Oral Vancomycin Therapy in a Child with Primary Sclerosing Cholangitis and Severe Ulcerative Colitis

  • Primary sclerosing cholangitis (PSC), a rare progressive liver disease characterized by cholestasis and bile duct fibrosis, has no accepted, effective therapy known to delay or arrest its progression. We report a 15 year old female patient diagnosed with PSC and moderate chronic active ulcerative colitis (UC) who achieved normalization of her liver enzymes and bile ducts, and resolution of her UC symptoms with colonic mucosal healing, after treatment with a single drug therapy of the antibiotic oral vancomycin. We postulate that the oral vancomycin may be acting both as an antibiotic by altering the intestinal microbiome and as an immunomodulator. Oral vancomycin may be a promising treatment for PSC that needs to be further studied in randomized trials.

  • Probiotics are traditionally defined as viable microorganisms that have a beneficial effect in the prevention and treatment of pathologic conditions when they are ingested. Although there is a relatively large volume of literature that supports the use of probiotics to prevent or treat intestinal disorders, the scientific basis behind probiotic use has only recently been established, and clinical studies on this topic are just beginning to get published. Currently, the best studied probiotics are lactic acid bacteria, particularly Lactobacillus and Bifidobacterium species. Other organisms used as probiotics in humans include Escherichia coli, Streptococcus sp., Enterococcus sp., Bacteroides sp., Bacillus sp., Propionibacterium sp., and various fungi, and some probiotic preparations contain more than one bacterial strain. Probiotic use for the prevention and treatment of antibiotic-associated diarrhea caused by Clostridium difficile induced intestinal disease as well as for other gastrointestinal disorders has been discussed in this review.

살모넬라 감염성 복부 대동맥류 환자에서 F-18 FDG PET/CT의 역할 (Role of F-18 FDG PET/CT in the Management of Infected Abdominal Aortic Aneurysm due to Salmonella)

  • We present a case of infected abdominal aortic aneurysm due to salmonella enteritidis. F-18 FDG PET/CT was performed to diagnosis and during follow-up after antibiotic treatment. Computed tomography (CT) is considered to be the best diagnostic imaging modality in infected aortic lesions. In this case, a combination of CT and FDG PET/CT provided accurate information for the diagnosis of infected abdominal aortic aneurysm. Moreover, FDG PET/CT made an important contribution to monitoring disease activity during antibiotic treatment.

  • A 59-year-old patient with a history of hepatocellular carcinoma presented with decreased consciousness and left hemiparesis. A rim-enhanced mass lesion without diffusion restriction was observed in contrast-enhanced MRI including diffusion-weighted imaging. Based on these findings, metastatic brain tumor was suspected. However, brain abscess (BA) was diagnosed after multiple bacterial colonies were observed in aspiration biopsy. Initial conventional antibiotic treatment including vancomycin had failed, so linezolid was used as second-line therapy. As a result, infection signs and clinical symptoms were resolved. We report a case with atypical imaging features and antibiotic susceptibility of a BA in an immunocompromised patient undergoing chemotherapy.

Treatment of Mycobacterium avium Complex Pulmonary Disease

  • The pathogen Mycobacterium avium complex (MAC) is the most common cause of nontuberculous mycobacterial pulmonary disease worldwide. The decision to initiate long-term antibiotic treatment is difficult for the physician due to inconsistent disease progression and adverse effects associated with the antibiotic treatment. The prognostic factors for the progression of MAC pulmonary disease are low body mass index, poor nutritional status, presence of cavitary lesion(s), extensive disease, and a positive acid-fast bacilli smear. A regimen consisting of macrolides (clarithromycin or azithromycin) with rifampin and ethambutol has been recommended; this regimen significantly improves the treatment of MAC pulmonary disease and should be maintained for at least 12 months after negative sputum culture conversion. However, the rates of default and disease recurrence after treatment completion are still high. Moreover, treatment failure or macrolide resistance can occur, although in some refractory cases, surgical lung resection can improve treatment outcomes. However, surgical resection should be carefully performed in a well-equipped center and be based on a rigorous risk-benefit analysis in a multidisciplinary setting. New therapies, including clofazimine, inhaled amikacin, and bedaquiline, have shown promising results for the treatment of MAC pulmonary disease, especially in patients with treatment failure or macrolide-resistant MAC pulmonary disease. However, further evidence of the efficacy and safety of these new treatment regimens is needed. Also, a new consensus is needed for treatment outcome definitions as widespread use of these definitions could increase the quality of evidence for the treatment of MAC pulmonary disease.

한양방 병행치료로 호전된 당뇨병성 족부감염 환자 증례보고 (Integrated Medicine Therapy for a Patient with a Diabetic Foot Infection)

  • Objectives: The purpose of this study is to report the clinical application of acupuncture, herbal medication, and antibiotic treatment to a patient with a diabetic foot infection. Methods: We treated the patient-who had ulcerations of the 1st, 2nd, and 4th toes and osteomyelitis on the right 1st and 2nd toes-with acupuncture, herbal medication, and antibiotics. We measured the state of this case by changes in his MRI signal and ulceration of his toes. Results: After treatment, the MRI signal indicated that the osteomyelitis had disappeared and the ulceration was diminished. Conclusions: A combination of acupuncture, herbal medication, and antibiotic treatment is effective for treatment of ulceration and osteomyelitis caused by diabetic foot infection.

Revitalization of necrotic mature permanent incisors with apical periodontitis: a case report

  • Despite considerable focus on the regenerative endodontic treatment of immature teeth with necrotic infected pulps and apical periodontitis, little data exist with regard to its possible implementation in necrotic permanent teeth with complete apical and radicular development. The present report describes the procedures and outcome of a regenerative endodontic treatment approach in 2 previously-traumatized incisors with closed apex with apical periodontitis. A 2-visit treatment procedure was employed. At initial visit, the root canals were copiously irrigated, followed by placement of a triple antibiotic paste containing ciprofloxacin, metronidazole, and clindamycin into the root canals. After 4 weeks, the antibiotic paste was removed, and apical bleeding was initiated with size 10 hand files beyond the apices. The root canals were coronally sealed with mineral trioxide aggregate, and the access cavities were restored with bonded resin composite. At post-operative 60 months, both teeth were remained asymptomatic, with the recall radiographs showing complete resolution of apical radiolucency and reestablishment of periradicular tissues. In both teeth, the dimensions of root space remained unchanged as verified by image analysis. The revitalization protocol utilizing root canal disinfection and induced apical bleeding in necrotic, closed-apex incisors may offer a clinically acceptable alternative to conventional root canal treatment.

바이오플락 환경에서 넙치(Paralichthys olivaceus)의 에드워드병에 대한 항생제 치료 효과 (Effect of antibiotics treatment for edwardsiellosis of olive flounder Paralichthys olivaceus in biofloc environment)

  • In biofloc culture for olive flounder, Paralichthys olivaceus, the possibility of antibiotics treatment was investigated against edwardsiellosis. After inducing edwardsiellosis by immersion in Edwardsiella tarda 1.2 × 105/mL suspension, the survival trends on various biofloc water management and some physiological changes were observed. For biofloc water management, six types of water treatments were carried out, which were no exchange without antibiotics as negative control, the exchange to stored biofloc water, the exchange to stored biofloc water with 20% flow-through, the exchange to fresh biofloc water, half fresh biofloc water and half sea water, and the complete flow-through. There was no significant physicochemical change on water qualities in any type. The exchange to fresh biofloc water was shown the highest survival ratio as 72.3%, and in case of stored biofloc water with 20% flow-through, the survival ratio was also significantly high as 62%. Plasma glucose, cholesterol, total protein, calcium, and magnesium were analyzed as physiological index. Mostly, there was no significant change, but plasma cholesterol showed an initial decrease in low survival group, and an initial increase with high survival group. Consequently, antibiotic treatment against a bacterial disease during biofloc culture is possible as long as the biofloc water management follow along properly.

만성 기관지염의 급성 악화에서 항생제 투여에 의한 유도객담 내 Matrix metalloproteinase와 Tissue inhibitor of matrix metalloproteinase의 변화 (Changes of Sputum Matrix Metalloproteinases and Tissue Inhibitor of Matrix Metalloproteinase-1 by Antibiotic Treatment in Acute Exacerbation of Chronic Bronchitis)

  • 연구배경 :임상적으로 만성 기관지염 환자의 호흡기 증상이 급격히 악화되는 것으로 정의되는 만성 기관지염의 급성악화는 기도의 폐쇄를 조장하여 만성 폐쇄성 폐질환으로의 진행을 촉진시키는 것으로 알려져 있다. 방 법 :만성 기관지염의 급성악화 환자 40명을 대상으로 하였으며 대상 환자를 무작위로 moxifloxacin군과 clarithromycin군으로 나누어 항생제를 경구투여 하였다.항생제를 복용 하기 전과 항생제를 7일간 복용한 후 각각 객담을 유도 처리하여 유도객담 상층액 내의 IL-8, SLPI, MMP-1, MMP-9, TIMP-1의 농도를 ELISA 방법으로 측정하였다. 결 과 : 항생제 투여에 의해 유도객담 내 TIMP-l의 농도와 TIMP-1/MMP-1의 분자량 비율이 유의하게 감소하였고 (p<0.05), 유도객담 내 SLPI의 농도는 유의하게 증가하였다(p<0.01) 급성악화 시 유도객담 내의 TIMP-1의 농도(p<0.01, r=0.751)와 TIMP-l/MMP-1의 분자량 비율(p<0.01, r=0.752)은 IL-8과 유의한 상관관계를 보이고 있었으며 항생제 치료로 호전이 된 이후에도 IL-8과의 상관관계는 계속 관찰되었다. 급성악화 시 유도객담 내 SLPI의 농도는 유도객담 내 TIMP-1(p<0.01, r=-0.496)과 TIMP-1/MMP-1(p<0.01, r=-0.456)의 분자량 비율 변화와 유약한 상관관계가 있었다. 그러나 유도객담 내 MMP-1, MMP-9의 농도 그리고 TIMP-l/MMP-9의 분자량 비율은 IL-8이나 SLPI의 농도와 유의한 상관관계가 없었다. 결 론 : 만성 기관지염의 급성 악화에 의해 TIMP와 MMPs의 불균형이 초래되며 TIMP가 상대적으로 많이 증가함으로써 기도 내 ECM이 축적되는데 적절한 항생제를 사용하지 않았을 때에는 이러한 기도벽의 재구성이 반복되어 기도의 폐쇄가 심해질 것으로 생각된다. TIMP와 MMPs의 불균형은 항생제 치료에 의하여 호전이 됨으로 적절한 항생제 치료는 만성 기관지염의 급성악화에 의한 비가역성 기도 폐쇄가 진행되는 것을 어느 정도 예방할 수 있을 것으로 생각된다.

당뇨병성 족부 감염에서 항생제 혼합 시멘트 충전물 사용의 치료 실패 위험 인자 분석 (Risk Factors for the Treatment Failure of Antibiotic-Loaded Cement Spacer Insertion in Diabetic Foot Infection)

  • Purpose: To evaluate the efficacy of antibiotic-loaded cement spacers (ALCSs) for the treatment of diabetic foot infections with osteomyelitis as a salvage procedure and to analyze the risk factors of treatment failure. Materials and Methods: This study reviewed retrospectively 39 cases of diabetic foot infections with osteomyelitis who underwent surgical treatment from 2009 to 2017. The mean age and follow-up period were $62{\pm}13years$ and $19.2{\pm}23.3months$, respectively. Wounds were graded using the Wagner and Strauss classification. X-ray, magnetic resonance imaging (or bone scan) and deep tissue cultures were taken preoperatively to diagnose osteomyelitis. The ankle-brachial index, toe-brachial index (TBI), and current perception threshold were checked. Lower extremity angiography was performed and if necessary, percutaneous transluminal angioplasty was conducted preoperatively. As a surgical treatment, meticulous debridement, bone curettage, and ALCS placement were employed in all cases. Between six and eight weeks after surgery, ALCS removal and autogenous iliac bone graft were performed. The treatment was considered successful if the wounds had healed completely within three months without signs of infection and no additional amputation within six months. Results: The treatment success rate was 82.1% (n=32); 12.8% (n=5) required additional amputation and 5.1% (n=2) showed delayed wound healing. Bacterial growth was confirmed in 82.1% (n=32) with methicillin-resistant Staphylococcus aureus being the most commonly identified strain (23.1%, n=9). The lesions were divided anatomically into four groups; the largest number was the toes: (1) toes (41.0%, n=16), (2) metatarsals (35.9%, n=14), (3) midfoot (5.1%, n=2), and (4) hindfoot (17.9%, n=7). A significant difference in the Strauss wound score and TBI was observed between the treatment success group and failure group. Conclusion: The insertion of ALCSs can be a useful treatment option in diabetic foot infections with osteomyelitis. Low scores in the Strauss classification and low TBI are risk factors of treatment failure.
