PRIMARY SYNOVIAL CHONDROMATOSIS OF THE TEMPOROMANDIBULAR JOINT WITH TEMPORAL INVOLVEMENT (측두골을 포함한 원발성 악관절 활막 연골종증)

  • Synovial chondromatosis is a rare, benign, monoarticular arthropathy that is characterized by the development of highly cellular, metaplastic cartilaginous nodules in the synovial membrane. It commonly affects larger joints such as the knee, elbow, wrist, shoulder, and hip. Synovial chondromatosis of the temporomandibular joint(TMJ) is rare. Moreover, the temporal involvement of synovial chondromatosis without connection with joint is greatly rare. A 44-year-old women had experienced pain of the right TMJ area and limitation of mouth opening. MRI and CT revealed multiple calcified loose bodies and widening in right upper joint space of TMJ and osteolytic lesion in right temporal bone. Treatment consisted of removal of multiple loose bodies, resection of the osteolytic lesion through the preauricular approach. She was diagnosed with primary transitional synovial chondromatosis of TMJ with involvement of temporal bone. In spite of remaining of the loose bodies, pain and mouth opening improved and there have been no recurrence of signs and symptoms for 5 years follow up.

A STUDY OF THE RADIATION EFFECTS ON THE BASAL CELL OF THE RAT TONGUE EPITHELIUM ACCORDING TO THE EXPOSURE TIMING (방사선 조사시기에 따른 백서 설상피의 기저세포에 미치는 영향에 관한 연구)

  • The purpose of this study was to investigate chronic radiation effects on the basal cell of the rat tongue epithelium according to different irradiation timing. Forty-two female rats were devided into 5 experimental groups according to different irradiation timing and were irradiated single dose of 396cGy by MK cell irradiator using Cs-137. Experimental rats were sacrificed at the 2nd week, 4th week and 6th week after birth. The specimens were examined with light microscope and transmission electron microscope. The following results were obtained. 1. The first changes after irraditation were vacuoles. The vacuoles were chiefly observed in the cytoplasm, perinuclei area, and nuclei. 2. The most severe degenerative changes in the basal cell layer were observed in all experimental groups. ; cellular disarrangement, vacuole formation, widening of intercellular space, enlarged mitochondria & rER, and chromatin clumping were seen. 3. The cellular degenerative changes were most severe at the 4th week after birth in all experimental group, and the basal cell hyperplasia was seen at the 6th week in the most of experimental groups 4. The experimental groups 3 and 4 show more severe and more prolonged cellular degeneration than experimental groups 1 and 2, which were irradiated in pregnancy, and experimental group 5, which was irradiated after tongue maturation.

Study on the Beacon Signal Characteristic for Efficiency Analysis of Indoor Positioning (실내 위치 측위 효율성 분석을 위한 비콘 신호 특성 연구)

  • Recently, the activity area in the indoor space has been widening. Various types of service technologies have been developed and applied user location, tracking, spatial pattern analysis and environmental analysis in indoor space. BLE-based beacon is used for the service technology in the indoor space. Therefore, in this paper, we collected and analyzed signals for indoor positioning based on beacon. For this purpose, a module was designed to analyze the efficiency of indoor location measurement using beacon. Experimental results show that the accuracy of the measured data at close range is improved. Also, it was analyzed that the more accurate the position data is extracted without the obstacle in the indoor space.

A Quantitative Review on Deep Learning and Smart Factory from 2010 to 2023

  • The convergence of deep learning and smart factory is drawing a lot of attentions from not only industrial but also academic circles. The objective of this article is to quantitatively review on deep learning and smart factory from 2010 to 2023. This research analyzed the 138 articles, extracted from the Core Collection of Web of Science, in terms of four dimensions such as the main trend in article publications, the main trend in article citations, the distribution of article publications by research area, and the keywords representing the main contents of published articles. The quantitative review results reveal the following four points: First, the article publications drastically grew from 2019 to 2022 in its annual trend. Second, the article citations have rapidly grown since 2018. Third, Engineering, Computer Science, and Telecommunications are the top 3 research areas composing the 138 articles. Fourth, it is the top 10 keywords such as 'deep', 'learning', 'smart', 'detection', factory', 'data', 'system', 'manufacturing', 'neural', and 'network' that represent the main contents of the 138 articles published from 2010 to 2023 in deep learning and smart factory. These findings revealed by this quantitative review will be significantly useful for deepening and widening relevant future research on deep learning and smart factory.

A Study of Structural Changes in the Seoul Metropolitan Area's Economy Since the 1990's : A Decomposition Analysis of Labor Productivity (1990년대 이후 수도권 경제의 구조적 변화에 관한 연구 : 노동생산성의 요소분해를 중심으로)

  • The Seoul Metropolitan Area's economy has undergone structural changes since the 1990s. With recent structural changes in the Korean economy in mind, this paper employs a decomposition analysis of Gross Regional Domestic Product per head and labor productivity to examine how the region's economic performance has changed relative to that of Korea since the 1900s. There are some findings. First, a strong cumulative causation process, derived from industrial linkages and agglomeration effects, has oocurred in the Seoul Metropolitan Area, widening economic disparities between the Seoul Metropolitan Area and the other regions especially since the Korean financial crisis in 1997 mainly due to Gyeonggi province's overwhelming cumulative GRDP growth relative to the national average. Second, the Seoul Metropolitan Area has begun w display good economic performance in terms of labour productivity relative w the national average since the late 1990s, with the region's industrial upgrading being implemented. Finally, manufacturing, financial intermediation and communications have made sensitive contributions to relative Seoul Metropolitan Area's labor productivity growth.

The Effect of Left Ventricular Volume on Postoperative Cardiac Function in Tetralogy of Fallot (활로씨 4징증의 좌심실용적이 수술후 심장기능에 미치는 영향)

  • Tetralogy of Fallot is a cyanotic congenital heart disease characterized by large ventricular septal defect[VSD] and stenosis of right ventricular outflow tract[RVOT] and the degree of RVOT stenosis and the state of pulmonary arteries are the major determinant of prognosis of this anomaly after operation. The sum of blood flow through RVOT and collateral flow from systemic arteries determine the total pulmonary blood flow and it is drained to left atrium and left ventricle. Therefore the degree of development of left ventricle not only reflects pulmonary blood flow and the status of peripheral pulmonary arteries but also affects postoperative prognosis as a systemic ventricle. In this article, left ventricular volume and its influence on postoperative cardiac function in tetralogy of Fallot were studied in 34 patients operated on at Department of Thoracic and Cardiovascular Surgery, Seoul National University Hospital in 1985. Mean age of the patients was 5 1/12*3 9/12 years[range 9/12 - 14 8/12 years], mean body surface area[BSA] 0.65*0.20m2[range 0.38 - 1.22m2], mean body weight 15.6k6.48kg[range 7.0 - 36kg]and mean hematocrit 50.6*9.77%[range 32.0 - 73.5%]. Left ventricular end-diastolic volume[LVEDV] of them were from 11.2 to 113cc and there was a good linear correlation between BSA[m2, X]and LVEDV[cc, Y][Y= - 20.0+923x, r= 0.84, p < 0.005]. Mean LVEDV/m2 was [57.6 * 18.3 cc / m2[range 28.7 - 95.8 cc / m2] and there was a significant reduction of volume compared with normal value. As body surface increases, there was a increasing tendency in LVEDV/m2 but there was no statistical significance. Mean total amount of postoperatively infused dopamine in these 33 patients[except one who expired postoperatively] was 65.6*74.5mg / kg and it was 40.6*44.0mg / kg in routine RVOT widening group [Group I] and 205*49.3mg / kg in transannular RVOT widening group[Group II]. There was a statistically significant difference between two groups. In group I patients there was a good linear inverse correlation between dopamine total amount[mg / kg, Y] and LV volume[cc / m2, X] [Y = 150 - 1.89 X, r = - 0. 77, p < 0.005]. But there were no correlations between dopamine total amount and Hct, cardiopulmonary bypass time and aorta cross clamp time. In conclusion, the patient with small preoperative left ventricular volume required more amount of dopamine as an inotropic agent for the maintenance of a cardiac function in postoperative period. But this is a result of immediate postoperative period and does not reflect the long term effect of left ventricular volume in tetralogy of Fallot. There must be more study for the evaluation of its long term effect.

  • This paper was undertaken to observe the displacement of craniofacial complex with cervical headgear and to compare narrowing or widening effect of palate by use of contraction or expansion face-bow, respectively. The 3-dimensional finite element method(FEM) was used for a mathematical model composed of 597 nodes and 790 elements and an electrical resistance strain gauge investigation was performed to validate the finite element model. The outer bow of cervical headgear was adjusted to be placed below the occlusal plane by $25^{\circ}$ and met the midsagittal plane by $40^{\circ}$, and was loaded 1kg on each right and left hook toward posterior direction. The results were as follows 1. Generally, the maxillary teeth and facial bone were displaced in posterior, medial and downward direction. 2. It was the maxillary 2nd bicuspid that moved bodily. 3. The craniofacial complex rotated in a clockwise direction around the rotating axis which lay from the most posterior and lowest point connecting nasal crest of maxillary bone and vomer, progressively toward a more posterior, lateral and upward direction, anterior and upper area of pterygomaxillary fissure, base of medial pterygoid plate and laterally to the contact area of zygomatic arch with squamous part of temporal bone. 4. No contraction effect was observed by contraction face-bow when compared to the standard face-bow. 5. In case of expansion face-bow, the areas of maxillary 2nd bicuspid, molars and palate were expanded remarkably.

The Financial Support of Regional Bank for SMEs and Regional Financial Policy (지방은행의 지역중소벤처기업지원과 지역금융정책)

  • This study investigates regional financial markets and proposes some policy measures for boosting up the regional financial system. Financial supports for start-ups, small and medium sized enterprises(SMEs) in early stage growth have been constrained due to expanded financial gaps among regions during financial restructuring period. The bank consolidation through M&A is associated with widening financial gaps between Seoul area and others, and between regions where regional banks survives or not. Loans to small firms are associated with relationship lending techniques that may be better supported by smaller regional banks. The loan rate to SMEs in locals where regional banks are activated has tended to be 10%point higher than the locals where regional banks closed, and also drive the greater contribution for preventing local capital outflow and promoting local capital investment by local penetrating strategy. Government should develop regional financial policies to boost up regional financial system, and expand the business area of regional financial institutions for supporting start-ups and SMEs.

Juxtacortical Osteogenic Sarcoma of Chondroblastic Type on the Maxilla (상악골에 발생된 연골형성현 측피질골육종)

  • 39-year-old female had been treated for the exophytic mass on buccal aspect of the left, maxillary posterior area 2 years and 8 months ago. Tentative diagnosis was obtained as fibrous dysplasia on clinical and radiographic examinations and histopathologic findings revealed as osteochondroma after bone trimming at that time. She revisited for the treatment of recurred lesions. We reviewed this case with clinical. radiologic and histopathologic standpoints retrospectively, and came to a conclusion that the tumor primarily occurred was juxtacortical osteogenic sarcoma and recurred due to inadequate treatment and then expanded over intramedullary. This case shows that the diagnosis of osteosarcoma should take account of the patient history, clinical. radiographic and histopathologic findings and it requires attentive follow up check. Retrospectively reviewed results were as follows ; At first visit, oral examination revealed a bony hard swelling on the buccal aspect of the left maxillary posterior area. Radiographically, a dense radiopaque mass was noted on the site. The lesion showed hot uptake of /sup 99m/Tc-MDP. Histopathologic diagnosis was done as osteochondroma, but it was considered as osteogenic sarcoma when compared with the recurrent lesion. When she revisited for the treatment of multiple bony swelling on the left maxilla, radiograms showed typical features of malignancy such as widening of periodontal ligament space and sunray appearace, and coincided with benign characters as follows; relatively well circumscribed lesion and expansion and displacement of the adjacent structures. Finally, histopathologic findings of the lesion was well differentiated chondroblastic osteogenic sarcoma.

Clinical Experience of Buccal Fat Pad Pedicled Flap for Denuded Area in Palatoplasty (입천장성형술 시 발생한 골 노출부의 피복을 위한 협지방대 유경피판의 경험)

  • Purpose: The primary goal of palatoplasty is to enable normal speech with harmonious growth of face. Some children who had palatoplasty display typical findings of transverse maxillary deficiency requiring orthodontic widening of the maxilla. Levi (2009) described a cleft palate repair coupled with pedicled buccal fat pad flaps to cover bone exposed areas of the hard palate. Hence we report clinical experiences of cleft palate repair using pedicled buccal fat pad flap. Methods: Four Veau class II and a Veau class I cleft palate patients underwent palatoplasty with buccal fat pad flap by single surgeon from April 2009 to August 2009. Two patients received 2-flap palatoplasty and three patients 1-flap palatoplasty, respectively. After the cleft palate repair, sharp mosquito scissors was placed in the superior buccal sulcus just lateral to the maxillary tuberosity and inserted directly through the mucosa resulting in buccal fat pad extrusion. The elevated flap was moved to cover mucoperiosteal defect in hard palatal area. Results: Five patients underwent primary palatoplasty using buccal fat pad flap. Flap harvest and inset took on average 9 minutes per flap. Mucosal epithelization took 18 days on average. No patients had complications related to the buccal fat pad flap. Conclusion: Buccal fat pad pedicled flap has significant potential to function as an added vascularized tissue layer in cleft palate repair and we can expect better growth of maxilla with this method although longer duration of follow-up was unavailable.
