Objectives: To propose Korean medicine treatments based on effects of Yanshimtang-Gamibang on three patients diagnosed with psychogenic dizziness. Methods: Korean medicine treatments based on Yanshimtang-Gamibang were applied to three patients diagnosed with psychogenic dizziness after experiencing a traffic accident. Effects of Korean medicine treatments based on Yanshimtang-Gamibang were evaluated using DHI (Dizziness Handicap Inventory), BAI (Beck Anxiety Inventory), NRS (Numerical rating scale), NDI (Neck Disability Index), ODI (Oswestry Disability Index), and EQ-5D (EuroQol-Five Dimensions). Quality of sleep was assessed with total sleep time and number of times waking up during sleep. Results: After the treatment, there were significant decreases in DHI and BAI scores as well as values of NRS, NDI, ODI, EQ-5D. The sleep quality of three patients also showed improvement. Conclusions: Korean medicine treatments based on Yanshimtang-Gamibang might be effective in people with psychogenic dizziness.
Objectives: The objective of this study is to report the effectiveness of Korean medicine therapy on Psychologic tremor patients with MCVT. Methods: Korean medicine therapy including MCVT, herb, acupuncture, and psychotherapy are applied to psychologic tremor patients. Vas and AIMS are used as assessment tools to evaluate the effects of therapy. MCVT is a traditional Korean medicine technique usually for chronic diseases. Similarly, autogenic training by J.H.Schultz and Giungoroen-therapy was conducted as psychotherapy. Results: In case 1, the patient lost all symptoms from vas10 to vas0, while in case 2, symptoms of the patient decreased from vas10 to vas3. Additionally, there was a significant decrease in the AIMS measurement score for both patients. Psychotherapy helped to reduce depression and anxiety. Conclusions: Psychologic tremor can be treated using Korean traditional medicine such as MCVT, Sosiho-tang and psychotherapy.
Since 2000, the frequency of earthquakes beyond the 5.0 magnitude quake has been increasing in the Korean peninsula. For instance, the 5.0-magnitude earthquake in Baekryong-do in 2003 has occurred, and recent earthquake with Gyeongju(2016) and Pohang(2017) measured respectively magnitude of 5.2 and 5.8 on the Richter scale. As results, the public concern and anxiety about earthquakes are increasing, and therefore it is necessarily required for social infrastructure to reinforce seismic design and energy production facilities directly related to the national economy and security. This study represents the analysis of seismic performance evaluation methodology such as Seismic Margin Assessment (SMA), Seismic Probabilistic Risk Assessment (SPRA), High Confidence Low Probability Failure (HCLPF) in nuclear power plants in order to develop optimal seismic performance improvement. Current methodologies to evaluate nuclear power plants are also addressed. Through review of the nuclear structure evaluation past and current trend, it contributes to be the basis for the improvement of evaluation techniques on the next generation of nuclear power plants.
Background: The aim of this study was to investigate the relationship between pain catastrophizing, acceptance, and quality of life in relation to chronic low back pain in Jakarta, the capital city of Indonesia. We also analyze the effect of personality in catastrophizing and acceptance. Methods: A total of 52 chronic low back pain patients were enrolled as participants from 2 hospitals in Jakarta (43 females, 9 males, mean age 54.38 years). Participants completed a set of self-reported questionnaires: the NEO Five Factor Inventory (NEO-FFI), Chronic Pain Acceptance Questionnaire-Revised (CPAQ-R), Pain Catastrophizing Scale (PCS), and Pain Discomfort Module (PDM). Results: This study showed that acceptance increased the patient's quality of life by giving physical relief from pain. In contrast, pain catastrophizing decreased the quality of life, and increased the patients' tendency to get frustrated, irritated, and anxious about the pain. From a personality perspective, the trait neuroticism may lead to a higher level of pain catastrophizing. Conclusions: This study showed that catastrophizing, compared with acceptance, had a greater impact on the patient's life by reducing its quality.
Objectives: This study was conducted to analyze the symptoms and care needs of home-based hospice palliative patients in Busan and to provide a basic reference for developing practical guidelines for their care. Methods: By examining the registration cards of 409 hospice palliative patients, who were registered in community health centers in Busan as of 2016, this study retrospectively analyzed their characteristics, symptoms and care needs. Results: The average age was 70.6 years, 59.4% were receiving medical benefits, and 48.4% lived alone. As per the data obtained from the Palliative Performance Scale, many were able to mobile. Fatigue was the most severe and depression and anxiety were reported together, and their care needs were also high. Most subjects reported mild or low pain, but care needs were high. Furthermore, the medical benefits group showed a high level of symptoms and care needs across areas. Conclusions: To help subjects to live in their homes for as long as possible, it is necessary to identify symptoms and care needs and provide services in accordance with their severity and situation. Thus, it is necessary to develop practical guidelines for standardized community hospice palliative care services.
Many approaches to local anesthesia have been studied in dentistry. In this study, we introduce a new local anesthetic method, "Point-Inject Technique (PIT)", and compare it with traditional injection techniques. The PIT method utilizes both the vasoconstrictive and antinociceptive properties of local anesthetics as well as the application of controlled pressure during injection, reducing the time to complete anesthesia. Fifty patients were selected as the experimental group who were anesthetized using PIT, and the other 50 patients were selected as the control group using the direct injection method with a carpool syringe. The PIT group received 0.25 cartridges of 2% lidocaine with 1:100,000 epinephrine. The control group received 1.5~2 cartridges of 2% lidocaine with 1:100,000 epinephrine. Both groups were asked to mark the intensity of the pain caused by anesthesia using the Numeric Pain Rating Scale. The average time to recover from anesthesia was 40 minutes in the experimental group and 90 minutes in the control group. Additionally, 96% of the experimental group reported feeling no pain, while 78% of the control group reported having some form of pain during injection. The PIT method reduced both the reported pain scores of patients as well as time to recover from local anesthesia than the widely-used syringe injection method.
Objective: This study aimed to investigate the potential benefits of microcurrent stimulation as a non-invasive therapeutic approach for managing pain and improving the quality of life in women suffering from primary dysmenorrhea. Design: A case study. Methods: This study was conducted, involving a cohort of 6 women diagnosed with primary dysmenorrhea, aged between 20 to 30 years. Participants were received microcurrent stimulation using low-intensity microcurrents for 30 minutes every day for 4 weeks. The intensity of microcurrent stimulation was 25 μA and the frequency was 8 Hz. The intervention was administered between the menstruations, with pain intensity and quality of life being assessed at baseline, and then at the end of menstrual cycle. Pain intensity was evaluated using a visual analog scale (VAS) and menstrual symptom questionnaire (MSQ), while the quality of life was assessed through the stress response inventory (SRI), state trait anxiety inventory (STAI), center for epidemiologic studies depression (CES-D), and menstrual distress questionnaire (MEDI-Q). Results: After the intervention, participants demonstrated a statistically significant reduction in pain intensity, as evidenced by improved VAS scores (p<0.05). However, the changes in MSQ scores did not reach statistical significance. Regarding quality of life measures, no statistically significant differences were found in the SRI, STAI, CES-D, and MEDI-Q scores after the intervention (p>0.05). Conclusions: This study suggest that microcurrent stimulation holds promise as a potential treatment option for alleviating pain associated with primary dysmenorrhea.
This case report demonstrates the effect of modified Bangibokryeong-tang (Fangji Fuling Decoction), a traditional herbal prescription in treating dyspnea and palpitations, symptoms that often lead to anxiety and reduced quality of life of cardiac patients. A female patient in her late 70s with congenital valve malformation and atrial fibrillation presented with dyspnea and palpitations, each rated at a severity of 8 on a numeric rating scale (NRS). After voluntarily discontinuing Western medication, she received modified BGBRT for 25 days, leading to significant symptom relief and NRS improvement to 0-2. The patient reported maintaining an improved condition and showed a significant increase in vitality. This improvement was sustained for 7 months, but the symptoms recurred; thus, modified BGBRT was restarted. Modified BGBRT showed substantial effects on persistent cardiac-origin dyspnea and palpitations. Thus, this TKM has the potential to manage symptoms and enhance the quality of life of cardiac patients.
Objectives : The purpose of this research is to examine the effect of Bunsimgi-eum on the chest discomfort of Hwa-byung's major symptom. Methods : In this randomized, double blinded, placebo-controlled study, we planned to give Bunsimgi-eum or controlled medication for major symptoms of Hwa-byung. As preparatory research, 100 mm Visual Analogue Scale(VAS) for chest discomfort, the Hwa-byung' major symptom was measured as the 1st evaluative instrument, and Likert scale for major symptom of Hwa-byung, Korean State-Trait Anger Expression Inventory(STAXI-K), Korean State-Trait Anxiety Inventory(STAI-K), Korean Beck's Depression Inventory(BDI-K) and Heart Rate Variability(HRV) were also measured as the 2nd evaluative instrument at the before treatment. Results : Clinical characteristics-vital signs & demographic characteristics showed no significant difference between both groups. The characteristics of disease-chief complaint, pattern Identification, period, etiological factor, etc, also showed no significant difference between both groups. There were almost no Coincidence between pattern identification & weight of Hwa-byung's major symptoms. The results of Chest PA, EKG and clinicopathologic examination showed no significant difference between both groups. There were no significant difference between both groups in all valuation scales; 100 mm Visual Analogue Scale for chest discomfort, the Hwa-byung' major symptom was measured as the 1st evaluative instrument, and Likert scale for major symptom of Hwa-byung, STAXI-K, STAI-K, BDI-K and HRV. Conclusions : We considered that establishment of experimental group and controlled group was objective and worth conducting this research. And this methodology is expected to be applied to the subsequent research. And also, we hope to make up for this study through various study and discussion.
자기보고식 설문평가와 컴퓨터 기반 심박변이도(Heart rate variability, HRV) 분석평가를 통해 원예치료가 우울증 환자의 정서개선에 미치는 효과를 알아보았다. 자기보고식 설문 평가는 정신적 스트레스, 신체적 스트레스, 불안, 우울 등 4가지 평가도구를 사용하였다. HRV 분석평가는 standard deviation of the N-N intervals(SDNN), square root of mean squared difference of successive N-N intervals(RMSSD), total power(TP), low-frequency/high-frequency ratio(LF/HF ratio)의 4개 분석지표를 이용하였다. 동일한 병원 소속 우울증 환자 30명을 대상으로 실험을 수행하였다. 이 가운데 실험군 15명을 대상으로 주 1회씩 12회 원예치료 프로그램을 실시하였고, 대조군에는 실시하지 않았다. 그 결과, 원예치료를 실시 한 실험군의 경우 자기보고식 설문평가는 물론 HRV 분석평가에서도 원예치료 후 우울 정서 개선 효과가 확인되었다. 특히 자기보고식 평가에서는 정신적 스트레스, 불안, 우울 척도에서 통계적으로도 유의한 효과가 나타났으며($p$<0.001), HRV 분석평가의 경우 TP 지표에서 통계적 유의성이 확인되었다($p$<0.05). SDNN, RMSSD, LF/HF ratio 지표 역시 눈에 띄게 호전되었으나 통계적 유의성은 없었다. 비록 HRV의 모든 분석지표들이 통계적 유의성을 나타내지는 않았지만, 이 연구를 통해 HRV 측정으로 원예치료 효과를 객관화된 수치로 제시할 수 있는 가능성을 확인하였다. 또한 주관적 평가와 객관적 평가를 도입한 본 실험 결과 원예치료가 우울증 환자의 심리적, 신체적 재활에 효과적임이 입증되었다.
