Resuscitative Endovascular Balloon Occlusion of the Aorta (REBOA)와 혈관색전술 후 수술적 치료로 호전된 급성 출혈을 동반한 외상성 췌장 손상 (Pancreatic trauma with acute hemorrhage successfully treated surgically after Resuscitative Endovascular Balloon Occlusion of the Aorta (REBOA) and angioembolization)

  • 혈역학적으로 불안정한 췌장손상의 치료에서 Resuscitative Endovascular Balloon Occlusion of the Aorta (REBOA)의 역할은 아직 불명확하다. 저자들은 출혈을 동반한 췌장손상에서 REBOA와 경도관 동맥색전술을 시행하여 출혈을 먼저 제어한 후 수술을 시행한 증례를 보고하고자 한다. 65세 남자가 20 m 높이에서 추락하면서 발생한 복통과 흉통을 호소하며 본원 응급실을 통하여 내원하였다. 전산화단층촬영에서 췌장 미부에 grade III 손상이 의심되고 비장동맥으로부터 조영제 누출이 관찰되었다. 전산화단층촬영 시행 후 수축기혈압이 60 mmHg까지 하강하여 혈역학적 안정을 위해 REBOA를 시행하였다. REBOA 시행 후 수축기혈압이 130 mmHg로 상승하였으며, 비장동맥에 대해 색전술을 시행하였다. 혈관조영술에서 더 이상 출혈이 없는 것을 확인하고, REBOA 제거 후 응급 개복 및 췌장미부절제술을 시행하였다. 수술 소견에서 췌장미부의 주췌관 손상을 동반한 열상이 관찰되었으며, 수술 중 큰 출혈은 관찰되지 않았고 주 췌관의 손상이 확인되었다. 술 후 환자는 합병증 없이 회복하였다. 본 증례에서 저자들은 혈역학적으로 불안정한 출혈을 동반한 췌장손상 환자에서 REBOA와 동맥색전술 시행 후 췌장미부절제술을 안전하고 효과적으로 진행할 수 있었다.

First Results of the Single Heartstring Aortotomy for Multiple Off-Pump Vein Grafts: A Case Series

  • To validate the technique of the single Heartstring aortotomy for multiple off-pump venous bypass grafts (described in 2015), the results of a 38-month follow-up study of 18 patients, including high-risk patients, are presented. No early deaths or cardiac or cerebral complications occurred. During the follow-up period, 2 patients died of non-cardiac causes, and 3 developed coronary ischemia. Ischemia occurred due to late graft occlusion in 2 patients, both of whom had normal postoperative courses and correct graft flow. The presence of acute symptoms 24 months after surgery in these patients indicated that technical graft failure was unlikely. This safe technique combines the advantages of simple and reproducible revascularization, the off-pump approach, and minimal aortic manipulation.

Transvenous occlusion of patent ductus arteriosus using an embolization coil in a Maltese dog

  • A 6-year-old female Maltese dog (body weight 2.0 kg) was referred to the Veterinary Teaching Hospital, Kangwon National University with primary complaints including exercise intolerance and heart murmur. Based on clinical and diagnostic findings including grade V/VI left basal continuous murmur, bounding femoral pulsation, left ventricular enlargement pattern in electrocardiogram, cardiomegaly with aortic bulging on the thoracic radiography, and shunt flow between aorta and pulmonary artery on the echocardiography, the dog was diagnosed as the left-to-right patent ductus arteriosus. The patent ductus arteriosus was successfully treated by lodging a single embolization coil with transjugular approach.

Cuff Technique for Small-Diameter Vascular Grafts in the Systemic Arterial Circulation of the Rat

  • This study determined the feasibility of the cuff technique for small-caliber vascular grafts in a rat model. A graft was implanted with the cuff technique or suture technique in a 1-cm segment of the abdominal aorta in 12 rats. The mean aortic clamp time was 29 minutes with the cuff technique and 44 minutes with the suture technique; the cuff technique was significantly shorter. Abdominal angiography at 1 week after implantation showed no significant stenosis in 9 rats, focal stenosis of the mid-portion of the graft in 1 rat with each technique, and total occlusion of the graft in 1 rat with the suture technique. We have successfully used the cuff technique for anastomosis for small-caliber vascular grafts in an animal model.

대동맥 근부치환술에 대한 Cabrol 술식의 성적 (The Results of using the Cabrol Technique for Aortic Root Replacement)

  • 배경: 대동맥근부의 여러 질환에 있어서 인공판막도관을 이용한 대동맥근부 치환술은 현재 가장 보편적으로 사용되고 있으며, 여러 가지 변형 방법이 소개되고 있다. 그 중 13년간 Cabrol 술식을 이용하여 본원에서 대동맥근부 치환술을 시행하였던 환자들을 대상으로 수술 결과를 분석, 평가하였다. 대상 및 방법: 1994년 1월부터 2006년 12월까지 본원에서 Cabrol 술식을 적용한 25예를 조사 대상으로 하였다. 환자들의 평균연령은 $43.7{\pm}14.1$(6세${\sim}$65세)였으며 성별은 남자 21명, 여자는 4명이었다. 생존한 모든 환자에서 추적관찰이 이루어졌으며, 평균추적관찰기관은 $60.7{\pm}50.4\;(1{\sim}162)$개월이었다. 수술 전 진단은 대동맥판막 폐쇄부전을 동반한 대동맥판륜확장증이 18예로 가장 많았으며, 대동맥판륜확장을 동반한 대동맥박리가 7예 있었다. 결과: 수술사망은 1예(4.0%)가 있었으며 MDCT를 촬영한 9예 중 4예에서 관상동맥문합부의 의미 있는 협착 및 폐색이 확인되었다. 결론: Cabrol 술식은 비록 낮은 수술사망률을 보이나, 관상동맥문합부의 협착 및 폐색 등의 장기 합병증에 대해서는 다소 높은 이완률이 확인되었으며 이에 따라 Cabrol 술식의 적용은 대동맥근부에 대한 재수술 및 대동맥판륜과 관상동맥 입구부가 매우 가까운 경우 등 제한적인 경우에 사용하는 것이 바람직하다고 판단된다.

비정상 좌측 쇄골하동맥을 동반한 Kommerell 게실의 하이브리드 수술법 (Hybrid Operation of a Kommerell’s Diverticulum with Left Aberrant Subclavian Artery)

  • 53세 된 여자 환자가 우측 대동맥 궁, Kommerell 게실 및 비정상 좌측 쇄골하동맥 등의 병변으로 내원하였으며 증상은 없었다. 6 mm ringed Gore-Tex 인조혈관을 이용하여 비정상 좌측 쇄골하동맥과 좌측 총경동맥을 연결하여 우회하고, 대동맥 궁의 Kommerell 게실을 막고자 stent graft를 삽입하는 hybrid 수술을 시행 하였다. 수술 직후 시행한 혈관 조영술과 수술 후 첫날 시행한 전산화 단층 촬영상 성공적인 대동맥류 차단 및 좌측 쇄골하동맥의 우회소견을 보여주었다. 수술 및 시술에 동반된 합병증은 없었다.

혈관수술에 대한 임상적 고찰 (Clinical Experience with Vascular Surgery)

  • The vascular surgery is the field that has developed in early 20 century and is progressing nowadays. Recent advance in surgical technique accompanying with excellent medical diagnosis and treatment, prompt angiographic usage, development of variable prosthetic material, and concomitant use of anti-coagulant have made remarkable results of vascular surgery. 83 cases of vascular surgery have been performed at Thoracic and Cardiovascular Surgery Department of Pusan National Unversity Hosaital since 1971 till 1990, for 20 years and their results are followed. Patient ductus arteriosus and Buerger`s disease were omited in this study. 1. The age distribution shows that the fifth and sixth decades are most frequently affected and mean age was 56.1 years old. Male to female ratio is 1: 2.32. 2. Among the 83 cases of all, number of occlusive vascular disease is 46 and that of aneurysmal disease is 33. 3. In clinical manifestation, most common symptom of occlusive disease is pulselessness and pain was next. Mass sensation is most commonly complained by patients of aneurysmal disease. 4. CT scan was more important in diagnosis of aneurysmal diseases and angiogram was more commonly used in occlusive diseases. 5. The common site of arterial occlusion was common iliac artery, femoral artery, aortic bifurcation, and external iliac artery, as its frequency rate. The most commonly affecting portion of aortic aneurysm was abdminal aorta, and descending thoracic aorta and femoral artery were next 6. Preoperative associated diseases were atherosclerosis[41 cases], hypertension[21 cases], valvular heart disease[11 cases], and diabetes mellitus[9 cases], etc, 7. Operative methods in ocllusive diseases were thrombectomy[36.9%], endarterectomy [10.9%], and bypass graft insertion[52.7%]. Among the bypass graft, Y-graft was used in 7 case, straight graft was used in 17 cases, and saphenous venous graft was used in 2 cases. 8. Postoperative complications were developed in 17 cases, and morbidity rate was 36. 9. Eleven patient were died within 1 month after operation, so operative mortality rate was 13.3%. 10. Duration of patency was beteween 7 and 58 months[average 27.5 months] in occlusive diseases and their 5-year patency rate was 56.3%. Duration of patency of aneurysmal disease was 20 months in aveage and their 5-year patency rate was 51.3%. 11. Patients of eleven cases of occlusive disease and two cases of aneurysmal disease required reoperation for variable reason. 12. 35 cases of patient have used anticoagulants: coumadin, ticlid, and persanthin-ASA combination.

Effect of the Proximal Anastomosis Configuration of the Radial Artery in Patients Undergoing Coronary Artery Bypass Grafting

  • Background: Several factors, such as the degree of target vessel stenosis, are known to be associated with radial artery (RA) graft patency in coronary artery bypass grafting (CABG). There is a lack of data regarding the effect of the RA proximal configuration (aortic anastomosis versus T-anastomosis). This study evaluated the effects of the RA proximal configuration on the patency rate and clinical outcomes after CABG. Methods: We conducted a retrospective study, analyzing 328 patients who had undergone CABG with an RA graft. We divided the patients into 2 groups. The primary endpoint was RA patency and the secondary endpoints were overall mortality and major adverse cardiac and cerebrovascular events (MACCE). We performed a propensity score-matched comparison. Results: Aorta-RA anastomosis was performed in 275 patients, whereas the rest of the 53 patients received T-RA anastomosis. The mean age was 67.3±8.7 years in the T-RA anastomosis group and 63.8±9.5 years in the aorta-RA anastomosis group (p=0.02). The mean follow-up duration was 5.13±3.07 years. Target vessel stenosis ≥70% (hazard ratio [HR], 0.42; 95% confidence interval [CI], 0.20-0.91; p=0.03) and T-RA anastomosis configuration (HR, 2.34; 95% CI, 1.01-5.19; p=0.04) were significantly associated with RA occlusion in the multivariable analysis. However, T-RA anastomosis was not associated with higher risks of overall mortality and MACCE following CABG (p=0.30 and p=0.07 in the matched group, respectively). Conclusion: Aorta-RA anastomosis showed a superior patency rate compared to T-RA anastomosis. However, the RA proximal anastomosis configuration was not associated with mortality or MACCE.

Clinical Outcomes of Surgical Repair with a Composite Graft for Abdominal Aortic Aneurysm Accompanied by Iliac Artery Aneurysm

  • Background: Iliac artery aneurysm is frequently found in patients undergoing surgical repair of an abdominal aortic aneurysm. The use of commercial bifurcated grafts is insufficient for aorto-biiliac replacement with complete iliac artery aneurysm resection. We evaluated the effectiveness of handmade composite grafts for this purpose. Methods: A total of 233 patients underwent open surgery for abdominal aortic aneurysm between 2003 and 2019, including 155 patients (67%) treated with commercial grafts and 78 patients (33%) treated with handmade composite grafts. Their operative characteristics, postoperative outcomes, and late outcomes were retrospectively reviewed. Results: The early mortality rate did not differ significantly between the groups. On average, the handmade composite graft technique took approximately 15 minutes longer than the commercial graft technique (p=0.037). Among patients who underwent elective surgery, no significant differences between the conventional and composite groups were observed in the major outcomes, including red blood cell transfusion volume (2.8±4.7 units vs. 3.1±4.7 units, respectively; p=0.680), reoperation for bleeding (2.7% vs. 3.1%, respectively; p>0.999), bowel ischemia (0% vs. 1.6%, respectively; p=0.364), and intensive care unit stay duration (1.9±6.6 days vs. 1.6±2.4 days, respectively; p=0.680). The incidence of target vessel occlusion also did not differ significantly between groups. Conclusion: The increased technical demand involved with handmade composite grafting did not negatively impact the outcomes. This technique may be a viable option because it overcomes problems associated with commercial grafts.

개실술에서 우측흉골우연소절개법을 이용한 최소침투수술 (Minimally Invasive Cardiac Surgery through A Small Right Parasternal Incision)

  • Background: Minimally invasive techniques for open heart surgery are widely accepted in these days. There are minimally invasive approaches by the right or left parasternal incision and another approaches by mini-sternotomy of upper or lower half or sternum. We report the safety and efficacy of minimally invasive technique with right parasternal incision compared with the routine full sternotomy. Material and Method: From April 1997 through February 1999, 20 patients(Group A) underwent minimally invasive cardiac operations. We chose 41 patients(Group B) whose preoperative diagnosis were the same and general conditions were similar and who underwent routine full sternotomy before April 1997. We compared A group and B group in many aspects. We performed routine full median sternotomy in B group but we did a minimally invasive technique through a small right parasternal incision in A group. Result: mean age was 36.1 years in both groups. In disease entities, there were 11 cases of ASD, 9 cases of mitral valve disease in group A, and 16 cases of ASD, 25 cases of mitral valve diseases in group B. In ASD, operation time, cardiopulmonary bypass time of aortic occulusion time were 263 min, 82 min, and 41 min in group A and 180 min, 53 min, and 32 min in group B. In mitral valve disease, operation time, cardiopulmonary bypass time and aortic occlusion time were 267min, 106 min, and 70min in A group and were 207 min, 82 min, and 69 min in group B. There were significant differences in operation time, CPB time, and ACC time between group A and group B. There was a significant difference in the amount of bleeding in postoperative day 1 between group A and group B of mitral diasease. However, there was no significant difference in the amount of bleeding in other comparisons. Mean length of incision was 8.7 cm in group A. There was no significant difference in postoperative complications between A group and B group. There was no mortality in either group. Conclusion: We conclude that this minimally invasive technique with right parasternal incision is cosmetically excellent but it is not effective in reducing operative time and there was no significant difference in recovery time and postoperative complications compared with routine full sternotomy.
