Relationship Between Shift Intensity and Insomnia Among Hospital Nurses in Korea: A Cross-sectional Study

  • Objectives: This study explored the relationship between shift intensity and insomnia among hospital nurses. Methods: The participants were 386 female hospital nurses who underwent a special health examination for night workers in 2015. The Korean Insomnia Severity Index (ISI), indices of shift work intensity, and other covariates such as amount of exercise, level of alcohol consumption, employment duration, and hours worked were extracted from the health examination data. The indices for shift intensity were (1) number of 3 consecutive night shifts and (2) number of short recovery periods after a previous shift, both assessed over the prior 3 months. Multiple logistic regression analysis adjusted for the aforementioned covariates was performed to evaluate the association of shift intensity with insomnia, defined as an ISI score of ≥8. Results: The nurses with insomnia tended to be younger (p=0.029), to have worked 3 consecutive night shifts more frequently (p<0.001), to have experienced a greater number of short recovery periods after the previous shift (p=0.021), and to have worked for more hours (p=0.006) than the nurses without insomnia. Among the other variables, no statistically significant differences between groups were observed. Experiences of 3 or more consecutive night shifts (odds ratio [OR], 2.33; 95% confidence interval [CI], 1.29 to 4.20) and 3 or more short recovery periods (OR, 2.01; 95% CI, 1.08 to 3.73) were associated with increased odds of insomnia. Conclusions: The results suggest that decreasing the shift intensity may reduce insomnia among hospital nurses working rotating shifts.

  • The purpose of this study is to present the effective management strategy of RI based on the results of research on the causal relationship between the regional medical care capacity and the hospital management performance by calculating the RI of the general hospitals in Korea, This research has significance. The results of this study are as follows: First, statistically significant differences were found in Number of beds and recurring profits in urban areas by the general characteristics. Second, the correlations between the RI and the variables of the regional medical use are as follows: Recurring profit ratio, New Outpatient Visits, Operating Margin, Daily Outpatient Visits per 100 Beds, Daily Inpatient Days per 100 Beds, and Average Charge per Inpatient Day. Based on the results of this study, the significance of this study is as follows. First, we calculated the affinity for local medical use, which is the index of local medical utilization. Secondly, it is analyzed according to internal and external environmental factors such as city size, hospital size, etc. It can be said that the hospital provided basic data for establishment of hospital management strategy to increase the utilization rate of local medical care.

  • The purpose of this study was to evaluate the clinical outcomes of patients with temporomandibular joint disorder before and after performance of arthrocentesis and stabilization splint therapy. The subjects of this study were 33 patients with limited mouth opening or pain or joint effusion who visited the Department of Oral and Maxillofacial Surgery, St Mary's Hospital, Catholic University of Korea. The arthrocentesis was performed and the stabilization splint was worn immediately after the arthrocentesis. Comparing the clinical outcomes of treatment at the times of first medical examination, 3 months and 6 months later on treatment. The results are as follows; 1. Statistically significant increase in the amount of maximum mouth opening occurred 3 months and 6 months later on treatment. 2. Statistically significant decrease in the average value of pain on test measured by VAS during maximum mouth opening, protrusive movement, right lateral movement, left lateral movement occured 3 months and 6 months later on treatment. The results suggested that arthrocentesis and stabilization splint therapy provide a improvement on patients with temporomandibular joint disorder when the exact diagnosis were provided.

  • Phlegm smear testing for acid-fast bacilli (AFB) requires careful examination of tubercle bacilli under a microscope to distinguish between positive and negative findings. The biggest weakness of this method is the visual limitations of the examiners. It is also time-consuming, and mistakes may easily occur. This paper proposes a method of identifying tubercle bacilli that uses a computer instead of a human. To address the challenges of AFB testing, this study designs and investigates image systems that can be used to identify tubercle bacilli. The proposed system uses an electronic microscope to capture digital images that are then processed through feature extraction, image segmentation, image recognition, and neural networks to analyze tubercle bacilli. The proposed system can detect the amount of tubercle bacilli and find their locations. This paper analyzes 184 tubercle bacilli images. Fifty images are used to train the artificial neural network, and the rest are used for testing. The proposed system has a 95.6% successful identification rate, and only takes 0.8 seconds to identify an image.

  • Backgrounds : The goal of drug therapy in pneumoconiosis is to inhibit the progression of pulmonary fibrosis related to a toxic effect of the inhaled substance. Although there have been many studies on the therapy of pneumoconiosis, it is still elusive. Quinolyl piperazine phosphate (QP), a derivative of chloroquine, is less toxic, more effective, and longer action than chloroquine. This investigation was performed to examine the effect of the quinolyl piperazine phosphate in silicotic rats. Methods : The silica group was administered intratracheally by 40 mg free silica dust with 0.5 ml normal saline, and the QP group was orally administered QP 10 mg per week after free silica instillation. The animals in the silica group and the QP group were killed at the 1st, 3rd, 8th and 20th week after free silica instillation. We observed the total cell count in bronchoalveolar lavage fluid, luminol-dependent chemiluminescence by viable alveolar inflammatory cells, the dry weights and the amount of hydroxyproline in the left lung and the histopathologic examination in the right lung. Results : 1) The total number of cells of bronchoalveolar lavage fluid in the QP group tended to be decreased in comparison with the silical group. But, It was not significant. 2) Luminol-induced chemiluminescence by viable alveolar inflammatory cells in the QP group was similiar to that in the silical group. 3) The dry weights in the left lung at the 3th and 8th week in the QP group were significantly decreased compared to the silical group. 4) The total amount of hydroxyproline at the 3rd week of the QP group were significantly decreased compared to the silical group. In the silica group, the total amount of hydroxyproline was significantly increased at the 3rd week compared with the 1st group. But, in the QP group, it was significantly increased at the 8th week. 5) In tissue pathology, the infiltration of inflammatory cells around bronchiole, and the number and the size of silicotic nodule in the QP group were similar to the silica group. But, the extent of fibrosis is less than the silica group. Especially we observed progressive massive fibrosis which located in the periphery in 3 cases among the silica group, but couldn't observe in the QP group. Conclusions : QP doesn't significantly suppress the pulmonary fibrosis consequent to the intratracheal instillation of free silica dust, but delay the progression of fibrosis.

The Food Safety of Superfine Powder (Phellinus linteus) Processed by Nanomill in C57BL/6 Mice (C57BL/6 마우스에서 나노밀 가공된 초미세분말(상황버섯)의 식이 안전성 연구)

  • A officinal mushroom, Phellinus linteus (PL) has been known to exhibit potent biological activities including antioxidative and anticancer effect. PL is consumed as a type of powder or extract for the purpose of health promotion and disease treatment. Recently superfine PL products was commercialized according to the development of pulverizing technology such as nanomill, so the evaluation of food safety is suggested. This study was conducted to evaluate the food safety of superfine PL (SPL) through hematological, biochemical and histological examination in mice as compared with fine PL (FPL). In the particle size distribution in volume after nanomill processing, the mean diameter of SPL and FPL particles was 11.78 ${\mu}m$ and 216.1 ${\mu}m$, and d (0.5), the particle diameter measured at 50% of distribution was 5.5 ${\mu}m$ and 147.9 ${\mu}m$, respectively. As the result of body weight, food intake and the weight of organs, SPL group didn't show any statistical difference compared with FPL group and normal group (N). Hematological and biochemical values were also involved in the normal range, although ALT (N vs. FPL, P<0.001) and BUN (N vs. FPL, P<0.01; N vs. SPL, P<0.01) showed significance compared with N group but there are no significance between FPL and SPL group. In the result of histological examination with liver, kidney, spleen, and small and large intestine, abnormal findings such as inflammatory reaction and histological changes were not observed. Our results suggest that the oral intake of SPL diet is not harmful to the animal in the hematological, biochemical and histological aspects although particle size was reduced to the level of superfine. However, further study will be necessary to confirm the histological safety in relation to the gastrointestinal contact of superfine particles in the case of large amount and long-term intake.

  • Purpose: Lymphoscintigraphy is absolutely being used standard examination in lymphatic diagnosis, evaluation after treatment, and it is useful for lymphedema to plan therapy. In case of lymphoscintigraphy of lower-extremity lymphedema, it had an effect on results if patients had not pose same position on the examination of 1 min, 1 hour and 2 hours after injection. So we'll study the methods to improve confidence with minimized quantitative analysis errors by influence factors. Materials and Methods: Being used the Infinia of GE Co. we injected $^{99m}Tc$-phytate 37 MBq (1.0 mCi) 4 sylinges into 40 people's feet hypodermically from June to August 2010 in Samsung Medical Center. After we acquired images of fixed and unfixed condition, we confirmed the count values change by attenuation of soft tissue and bone according to different feet position. And we estimated 5 times increasing 2 cm of distance between $^{99m}Tc$ point source and detector each time to check counts difference according to distance change by different feet position. Finally, we compared 1 and 6 min lymphoscintigraphy images with same position to check the effect of quantitative analysis results owing to difference of amounts of movement of the $^{99m}Tc$-phytate in the lymphatic duct. Results: Percentage difference regarding error values showed minimum 2.7% and maximum 25.8% when comparing fixed and unfixed feet position of lymphoscintigraphy examination at 1 min after injection. And count values according to distance were 173,661 (2 cm), 172,095 (4 cm), 170,996 (6 cm), 167,677 (8 cm), 169,208 counts (10 cm) which distance was increased interval of 2 cm and basal value was mean 176,587 counts, and percentage difference values were not over 2.5% such as 1.27, 1.79, 2.04, 2.42, 2.35%. Also, Assessment results about amounts of movement in lymphatic duct within 6 min until scanning after injection showed minimum 0.15%, and maximum 2.3% which were amounts of movement. We can recognize that error values represent over 20% due to only attenuation of soft tissue and bone except for distance difference (2.42%) and amounts of movement in lymphatic duct (2.3%). Conclusion: It was show that if same patients posed different feet position on the examination of 1 min, 1 hour and 2 hours after injection in the lymphoscintigraphy which is evaluating lymphatic flow of patients with lymphedema and analyzing amount of intake by lymphatic system, maximum error value represented 25.8% due to attenuation of soft tissue and bone, and PASW (Predictive Analytics Software) showed that fixed and unfixed feet position was different each other. And difference of distance between detector and feet and change of count values by difference of examination beginning time after injection influence on quantitative analysis results partially. Therefore, we'll make an effort to fix feet position and make the most of fixing board in lymphoscintigraphy with quantitative analysis.

Characterizing the Stimulation Intensity in Acupuncture Manipulation Techniques for Tonification and Sedation Therapy (침자 보사에서 자침 자극 강도의 특성)

  • Objectives : The purpose of this study was to characterize the stimulation intensity in acupuncture manipulation techniques for tonification and sedation therapy. Methods : To describe the level of stimulation used in acupuncture manipulation techniques for tonification and sedation therapy, we reviewed the acupuncture manipulation techniques described in classical medical textbooks. Results : Based on the patients' conditions and pattern identification, acupuncture manipulation strategies for tonification and sedation therapy were chosen. For example, the excess condition was treated with sedative therapy, and the deficiency condition was treated with tonification therapy. For tonification therapy, weak to modest stimulation was applied in acupuncture manipulation techniques, whereas intense stimulation was applied for sedative therapy. Even though the intensity of acupuncture stimulation was chosen based on the clinical examination, deqi sensation is a crucial component of acupuncture treatment, and during acupuncture practice, the practitioner should choose the right intensity of acupuncture stimulation based on deqi response in each patient. Conclusions : We concluded that the tonifying and sedative effects of acupuncture treatment are related to the stimulation intensity of acupuncture manipulation techniques. For individualized medicine, the right amount of acupuncture stimulation should be administered based on the patients' conditions and responses, such as deqi responses.

Urine Cotinine Should Be Involved in Initial Evaluation of Tinnitus in Adolescents

  • Objectives. Smoking is associated with hearing loss, while the correlation between tinnitus and smoking is not fully elucidated. This study aimed to evaluate risk factors of tinnitus in adolescents in terms of smoking, and we identified a rectifiable parameter that can be serially monitored. Methods. A cross-sectional study was conducted using data from the Korea National Health and Nutrition Examination Survey, with 2,782 participants aged 12 to 18 years, from 2008 through 2011. Participants with history of ear disease, hearing loss, and inadequate responses to questionnaires were excluded. We investigated the prevalence of tinnitus and tinnitus-related annoyance by questionnaire and sought potential risk factors in blood and urine tests and smoking history. Results. The prevalence of tinnitus in the 12- to 18-year-old population was 17.5%, with 3.3% reporting tinnitus-related annoyance. On univariate analysis, the prevalence of tinnitus increased with age (P<0.001) and was higher among girls (P=0.012). Blood tests and urinalysis showed significant correlation between tinnitus and red blood cell count, alkaline phosphatase levels, and urine cotinine (P=0.002, P<0.001, P=0.018, respectively). In multivariate analysis, the urine cotinine level was the only parameter associated with tinnitus (odds ratio, 1.000; 95% confidence interval, 0.999 to 1.000; P=0.038). Smoking was also significantly correlated with tinnitus (P=0.043), and amount of smoking with tinnitus-related annoyance (P=0.045). However, current smoking and past smoking were not correlated with tinnitus. Conclusion. Urine cotinine may be a rectifiable marker for management of tinnitus in adolescents. This suggests that smoking cessation should be incorporated in the management of tinnitus in adolescents.
