Study for Association between Adverse Drug Reactions and Causative Drugs in the Elderly Using Data-mining Analysis (데이터마이닝 분석을 이용한 노인약물유해반응과 원인약물의 연관성연구)

  • Purpose: To investigate adverse drug reactions (ADR) and causative drugs in the elderly 65 years of age or older, using Korean spontaneous reporting adverse events reporting database from June 2009 to December 2010. Methods: We estimated the association between ADRs and implicated medications by calculating a proportional reporting ratio (PRR), reporting odds ratio (ROR), and information component (IC). We reexamined the most frequently implicated medications and ADRs, and the seriousness of ADRs. Then, we assessed reports and concordant rate of ADRs due to medications designated as "high-risk" in elderly by 2012 healthcare effectiveness data and information set (HEDIS) or "potentially inappropriate" by 2012 American Geriatrics Society updated Beers criteria for potentially inappropriate medications (PIMs). Results: Among 15,484 elderly reports, data-mining analysis by PRR, ROR and IC showed that 421 drug-ADR pairs were detected as signals (3,189). The most frequently reported ADR and causative drug were urticaria (470) and contrast media agents (647), respectively. One hundred eighty nine ADR cases were graded as serious. Twenty-two kinds of high-risk medications were shown to be implicated in only 0.9% of ADRs. Only thirty-nine cases were consistent with 2012 Beers criteria or HEDIS. Conclusion: These results suggest that management of the other medications including contrast media agents as well as close monitoring of PIMs are necessary for reducing ADRs in the elderly.

Surgical Treatment of Ankle Fractures in the Elderly (고령 환자 족관절 골절의 수술적 치료)

  • Purpose: The optimal management for ankle fracture in elderly patients remains controversial. This study was undertaken to review the results of surgical treatment of ankle fracture in the elderly and to compare with other studies. Materials and Methods: The participants in this study were 33 patients over the age of 65(average 71.5 years) who underwent surgical treatment of ankle fracture from January 2004 to December 2011. The study was a retrospective review of outcomes after open reduction and internal fixation (ORIF) of ankle fractures. To measure the clinical outcomes, we assessed postoperative complications, the pre- and post-operative mobility status, fracture union status, the time of fracture union and the AOFAS (American Orthopaedic Foot and Ankle Society) Ankle-Hindfoot scale. The level of patient satisfaction was also identified. Results: Delayed wound healing occurred in three patients(9.1%) but their wounds healed with repeated dressings without additional surgical treatment. Malunion occurred in one patient(3%). One patient(3%) had postoperative infection but healed with antibiotic treatment. 24 patients(96%) returned to preoperative mobility status. VAS score was lower than 2 in all patients. Bone union occurred with the 3.8 months (average months) after the surgery in all patients. Average AOFAS score was 87.4 and these were similar results as other studies of young patients. All patients were satisfied with surgical outcomes according to interviews. Conclusion: Surgical treatment of ankle fractures in the elderly can carry a significant risk of delayed wound healing and infection but incidence is relatively low. Internal fixation of ankle fractures in the elderly can be undertaken safely and the majority of patients can expect good outcome.

Could Adjuvant Chemotherapy after Surgery Benefit Elderly Patients with Advanced Gastric Cancer?

  • Purpose: The aim of this study was to evaluate tolerance to adjuvant chemotherapy, and to compare survival between treatments using only surgery and using surgery with adjuvant chemotherapy, in elderly patients with advanced gastric cancer who were ${\geq}75years$ of age. Materials and Methods: Patients ${\geq}75years$ of age who were diagnosed with pathological stage II or III gastric cancer were identified retrospectively and categorized into the surgery only and surgery with adjuvant chemotherapy groups. Clinicopathological and survival data were compared between these two groups. Results: Among the 130 patients studied, 67 patients underwent curative surgery only, and 63 patients received adjuvant chemotherapy after curative surgery. In the latter group, adverse events were reported in 24 patients (38.1%). The treatments were discontinued in 19 patients (30.2%) owing to any reason. The overall 5-year survival rates of the surgery only and the surgery with adjuvant chemotherapy groups did not differ significantly (44.1% vs. 30.7%, respectively; P=0.804). Among 90 death events, deaths from recurrences of gastric cancer occurred in 42 patients. Multivariate analyses revealed that the American Society of Anesthesiologists score and the depths of tumor invasions were related to survival, and the addition of adjuvant chemotherapy after surgery did not influence survival. Conclusions: The decision for the addition of adjuvant chemotherapy for elderly patients should be taken after considering the condition of individual patients and their life expectancies.

Onlay patch augmentation in rotator cuff repair for moderate to large tears in elderly patients: clinical and radiologic outcomes

  • Background: This study evaluated the clinical and radiologic outcomes of onlay patch augmentation in rotator cuff repair for moderate-to-large tears in elderly patients. Methods: We reviewed 24 patients who underwent onlay augmentation with dermal allograft after arthroscopic rotator cuff repair from January 2017 to March 2020. Inclusion criteria were patients aged >65 years with tears >2.5 cm, who were followed for >12 months after surgery, and patients who could raise their arms above 90° preoperatively. American Shoulder and Elbow Surgeons (ASES) score, Constant-Murley score, pain visual analog scale (VAS), and VAS for satisfaction were used as clinical outcomes. For the evaluation of cuff integrity, magnetic resonance imaging scans were performed every 3 months after surgery. The results were compared before and after surgery in all patients and between the retear and intact groups. Results: The average follow-up period was 16.38 months, and the mean age of patients was 71.05 years. All patients showed significant improvement in ASES score, Constant-Murley score, and pain VAS at the last evaluation. The average value of satisfaction VAS was 7.27/10. The retear rate was 25% (6/24) if Sugaya type 3 was categorized in the retear group, otherwise 16.7% (4/24), if Sugaya type 3 was categorized into the intact group. Irrespective of Sugaya type 3 being included in the retear group, there was no significant difference in outcome variables between the intact and retear groups during follow-up. Conclusions: In moderate-to-large rotator cuff tear in elderly patients, onlay patch augmentation improved clinical outcomes. Retear did not adversely affect clinical outcomes.

  • 미국의 65세 이상 노인인구의 구성을 보면, 민족이 더욱 다양화되고 있는 추세이다. 다양화 되고 있는 민족 가운데 한국계 미국인의 수는 급속히 증가하고 있으나, 그들의 건강문제에 대한 정보는 매우 부족한 실정이다. 연구목적: 연구의 목적은 한국계 미국 노인의 건강에 대한 현재까지의 연구를 정리하고, 이를 바탕으로 앞으로의 연구방향에 대해 논의하는 것이다. 연구방법: 컴퓨터 데이터베이스를 이용한 검색과 수작업에 의한 검색을 통해 수집된 21개의 자료를 분석하였다. 결론 및 제언: 21개의 자료는 네 개의 항목으로 나눠 정리하였다. 기존의 연구는 주제의 다양성이나 각 주제별 연구의 수에 있어서 매우 미흡하였다. 중요한 주제임에도 다뤄지지 않고 있는 분야에 대한 관심이 요구되며, 또한 각 분야에 대한 심도 있는 연구가 이루어져야 할 것이다.

Early Surgery in Valvular Heart Disease

  • The burden of valvular heart disease (VHD) is increasing with age, and the elderly patients with moderate or severe VHD are notably common. When to operate in asymptomatic patients with VHD remains controversial. The controversy is whether early surgical intervention should be preferred, or a watchful waiting approach should be followed. The beneficial effects of early surgery should be balanced against operative mortality and long-term results. Indications of early surgery in each of the VHD will be discussed in this review on the basis of the latest American and European guidelines.

Religious Participation and Depression among American Older Adults (미국노인의 종교활동참여와 우울증)

  • The main purpose of this study was to examine the impact of religious participation on the depression of elder adults in USA. Specifically, this study examined how the influence of religious participation varied according to continuity or discontinuity of participation. Data from N=1,658 adults aged 65-90 who were respondents to two waves of the U.S. National survey of Families and Households 1987-1993 were used for these analyses. Depression was measured with a 12-item (of the original 20) modified version of the CES-D (Center for Epidemiological Studies-Depression). Multivariate regression models controlling for several demographic variables were estimated. Some clear evidence was found supporting activity theory and continuity theory That is, participating in a religious organization role at Time 2 but not Time 1 (T1 No - T2 Yes) and being continuously involved in religious organizations both at Time 1 and Time 2 (T1 Yes -T2 Yes) were associated with reduced depression, compared to continuous nonparticipation in religious organizations (71 No -72 No).

Displaced Scapula Fracture (Ideberg Type IIb) Combined with a Large Rotator Cuff Tear in Anterior Shoulder Dislocation: A Case Report

  • Traumatic anterior shoulder dislocation combined with scapular fracture in elderly patients is relatively rare. In this case, a patient visited Emergency Room of Dong-A University Hospital for shoulder pain after falling off a ladder. Radiographs demonstrated anterior shoulder dislocation with displaced Ideberg type IIb scapula (glenoid fossa) fracture combined with a large rotator cuff tear on magnetic resonance imaging. We performed arthroscopic rotator cuff repair, but a large fragment in the inferior glenoid was left untreated. At the 1 year follow-up visit, the pain visual analogue scale of the patient was 2, the American Shoulder and Elbow Society score was 88 and the patient had gained nearly full range of motion without any apprehension.

Comparative Analysis of Curriculum and Research Trends Related to Clothing for Persons with Special Needs between Korea and America (특수의복에 관한 실태조사 연구 -한국과 미국의 교과과정과 연구경향분석을 중심으로-)

  • With the purpose of identifying clothing for persons with special needs one of new directions in Clothing and Textiles, this pater comparatively analyzed current curriculum and research trends related to this field between Korea and America. Questionaire was distributed to 25 Korean colleges with Clothing and Textiles department and 4 American colleges known for special clothing study. Questions regarded the name of course, level (grade), number of credit houre, prerequisits and supporting courses, subjects of lecture and projects, way of managing lecture and projects, evaluation system, other special characteristics. Resources for analyzing research trends are Thesis and Dissertations in Clothing and Textiles, Research Journal of Clothing & Textiles/Home Economics Association, College Professors' research reports collection, of both Korea and America. Years of publications are 1972∼1986 (America), 1978∼1988 (Korea). Findings are as follows; 1. Clothing for special needs can be classified into various groups; Clothing for physically handicapped, mentally retarded, elderly, unusual-sized, pregnant/nursing mothers, institutionalized, protective clothing, uniform/career apparel. 2. Management of coursework related to special clothing has several common characteristics specially in America; Building-up coursework, problem-solving orientation, cooperative team-preject, interdisciplinary approach, client-versus-designer preference, community involvement. In Korea, a few school only has special clothing coursework and its scope of study was very limited. 3. Majority of research reports done with the subjects of special clothing had made for physically handicapped people and the elderly. Protective clothing and uniform have recently been paid much interests. 4. Researchers had mainly concerned on Design and Construction Aspects and nextly on social-psychological aspects. However, in America, selection and care aspects and marketing aspects of special clothing have been very procuctive issues recently. 5. Study on clothing for special needs has several opportunities to be stressed and improvements to be strengthened. Opportunities included active participation to the welfare of community society, strong publicity of accurate professional image, share with business and industry, interdisciplinary approach and new job market. Improvements were discussed in terms of strengthening supporting courses/prerequistes, intensifying interdisciplinary cooperation, provoking strong community involvement and securing financial support and research funds.

Use of Hand Massage with Presence to Increase Relaxation in Korean -American Elderly (재미 한국이민노인에게 시행한 손마사지가 이완에 미치는 영향)

  • 간호사가 사용하는 중재로서 관심을 표현하는 방법이며 의사소통의 통로인 접촉의 한 형태로서의 마사지가 이완과 안녕감을 증진시킬 수 있는지를 알아보기 위해 간편하면서도 장소에 거의 구애를 받지 않는 손마사지를 미국 미네소타주에 살고 있는 22명의 한국 이민노인에게 시행하였다. 자료수집 기간은 1992년 12월부터 1993년 6월까지 약 6개월이 소요되었다. 편의표출방법으로 대상자를 선정한 원시실험연구설계로, 독립 변수로 손마사지를 사용하였고 이완행동, 수축기혈압, 이완기 혈압, 맥박수로 측정한 이완의 정도와 안녕감을 종속변수로 사용하였다. 손마사지는 Snyder등이 개발한 방법으로 한 손에 2분 30초씩 5분이 소요되는데 이틀에 한번씩 6회 실시하였다. 안녕 감을 측정하는 Cantril ladder는 10개의 사다리로 된 도구로 첫번 마사지전, 세번째 마사지후, 그리고 여섯번째 마사지후에 대상자로 하여금 직접 사다리 점수를 매기도록 하였다. Luiselli등이 개발한 이완행동 측정도구로 각 마사지 전후에 대상자의 이완정도를 관찰하였고, 마사지 전후에 혈압과 맥박수를 측정하였다. 자료는 paired t-test와 ANOVA, repeated measures ANOVA로 분석하였다. 연구의 결과는 다음과 같다. 1. 1-6회 동안의 마사지전의 이완점수의 평균값과 1-6회 동안의 마사지후의 이완점수의 평균값을 비교한 결과 마사지후에 이완의 정도가 유의하게 증가하였고 (t=10.70, p=.000), repeated measures ANOVA로 검증한 시간에 따른 효과도 유의한 차이를 보였다. 2. 1-6회 동안의 마사지전의 수축기 혈압의 평균값과 1-6회 동안의 마사지후의 수축기 혈압의 평균값을 비교한 결과 마사지후에 수축기 혈압이 유의하게 저하되었 다(t=6.09, p=.000). 3. 1-6회 동안의 마사지전의 이완기 혈압의 평균값과 1-6회 동안의 마사지후의 이완기 혈압의 평균값을 비교한 결과 유의한 차이를 보이지 않았다(t=.27, p=.793). 4. 1-6회 동안의 마사지전의 맥박수의 평균값과 1-6회 동안의 마사지후의 맥박수의 평균값을 비교한 결과 마사지후의 맥박수가 유의하게 감소하였다. (t=4.45, p=.001). 5. Cantril 안녕감의 점수는 유의한 차이를 보이지 않았다(F=2.42, p=1.01)
