Bentall Operation in a Patient with a Unicommissural Unicuspid Aortic Valve

  • A unicuspid aortic valve is a rare congenital malformation that frequently presents with valvular dysfunction and dilatation or aortic aneurysm, requiring combined aortic valve surgery and aortic repair. Some patients show severe valve calcification extending into the interventricular septum, possibly resulting in damage to the conduction system during debridement for valve replacement. We present a rare case of severe aortic stenosis with a unicommissural unicuspid aortic valve diagnosed by preoperative transesophageal echocardiography in a 36-year-old man. After composite graft replacement of the aortic valve, aortic root, and ascending aorta, a permanent pacemaker was placed because of postoperative complete heart block.

  • 대동맥근부를 포함한 대동맥 판막 심내막염은 적극적인 내과적 및 외과적 치료를 해도 염증의 재발이나 사망률이 높은 질환이다. 자가폐동맥판막을 이용하여 전대동맥근부치환술은 이식편이 생체 조직이고, 면역학적으로도 안전한 조직이라 술후 항응고요법이 요구되지 않는 술식이다. 저자들은 38세된 여자환자에서 대동맥근부를 포함한 대동맥 판막 심내막염 환자에서 Ross술식을 이용하여 성공적으로 치험하였고 수술후 3개월간 추적중 양호한 상태이다.

Long-Term Outcomes of Homografts in the Aortic Valve and Root Position: A 20-Year Experience

  • Background: The advantages of using a homograft in valve replacement surgery are the excellent hemodynamic profile, low risk of thromboembolism, and low risk of prosthetic valve infection. The aim of this study was to evaluate the long-term outcomes of homograft implantation in the aortic valve position. Methods: This is a retrospective study of 33 patients (>20 years old) who underwent aortic valve replacement or root replacement with homografts between April 1995 and May 2015. Valves were collected within 24 hours from explanted hearts of heart transplant recipients (<60 years) and organ donors who were not suitable for heart transplantation. The median follow-up duration was 35.6 months (range, 0 to 168 months). Results: Aortic homografts were used in all patients. The 30-day mortality rate was 9.1%. The 1- and 5-year survival rates were $80.0%{\pm}7.3%$ and $60.8%{\pm}10.1%$, respectively. The 1-, 5-, and 10-year freedom from reoperation rates were $92.3%{\pm}5.2%$, $68.9%{\pm}10.2%$, and $50.3%{\pm}13.6%$, respectively. The 1-, 5-, and 10-year freedom from significant aortic dysfunction rates were $91.7%{\pm}8.0%$, $41.7%{\pm}14.2%$, and $25.0%{\pm}12.5%$, respectively. Conclusion: Homografts had the advantages of a good hemodynamic profile and low risk of thromboembolic events, and with good outcomes in cases of aortitis.

Reoperative Aortic Root Replacement in Patients with Previous Aortic Root or Aortic Valve Procedures

  • Background: Generalization of standardized surgical techniques to treat aortic valve (AV) and aortic root diseases has benefited large numbers of patients. As a consequence of the proliferation of patients receiving aortic root surgeries, surgeons are more frequently challenged by reoperative aortic root procedures. The aim of this study was to evaluate the outcomes of redo-aortic root replacement (ARR). Methods: We retrospectively reviewed 66 patients (36 male; mean age, $44.5{\pm}9.5years$) who underwent redo-ARR following AV or aortic root procedures between April 1995 and June 2015. Results: Emergency surgeries comprised 43.9% (n=29). Indications for the redo-ARR were aneurysm (n=12), pseudoaneurysm (n=1), or dissection (n=6) of the residual native aortic sinus in 19 patients (28.8%), native AV dysfunction in 8 patients (12.1%), structural dysfunction of an implanted bioprosthetic AV in 19 patients (28.8%), and infection of previously replaced AV or proximal aortic grafts in 30 patients (45.5%). There were 3 early deaths (4.5%). During follow-up (median, 54.65 months; quartile 1-3, 17.93 to 95.71 months), there were 14 late deaths (21.2%), and 9 valve-related complications including reoperation of the aortic root in 1 patient, infective endocarditis in 3 patients, and hemorrhagic events in 5 patients. Overall survival and event-free survival rates at 5 years were $81.5%{\pm}5.1%$ and $76.4%{\pm}5.4%$, respectively. Conclusion: Despite technical challenges and a high rate of emergency conditions in patients requiring redo-ARR, early and late outcomes were acceptable in these patients.

True Aneurysm of the Common Coronary Button in a Marfan Patient with an Anomalous Right Coronary Artery after a Bentall Procedure: A Case Report

  • True aneurysms of the coronary artery after aortic root replacement in Marfan syndrome patients are very rare. An anomalous origin of the right coronary artery (RCA) from the left sinus of Valsalva adds complexity during aortic root surgery. We present a case of a 37-year-old male patient with Marfan syndrome who had an RCA anomaly and a 4.5-cm true aneurysm of the common coronary button 14 years after a previous Bentall procedure. A redo Bentall operation and hemi-arch replacement were successfully performed. The anomalous origin of the RCA from the left sinus of Valsalva was safely divided and anastomosed as separate coronary buttons to the prosthetic composite valve graft. To prevent coronary button aneurysms after aortic root surgery in Marfan patients, the coronary buttons and the corresponding side holes on the prosthetic graft must be reduced to the maximum possible extent.

  • 배경: Behcet씨 병과 동반된 대동맥판막 폐쇄부전에서 인공 대동맥판막 치환술 후 발생하는 인공판륜 주위 누출이나 가성동맥류는 가장 심각한 합병증의 하나로써, 재수술을 필요로 한다. 저자들은 Behcet씨 병과 동반된 대동맥판막 폐쇄부전 환자에서 동종이식편을 이용한 대동맥근부 치환술 결과를 알아보고자 하였다. 방법 및 대상: 1992년 1월부터 2001년 12월까지 저자들은 6명의 Behcet씨 병 환자에서 동종이식편을 이용한 근부치환술 7례와 1례의 Ross 술식을 시행하였다. 남자 5명, 여자가 1명이었으며 수술 당시 평균 연령은 37$\pm$9세(27~51세)였다. 2명은 1차 수술에서 동종이식편을 이용한 근부치환술을 시행하였고 다른 4명은 대동맥판막 치환술 후 발생한 인공판륜 주위 누출로 동종이식편을 이용하여 근부치환술을 시행하였다. 인공판막치환술 후 재수술까지 평균 기간은 21 $\pm$29개월(5 ~73.3개월, 중앙값, 7.6 개월)이였다. 결과: 동종이식편을 이용한 근부치환술 후 조기 사망은 없었다. 6명에서 평균 18.9$\pm$24.0개월(1.9~68.9개월, 중앙값, 8.4개월)을 추적 관찰하였다. 판막치 환술 후 동종이식편을 이용한 근부치환술 환자 4명중 2명에서 상행대동맥에 발생한 가성동맥류와 폐동맥 동종이식편의 기능 부전으로 재수술이 필요하였다. 1명은 대동맥 동종이식편을 이용하여 대동맥근부, 상행대동맥 및 부분 대동맥궁을 재치환하였으며 다른 1명은 Ross수술을 시행하였다. 결론: 본 연구 결과 Behcet씨 병과 동반된 대동맥판막 폐쇄부전에서 동종이식편이나 폐동맥 자가이식편을 이용한 대동맥근부.치환술은 양호한 조기 수술 결과를 보이며, 인공판막 치환술 후 발생할 수 있는 인공판륜 주위 누출이나 가성동맥류의 발생을 줄일 수 있다. 그렇지만 Behcet씨 병의 정확한 진단과 수술 전후 적절한 내과적인 치료 및 수술시 염증 조직의 완전한 제거가 장기 결과의 향상을 위해 매우 중요할 것으로 사료된다.

대동맥 근부 복합 인공 판막 도관 치환술에 관한 임상적 고찰 (Clinical Study of Composite Valve Graft Replacement of the Aortic Root)

  • 배경: 복합 인공 판막 도관(composite valve graft) 치환술은 다양한 대동맥 근부 질환들에 사용되어 왔다. 이 연구의 목적은 이 술기에 대한 중기 결과를 알아보는 데 있다. 대상 및 방법: 1992년부터 2009년까지 대동맥 근부 복합 인공 판막 도관 치환술을 받은 39명의 환자를 대상으로 후향적 조사를 하였다. 환자들의 평균 나이는 $49.2{\pm}16.4$세였고, 평균 추적 관찰 기간은 $64.2{\pm}53.4$개월이었다(최대 기간, 176개월). 결과: 4명(8.6%)의 병원 내 사망이 있었고 모두 응급 대동맥 박리인 경우였다. 4명의 만기 사망이 있었는데 2명은 잔류 대동맥류, 1명은 경막하 출혈, 1명은 판막 주위 역류가 원인이었다. 생존율은 각각 1년, 5년, 10년에 93.5%, 85.0%, 85.0%였다. 추적 관찰 기간 동안 3명의 환자에서 뇌혈관 사고가 발생하였고, 뇌혈관 사고로부터의 자유율은 각각 1년, 5년, 10년에 97.0%, 92.0%, 80.0%였다. 모든 뇌혈관사고는 출혈과 연관된 합병증이었다. 결론: 복합 인공 판막 도관을 이용한 대동맥 근부 치환술은 좋은 결과를 보여 왔다. 그러므로 이 술기는 다양한 대동맥 근부 질환의 치료에 좋은 방법이라 할 수 있다.

Composite graft를 이용한 대동맥근부 치환술: Button 술식의 중단기 결과 (Composite Graft Aortic Root Replacement with Coronary Button Reimplantation: The Early and Mid-Term Results)

  • 목적 본 연구는 대동맥근부를 침범한 상행대동맥류 환자에서 composite graft를 이용한 대동맥근부 치환술시 button 술식을 이용한 수술의 중단기 결과를 분석하고자 하였다. 방법 및 대상 1995년 4월부터 2001년 9월까지 composite valve graft를 이용한 대동맥근부 치환술 환자중 button 술식을 이용한 54명을 대상으로 후향적으로 조사하였다. 술전 좌심실 기능부전 14명(25.9%), 대동맥판막 폐쇄부전 48명(89%), 그리고 Mafarn 증후관17명(31.5%)에서 동반되었다. 진단은 대동맥판륜 확장증 29명(53.7%), 대동맥 박리증 11명(20.4%), 대동맥근부를 침범한 상행대동맥류 12명(22.2%), 그리고 대동맥염이 2명(3.7%)이었다. 과거에 심장이나 상행대동맥 수술을 받은 환자는 6명(11.1%)이었다. 근부치환술은 모든 환자에서 composite graft를 이용하여 button 술식으로 시행하였다. 동반수술은 대동맥궁 치환술 21명(38.9%), 관상동맥우회술 7명(13.%), 승모판 성형 혹은 치환술 4명, 그리고 기타 6명이었다. 평균 순환정지, 체외순환 및 대동맥차단 시간은 각각 18 $\pm$ 9분, 177 $\pm$ 42분과 127 $\pm$ 31분이었다. 결과 조기 사망은 1명(1.9%)에서 있었다. 술후 생존자 53명중 52명(98.1%)에서 평균 24.6 $\pm$ 19.5 개월(1-78개월)을 추적하였다. 만기 사망은 외상성 뇌출혈로 사망한 1명을 포함해 2명(3.8%)이었으며 대동맥근부 치환술과 관련한 만기 사망률은 1.9%였다. 한편 술후 1년과 6년 survival rate는 각각 98.0 $\pm$ 2.0%와 93.1 $\pm$ 5.1%였다. 대동맥근부 치환술과 관련한 만기 합병증은 근부 봉합 부위에 발생한 가성동맥류와 인공대동맥 판막 기능부전이 각각 1명에서 발생하여 재수술이 필요하였으며(3.8%), 1년과 6년 후 재수술의 freedom rate는 각각 97.9 $\pm$ 2.1%와 65.3 $\pm$ 26.7%였다. 잔여 대동맥에 대한 수술은 1명에서 박리성 흉복부대동맥류에 대해 단계적으로 치환술을 시행하였다.

베체트씨병에 의한 대동맥판 폐쇄부전의 수술적 치료 (Surgical Management of Aortic Insufficiency in Behcet's Disease)

  • Background: Cardiac involvement of Behcets disease is very rate, however, the prognosis of Behcet disease depends on cardiovascular complications. In this article, we described surgical treatment of aortic insufficiency with Behcets disease. Material and Method: From March 1986 to February 1998, we operated on 10 patients of aortic insufficiency with Behcets disease. Male to female ratio was 8 to 2, and age ranged from 21 to 40 years(mean 32.8 years). There were 8 patients with evidence of Behcets disease and another 2 patients had some suspicious findings of Behcets disease(i.e., prosthetic value dehiscence, hypertrophied aortic wall). Adequate preoperative medical treatment for Behcets disease was done in 3 patients. Result: We performed 24 open heart surgeries in 10 patients. Redo value replacements using prosthetic valves were done in 4 patients. Among them, 2 patients were operated on for a second redo valve replacement and one of them operated on for a 4th and 5th operation because of recurrent paravalvular leakage. These 4 patients expired. 1 patient who had undergons tissue value replacement is alive. 1 patient who underwent Cabrol operation expired dut to rupture of graft anastomosis site. We used homografts in 3 patients. In 2 of them, we performed aortic root replacement and subcoronary valve replacement in another patient. The patient who underwent subcoronary valve insertion had remnant aortic insufficiency, so we are closely observing him. We also performed Ross operation in a 24 year old female who suffered severs aortic insufficiency and endocarditis after aortic valvuloplasty. 5 patients are alive and mean follow up duration is 49.0 months. Among them, we used homografts or sutografts in 4 patients. We could observe excellent clinical results in the patients who underwent aortic root replacement using homograft and they were treated medically for Behcets disease. Conclusion: We concluded that adequate preporative diagnosis, clinical suspicion, and periopertive medical treatment for Behcets disease are very important for the result of surgical management of aortic insufficiency with Behcets disease. The use of homograft or autograft was helpful for the healing of anastomosis site and we should carefully observe the long term follow up results.

Subvalvular Septal Myectomy and Enlargement of the Narrow Aortic Root in Patients with Aortic Valve Replacement

  • In candidates for aortic valve replacement [AVR]it is our primary intention to implant the largest possible vale prosthesis of at least 23 mm in diameter in patients with severe valvular aortic stenosis. However, in many patients there is an additional subvalvular asymmetric septal hypertrophy which in some cases may cause an postextrasystolic increase of the LV-aortic gradient. Another component of the aortic stenosis syndrome is a narrow valvular ring, or a combination of both. After complete removal of the diseased valve and decalcification the narrow aortic ring [< 23 mm] can be widened firstly by transaortic subvalvular septal myectomy- [TSM] thus unfolding the left ventricular outflow tract[LVOT]and secondly by extending the oblique aortic incision into the aortic valve ring or further down into the anterior leaflet of the mitral valve. The sub-and supra-valvular defect will be closed by patch enlargement of the aortic root [PEAR] using autologous pericardium. These techniques allow a considerable enlargement of the valvular ring of about 4 to 10 mm in circumference. In a retrospective study using a computerized program, 847 patients with AVR [1980-1984]were reviewed to evaluate the intraoperative hemodynamic results mainly concerning relief of the transvalvular gradient. In 626 patients AVR was performed, 151 patients had double valve replacement [AVR+MVR], and 70 patients had AVR plus additional surgical procedures. Concentrating on the AVR-group [n=626] there were 103 patients with TSM, 24 patients with PEAR and 20 patients with TSM+PEAR which demonstrated that in a total, of 147 patients of this groups [23.5%] an additional procedure was necessary. The Statistical evaluation of the intraoperative pressure measurements before and after AVR in relation to the size of the implanted prostheses indicated the lowest preoperative mean gradient in patients with AVR alone, the highest in patients who afforded TSM plus PEAR. However, after AVR the mean gradients in all three groups were very low [mean 5 to 10 mmHg]. These data indicate that in patients with a narrow aortic ring and additional considerable ASH, TSM and PEAR are suitable techniques to enlarge the aortic root to enable the implantation of an adequate aortic valve prosthesis. Long-term controls have shown that autologous pericardium is a qualified graft material for the ascending aorta.
