  • 1. Panax ginseng의 재배년수에 따른 내부형태적 차이를 검토한 결과, 소엽신(小葉身)의 주맥부(主脈部), 소엽병(小葉柄), 엽병(葉柄)의 횡절면(橫切面)에 있어서 각각의 특징에 의하여 발육년수(年數)를 판단할 수가 있었다. (Tabel II) 2. 내부형태를 비교검토한 결과, 서울, 부산, 대구, 금산의 $\ulcorner$인삼잎$\lrcorner$은 원식물(原植物)이 Panax ginseng으로서 대부분이 5년 및 6년생이었으며 부분적으로 4년생이 함유되어 있었다.

A Case Report of the Angiosarcoma Involving Epicranial Muscle and Fascia : Is the Occipitofrontalis Muscle Composed of Two Different Muscles?

  • The occipitofrontalis muscle is generally regarded as one muscle composed of two muscle bellies joined through the galea aponeurotica. However, two muscle bellies have different embryological origin, anatomical function and innervations. We report a case of angiosarcoma of the scalp in a 63-year-old man whose MR showed that the superficial fascia overlying the occipital belly becomes the temporoparietal fascia and ends at the superior end of the frontal belly. Beneath the superficial fascia, the occipital belly of the occipitofrontalis muscle becomes the galea aponeurotica and inserts into the underside of the frontal belly. The presented case report supported the concept of which the occipitofrontalis muscle appears to be composed of two anatomically different muscles.

Cystic Abducens Schwannoma without Abducens Paresis : Possible Role of Cisternal Structures in Clinical Manifestation

  • The abducens nerve paresis generally can aid in the presumptive diagnosis of abducens schwannoma along with the typical radiological features of schwannomas. The authors present a case of a 76-year-old male patient with a abducens schwannoma without abducens nerve paresis. Peroperatively, abducens nerve located in the cerebellopontine cistern had normal in contour and diameter, despite the mass originated from this nerve. We hypothesize that anatomic location of abducens nerve may affect the vector of tumor growth to prevent destruction of its origin, the abducens nerve.

Morphometric Study of Hypoglossal Nerve and Facial Nerve on the Submandibular Region in Korean

  • Objective : This study was performed to determine the anatomical landmarks and optimal dissection points of the facial nerve (FN) and the hypoglossal nerve (HGN) in the submandibular region to provide guidance for hypoglossal-facial nerve anastomosis (HFNA). Methods : Twenty-nine specimens were obtained from 15 formalin-fixed adult cadavers. Distances were measured based on the mastoid process tip (MPT), common carotid artery bifurcation (CCAB), and the digastric muscle posterior belly (DMPB). Results : The shortest distance from the MPT to the stylomastoid foramen was $14.1{\pm}2.9$ mm. The distance from the MPT to the FN origin was $8.6{\pm}2.8$ mm anteriorly and $5.9{\pm}2.8$ mm superiorly. The distance from the CCAB to the crossing point of the HGN and the internal carotid artery was $18.5{\pm}6.7$ mm, and that to the crossing point of the HGN and the external carotid artery was $15.1{\pm}5.7$ mm. The distance from the CCAB to the HGN bifurcation was $26.6{\pm}7.5$ mm. The distance from the digastric groove to the HGN, which was found under the DMPB, was about $35.8{\pm}5.7$ mm. The distance from the digastric groove to the HGN, which was found under the DMPB, corresponded to about 65.5% of the whole length of the DMPB. Conclusion : This study provides useful information regarding the morphometric anatomy of the submandibular region, and the presented morphological data on the nerves and surrounding structures will aid in understanding the anatomical structures more accurately to prevent complications of HFNA.

Morphometric Relationship between the Cervicothoracic Cord Segments and Vertebral Bodies

  • Objective : The objective of this study was to investigate the morphologic characteristics between the vertebral body and the regions of the cervical and thoracic spinal cords where each rootlets branch out. Methods : Sixteen adult cadavers (12 males and 4 females) with a mean age of 57.9 (range of 33 to 70 years old) were used in this study. The anatomical relationship between the exit points of the nerve roots from the posterior root entry zone at each spinal cord segment and their corresponding relevant vertebral bodies were also analyzed. Results : Vertical span of the posterior root entry zone between the upper and lower rootlet originating from each spinal segment ranged from 10-12 mm. The lengths of the rootlets from their point of origin at the spinal cord to their entrance into the intervertebral foramen were 5.9 mm at the third cervical nerve root and increased to 14.5 mm at the eighth cervical nerve root. At the lower segments of the nerve roots (T3 to T12), the posterior root entry zone of the relevant nerve roots had a corresponding anatomical relationship with the vertebral body that is two segments above. The posterior root entry zones of the sixth (94%) and seventh (81%) cervical nerve roots were located at a vertebral body a segment above from relevant segment. Conclusion : Through these investigations, a more accurate diagnosis, the establishment of a better therapeutic plan, and a decrease in surgical complications can be expected when pathologic lesions occur in the spinal cord or vertebral body.

Evaluation of Human Papilloma Virus Infection in Patients with Esophageal Squamous Cell Carcinoma from the Caspian Sea Area, North of Iran

  • Introduction: HPV has been found repeatedly in esophageal squamous cell carcinoma (ESCC) tissues. However, reported detection rates of HPV DNA in these tumors have varied markedly. Differences in detection methods, sample types, and geographic regions of sample origin have been suggested as potential causes of variation. We have reported that infection of HPV DNA in ESCC tumors depends on anatomical sites of esophagus of the patients from Mazandaran, north of Iran. Materials and Methods: HPV DNA was examined in 46 upper, 69 middle and 62 lower third anatomical sites of esophageal squamous cell carcinoma specimens collected from Mazandaran province in north Iran, near the Caspian Littoral as a region with high incidence of ESCC. HPV L1 DNA was detected using Qualitative Real time PCR and MY09/MY11 primers. Results: 28.3% of upper, 29% of middle and 25.8% of lower third of ESCC samples were positive for HPV DNA. 13.6% for males and 14.1% for females were HPV positive in all samples. Conclusions: HPV infection is about one third of ESCC in this area. Findings in this study increase the possibility that HPV is involved in esophageal carcinogenesis. Further investigation with a larger sample size is necessary.

Variations in Entrance of Vertebral Artery in Korean Cervical Spine: MDCT-based Analysis

  • Background: Knowledge of the anatomical variation of the vertebral artery has clinical importance not only for the performance of interventional or surgical procedures itself but also to ensure their safety. We conducted a study of the anatomical variation by reviewing multi-detector computed tomography (MDCT) images of the cervical spine from 460 Korean patients. Methods: 16-row MDCT data from 460 patients were used in this study. We observed 920 vertebral arteries. Examination points included level of entrance of the artery into the transverse foramen of the cervical vertebra, origin site of the vertebral artery, course of a vertebral artery with aberrant entrance. Result: The vertebral artery in 2 (0.2%) cases in this study entered into the transverse foramen of the 7th cervical vertebra from the left. In 45 (4.9%) cases, the vertebral artery entered into the transverse foramen of the 5th cervical vertebra. Of these, the entrance was on the right in 15 (1.6%) and on the left in 30 (3.3%). We found 17 (1.8%) cases in which the artery entered into the transverse foramen of the 4th cervical vertebra, 10 (1.1%) on the right and 7 (0.7%) on the left side. As is commonly acknowledged, the 6th cervical vertebra was the most common site of entry; the vertebral artery entered the transverse foramen of the 6th cervical vertebra in the remaining 855 (93.0%) cases, on the right in 434 (47.2%) and on the left in 421 (45.8%). Conclusions: In conclusion, the possibility of an atypical course of the vertebral artery in segments V1 and V2 should be evaluated with magnetic resonance imaging (MRI) or CT images before carrying out procedures involving the anterior cervical vertebrae.

Facial artery: anatomical variations in the perioral region in cadavers

  • Background: In recent years, there has been an increase in reports of perioral vascular complications resulting from filler injections, such as necrosis of the lip or alar rim, occlusion, and in severe cases, blindness. Conversely, the use of perioral arterial flaps is becoming more prevalent in the treatment of cleft lips, cancer, and trauma. A thorough understanding of perioral arteries is essential to minimize complications and maximize the success of these flaps. However, the course of the facial artery (FA) in the perioral region remains incompletely understood. The aim of this study was to describe the variations of the FA in the perioral region. Methods: We dissected 52 embalmed and formaldehyde-fixed Vietnamese cadavers. We then studied the size and distribution of perioral arteries in 102 specimens. Results: The superior labial artery (SLA) was the most common branch, occurring in 87.25% of cadavers, followed by the inferior labial artery (ILA) at 78.43%. The SLA primarily originated above the mouth corner (cheilion), accounting for 91.01% of cases, and predominantly exhibited a tortuous course within the submucosa (78.65%). The ILA's branching pattern varied, but it was primarily located below the cheilion (91.25%). The ILA also followed a twisted path, generally within the submucosa. The ILA exhibited two patterns: the typical pattern, distributed at the vermilion border of the lower lip (8.82%), and the horizontal labiomental artery pattern, which ran horizontally in the middle of the lower lip area (69.61%). At their origin, the SLA and ILA had average external diameters of 1.29 mm and 1.28 mm, respectively. Conclusion: Numerous anatomical variations in the FA in the perioral region were found. A detailed anatomic description, suggested landmarks, and angiography before the procedure will be useful to help doctors avoid complications.

흰쥐 왼쪽관상동맥의 분지 양상에 관한 해부학적 연구 (An anatomical study on the branching patterns of left coronary artery in the rats)

  • The left main descending artery (LMDA) of left coronary artery (LCA) in rats runs around the left side of conus arteriosus after arising from the aortic sinus and descends to the apex of heart with branching several branches into the wall of left ventricle (LV). The ligation site of LMDA for myocardial infarction (MI) is the 2~4 mm from LCA origin, between the pulmonary trunk and left auricle. The characteristics that rat heart has no interventricular groove on the surface and its coronary arteries run intramyocardially with branching several branches give the difficulty in surgery for MI which resulted in expected size. This study was aimed to elucidate the branching patterns of the left coronary artery for analysis of MI size and for giving the basic data to producing small MI intentionally in 2 male species that are widely used, Sprague-Dowley (SD) and Wistar-Kyoto (WKY), in the world. Red latex casting was followed by the microdissection in 27 and 28 hearts of SD and WKY male rats, respectively. The branching patterns of LMDA were classified into 3 major types and others based on the left ventricular branches (L). The Type I, Type II, Type III and others are shown in 55.6%, 22.2%, 14.8%, and 7.4% in SD, 60.7%, 10.7%, 7.1%, and 21.5% in WKY, respectively. The branching number of the first left ventricular branch (L1) that are distribute the upper one third of LV was 1.2~1.5, and its branching sites were ranging 0.9~2.1 ßÆ from LCA origin. L2, the second left ventricular branch distributing middle one third of LV, was the number of 1.2~1.4 and branching out ranging 5.1~5.7 mm. L3, the third left ventricular branch of LMDA distributing lower one third of LV, was the number of 1~1.5 and branching out ranging 7.0~9.3 mm from LCA origin. The common branch of L1 and L2 was branched from LMDA with the number of 1.1, and its site was located in the distance of mean of 1.5 mm and 2.8 mm in SD and WKY, respectively. The common branch of L2 and L3 was branched from LMDA with the number of 1, and its site was located in the distance of mean of 7.2 mm and 2.9 mm in SD and WKY, respectively. The right ventricular branches (R) of LMDA were short and branched in irregularly compared with L. The number of 1~4 of R were branched from LMDA. With regarding to the distribution area of L and the ligation site for MI, moderate MI (25~35% of LV) might be resulted in 70.4% and 60.7% in SD and WKY rats. Small MI might be produced intentionally if the ligation would be located at the 4~6 mm from LCA origin in the left side of LMDA. These data wold be helpful to expect the size of MI and to reproduce of small MI, intentionally, in rat hearts.

당귀의 해부형태학적 특징에 따른 기원판별 (Discrimination of Angelicae Radix Based on Anatomical Characters)

  • 당귀류 약재의 기원식물에 대해 해부형태적 특징을 관찰하고, 이를 통하여 당귀 기원 및 국내유통 당귀류 약재의 판별에 이용코자 실험한 결과는 다음과 같다. 기원식물 뿌리의 해부형태특징은 정상 2기생장, 주피발달, 유세포내 전분함유 등이 공통점이었고, 후각세포층은 참당귀와 왜당귀에서 관찰되었으며 피층의 유실분포는 참당귀와 중국당귀에서 관찰되었다. 또한 참당귀에서만 속성후벽세포가 다량 관찰되었다. 잎의 표피세포는 중국당귀가 가장 컸으며 세포표면의 형태는 참당귀와 중국당귀는 거친 주름이져 있었고, 왜당귀는 완만한 굴곡이 관찰되었다. 3종의 기공은 모두 불규칙형 부세포를 가지고 있었고 참당귀가 $mm^2$당 600개 이상으로 가장 많은 기공이 분포하고 있었다. 모용은 참당귀에서만 원뿔형으로 관찰되었다. 엽병은 3종식물에서 모두 유조직이 잘 발달된 형태였고 환상으로 병립유관속이 발달하고 있었다. 국내 유통 당귀류 약재의 판별에 있어서 해부형태적 특징을 정리하여 간단한 동정 KEY를 적용하여 만든 검색표가 유용하게 활용될 수 있었다. 검색표를 이용하여 전국 7개 지점에서 수집한 약재를 판별한 결과, 당귀 (국산) 또는 토당귀로 유통되는 약재는 참당귀 (A. gigas)로 동정되었고, 일당귀로 유통되는 약재는 왜당귀(A. acutiloba)로 동정되었다. 중국당귀는 현재 시중 유통되지 않고 있으며 본연구에서 사용된 약재 중국당귀는 중국 현지에서 직접 구입해 온 것으로 중국당귀 (A. sinensis)의 뿌리와 동일한 횡단면 형태를 관찰할 수 있었다. 그러므로, 현재 국내 유통되는 당귀류 약재는 (토)당귀의 경우는 KP에서 규정하는 것과 동일기원이었으며, 일당귀는 일본약국방(JP,2001)에서 규정하는 것과 동일한 기원으로 당귀류 유통약재의 기원상에 오류는 없었다.
