In industrial designing of ready-to-wear shoes, one important factor to consider is that the population has diverse fooot shapes as well as foot length. The general shape of a foot could be represented by "foot ratio", i.e. the ratio of width to length. In this study, we measured several dey aspects of young Korean men's foot, and compared the results with their shoe sizes and general foot shapes. To this end, 172 male subjects were categorized according to their shoe size (small, medium, large) or foot ratio (wide, narrow, intermediate). The statistics of this survey indicated that the people with narrow foot shap have significantly greater foot length compared with the one having wide foot shape. Conversely, subjects having wide foot shape manifested significantly greater foot and ankle girth as well as significantly greater foot breadth. However, different foot shape groups showed no significant differences in heel width, heel ankle girth, instep height, and malleolus height. On the other hand, subjects wearing larger shoe size showed significantly larger foot measurements except instep and ankle heights, whereas subjects with shoe sizes 260 and below measured significantly more narrow heel and lateral metatarsal breadths. The deviation between foot length and dress shoe size(length) was greater in groups with wide foot shape and in groups wearing large shoe sizes. The results of this survey indicated that the subjects with wide foot shape apparently choose a size or two larger shoes for them as a compromise for a better breadth fit.eadth fit.
Purpose: This retrospective study reports the intermediate-term clinical outcomes including the practical function in daily and sports activities after total ankle arthroplasty for end-stage rheumatoid arthritis, as well as the effects of modification of perioperative anti-rheumatic medications. Materials and Methods: Twelve patients were followed for a minimum of 2 years after total ankle replacement for end-stage rheumatoid arthritis. Perioperative anti-rheumatic medications in all patients were modified based on a specific guideline. Clinical evaluations consisted of American Orthopaedic Foot and Ankle Society (AOFAS) scores, Foot and Ankle Outcome Score (FAOS), and Foot and Ankle Ability Measure (FAAM) scores. Periodic radiographic evaluation was conducted to detect changes in ankle alignment and postoperative complications. Results: Mean AOFAS, FAOS, and FAAM scores improved significantly from 37.5 to 81.2, 39.1 to 72.4, and 33.8 to 64.0 points at final follow-up, respectively (p<0.001). Functional outcomes in daily and sports activities at final follow-up were found to be 76.5 and 55.8 points for the FAOS and 70.5 and 57.5 points for the FAAM, respectively. As early postoperative complications, there was one case of local wound necrosis, one case of medial malleolar fracture, and one case of deep peroneal nerve injury. Radiological evaluation revealed two cases of asymptomatic heterotopic ossification and one case of progressive arthritis in the talonavicular joint. Reoperation was performed in only one patient (8.3%) with a medial soft tissue impingement at a mean of 35.6 months follow-up. Conclusion: Total ankle arthroplasty appears to be an effective surgical option for end-stage rheumatoid arthritis. Practical functions in daily and sports activities were significantly improved at intermediate-term follow-up. Modification of perioperative anti-rheumatic medications can be one of the solutions to reduce the postoperative complication rate.
Purpose: To evaluate the safety and prognostic factors of intramedullary nailing of distal tibia fractures in terms of function and symptoms of the ankle joint. Materials and Methods: We retrospectively analyzed 22 distal tibial fractures with intramedullary nailing. The mean duration of follow-up was 43 months. We reviewed medical records to describe each case. We measured radiographic parameters such as fracture configuration, arthritic change of the ankle joint and status of reduction. We also assessed clinical results by AOFAS ankle hind foot scoring system, degree of pain by VAS and range of motion to find out prognostic factors for functional result of the ankle joint. Results: Bone healing was obtained in all cases without any wound complications. Mean AOFAS ankle score was 94. There were 4 cases with mild (VAS<3/10) ankle pain and 2 cases with mild limitation of ankle motion. The comminution of fracture had a significant relationship with delayed angular deformity of ankle joint (p=0.032). There was no other significant parameter affecting ankle joint function except the location of nail-end. Conclusion: Intramedullary nailing in distal tibia fracture is a safe and effective procedure. But further study may need to evaluate the relationship between the position of nail-end and the function of ankle joint.
The purpose of this study was to investigate the effects of foot type and ankle joint fatigue levels on the trajectories of center of pressure and center of mass during a single-leg stance. The study subjects included 24 healthy women (normal foot group, n=10; pronated foot group, n=14). Ankle joint muscle fatigue was induced by using an isokinetic dynamometer, where the fatigue levels were measured on plantar flexion and dorsiflexion at angular velocities of $30^{\circ}/s$ at 50% and 30% of the peak torque of ankle plantar flexion. Following assessments in the anteroposterior direction according to the level of fatigue, the pronated foot group showed decreased single-leg stance ability at 50% and 30% of the fatigue level. Moreover, the normal foot group showed better single-leg stance ability than the pronated foot group at 30% of the fatigue level. Following assessments in the mediolateral direction, we noted that the single-leg stance ability did not differ significantly according to the levels of fatigue or foot type. In conclusion, ankle plantar flexion at 50% and 30% of the peak torque reduced the ability of the pronated foot group to achieve a single leg stance in the anteroposterior direction. Moreover, the normal foot group showed better single-leg stance ability than the pronated foot group.
Purpose: Despite continuous updates of standard treatment guidelines for acute ankle sprain and chronic ankle instability (CAI), in practice preferred treatment protocols vary widely. Based on a Korean Foot and Ankle Society (KFAS) member survey, this study reports current trends in the management of ankle ligament injuries. Materials and Methods: A web-based questionnaire containing 34 questions was sent to all KFAS members in September 2021. Questions mainly addressed clinical experience and preferences for the diagnosis and treatment of ankle ligament injuries. Answers with a prevalence of ≥50% among respondents were considered to reflect tendencies. Results: Eighty-four of the 550 members (15.3%) responded. Answers that showed a tendency were as follows: commonest additional image study (ultrasound), conservative treatment modality (immobilization, oral medication), frequency of surgical treatment (<5 cases per annum), most important factor when deciding on surgical treatment (activity level, e.g., occupation or sport), and commonest surgical procedure (open ligament repair). Answers that showed a tendency for CAI were as follows: most important symptom (repeated sprain, giving way), radiological factors (talar tilt, osteochondral lesion, anterior talar translation), and patient factors (occupation, sports activities, recurrent instability after surgery, etc.). For decision making regarding surgical treatment and method, the most preferred surgical procedure was the modified Broström procedure, and the most common repair technique was suture anchor technique. The following were considered poor prognostic factors; generalized laxity, failed previous surgery, cavovarus, severe mechanical instability, heavy work, obesity, and dissatisfaction after surgery because of residual pain. Conclusion: This study updates information regarding current trends in the management of ankle ligament injuries in Korea, and reveals consensus opinions and variations in approaches to patients with an acute or chronic injury. The divergence of approaches identified indicates the need for further studies to determine standard guidelines and long-term results.
Intraneural ganglilon of superficial peroneal nerve was rare condition around foot and ankle. we experienced a case of recurred intraneural ganglion of superficial peroneal nerve on foot. We treated the case with idendify of intraarticular branch of ganglion. We report the case with a review of literature.
Purpose : There is no validated method for measuring the toe flexor strength that can isolate the intrinsic muscles of the foot from the extrinsic muscles. This study compared the hallux flexion force (HFF) and muscle activity in the foot and ankle according to ankle position [plantarflexion (PF), neutral, and dorsiflexion (DF)]. Method : The study enrolled 17 subjects. In the sitting position, the HFF and activities of the abductor hallucis (AbdH), tibialis anterior (TA), and gastrocnemius (GCM) muscles were measured using a digital dynamometer and a wireless electromyography system, respectively. Subjects were instructed to flex the great toe maximally in three different ankle positions. Three 5-second trials were performed to measure the HFF and muscle activities in each condition. Repeated-measures ANOVA was used to compare the variables and paired t-tests with the Bonferroni correction were used for post-hoc pair-wise comparisons. The significance level was set at .016. Result : The HFF in DF was significantly greater than in any other ankle position (p<.01). The TA activity was greatest in ankle DF and that of the GCM was greatest in PF (both p<.01). However, there was no significant difference in AbdH activity according to ankle position. Conclusion : These results suggest that selective strength measurement of the foot intrinsic muscles in HFF should be performed with the ankle in the neutral position.
This study was designed to investigate the effects of an ankle foot orthosis(AFO) with variable ankle joint angles on balance performance in healthy adults. Eighteen healthy adults were recruited in this repeated measures design with subjects as their own controls. An AFO with four kinds of ankle joint angles(-5, 0, 5, and 10 degree) were used and balance performance was measured during single limb standing. Three trials were obtained and then averaged for data analysis. Foot pressure was measured using an F-scan system and muscle activity was measured using an MP150 system. There were significant differences in balance performance with ankle joint angles. An AFO with -5 degrees was associated with significant increases in postural sway(anterior-posterior), and in muscle activity for the medial gastrocnemius and tibialis anterior compared with other degrees of angle. Findings of this study show that angles of an AFO are related to balance performance and a joint angle of 10 degree is effective for promoting joint stability and postural control. This information can be used by clinicians to prescribe AFOs.
The purpose of this study was to determine the effect of HPAFO(Hinged Plastic Ankle Foot orthosis) and SPAFO(Solid Plastic Ankle Foot Orthosis) on standing balance and step moving in hemiplegia patients. Twenty hemiplegic patients were either inpatient or outpatient at the Rehabilitation medicine, MokPo J General Hospital and JeonJu J Medical Center from July 15, 2002 to September 15, 2002. Patients were able to ambulate independently for at least 10 meters and to stand independently for at least 10 minutes with balance measurement tool. The static balance and dynamic activity measurement was determined by SAKAI active balancer(Japan) with wearing HPAFO and wearing SPAFO. The static balance and dynamic activity analysis was analyzed by independent t-test. The results were as follows: 1. There were no significant difference in body weight bearing percent between wearing HPAFO and wearing SPAFO(p>0.05). 2. There were significant difference in whole path length between wearing HPAFO and wearing SPAFO(p<0.05). 3. There were significant difference in Effective Value Area between wearing HPAFO and wearing SPAFO(p>0.001). 4. There were significant difference in repeated functional time between wearing HPAFO and wearing SPAFO(p>0.001). 5. There were significant difference in cadence between wearing HPAFO and wearing SPAFO(p>0.001). The result of this study showed that wearing HPAFO and wearing SPAFO gave fair amount of improvement to static standing balance and dynamic movement ability of hemiplegic patients. The comparison result showed significant differences in between HPAFO and SPAHFO. The result of this study had a general limitation due to the restricted number of cases. The future study needs more detailed research and comparison with various variance between these two orthoses.
Purpose: We analyzed 35 patients of malignant tumors of the foot and ankle to evaluate clinical manifestation. result of treatment and prognosis. Materials and Methods: Thirty five patients who were histologically confirmed for malignant tumors of the foot and ankle from September 1984 to May 1999 were investigated. Results: There were 16 males and 19 females. with an average age 38.3 years. Ten tumors were originated from bone and 25 from soft tissue; osteosarcoma (8) was the most common bone tumor and synovial sarcoma (8) and malignant melanoma (6) were common in soft tissue tumors. Surgical procedures included; marginal resection (2), limb salvage procedure after wide resection (5) for bone tumors, and amputation (12), wide resection (4), marginal resection (5) for soft tissue tumors. In some cases, perioperative chemotherapy and radiotherapy were given. There were 2 local recurrences and 11 metastases; 5 metastases were found at the time of initial diagnosis. Average follow-up was 3.5 years. Conclusion: We conclude that suspicion and early diagnosis are important in malignant tumors of the foot and ankle, and the resection margin must be obtained more thoroughly during surgery with perioperative adjuvant therapy, if necessary.
