PET-CT 검사 시 호흡 동조 시스템들의 유용성 평가 (An Assessment of the Utility of Respiratory Synchronized Systems in the PET/CT Examination)

  • PET/CT 검사 시 호흡으로 인한 내부장기의 움직임은 여러 호흡주기 동안 영상화되어 종양크기는 실제보다 증가하고 SUV에도 영향을 미치게 된다. 호흡 동조 시스템들을 이용하여 종양크기와 SUV 변화 유무를 평가해보고자 한다. 장비는 Biograph mCT 64를 사용하였고 호흡 동조 시스템은 RPM과 Anzai 시스템을 사용하였다. 실험을 위해 Point source와 Micro-phantom을 환자는 2016년 8월에서 9월까지 폐 기저부 또는 간 상부에 고형종양이 확인된 환자 12명을 대상으로 호기-호흡상태에서의 PET영상과 호기 후 멈춤 상태의 CT영상을 얻어 기존 Static, RPM, Anzai방식에서의 방사능 농도(kBq/mL), SUVmax, Cylinder diameter(mm), Tumor diameter (mm) 변화 유무를 평가하였다. Point source 방사능 농도 측정 결과 Static 대비 RPM 94%, Anzai 91% 상승하였고 Micro-phantom에서 방사능량을 달리한 2개의 Cylinder에서 SUVmax값은 Static 대비 RPM 61%, 78%, Anzai 58%, 77%로 각각 상승하였고 Cylinder diameter는 RPM -26%, -28%, Anzai -28%, -26% 감소하였다. 환자의 경우 SUVmax값은 Static 대비 RPM은 최소 8.2%에서 최대 94.4%, Anzai는 최소7.6%에서 최대 68.3% 상승하였고 Tumor diameter는 RPM은 최소 -7.6%에서 최대 -28.9%, Anzai는 최소 -9.6%에서 최대 -27.7% 감소하였다. 호흡 동조 시스템 RPM과 Anzai에서 phantom study는 별 차이가 없었지만 환자의 종양에서는 유의미한 차이가 있었다(P<0.05). 호흡 동조 시스템 RPM과 Anzai는 호흡이 일정한 주기로 이루어지는 phantom study에서 별 차이가 없었지만 환자의 경우 일정하지 않은 호흡주기와 시스템간 차이 때문에 유의미한 차이가 발생함을 알 수 있었다. 하지만 호흡 동조 시스템은 기존 Static 대비 종양의 크기는 감소하고 SUV는 증가하여 정확한 진단과 SUV측정에 유용할 것으로 사료된다.

  • Penetrating chest trauma may result in significant intracardiac injury. A traumatic ventricular septal defect is a rare complication that requires surgical management, particularly if heart failure ensues. We report a case of delayed repair of an outlet-type ventricular septal defect and perforation of the aortic and pulmonary valve leaflets following a stab wound. This report highlights diagnostic and surgical considerations and also presents an opportunity to review the conotruncal anatomy, which may be relatively unfamiliar to many adult cardiac surgeons.

자기연마를 이용한 ELID 연삭면의 나노경면연마 (Nano-level mirror finishing for ELID ground surfsce using magnetic assisted polishing)

  • ELID(ELectrolytic In-process Dressing) grinding is an excellent technique for mirror grinding of various advanced metallic or nonmetallic materials. A polishing process is also required for elimination of scratches present on ELID grinded surfaces. MAP(Magnetic Assisted Polishing) has been used as a polishing method due to its high polishing efficiency and to its resulting in a superior surface quality. This study describes an effective fabrication method combining ELID and MAP of nano-precision mirror grinding for glass-lens molding mould, such as WC-Co, which are extensively used in precision tooling material. And for the optics glass-ceramic named Zerodure, which is extensively used in precision optics components too. The experimental results show that the combined method is very effective in reducing the time required for final polishing. The best surface roughness of the polished glass-ceramic was within 1.7nm Ra in this study.

Composite Neural Networks for Controlling Semi-Linear Dynamical Systrms: Example from Inverted Pendulum Problem

  • In this paper, we propose a neural network for learning to control semi-linear dynamical systems. The network is a composite system of four three-layer backpropagation subnetworks, and is able to control inverted pendulums better than systems based on modern control theory at least in some ranges of parameters. Three of the four subnetworks in our network system process angles, velocities, and positions of a moving inverted pendulum, respectively. The outputs from those three subnetworks are input to the remaining subnetwork that makes control decisions. Each of the four subnetworks learns connection weights independently by backpropagation algorithms. Teaching signals are given by the human operator. Also, input signals are generated by the human operator, but they are converted by preprocessors to actual input data for the three subnetworks except for the network for control decisions. The whole system is implemented on both of 16 bit personal computers and 32 bit workstations. First, we briefly provide the research background and the inverted pendulum problem itself, followed by the description of our composite neural network model. Next, some results from the simulation are given, which are subsequently compared with the results from a control system based on modern control theory. Then, some discussions and conclusion follow.

금형면의 자기연마가공 고효율화에 관한 연구 (A Study on Improving the Efficiency of Magnetic Abrasive Polishing for Die & Mold Surfaces)

  • There are many difficulties in automatic polishing for die & mold surfaces. Even though the process has been studied in the past 15 years, it has not been achieved yet, but by the process of actual hand work of well-skilled workers. A new magentic assisted polishing process, which is one of the potential methods for automation of surface finishing has been studied in the past 10 years by colleagues. The process has many merits, but on the other hand also has demerits, one being low efficiency of grindability by comparision with wheel polish. Therefore, some attempts were tried to improve the grindability by adopting electropolishing, ultra-high speed milling, 5-axis controlled machine etc... most recently by colleagues. This paper also aims to improve the efficiency of polishing by introducing the easily-polished shape surface cutting method equalizing the tool feed per revolution to the pick feed. This cutting method was experimentally confirmed to have sufficient grindability to polish milled surface (with $10{{\mu}m}$Rmax surface roughness) into mirror surface (with $0.4{{\mu}m}$Rmax surface roughness).

Enzyme Sensors Modified with Avidin/Biotin Systembased Protein Multilayers

  • Enzyme multilayers composed of avidin and biotin-labeled enzymes were prepared on the surface of electrode, through a strong affinity between avidin and biotin (binding constant: ca $10^{15} M^{-1}$). The enzyme multilayers were useful for the improvement of the performance characteristies of enzyme sensors. The output current of the enzyme sensors depended linearly on the number of enzyme layers deposited. Thus, lactate oxidase (LOx) and alcohol oxidase (AlOx) were deposited after being modified with biotin for constructing enzyme sensors sensitive to L-lactate and ethanol respectively. It was also possible to deposit two different kinds of enzymes successively in a single multilayer. The glucose oxidase (GOx) and ascorbate oxidase (AsOx) were built into a multilayer structure on a Platinum electrode. The GOx, AsOx multilayer-modified electrode was useful for the elimination of ascorbic acid interference of the glucose sensor.

Parametric study of porous media as substitutes for flow-diverter stent

  • For engineers, generating a mesh in porous media (PMs) sometimes represents a smaller computational load than generating realistic stent geometries with computer fluid dynamics (CFD). For this reason, PMs have recently become attractive to mimic flow-diverter stents (FDs), which are used to treat intracranial aneurysms. PMs function by introducing a hydraulic resistance using Darcy's law; therefore, the pressure drop may be computed by test sections parallel and perpendicular to the main flow direction. However, in previous studies, the pressure drop parallel to the flow may have depended on the width of the gap between the stent and the wall of the test section. Furthermore, the influence of parameters such as the test section geometry and the distance over which the pressure drops was not clear. Given these problems, computing the pressure drop parallel to the flow becomes extremely difficult. The aim of the present study is to resolve this lack of information for stent modeling using PM and to compute the pressure drop using several methods to estimate the influence of the relevant parameters. To determine the pressure drop as a function of distance, an FD was placed parallel and perpendicular to the flow in test sections with rectangular geometries. The inclined angle method was employed to extrapolate the flow patterns in the parallel direction. A similar approach was applied with a cylindrical geometry to estimate loss due to pipe friction. Additionally, the pressure drops were computed by using CFD. To determine if the balance of pressure drops (parallel vs perpendicular) affects flow patterns, we calculated the flow patterns for an ideal aneurysm using PMs with various ratios of parallel pressure drop to perpendicular pressure drop. The results show that pressure drop in the parallel direction depends on test section. The PM thickness and the ratio of parallel permeability to perpendicular permeability affect the flow pattern in an ideal aneurysm. Based on the permeability ratio and the flow patterns, the pressure drop in the parallel direction can be determined.

호흡동조방사선치료 시 Real Time Monitor와 Ventilator의 유용성 평가 (Utility Estimation of the Application of Auditory-Visual-Tactile Sense Feedback in Respiratory Gated Radiation Therapy)

  • 목 적: 호흡동조방사선치료 시 호흡에 따른 종양의 위치를 정확하게 파악하여 치료하기 위해서는 안정되고 일정한 호흡유지가 중요하다. 이에 본 연구에서 Real time monitor와 Ventilator를 이용하여 측정한 호흡유지방법을 소개하고 그 유용성을 평가하고자 한다. 대상 및 방법: 7명의 실험자를 대상으로 하여 Gate on time (Beam on time) 600초를 기준으로 실험자를 총 5개의 그룹 -자연 호흡, Monitor를 이용하여 유도한 호흡, Ventilator를 이용하여 유도한 호흡, Monitor와 Ventilator를 동시에 이용하여 유도한 호흡, 실험자 스스로 Real time monitor를 이용하여 주기적 Breath-hold 상태를 유지하는 호흡- 으로 분류 하였다. 분류 후 ANZAI 4D system을 이용하여 각각의 호흡신호를 획득 하였다. 호흡신호별 총 소요 시간을 비교하고 호흡주기 상의 Gate on/off time 평균값, 표준편차, 분산, 평균 호흡 주기의 횟수와 변화 등을 각각 분석하여 호흡의 안정성을 평가 하였다. 동시에 실험자들을 대상으로 설문조사를 실시하여 각 장치들의 의존성을 정성적으로 조사하였다. 결 과: 5개 그룹의 호흡신호 편차변화를 비교하여 실험자의 각 호흡신호 안정성을 평가하였다. 그 결과 모든 실험자는 Gate on time 600초 동안 소요되는 시간이 Monitor와 Ventilator를 동시에 사용 했을 경우에 실험시간이 가장 짧게 걸렸고, 가장 안정된 호흡을 유지한 것으로 나타났다. 결 론: 본 연구에서 Real time monitor와 Ventilator를 사용하여 호흡유도를 진행해본 결과 실험대상자들의 안정적인 호흡유지에 유용성이 있는 것으로 나타났다. 2개의 장비를 독립적으로 사용하였을 경우 보다 동시에 사용할 경우 호흡유지 안정도가 더 우수한 것으로 나타났다. Real time monitor와 Ventilator를 이용하여 환자의 호흡조건을 설정하여 환자에게 적용시킨다면 호흡조절방사선치료 시 치료의 정확도를 높일 수 있으리라 예상되며 동시에 치료시간도 단축될 것으로 사료된다. 특히 규칙적인 호흡조절이 가능한 환자의 경우 Real time monitor를 활용한 Breath-hold 방법의 활용은 치료시간 단축의 효과를 현저히 높일 수 있을 것이라 사료된다.

PET/CT 영상의 부분체적효과와 장기의 움직임으로 인해 감소된 SUV의 보정을 위한 회복계수의 비교 (Comparison of Recovery Coefficients for Correction of Reduced SUV by Partial Volume Effect and Organ Movements in PET/CT Images)

  • In this study, a recovery coefficient (RC) calculation was conducted that can correct the underestimation of the standardized uptake value (SUV) due to the partial volume effect (PVE) through phantom measurements and formulas. The experiment was conducted using a dynamic phantom capable of implement cranio-caudal movement at a respiratory rate of 15 times per minute along with the measured phantom experiment of the stopped state, and the RC of the moving state is calculated and compared. Ingenuity TF (Philips Healthcare, Netherland) was used as a positron emission tomography/computed tomography (PET/CT) device. PET-CT Phantom (Biodex Medical System, USA) was used as a phantom for measurement. A phantom image in a stationary state was acquired, and a moving phantom image was acquired using the AZ-733V Respiratory Phantom (Anzai Medical Co, Japan) capable of breathing movement in the cranio-caudal direction under the same acquisition parameters. For RC calculation, the sphere maximum radioactivity concentration and the background mean radioactivity concentration of the acquired images were measured, and the initially determined sphere and background radioactivity concentrations were calculated. The calculated RC was 0.08 to 0.72. The size of sphere smaller, it was confirmed that the RC reduced. And the RC in the moving state reduced than in the stationary state. As a result of this study, the change of the RC was confirmed according to the size of spheres and the phantom moving. Using the RC derived by implement movement of breathing with the respiratory phantom, it is possible to considering correction of underestimated SUV by the partial volume effect of PET images and the patient movements.
