  • We report a case of concurrent saccular aneurysms caused by a penetrating atherosclerotic ulcer of the thoracic and abdominal aorta that were successfully treated by staged endovascular repair. Even though surgical open repair or endovascular repair is the treatment option, use of endovascular repair is now accepted as an alternative treatment to surgery in selected patients. To prevent contrast medium-induced nephropathy and spinal cord ischemia caused by a simultaneous endovascular procedure, a saccular aneurysm of the descending thoracic aorta was excluded by stent graft, followed by the placement of a bifurcated stent graft in the infrarenal abdominal aorta one month later.

Effect of Saline Flush on the Enhancement of Vascular and Liver via Saphenous Vein for Abdominal CT in Dogs

  • The aim of this study was to evaluate the contrast effect if a saline flush following low-volume contrast medium bolus improves vascular and parenchymal enhancement using a saphenous vein in abdominal CT for small animals. Six clinically healthy beagle dogs underwent abdominal contrast-enhanced CT. They were divided into nine groups (each group, n = 6), according to the volume of contrast medium 1, 2, and 3 mL/kg, and volume of the saline solution 0, 5, and 10 mL. Dynamic CT scanning was performed at the hepatic hilum level. The maximum contrast enhancement, time to maximum enhancement, and time to equilibrium phase were calculated from the time attenuation curves. Mean attenuation values for all groups were measured in the aorta, portal vein, and liver. After contrast enhancement, grading of image quality regarding surrounding artifacts and evaluation of the hepatic hilum structures was performed. For comparison of the effect of the contrast material and saline solution doses, differences in mean attenuation values between the contrast medium 2 mL/kg without saline flush group and the remaining groups, and between contrast medium 3 mL/kg without saline flush group and the remaining groups, were analyzed for statistical significance. There were no significant differences between with and without saline flushing at the same contrast medium dose groups. There were no significant differences in peak values between the 3 mL/kg dose of contrast medium alone and the 2 mL/kg dose of contrast medium with saline solution flush. However, there was a significant difference in peak values between the 3 mL/kg dose of the contrast medium without the saline flush group and the 2 mL/kg dose of the contrast medium alone group. Grades of the artifacts were not significantly different in the saline flush regardless of the dose of the contrast medium. Using 2 mL/kg of contrast medium with saline solution flush resulted in similar liver parenchyma attenuation, compared with using 3 mL/kg of contrast medium without saline solution flush. In CT evaluation of hepatic parenchymal diseases, using 2 mL/kg of contrast medium with saline solution flush may yield decreased risk of contrast nephropathy and cost-saving.

복부 CT 검사 시 이중에너지 기법을 통한 적정한 조영제 양에 관한 연구 (Study on Optimum Contrast Medium Quantity during Abdominal CT using Dual Energy Technique)

  • 이중에너지 기법을 이용한 복부 CT 검사에서 적정한 조영제 양에 대하여 알아보고자 하였다. 일반적인 단일에너지 복부 CT 검사를 시행했던 환자들 중 이중에너지 기법을 이용하여 추적검사를 시행한 30명을 대상으로 하였다. 단일에너지 복부 검사에서 사용했던 조영제 양 대비 30%, 40%, 50%, 60%, 70%로 각각 설정하여 영상을 획득한 후 대동맥, 하대정맥, 간문맥, 간실질에 관심영역을 설정하여 조영증강정도(hounsfield unit; HU)를 구하여 단일에너지에서 측정된 부위와의 값을 비교 분석하였다. 그 결과 기존대비 60%로 조영제를 설정한 군에서는 대동맥 HU : $210.80{\pm}13.609$, 하대정맥 HU : $190.40{\pm}25.215$, 간문맥 $198.40{\pm}21.232$, 간실질 HU : $119.20{\pm}7.98$로 각각 측정되었으며. 단일에너지 복부 CT 검사는 대동맥 HU : $205.40{\pm}16.426$, 하대정맥 HU : $188.20{\pm}21.476$, 간문맥 HU : $195.40{\pm}22.744$, 간실질 HU : $121.00{\pm}6.595$ 이었다. 따라서 이중에너지를 이용하여 복부 조영 CT검사를 시행할 때 조영제의 양을 60%로 설정한 후 검사하는 것이 각 장기에서의 단일에너지 조영검사시와 비슷한 조영증강정도를 획득할 수 있어 기존 조영제 양의 감소와 동시에 조영제를 낮은 속도로 주입할 수 있어 신장기능 저하환자, 정맥혈관이 약한 환자, 과거 조영제 부작용이 있었던 환자에게 유용한 검사라 사료된다.

소아 흉부 CT검사에서 조영제 주입에 관한 프로토콜의 최적화 (Optimization of Protocol for Injection of Iodinated Contrast Medium in Pediatric Thoracic CT Examination)

  • 본 논문의 목적은 흉부 CT검사를 시행하는 소아환자의 조영제 투여를 최소화하고 조영 증강은 최적화하기 위하여, 몸무게의 변화에 따른 생리학적 주입 프로토콜을 정립하는 것이다. 만 10세 이하의 소아환자 80명을 대상으로 연구하였다. 혈관 조영제 300 mgI/ml를 사용하였으며, A그룹은 몸무게 1.5배의 용량을 주입하였고, B, C, D 그룹은 10%씩 감소하면서 생리식염수 5 ~ 15 ml를 추가 주입하였다. 조영제와 생리식염수의 주입량과 속도 및 지연시간을 몸무게와 연동하여 계산할 수 있는 생리학적 모델의 엑셀시트를 적용하였다. 화질평가는 상대정맥, 상행대동맥, 폐동맥, 우심방, 우심실, 좌심방, 좌심실, 하행대동맥, 쇄골하정맥의 CT number와 SNR을 측정하여 그룹간 비교하였으며, 생리식염수 주입에 따른 인공물 감소효과를 파악하기 위하여 쇄골하정맥과 상대정맥의 CT number를 비교하였다. 또한 조영 증강의 정도와 인공물에 대하여 정성적 평가를 추가로 수행하였다. 조영제 주입 프로토콜에 따른 SNR을 비교 평가한 결과, 상대정맥과 폐동맥, 하행대동맥, 우심실, 좌심실에서 유의한 차이를 보였으며, CT number는 모든 장기에서 유의한 차이를 보였다. 특히 조영제를 10% 감소하고 생리식염수를 추가 주입한 그룹은 조영 증강 정도는 차이가 없었으나, 20% 이상 감소한 그룹은 조영 증강의 정도가 낮게 나타났다. 또한 생리식염수 주입한 그룹은 상대정맥과 쇄골하정맥의 조영 증강이 매우 감소되었고, 조영제에 의한 빔 경화 인공물이 상당히 감쇄되었다. 결론적으로 소아의 흉부 CT 검사에서 생리학적 조영제 주입 프로토콜의 적용은 조영제에 의한 인공물을 감소시키면서, 불필요한 조영제 사용을 예방하고 조영 증강의 효과는 향상시킬 수 있었다.

조영제 사용이 양성자 Range와 SOBP(Spread-Out Bragg peak)에 미치는 영향 (Influence of Intravenous Contrast Medium on Proton range and SOBP(Spread-Out Bragg peak))

  • 목 적 : 조영제는 CT모의치료시 정상조직과 악성종양을 구분하는데 뛰어난 대조도를 제공하기 때문에 CT모의치료에 널리 사용하고 있다. 하지만 조영제는 치료계획 CT에서 일시적으로 존재하기 때문에, 치료계획과 실제 치료 사이에는 Hounsfield Unit(HU)의 차이가 발생하게 된다. 이에 본 연구에서는 조영제 사용으로 인한 HU의 변화가 양성자 치료계획에 미치는 영향을 확인 하고자한다. 대상 및 방법 : 20명의 간암 환자에게서 Phase에 따른 HU값의 변화를 측정하였다, 그리고 5명의 양성자치료계획에서 HU변화에 따른 Range와 Spread-Out Bragg peak(SOBP)의 변화를 살펴보았다. Hand made water phantom을 이용하여 깊이와 HU의 변화에 따른 Range와 SOBP의 변화를 확인하였다. 결 과 : 20명 간암 환자의 HU 변화(Pre contrast, Arterial phase, Porta phase)는 간 조직에서 ($58{\pm}5.7$, $75{\pm}9.5$, $117{\pm}14.6$)이었으며 대동맥에서($40{\pm}6.1$, $279{\pm}49.0$, $154{\pm}22.8$)이었다. 5명의 간암 양성자치료계획에서 HU 변화에 따른 Range 변화는 평균 2.5 mm, SOBP 변화는 평균 1.4 mm 이었다. Hand made water phantom study에서는 HU가 증가함에 따라 Range는 감소되었고 SOBP는 줄어들었다. 결 론 : 양성자 치료계획에서 HU값의 변화는 양성자 Range와 SOBP를 변화시켰다. 실제 양성자 빔에서 HU 변화에 따른 Range와 SOBP의 변화에 대한 추가적인 연구가 필요할 것으로 사료 된다.

아연의 1차혈관평활근세포 증식에 대한 기능 (The function of zinc in the primary vascular smooth muscle cell proliferation in rats)

  • Purpose: The vascular smooth muscle cells (VSMCs) in mature animals have implicated to play a major role in the progression of cardiovascular diseases such as atherosclerosis. This study aimed at optimizing the protocol in culturing primary VSMCs (pVSMCs) from rat thoracic aorta and investigating the effect of cellular zinc (Zn) deficiency on cell proliferation of the isolated pVSMCs. Methods: The thoracic aorta from 7-month-old Sprague Dawley rats was isolated, minced and digested by the enzymatic process of collagenase I and elastase, and then inoculated with the culture Dulbecco Modified Eagle Medium (DMEM) at 37℃ in an incubator. The primary cell culture morphology was observed using phase-contrast microscopy and cellular Zn was depleted using Chelex-100 resin (extracellular zinc depletion only) or 3 µM N,N,N',N'-tetrakis(2-pyridinylmethyl)-1,2-ethanediamine (TPEN) (extracellular and intracellular zinc depletion). Western blot analysis was used for the detection of SM22α and calponin as smooth muscle cell marker proteins and von Willebrand factor as endothelial cell marker protein to detect the culture purity. Cell proliferation by Zn depletion (1 day) was measured by MTT assay. Results: A primary culture protocol for pVSMCs from rat thoracic aorta was developed and optimized. Isolated cultures exhibited hill and valley morphology as the major characteristics of pVSMCs and expressed the smooth muscle cell protein markers, SM22α and calponin, while the endothelial marker von Willebrand factor was hardly detected. Zn deprivation for 1 day culture decreased rat primary vascular smooth muscle cell proliferation and this pattern was more prominent under severe Zn depletion (3 µM TPEN), while less prominent under mild Zn depletion (Chelexing). Conclusion: Our results suggest that cellular Zn deprivation decreased pVSMC proliferation and this may be involved in phenotypic modulation of pVSMC in the aorta.

Contrast Media in Abdominal Computed Tomography: Optimization of Delivery Methods

  • Objective: To provide a systematic overview of the effects of various parameters on contrast enhancement within the same population, an animal experiment as well as a computer-aided simulation study was performed. Materials and Methods: In an animal experiment, single-level dynamic CT through the liver was performed at 5-second intervals just after the injection of contrast medium for 3 minutes. Combinations of three different amounts (1, 2, 3 mL/kg), concentrations (150, 200, 300 mgI/mL), and injection rates (0.5, 1, 2 mL/sec) were used. The CT number of the aorta (A), portal vein (P) and liver (L) was measured in each image, and time-attenuation curves for A, P and L were thus obtained. The degree of maximum enhancement (Imax) and time to reach peak enhancement (Tmax) of A, P and L were determined, and times to equilibrium (Teq) were analyzed. In the computed-aided simulation model, a program based on the amount, flow, and diffusion coefficient of body fluid in various compartments of the human body was designed. The input variables were the concentrations, volumes and injection rates of the contrast media used. The program generated the time-attenuation curves of A, P and L, as well as liver-to-hepatocellular carcinoma (HCC) contrast curves. On each curve, we calculated and plotted the optimal temporal window (time period above the lower threshold, which in this experiment was 10 Hounsfield units), the total area under the curve above the lower threshold, and the area within the optimal range. Results: A. Animal Experiment: At a given concentration and injection rate, an increased volume of contrast medium led to increases in Imax A, P and L. In addition, Tmax A, P, L and Teq were prolonged in parallel with increases in injection time The time-attenuation curve shifted upward and to the right. For a given volume and injection rate, an increased concentration of contrast medium increased the degree of aortic, portal and hepatic enhancement, though Tmax A, P and L remained the same. The time-attenuation curve shifted upward. For a given volume and concentration of contrast medium, changes in the injection rate had a prominent effect on aortic enhancement, and that of the portal vein and hepatic parenchyma also showed some increase, though the effect was less prominent. A increased in the rate of contrast injection led to shifting of the time enhancement curve to the left and upward. B. Computer Simulation: At a faster injection rate, there was minimal change in the degree of hepatic attenuation, though the duration of the optimal temporal window decreased. The area between 10 and 30 HU was greatest when contrast media was delivered at a rate of 2 3 mL/sec. Although the total area under the curve increased in proportion to the injection rate, most of this increase was above the upper threshould and thus the temporal window was narrow and the optimal area decreased. Conclusion: Increases in volume, concentration and injection rate all resulted in improved arterial enhancement. If cost was disregarded, increasing the injection volume was the most reliable way of obtaining good quality enhancement. The optimal way of delivering a given amount of contrast medium can be calculated using a computer-based mathematical model.

Vasorelaxant Activities of Aqueous Extracts from Twenty Medicinal Plants Used in Oriental Medicines in Isolated Rat Aorta

  • Water extracts from 20 medicinal plants, traditionally used for postmenopausal symptoms in Korea, were examined for their vasorelaxant activity in isolated rat thoracic aorta rings precontracted with norepinephrine (NE). Among the 20 medicinal plants, Cornus officinalis (CoEx, 0.3 mg/mL), Schisandra chinensis (ScEx, 0.3 mg/mL), Erythrina variegate (EvEx, 0.3 mg/mL), and Epimedium koreanum (EkEx, 0.3 mg/mL) showed rapid relaxation of endothelium-intact aorta ($69\pm4%$, $40\pm3%$, $25\pm2%$, and $23\pm3%$ of active tone induced by NE, respectively). In contrast, the extracts of Erythrina variegata (EvEx), Angelica gigas (AgEx), Pueraria thunbergiana (PtEx), and EkEx lead to gradual (i.e., long-term) relaxation to baseline in endothelium-intact vessels. The time to complete relaxation was 20~40 min. These 6 plant extracts were selected for the investigation of possible underlying mechanisms. The CoEx-, ScEx-, or EkEx-induced rapid relaxations were virtually abolished by endothelium denudation, and were significantly inhibited by pretreatment with nitric oxide (NO) synthase inhibitor $N^G$-nitro-L-arginine (L-NNA, 10 ${\mu}M$), indicating that increased formation of NO might contribute to the endothelium-mediated relaxation. In long-term responses, the endothelium denudation did not affect PtEx-induced relaxation, whereas it delayed responses by EvEx and AgEx, and significantly inhibited the effect of EkEx. Among EvEx, AgEx, and PtEx, EvEx attenuated the $CaCl_2$-induced vasoconstriction in high-potassium depolarized medium, implying that EvEx is involved in inhibition of the extracellular calcium influx to smooth muscle through voltage dependent calcium channels. These results provide the scientific rationale for the interrelationships between the use of 20 medicinal plants and their effects on cardiovascular health in estrogen deficient conditions.

Development and Validation of Generalized Linear Regression Models to Predict Vessel Enhancement on Coronary CT Angiography

  • Objective: We evaluated the effect of various patient characteristics and time-density curve (TDC)-factors on the test bolus-affected vessel enhancement on coronary computed tomography angiography (CCTA). We also assessed the value of generalized linear regression models (GLMs) for predicting enhancement on CCTA. Materials and Methods: We performed univariate and multivariate regression analysis to evaluate the effect of patient characteristics and to compare contrast enhancement per gram of iodine on test bolus (${\Delta}HUTEST$) and CCTA (${\Delta}HUCCTA$). We developed GLMs to predict ${\Delta}HUCCTA$. GLMs including independent variables were validated with 6-fold cross-validation using the correlation coefficient and Bland-Altman analysis. Results: In multivariate analysis, only total body weight (TBW) and ${\Delta}HUTEST$ maintained their independent predictive value (p < 0.001). In validation analysis, the highest correlation coefficient between ${\Delta}HUCCTA$ and the prediction values was seen in the GLM (r = 0.75), followed by TDC (r = 0.69) and TBW (r = 0.62). The lowest Bland-Altman limit of agreement was observed with GLM-3 (mean difference, $-0.0{\pm}5.1$ Hounsfield units/grams of iodine [HU/gI]; 95% confidence interval [CI], -10.1, 10.1), followed by ${\Delta}HUCCTA$ ($-0.0{\pm}5.9HU/gI$; 95% CI, -11.9, 11.9) and TBW ($1.1{\pm}6.2HU/gI$; 95% CI, -11.2, 13.4). Conclusion: We demonstrated that the patient's TBW and ${\Delta}HUTEST$ significantly affected contrast enhancement on CCTA images and that the combined use of clinical information and test bolus results is useful for predicting aortic enhancement.
