A Case Study of Wound Dressing Treatment in a Patient with Diabetic Foot Ulcer (당뇨병성 족부 궤양 환자의 창상 드레싱 치료 증례)

  • Objectives : The purpose of this study is to report on the treatment of wound dressing in patients with diabetic foot ulcers in the dermatology clinic of Korean medicine. Methods : First, sufficient marginal resection of the ulcer and necrotic tissue on the foot of a patient with diabetic peripheral neuropathy was performed. Inflammation was treated with Anti-inflammatory pharmacopuncture solution, and dressings were applied for one month using Cornu Cervi Parvum pharmacopuncture solution, and Haeboo ointment for granulation tissue formation and skin regeneration. Results : After 10 days of the treatment, the formation of new granulation tissue was observed in the necrotic tissue area, and the extent of the lesion decreased sharply from the 14th day. On the 40th day, epithelialization progressed so that the treatment was terminated, and complete keratinization of the site was observed at follow-up 3 weeks after the end of the treatment. Conclusions : In the treatment of wound dressings in diabetic foot ulcer patients, marginal resection treatment with the application of pharmacopuncture solution and herbal ointment showed good effect.

Antioxidant, antibacterial, and antiinflammatory effects of yoghurt made with vitamin tree (Hippophae rhamnoides L.) fruit powder

  • It is well known that the fruit of the vitamin tree (Hippophae rhamnoides L.) provides excellent anti-diabetic, antibacterial, immune regulation, anti-inflammatory, and anti-aging effects. In some countries including Europe the fruit has been added to certain foods to develop functional foods. The present research was carried out to elucidate the biological function of vitamin tree fruit powder added to fermented milk. It was found that there was an antioxidant effect of yoghurt made with vitamin tree fruit powder, and this effect was greater with increased incubation time and amount of vitamin powder, as shown by 1,1-diphenyl2-picrylhydrazyl (DPPH) and 2,2-anziobis (3-ehtylbenzothiazoline-6-sulfonic aicd) (ABTS) radical scavenging activities. The antibacterial effect of yoghurt containing vitamin tree fruit powder was shown to be effective against four pathogenic microorganisms, Escherichia coli, Staphylococcus aureus, Pseudomonas aeruginosa, and Salmonela Typhimurium. In particular, yoghurt supplemented with 5% of vitamin tree fruit powder showed the best antibacterial effect. The yogurt containing the vitamin tree fruit powder significantly inhibited the expression of the anti-inflammatory cytokines interleukin (IL)-6 (yogurt [Y] + Hippophae rhamnoides L. powder [HP] and yogurt containing 5% Hippophae rhamnoides L. powder [HPY]) and IL-1β (HP, Y + HP and HPY) in a concentration-dependent manner among tumor necrosis factor (TNF)-α, IL-6, IL-1, and induced nitric oxide synthase (iNOS). Our results clearly demonstrated that vitamin tree fruit powder could be a good functional ingredient for improving health through yoghurt manufactured with vitamin tree.

Inhibitory effects of extract from the Schizandra chinensis on rat small intestinal ${\alpha}-amylase$ activity and postprandial blood glucose

  • Postprandial hyperglycemia plays an important role in the development of type 2 diabetes and complications associated with the disease such micro-and macro-vascular disease. The present study investigated the effect and action mechanism of a ethanolic extract from the Schizandra chinensis(SC-E) on hypeglycemia in vivo and in vitro. In vitro, SE-E demonstrated a potent inhibitory effects on ${\alpha}-amylase$ activity ($IC_{50}$ : 4 ${\mu}g/ml$). Its inhibition on ${\alpha}-amylase$ was determined to be competitive type. Oral administration of SE-E markedly lowered plasma glucose levels in non-fasted streptozotocin induced diabetic rats (45 mg/kg BW). In addition when it was orally administrated to rats with starch (2g/kg BW), SC-E (50 and 100 mg/kg BW) significantly suppressed the increase of blood glucose levels after starch loading . These results suggest that some edible plants merit further evaluation for clinical usefulness as anti-diabetic drugs.

Dietary supplementation with astaxanthin may ameliorate sperm parameters and DNA integrity in streptozotocin-induced diabetic rats

  • Objective: Diabetes mellitus (DM) is known to cause many systemic complications as well as male infertility. Astaxanthin (ASTX) is a powerful antioxidant that is involved in a variety of biologically active processes, including those with anti-diabetes effects. The present study investigates the effect of ASTX on the spermatozoa function in streptozotocin (STZ)-induced diabetic rats. Methods: We divided 30 adult rats into three groups (10 rats per group), with a control group that received corn oil mixed with chow. DM was induced by intra-peritoneal injection of STZ. Eight weeks after the STZ injection, half of the diabetic animals were used as diabetic controls, and the rest were treated with ASTX for 56 days. Then the parameters and chromatin integrity of the epididymal sperm were analyzed using chromomycin A3, toluidine blue (TB), and acridine orange (AO) staining. Results: The count, viability, and motility of the epididymal sperm were decreased significantly in the STZ group in comparison with the control group (count and viability, p<0.001; motility, p<0.01). ASTX increased normal morphology and viable spermatozoa compared to the STZ group (morphology, p=0.001; viability, p<0.05). The percentage of abnormal chromatins in TB and AO staining was higher in the STZ group compared to the control group (p<0.001). The mean percentage of TB and AO positive spermatozoa in STZ rats was significantly lower in the STZ+ASTX group (TB, p=0.001; AO, p<0.05). Conclusion: This study observed that in vivo ASTX treatment partially attenuates some detrimental effect of diabetes. Conversely, ASTX improved sperm viability, normal morphology, and DNA integrity.

Properties and Activitiy Screening of Chromone Derivatives (크로몬 유도체의 물성 및 활성검색)

  • We have synthesized 4-isonitroso-4H-1-benzopyran and 4-amino-2,3-dihydro -4H-1-benzopyran of chromone derivatives by using condensation method. Physico-chemical properties of these compounds were measured and analyzed by UV and HPLC method. The correlation coefficient of their methanol solutions by UV were 0.9992 and 0.9994, respectively. And oxime compound was resolved within 4 min and had a detection limit of 3 ng at S/N=3 by HPLC using a reversed phase column with three solvents(MeOH, $H_2O$, HAc). The amino compound was resolved within 4.5 min and had a detection limit of 10 ng at S/N=3 by HPLC under the same conditions. Anti-diabetic effect of chromone derivatives were investigated in the streptozotocin (STZ)-induced diabetic rats. Diabetes was induced in male Sprague-Dawley rats by injections of STZ (45 mg/kg, i.v). The investigation of the hair growth effect of isonitrosobenzopyran and 4-aminobenzopyran on the hair of black mouse (C57BL/6) was also carried out. The administraion of their ethanol solution to the black mouse (C57BU/6) through skin them promoted the growth of hair.

Antihyperglycemic α-Glucosidase Inhibitory Activity of Ethanol Extract from Neolentinus lepideus (잣버섯(Neolentinus lepideus) 에탄올 추출물의 혈당상승 억제 효과)

  • For development a new anti-diabetic compound from edible mushroom, antihyperglycemic ${\alpha}$-glucosidase inhibitory activities of Pleurotus ostreatus, Pleurotus cornucopiae, Pleurotus salmoneostramineus, Pleurotus eryngii and Neolentinus lepideus were investigated on its water and ethanol extracts. ${\alpha}$-Glucosidase inhibitory activity of Neolentinus lepideus fruiting body showed the highest at 86.3% in the 95% ethanol extracts and water extract from Pleurotus cornucopiae was also higher at 48.5% among water extracts. Therefore, Neolentinus lepideus which showed very high ${\alpha}$-glucosidase inhibitory activity was selected as a new anti-diabetic agent-containing mushroom and the ${\alpha}$-glucosidase inhibitor was maximally extracted when treated with 95% ethanol at $30^{\circ}C$ for 48 hr. The ethanol extracts from Neolentinus lepideus fruiting body showed dosage-dependent hypoglycemic action after administration to 120 min in the SD-rat and streptozotocin-induced diabetic SD-rat.

Production and Its Anti-hyperglycemic Effects of γ-Aminobutyric Acid from the Wild Yeast Strain Pichia silvicola UL6-1 and Sporobolomyces carnicolor 402-JB-1

  • This study was done to produce ${\gamma}$-aminobutyric acid (GABA) from wild yeast as well as investigate its anti-hyperglycemic effects. Among ten GABA-producing yeast strains, Pichia silvicola UL6-1 and Sporobolomyces carnicolor 402-JB-1 produced high GABA concentration of $134.4{\mu}g/mL$ and $179.2{\mu}g/mL$, respectively. P. silvicola UL6-1 showed a maximum GABA yield of $136.5{\mu}g/mL$ and $200.8{\mu}g/mL$ from S. carnicolor 402-JB-1 when they were cultured for 30 hr at $30^{\circ}C$ in yeast extract-peptone-dextrose medium. The cell-free extract from P. silvicola UL6-1 and S. carnicolor 402-JB-1 showed very high anti-hyperglycemic ${\alpha}$-glucosidase inhibitory activity of 72.3% and 69.9%, respectively. Additionally, their cell-free extract-containing GABA showed the anti-hyperglycemic effect in streptozotocin-induced diabetic Sprague-Dawley rats.

The Metformin Use and Gastric Cancer Risk (메트포르민의 사용과 위암 발생)

  • Metformin is a widely used first-line anti-diabetic drug worldwide. Epidemiologic studies using the large population-based cohort database have shown the association between metformin uses and reduced risk of various type cancers including gastric cancer. In the gastric cancer prevention, metformin use was associated with the significant reduction of gastric cancer risk, especially for long-term metformin users. However, there is no well-designed randomized controlled clinical trial investigating the effect of metformin as a chemopreventive drug for gastric cancer. Therefore, further well-designed clinical trials will be needed to implement metformin for chemoprevention of gastric cancer.
