Anomalous Origin of the Right Subclavian Artery in a Patient with D-transposition of the Great Arteries

  • This case report concerns a young patient with an extremely rare combination of d-transposition of the great arteries (d-TGA) and anomalous origin of the right subclavian artery. In our patient, the right subclavian artery originated from the pulmonary artery, which is why he did not show reversed differential cyanosis. We conclude that the presence of an aortic arch anomaly should be considered in patients with d-TGA who do not present with reversed differential cyanosis. A further imaging work-up, including computed tomography or magnetic resonance imaging, might be helpful.

Anomalous Variations of the Origin and Course of Vertebral Arteries in Patients with Retroesophageal Right Subclavian Artery

  • We present two rare cases of anomalous vertebral artery (VA) with retroesophaqeal right subclavian artery. One patient had a right VA arising from the right common carotid artery (CCA), and a left VA originating from the third branch off the aorta. Both VAs ascended anteriorly to the transverse foramen of C5 to C6 vertebra and entered the transverse foramen of C4. The other patient had a right VA arising from the right CCA and entering the transverse foramen of C5. The presence of anomalous variations of the oriqin and course of vertebral artery might have serious implications in anqioqraphic and surgical procedures, and it is of great importance to be aware of such a possibility.

  • Anomalous aortic origin of the left subclavian artery (LSCA) from the left pulmonary artery (LPA) is a rare congenital cardiac malformation. We describe a case of LSCA from the LPA via ductus arteriosus in association with a double-outlet right ventricle, which never has been reported previously in Korea.

  • We report the case of a newborn with a rare anatomic variation: a right aortic arch with a retroesophageal left subclavian artery and an anomalous origin of the pulmonary artery from the aorta. This variation was diagnosed using echocardiography and computed tomography, and we treated the condition surgically.

대동맥궁에서 기시하는 양측 추골동맥의 이상기시: 증례 보고 (Anomalous Origins of the Bilateral Vertebral Arteries Arising from the Aortic Arch: A Case Report)

  • 추골동맥의 기시부 변이의 다양성은 여러 문헌에서 보고되어 왔다. 그러나, 양측 추골동맥이 직접 대동맥궁으로부터 기시하는 변이는 매우 드물다. 이번 증례는 갑작스럽게 발생한 우측 편마비를 주소로 내원한 60세의 남자 환자에서 우연히 발견된 드문 양측 추골동맥 기시부의 변이를 보여준다. CT 혈관조영술에서 우측 추골동맥은 대동맥궁의 좌측 쇄골하정맥의 원위부에서 기시하였고, 좌측 추골동맥은 대동맥궁의 좌측 총경동맥과 쇄골하정맥의 사이에서 기시하였다. 이 변이와 연관이 있을 수 있는 발생학적인 기전도 고찰하였다. 추골동맥 기시부의 드문 변이의 가능성은 추골동맥이 일반적인 위치에서 보이지 않을 때 고려되어야 한다. 이 변이를 이해하는 것은 혈관 내 치료나 수술 중에 예기치 않은 합병증을 피하기 위해 중요하다.
