열린사슬운동 및 닫힌사슬운동에서 위팔굽힘 각도에 따른 앞톱니근의 근활성도 비교 (A Comparison of the Serratus Anterior Muscle Activity according to the Shoulder Flexion Angles in a Closed Kinetic Chain Exercise and an Open Kinetic Chain Exercise)

  • PURPOSE: The purpose of present study was to ascertain how the activity of the serratus anterior muscle, the upper trapezius muscle and the pectoral major muscle was affected while the upper arm was being flexed at 70, 90 and 110 degrees respectively in a closed kinetic chain exercise (wall push up plus) and an open kinetic chain exercise (static hug). METHODS: Sixteen healthy young men subjects participated in the study. Surface electromyography (EMG) data were collected from the dominant-side muscles during a closed kinetic chain exercise and an open kinetic chain exercise. The activity of each muscle was measured quantitatively, and by the use of the two-way repeated ANOVA, the data were compared with each other according to exercises and shoulder flexion angles. RESULTS: Results indicated that the closed kinetic chain exercise did not interact with the open kinetic chain exercise (p>.05). In both the closed kinetic chain exercise and the open kinetic chain exercise, the activity of the serratus anterior muscle became different significantly according to angles (p<.05). Its activity increased in order of 70, 90 and 110 degrees (p<.05). In both exercises and all angles, muscle activity was significantly higher in the serratus anterior muscle than in the upper trapezius muscle and the pectoral major muscle (p<.05). CONCLUSION: The above results show that there is a need to selectively control the exercise stress of the serratus anterior muscle in the case of the patients with the shoulder impingement syndrome characterized by the winged scapula, insufficient scapular protraction and upward rotation.

경쟁적 게임을 적용한 집단 운동 프로그램이 만성 뇌졸중 환자의 균형에 미치는 영향 (The Effects of a Group Exercise Program with Competitive Games on Balance in Chronic Stroke Patients)

  • PURPOSE: The purpose of this study was to investigate the effects of a group exercise program with competitive games on balance in chronic stroke patients. METHODS: The subjects were chronic stroke patients living at home after their discharge from the hospital. A total of 32 subjects were divided into two groups. The experimental group performed a group exercise program with competitive games, and the control group performed individual exercises. The exercise program duration was one hour, and it comprised 10 minutes of stretching, 40 minutes of the main exercise, and 10 minutes of stretching at the end. The experimental group was divided into two teams of 8 players each and played competitive games (transfer a gym ball to another person, relay 10 m walking and return, transfer an object to the next person using both arms while sitting). The control group performed an arm/trunk exercise for 15 minutes and gait training for 25 minutes. These exercises were conducted twice a week (4 weeks). The Berg Balance Scale (BBS) and timed up-and-go (TUG) tests were performed to confirm the change in balance. RESULTS: In the experimental group, BBS showed a significant increase (p < .05) and TUG showed a significant decrease (p < .05). In the control group, there was no statistically significant change in the BBS. However, the TUG showed a significant decrease (p < .05). The BBS and TUG both showed statistically significant differences (p < .05) in the between-group comparison of the change in pre-post intervention values. CONCLUSION: A group exercise program with competitive games to improve the balance ability for fall prevention in chronic stroke patients could be used as a good intervention method.

Fermented Whey Protein Supplementation Improves Muscular Strength, Muscle Parameters, and Physical Performance in Middle-Aged Korean Adults: An 8-Week Double Blind Randomized Controlled Trial

  • The present study evaluated the effects of fermented whey protein using kimchi lactic acid bacteria Lactobacillus casei DK211 on skeletal muscle mass, muscle strength, and physical performance in healthy middle-aged males performing regular resistance exercises. Effective protein supplementation and regular exercise are two important factors for improving muscle health. Therefore, in this study, the effects of consuming fermented whey protein twice a day were investigated and compared with that of non-fermented supplementation. Forty-eight males (average age 44.8) were randomly assigned to two groups: Fermented whey protein supplementation (FWPS) and non-fermented whey protein concentration supplementation (WPCS) groups. Each group ingested 37 g of FWPS or WPCS twice a day for eight weeks. Body composition, muscle strength, and physical performance were assessed pre- and post-intervention. Independent t-tests or chi-square tests for the categorical variables were performed for analyzing the observations. FWPS was effective in promoting the physical performance in dynamic balance measurement and muscle health, indicated through the increment in grip strength (left), upper arm circumference, and flat leg circumference from the baseline. However, similar improvements were not observed in the WPCS group. These results imply that whey protein fermented by L. casei DK211 is an effective protein supplement for enhancing muscle health in males performing regular resistance exercises.

Comparison Study for Body Composition and Physical Function Fitness to the According of Exercise Type in Elderly Women

  • 본 연구의 목적은 여성노인을 대상으로 모던댄스, 아쿠아로빅, 복합운동 프로그램을 실시한 후 신체조성과 신체기능체력에 대한 변화 비교하여 여성노인에게 가장 적합한 운동이 무엇인지를 살펴보고자 실시되었다. 연구의 대상자는 J도에 거주하는 노인여성 47명을 대상으로 MDG(n=13) AEG(n=11), CEG(n=11), CG(n=12)을 무작위로 집단을 모집하여 12주간 주3회 1시간씩 운동을 실시하였다. 이 연구의 결과 통제그룹에 비해 grip strength, chair stand, arm curl, 2 minute step, 244cm up and go에서 유의한 차이를 보였으며, Back scratch는 MDG와 CEG에서만 운동 후 유의하게 감소하였으나 Chair sit and reach에서는 변화가 보이지 않았다. 본 연구결과 운동그룹 중 모던댄스(MDG)가 운동 후 가장 많은 신체기능체력이 향상된 것으로 나타났으며, 노인들에게 유연성과 관절의 가동성을 증진 시킬 수 있는 추가적인 운동프로그램이 필요하다. 또한 자신의 흥미에 맞는 운동을 자발적으로 참여하여 지속성 있게 하는 것이 중요할 것으로 생각된다.

The effect of short-term creatine intake on blood lactic acid and muscle fatigue measured by accelerometer-based tremor response to acute resistance exercise

  • [Purpose] The purpose of this study was to investigate the effects of short-term creatine intake on muscle fatigue induced by resistance exercise in healthy adolescent men, i.e., lactic acid concentration and wrist and head tremor measured by an accelerometer. [Methods] Twelve healthy adolescent men who had no experience with creatine intake were included. The subjects were randomly assigned to the creatine group and the placebo group, followed by 5 days of creatine and placebo intake, and 5 times of 5 sets of leg press, leg extension, bench press, and arm curl exercises at 70% repetition maximum (RM). The lactic acid concentration before and after exercising, rate of perceived exertion (RPE), and accelerometer-based wrist tremor and head tremor during exercise were measured. Subsequently, after 7 days to allow for creatine washout, the same exercise treatment and measurement were performed in each group after switching drug and placebo between the groups. [Results] The level of lactic acid before and after the acute resistance exercise trial was significantly lower in the creatine group than in the placebo group (P <0.05). The mean RPE during the resistance exercise was significantly lower in the creatine group than in the placebo group (P <0.05). There was no difference between the two groups in the mean wrist tremor during resistance exercise, but the mean head tremor values were significantly lower in the creatine group than in the placebo group in the arm curl, the last event of the exercise trials (P <0.05). [Conclusion] Short-term creatine intake reduces the blood fatigue factor increased by resistance exercise, and is thought to suppress fatigue, especially in the latter half of resistance exercise. Therefore, these findings indicate that short-term creatine intake can have an improved effect on anaerobic exercise performance.

어깨 부딪힘 증후군 환자에 대한 PNF 개념을 이용한 물리치료 사례보고 (Case Report of Physical Therapy Using the PNF Concept in a Patient with Shoulder Impingement Syndrome)

  • Purpose: The objective of this case report is to examine the impact of physical therapy using the proprioceptive neuromuscular facilitation (PNF) concept for a patient with shoulder impingement syndrome. Methods: The patient is a 35-year-old female who has felt pain in the right shoulder for one month. The physical examination evaluated sensory integration, pain, joint integration and mobility, posture, reflex integration, range of motion, muscular strength, analysis of movement, and shoulder function. Comprehensive physical therapy was given to the patient, including stretching, mobilization, strengthening, posture correction, coordination improvement, daily activities, and sports exercises. The therapy was given 5 times a week for the first 5 weeks, then 3 times a week for the next 5 weeks. In all, the intervention lasted for 10 weeks. Results: The patient's senses, posture, and muscular strength all improved to a normal level. The degree of pain fell from 3/10 to 0/10 for activities taking place below shoulder height, and from 8/10 to 1/10 for activities above the head. Additionally, joint integration, motility, range of motion, and movements also improved. The disabilities of the arm, shoulder and hand (DASH) for functional evaluation improved from 27.5 to 10.3. Conclusion: Physical therapy using the PNF concept is effective in improving the body structure, function, activity, and participation of patients with motor disorders of the shoulder impingement syndrome.

How Does 12-weeks of Taekwondo Training Effect Older Persons' Functional Fitness: A Preliminary Study

  • PURPOSE: This pre-post intervention study aimed to examine the effects of a 12-week supervised modified Taekwondo exercise program on the functional fitness of community-dwelling older adults. METHODS: 10 participants (mean age: 72.3 ± 4.6 years) completed the program over a 12-week period. The intervention consisted of Taekwondo-based exercises modified for older persons. Changes to functional fitness were measured using the finger-to-nose test, functional reach test, timed up and go test, 30-second sit-to-stand test, 30-second arm curl test, Chester step test, chair sit-and-reach test, and back scratch test. Participants were assessed prior to the training and after the final training session, changes were measured using descriptive statistics and paired sample 't' tests. RESULTS: Effect sizes ranged from small to large (Cohen's d = .22 to 1.23). The exercise program was well- tolerated by participants, with a high level of engagement and no attrition for the duration of the program. Results showed significant improvements in most measures of functional fitness (p < .05) except for the back scratch test (p = .051). CONCLUSION: These findings suggest a well-designed, supervised, modified Taekwondo exercise program can significantly improve functional fitness in older adults.

율동적 동작 훈련이 젊은 여성의 Wellness에 미치는 영향 (A Study of Dance Movement Training on the Wellness of young Women)

  • Exercises are achievement oriented, the process is frequently perceived as hard and difficult Participants drop out from exercise programs in the middle of the training period. Dance movement, which is the deliberate and systematic use of movement, is enjoyable during the movement and provides opportunities for persons to express them-selves. Regular long term dance movement may in-duce a training effect with a decreased drop out rate. Dance movement could be one way to attain wellness, however, there have been few studies to evaluate both physiological and psychological aspects of dance movement. This study focused on evaluating the effects of dance movement training on body weight, resting blood pressure and heart rate, limb circumference and strength, stress response and subjective feelings. This quasi-experimental study was designed as a nonequivalent control group pre test -post test study. Ten healthy fe-male subjects, aged between 19 and 31 years volunteered for an eight week dance movement program. Ten healthy female subjects, between 19 and 21 years of age paticipated as controls. None of the subjects had performed regular physical activity for six months prior to the study. Dance movement was created with reference to Heber's movement guide. The Dance movement program consisted of approximately 30 minutes of dance, three days per week, for eight weeks. During each 30 minute work out, there were approximately 5 minutes of warm-up dancing, 20 minutes of conditioning dance and 5 minutes of cool-down dancing. The intensity for the conditioning phase was at between 60% and 65% of age-adjusted maximum heart rates. Body weight, resting blood pressure and heart rate, circumference of mid upper arm, mid thigh and mid calf, muscle strength of upper and lower limb, physical and psychological response to stress were measured prior to, and following the experimental treatment. Body weight was measured by digital weight scale(Kyung In Corp., Korea). Resting systolic and diastolic blood pressure were measured by sphygmomanometer, Resting heart rate was measured for one minute in a relaxed sitting position using the radial artery. Circumference of mid upper arm, mid thigh and mid calf was determined by tape measure. Muscle strength of the upper extremities was measured by a grip dynamometer (Takei Corp. No.1857, Japan) and that of the extremities was measured by the length of time the leg could be held at 45° Physical and psychological responses to stress were measured using the Symptoms of Stress (SOS)Scale. Paticipants in the dance movement were interviewed by the facilitator following the eight weeks, and their thematic responses about the dance movement were recorded. Following the eight week dance movement train-ing, body weight decreased significantly, circumference of mid thigh and mid calf increased. The length of time leg - raising could be held tended to increase following the dance movement training. Resting systolic and resting heart rate showed a tendency to decrease. Total mean score of stress response tended to de-crease, and mean score of habitual patterns, do-pression, anxiety / fear, anger and cognitive disorganization decreased remarkably following the eight week dance movement. Thematic responses about the dance movement were positive following the training.

복싱 국가대표선수 라이트 스트레이트 펀치 동작의 Kinematic 특성분석-사례연구 (The Case Study of A Kinematic Analysis of the Right-Straight Punch in Korean National Representative Boxers)

  • The purpose of this study was to analyze the kinematic variables of the right-straight punch(RSP) in boxing with three-dimensional analysis technical methods. The subjects are boxers who have been playing in national boxing representative team and the RSP is their special favorite technique, The right-straight punches were filmed on 16mm video cameras(30frames/sec.) The kinematic variables were temporal, postural and center of gravity(COG). The mean and the standard deviation of variables have been obtained and used as basic factors for examining characteristics of the RSP by out-boxers. From the data analysis and discussion, the following conclusions have been drawn. 1) Temporal variables It is a significant characteristic that LDJ and KDM s' the amount of elapsed time(EF) needed for both an attack and a defense were similar : ET for stretch-out of attack-arm was $0.52{\pm}0.04\;sec$. and return was $0.54{\pm}0.01\;sec$. Therefore, a defense motion is as important as an attack motion. 2) Posture variables When the subjects performed a RSP, the significant characteristic of the ankle angle was that it wasn't completely returned to the original position after stretching-out. Therefore it is necessary to do supplementary exercises, such as side steps, to move the center of gravity more effectively. The hee angle was not fully stretched either. In regard to the hip angle, it should be rotated with all strength to harmonize with the direction of movement. 3) Center of Gravity(COG) variables When both LDJ and KDM performed a RSP, a significant characteristic was the transformation of sagittal view rather than transverse or frontal views.

Comparison of the Upper and Lower Trapezius and Serratus Anterior during Push-up Plus Exercise with and without an Elastic Shoulder Band

  • PURPOSE: Scapular stability is very important to arm function during activities of daily living. The push-up plus exercise is commonly used to stabilize the shoulder muscles. In the present study, we designed an elastic shoulder band (ESB) that could be used with the push-up plus exercise and studied muscle activities of the serratus anterior (SA) and the lower and the upper trapezius (LT and UT) on stable and unstable support. METHODS: Fifteen healthy people participated in this study. Three muscles were investigated using surface EMG with and without the ESB on stable and unstable support. All subjects performed each trial three times for 7 seconds per set and rested for 30 seconds. Repeated one-way ANOVA was used for statistical analyses. RESULTS: The EMG activity of the SA was significantly different during the push-up plus exercise on stable support with the ESB compared with that on unstable support without the ESB (p<.05). However, there were no differences in the activities of UT and LT on stable and unstable support (p>.05). CONCLUSION: The ESB was effective for the activity of SA during the push-up plus exercise on stable support; therefore, it has the potential for use by people during push-up plus exercises. However, further studies are required to investigate the rehabilitation tools for patients with shoulder instability.
