Three Dimensional MRI and Software for Studying Normal Anatomical Structures of an Entire Body (온몸의 정상 해부구조물을 익히기 위한 3차원 자기공명영상 및 소프트웨어)

  • For identifying the pathological findings in magnetic resonance images (MRIs), normal anatomical structures in MRIs should be identified in advance. For studying the anatomical structures in MRIs, a learning tool that includes the followings is necessary. First, MRIs of the entire body; second, horizontal, coronal, and sagittal MRIs; third, segmented images corresponding to the MRIs; fourth, three dimensional (3D) images of the anatomical structures in the MRIs; fifth, software incorporating the MRIs, segmented images, and 3D images. Such a learning tool, however, is hard to obtain. Therefore, in this research, such a learning tool which helps medical students and doctors study the normal anatomical structures in MRIs was made as follows. A healthy young Korean male adult with standard body shape was selected. Six hundred thirteen MRIs of the entire body were scanned (slice thickness 3 mm, interslice gap 0 mm, field of view 480 mm${\times}$480 mm, resolution 512${\times}$512, T1 weighted), and transferred to the personal computer. Sixty anatomical structures in the MRIs were segmented to make segmented images. Coronal, sagittal MRIs and coronal, sagittal segmented images were made. On the basis of the segmented images, forty-seven anatomical structures 3D images were made by manual surface reconstruction method. Software incorporating the MRIs, segmented images, and 3D images was composed. This learning tool that includes horizontal, coronal, sagittal MRIs of the entire body, corresponding segmented images, 3D images of the anatomical structures in the MRIs, and software is expected to help medical students and doctors study the normal anatomical structures in MRIs. This learning tool will be presented worldwide through Internet or CD titles.

3-D Kinematic comparison of One Hand Backhand Stroke and Two Hand Backhand Stroke in Tennis (테니스 한손 백핸드 스트로크와 양손 백핸드 스트로크 동작의 3차원 운동학적 비교 분석)

  • The purpose of this study was to investigate the relations between the segments of the body, the three dimensional anatomical angle during One Hand Backhand Stroke and Two Hand Backhand in tennis. For the movement analysis three dimensional cinematographical method(APAS) was used and for the calculation of the kinematic variables a self developed program was used with the LabVIEW 6.1 graphical programming(Johnson, 1999) program. By using Eular's equations the three dimensional anatomical Cardan angles of the joint and racket head direction were defined. 1. In three dimensional maximum linear velocity of racket head the X axis and Y axis(horizontal direction) showed $-11.04{\pm}2.69m/sec$, $-9.31{\pm}0.49m/sec$ before impact, the z axis(vertical direction) maximum linear velocity of racket head did not show at impact but after impact this will resulted influence upon hitting ball. It could be suggest that Y axis velocity of racket head influence on ball direction and z axis velocity influence on ball spin after impact. The stance distance between right foot and left foot was mean $75.4{\pm}5.86cm$ during one hand backhand stroke and $72.6{\pm}4.67cm$ during two hand backhand stroke. 2. The three dimensional anatomical angular displacement of trunk in interna rotation-external rotation showed most important role in backhand stroke. and is follwed by flexion-extension. the three dimensional anatomical angular displacement of trunk did not show significant difference between one hand backhand stroke and two hand backhand stroke but the three dimensional anatomical angular displacement of trunk was bigger than one hand backhand stroke. 3. while backhand stroke, the flexion-extension and adduction-abduction of right shoulder joint showed significant different between one hand backhand stroke and two hand backhand stroke. the three dimensional anatomical angular displacement of right shoulder joint showed more flex and abduct in one hand backhand stroke. 4. The three dimensional anatomical angular displacement of left shoulder showed flexion, adduction, and external rotation at impact. after impact, The angular displacement as adduction-abduction of left shoulder changed motion direction as abduction. angular displacement of left shoulder as flexion-extension showed bigger than the right shoulder.

  • Recently among several tennis techniques forehand stroke has been greatly changed in the aspect of spin, grip and stance. The most fundamental factor among the three factors is the stance which consists of open, square and closed stance. The purpose of this study was to investigate the relations between the segments of the body, the three dimensional anatomical angle according to open stance patterns during forehand stroke in tennis. The three dimensional anatomical angular displacement of racket increased flexion/abduction angle until the impact. after impact, The angular displacement of racket changed motion direction as extension/adduction. 3. The three dimensional anatomical angular displacement of trunk in flexion-extension showed extension all around the forehand stroke. The angular displacement of trunk in adduction-abduction showed abduction at the backswing top and adduction around impact. while there is no significant internal-external rotation 4. The three dimensional anatomical angular displacement of hip joint and knee joint increased extension angle after minimum of knee joint angle in the forehand stroke, The three dimensional anatomical angular displacement of ankle joint showed plantar flexion, internal rotation and eversion in forehand stroke. it could be suggest that the plantar pressure of open stance during forehand stroke would be distributed more largely to the fore foot. and lateral side.

Three-Port Laparoscopic Exploration is not Sufficient for Patients with T4 Gastric Cancer

  • Gastric cancer continues to be a leading cause of cancer death. The majority of patients with gastric adenocarcinoma in China present with advanced disease. Ruling out unresectable cancers from an unnecessary "open" exploration is very important. The aim of this study was to assess the value of five-port anatomical laparoscopic exploration in T4 gastric cancer in comparison with three-port laparoscopic exploration and laparotomy exploration. We conducted a retrospective study on 126 patients with T4 stage scheduled for D2 curative gastrectomy based on computed tomography (CT) staging at Department of Gastric Cancer and Soft Tissue Sarcoma, Fudan University Shanghai Cancer Center, from Apr. 2011 to Apr. 2013. Laparotomy exploration (Group I), three-port laparoscopic exploration (Group II) or five-port anatomical laparoscopic exploration (Group III) were performed prior to radical gastrectomy. Accuracy rate for feasibility of D2 curative gastrectomy in laparotomy exploration and five-port anatomical laparoscopic exploration groups was higher than that in the three-port laparoscopic exploration group. Five-port anatomical laparoscopic exploration group had the highest accuracy resection rate (Group I vs Group II vs Group III,92.6% vs78.6% vs 97.7%; p<0.05) and shorter length of hospitalization (Group I vs Group II vs Group III, $9.58{\pm}4.17$ vs $6.13{\pm}2.85$ vs $5.00{\pm}1.81$; p<0.001). Three-port laparoscopic exploration has low accuracy rate for assessing feasibility of D2 curative gastrectomy and five-port anatomical laparoscopic exploration should be performed on patients with T4 gastric cancer.

Clinicopathological Significance of Lymphangiogenesis and Tumor Lymphovascular Invasion in Indonesian Breast Cancers

  • Background: Lymphangiogenesis, assessed as lymphovascular density (LVD), is the initial step of generalized tumor lymphovascular invasion (LVI). It also involves VEGF-C as the most important protein family. Lymphangiogenesis among breast cancer cases correlations with several clinicopathological factors are important to determine prognosis and treatment strategies, but results have been controversial and require clarification. Aim: To define correlations between VEGF-C expression, LVD and LVI with several clinicopathological parameters from Indonesian breast cancer patients. Materials and Methods: Using a cross-sectional study, a total of 48 paraffin-embedded tissues of breast cancer from Dr. Sardjito General Hospital Indonesia were assessed for VEGF-C expression, LVD and LVI by immunohistochemistry. Correlations of these markers with clinicopathological parameters like patient age, tumor size, lymph node status, grade, ER/PR and Her-2 status, cell proliferation and p-53 expression were investigated by linear analysis. Correlations of VEGF-C expression and LVI with several clinicopathological parameters were analyzed with Coefficient Contingency Chi-Square test. Results: The mean of patients age was 53.0 year, pre and post-menopausal patients accounting for 56.3% and 43.8%, respectively. Some 10.4% were well, 41.7% moderate and 47.9% poorly differentiated. ER positivity was evident in 50% while PR and Her-2 positivity was found in 31.3% and 33.3%, respectively. Breast cancer cells with over-expression of p-53 was 64.6% and with high cell proliferation was 56.3%. Lymph node metastasis was noted in 63.5%, and LVI in 72.9%. Significant correlations were found between LVD and tumor size (p:0.037), grade (p:0.000), lymphnode status (p:0.036), LVI (p:0.003), as well as with p-53 and cell proliferation. There were also significant correlation of VEGF-C (p:0.011) and LVI (p:0.001) with tumor grade. Only ER status was found to have a correlation with tumor size (p:0.027). Conclusions: This study suggested that in Indonesian breast cancer patients, lymphangiogenesis is correlated with tumor size, grade, lymph node status and tumor lymphovascular invasion, the latter also being related with p-53 over expression and high cell proliferation.

  • The effect of Roentgen rays on carcinoma of the cervix has long been of great interest to both radiologists and gynecologists. Since most cervical carcinomas are treated by irradiation, any additional knowledge either concerning the radiosensitivity of cervical tumors or their ultimate prognosis would be of value. The vaginal smear is considered to be one of convenient and rapid methods to study the effects of radiation on cervical malignancy. We observed morphologic changes in 297 cytologic preparations obtained from 60 patients who had underwent irradiation for cancer of the cervix. With the morphologic parameters such as cytoplasmic vacuolization, cytoplasmic basophilia, multinucleated giant cell formation, polymorphonuclear leucocytes (PMNL) sticking and postradiation dysplasia, we analyzed the findings in relation to the follow up time interval. The most common effect was the cytoplasmic vacuolization with basophilia of basaloid cells, which were noted in more than 90% of followed patients. The multinucleated giant ceil formation and PMNL stickering were noted in 38 cases(63%) and 48 cases(80% ) respectively. The differential diagnosis of postradiation dysplasia from recurrent or persistent carcinoma, reparative atypical cells, and regressing tumor cells was difficult and further study seems to be needed to clarify the more accurate morphologic features and biologic behavior.

Study on Relationship between Five Su Points and Glomus (오수혈(五輸穴)과 글로뮈의 상관성에 대한 연구)

  • This study was to investigate the origin of meridian through the relationship between Five Su Points and arteriovenous anastomosis. We searched traditional studies about origin of merdian and papers on relationship origin of merdian and blood vessels. As bibliographic search results, we got the conclusion that it is funtion of arteriovenous anastomosis and its anatomical position. Next we compared relationship between it and merdian. We found that Jeong-Acupuncture Points of Five-Su Points and Geun-Acupuncture Points of merdian have close relate to anatomical position(similarities). When we compared relationship between arteriovenous anastomosis and hypertension, we found that Biao Acupuncture Points and Geun Acupuncture Points of merdian have close relate to regulation of blood pressure and blood circulation. According to this results, Five Su Points and arteriovenous anastomosis have close relationship in anatomical position and regulatory function of blood pressure.

The Relationship between Anatomical Characteristics and Compression Strength Parallel to Grain in Major Species of Lepidobalanus (참나무 아속(亞屬) 주요 수종의 조직적 성질과 종압축강도와의 관계)

  • This study was carried out to get some basic information on mechanical properties of Korea Lepidobalanus for rational utilization of wood. Five species of Quercus variabilis, Q. aliena, Q. serrata, Q. mongolica, Q. acutissima belong to Lepidobalanus were investigated in this research. Relationship of anatomical characteristics to compression strength parallel to grain was analyzed using stepwise regression technique. All possible combination of 21 independent variables were regressed on compression strength parallel to grain. The summarized results in this study were as follows: I. In the proportion of wood structural elements, the factors that had the greatest influence on compression strength parallel to grain appeared to be proportion of ray in earlywood in Q. variabilis and Q. acutissima, proportion of wood fiber in latewood in Q. aliena and Q. serrata and proportion of vessel in earlywood in Q. mongolica respectively. 2. In the size of wood element and its structure, the factors that had the greatest influence on compression strength parallel to grain were microfibril angle in Q. variabilis, Q. serrata and Q. mongolica and length of wood fiber in Q. aliena and Q. acutissima.

  • In clinical dentistry, botulinum toxin is generally used to treat the square jaw, bruxism, and temporomandibular joint diseases. Recently, this procedure has been expanded and applied for cosmetic purposes, and it is becoming a key task to be aware of the precise anatomical structure of the target muscles to be cautious during treatment and how to prevent side effects. Therefore, the purpose of this study is to observe the anatomical structure of the superficial layer of masseter muscle and to provide a most effective botulinum toxin injection method through clinical anatomical consideration. It was observed that the muscle belly of superficial part of the superficial layer was originated from the deep to the aponeurosis of masseter muscle and descend, then changed gradually into the tendon structure attaching to the inferior border of the mandible. In this study, we named this structure deep inferior tendon. This structure was observed in all specimens. We conclude that the use of superficial layer and deep layer injection should be considered to prevent paradoxical masseteric bulging in consideration of the deep inferior tendon of superficial part of superficial layer of masseter muscle.

Radionuclide (Tc-99m) Hysterosalpingography for Evaluation of Fallopian Tube Patency (동위원소(Tc-99m HAM)를 이용한 난관 조영술)

  • A prospecitive study was performed to evaluate the efficacy of Radionuclide Hysterosalpingography (RNHSG), that was modified by McCalley, et al. for the evaluation of fallopian tube. Using spontaneous migration of Tc99m labelled human albumin microsphers (HAM) and images got with Gamma camera, the study was made in 20 gynecological patients (44 tubes) in an effort to confirm the anatomical patency or obstruction as well as functional one at the Department of Obstetrics and Gynecology, National Medical Center from January 1, 1986 to July 31, 1986. The efficacy was also compared with that of contrast hysterosalpingography and surgical observation. Results obtained were as follows; 1) The Radionuclide Hysterosalpingography appeared to be a simple and accurate procedure as a mean with which anatomical tubal patency, as well as functional patency can be detected, although it's some limit in showing anatomical details of uterine cavity and tubes in comparison with contrast hysterosalpingography. 2) The predicted value of tubal obstruction was 82.4%. and that of tubal patency was 88.9% respectively. Overall efficacy (percent of tube correctly classified) of Radionuclide Hysterosalpingography was 88.6%. 3) There was no nuclide related complication during and after the procedure.
