비해 자체개발한 고정용구가 정위적체부방사선치료에 유용할 것으로 사료된다.
