Wandering Behavior in Korean Elders with Dementia Residing in Nursing Homes (요양시설 치매노인의 배회행동)

  • Purpose: This study investigated various factors relatied to wandering behavior of Korean elders with dementia (KED). Methods: A sample of 160 ambulatory residents with dementia from 14 long term care facilities was used to examine demographic, individual, cognitive, physical health, and environmental characteristics by comparing wanderers (N=108) to nonwanders (N=52). Subjects were evaluated by Korean versions of the Mini-mental State Exam (K-MMSE), the Physical and Instrumental Activities of Daily Living (K-PIADL), and the Revised Algase Wandering Scale Nursing Home version (KRAWS-NH) along its six dimensions. Demographic and environmental data were also obtained. Independent sample t-tests, Chi-square test, Fisher's Exact tests, and ANCOVAs were used to examine differences between wanderers and nonwanders. Results: Wanderers were significantly (p<.05) older and had more limitations in K-PADL and K-IADL. The degree of overall wandering and certain features of wandering were significantly different (p<.05) by total number of residents in the facility, type of bedroom (i.e., "Ondol"), and color of bedroom and living-room walls (i.e., sky blue). Conclusions: Findings of this study may be useful in understanding wandering behavior of KEDs and thus developing more culturally specific management strategies.

  • 서울대학병원의 마스터플랜 재구성에 대한 프로젝트로서 기존의 환경에 대한 조사와 분석 그리고 외국사례와의 비교를 통해 앞으로 서울대학병원이 나아가야 할 목표 및 방향을 단기(1-2년), 중기(2-5년), 장기(10년 이상)계획으로 나누어 각 단계별 항목들을 분석, 구체화하였다. 단계계획에서는 응급실의 확장과 어린이 병원증축에 대한 내용을 제시하였고, 중기계획에서는 외래진료센타(ambulatory care center)에 대한 공간구성을 통하여 기존 마스터플랜이 나아가야 할 방향을 제시하였다. 장기계획에서는 어린이 병원의 이전, 캠퍼스 기능구획을 연구, 교육, 병원으로의 분류, 공급시설의 개선과정을 제시하였다. 이 연구를 통하여, 한국의 병원시장에 대한 경향을 다음과 같이 분석할 수 있었다. 1) 증축공간이 부족한 도심에 밀집한 한국병원의 경우, 리노베이션을 통한 마스터플랜은 다양한 시설물을 개선, 관리, 통합할 수 있는 방법이다. 2) 편리성, 환자의 프라이버시, 전문성, 방문자 오락시설, 통합적인 치료환경의 구축이다. 3) 예방 의약품의 증가, 건강센타, 건강중심의 프로그램, 확장된 진료시설 등 건강유지분야의 개발이다. 마스터 플랜은 계속 진행되는 과정의 일부분이며, 변화하는 조건 속에서 적용되어, 수정되어져야 할 것이다.

A Study on Relations of Peripheral Arterial Disease Marker and Photoplethysmography Measured from the Lower Limb (하지에서의 광용적맥파와 말초동맥질환 표지자의 상관관계 연구)

  • In this study, photoplethysmography(PPG) was suggested as a way to replace the ankle-brachial index(ABI) in diagnosing PAD. The method using the PPG was presented for the simplification of the PAD diagnosis method which was used before. And the index related to the health condition of the artery from the PPG measured in both big toes of the subjects through the experiment was drawn. The indexes showing the significant relativeness in the Pearson correlation analysis with the ABI were the stiffness index(SI), reflection index(RI); it was confirmed each of them had the correlation coefficient of 0.688, and 0.637 at p < 0.05. The explanation ability of the linear regression equation derived using ABI, SI and RI was 52.5%. The explanation ability of the secondary curve regression equation derived using ABI, squared SI was 54.7%. It is expected to provide patients with significant results and draw the index associated with PAD by measuring PPG easily in the real life instead of the ambulatory care field.

  • The real-time monitoring about the activity of the human provides useful information about the activity quantity and an ability. This study developed a system for human physical activity assessment in ambulatory monitoring using portable sensing device combining a tri-axial accelerometer and wireless sensor node. This real-time system is able to identify several postures, posture transitions and movements with classification algorithm. In addition, this system also features fall detection capability. The results of the assessment for evaluating the performance of the system show high identification accuracy.

Effects of Intraoperative Music Therapy on Anxiety in Patients Undergoing Local Anesthesia (수술 중 음악요법이 국소마취 수술환자의 불안에 미치는 효과)

  • Purpose: This study was performed to identify the effects of intraoperative music therapy on the anxiety level of adult patients undergoing local anesthesia in the operating room. Method: The research design was a nonequivalent control group pretest-posttest. Data collection was done with 48 patients undergoing local anesthesia from June 2003 to September 2()03. The 24 patients in the experimental group were given music therapy through headphones from the beginning of the operation to the end of the operation with local anesthesia. The data were analyzed using SPSS/PC 10.0 for Windows. Results: Postoperative state anxiety scores were significantly different between the experimental group and the control group. Intraoperative pulse rates were significantly different between the two groups. But systolic and diastolic blood pressures and respiratory rates were not significantly different between the two groups. Conclusion: Considering these results, music therapy can be regarded as an effective nursing intervention that relieves anxiety for adult patients undergoing local anesthesia. However, to determine the best use of this therapy, further study is required for different types of anesthesia and operations such as ambulatory surgery.

  • Objectives : The objective of this study is to evaluate the treatment effect of Taping therapy in Herniation of Inter-Vertebral Disc. Methods : Taping therapy were performed for 31 days ambulatory care in a patient who had Herniation of Inter-Vertebral Disc. Results : Physical examination of Lumbar Spine has improved, VAS were also decreased. Conclusion: Taping therapy were effective in reducing the symptoms. We think that it need the further study and clinical trial for Herniation of Inter- Vertebral Disc.

Real-Time Heart Rate Monitoring System based on Ring-Type Pulse Oximeter Sensor

  • With the continuous aging of the populations in developed countries, the medical requirements of the aged are expected to increase. In this paper, a ring-type pulse oximeter finger sensor and a 24-hour ambulatory heart rate monitoring system for the aged are presented. We also demonstrate the feasibility of extracting accurate heart rate variability measurements from photoelectric plethysmography signals gathered using a ring-type pulse oximeter sensor attached to the finger. We designed the heart rate sensor using a CPU with built-in ZigBee stack for simplicity and low power consumption. We also analyzed the various distorted signals caused by motion artifacts using a FFT, and designed an algorithm using a least squares estimator to calibrate the signals for better accuracy.

Factors Affecting Health-Related Quality of Life of the BPH Patients (전립선 비대증 환자의 삶의 질 영향요인)

  • Purpose: This study was to investigate the factors affecting health-related quality of life in Benign Prostatic Hypertrophy(BPH) Patients. Method: Data was collected on 259 outpatients who were receiving ambulatory treatment after having been diagnosed with BPH. The measurement tool for the quality of life was developed by Epstein and Deverka(1992) and adapted by Kim Yeong-hye(1997). Data was analyzed with the number, percentage, and the mean and multiple regression, by using the SPSS 12.0 program. Result: The mean quality of life in the sexual-life domain respectively. Factors affecting the quality of life in BPH patients were the symptom of BPH(51.4%), the perceived health state(3.6%), and self-efficacy (1.6%). These three factors described 56.6% of quality of life. Conclusion: It's necessary to have an intervention plan that can enhance the most influential disease-symptom management in a patient, and self-efficacy when planning a nursing education program aiming to promote quality of life in BPH patients.

Safe anesthesia for office-based plastic surgery: Proceedings from the PRS Korea 2018 meeting in Seoul, Korea

  • There has been an exponential increase in plastic surgery cases over the last 20 years, surging from 2.8 million to 17.5 million cases per year. Seventy-two percent of these cases are being performed in the office-based or ambulatory setting. There are certain advantages to performing aesthetic procedures in the office, but several widely publicized fatalities and malpractice claims has put the spotlight on patient safety and the lack of uniform regulation of office-based practices. While 33 states currently have legislation for office-based surgery and anesthesia, 17 states have no mandate to report patient deaths or adverse outcomes. The literature on office-base surgery and anesthesia has demonstrated significant improvements in patient safety over the last 20 years. In the following review of the proceedings from the PRS Korea 2018 meeting, we discuss several key concepts regarding safe anesthesia for office-based cosmetic surgery. These include the safe delivery of oxygen, appropriate local anesthetic usage and the avoidance of local anesthetic toxicity, the implementation of Enhanced Recovery after Surgery protocols, multimodal analgesic techniques with less reliance on narcotic pain medications, the use of surgical safety checklists, and incorporating "the patient" into the surgical decision-making process through decision aids.

Retrospective Drug Utilization Review of Antibiotics for Respiratory Tract Infection(RTI) in Ambulatory Outpatient Care (외래 호흡기계 질환에서 항생제 사용에 대한 후향적 평가방안)

  • As respiratory tract infections (RTI) account for about 60% of all antibiotic prescriptions in outpatient care setting, there are significant concerns about emerging resistance that are largely due to the excessive or inappropriate use of antibacterial agents for viral respiratory infections. This study was aimed to develop retrospective drug utilization review (DUR) program of antibiotics for RTIs using Delphi methods. Retrospective DUR criteria of antibiotics for RTIs were identified based on clinical practice guidelines and opinion of experts. Expert panel members were clinical doctors and pharmacists and Delphi method was applied by survey on 16 members of panels. The claim data from Korean Health Insurance Review & Assessment (HIRA) were used to examine trends in outpatient antibiotic prescription between Janunary to December of 2008. As results, Quality index for RTI was assessed for the claim type, antibiotics use of quantity, duration, number and cost. Antibiotic prescription rate for RTIs, Defined Daily Dose (DDD), and duration of antibiotics use were more recognized as significant quality index by experts' opinion. Use of first line agents suggested by guidelines was low and duration of antibiotics use was shorter compared to the recommendations. Antibiotics were over prescribed for RITs. However, dose and duration of antibiotics were under-used.
