The Relationship between Anxiety, Depression and 24-hour Ambulatory Blood Pressure in Hotel Employees (호텔 종사자들에서 불안 및 우울과 24시간 활동혈압 관련성)

  • Objectives: Anxiety and depression are known to be associated with hypertension, and blood pressure can vary spontaneously throughout the day. The aim of this study was to evaluate anxiety, depression and 24-hour ambulatory blood pressure (24-h ambulatory BP) in employees at their worksite. Methods: A total of 107 volunteers among 136 employees at a hotel in Gyeongju, Korea were enrolled in this study between December 2009 and March 2010. The Beck Anxiety Inventory (BAI) and the Beck Depression Inventory (BDI) were used to assess anxiety and depression, respectively. Blood pressure was measured using a 24-h ambulatory BP monitoring system. Results: No significant differences in either BAI or BDI scores were found when hypertensive individuals were compared to normotensive individuals. The frequency of diastolic non-dipper was significantly higher in individuals with depressive symptoms compared to those without (p<0.05). Depression was significantly associated with diastolic non-dipper (OR: 6.85, 95% CI: 1.50-30.01). Conclusions: The results of this study indicate that depression should be considered when deciding upon blood pressure control regimens, and appropriate additive psychotherapy may be beneficial in the treatment of hypertensive patients.

Development of a Pharmaceutical Care Service Model for Patients with Diabetes in Ambulatory Care Settings (외래환자 대상 당뇨병 약료서비스 모델 개발)

  • The prevalence of diabetes and its related morbidity and mortality are being increased. Despite the advancement of evidence-based pharmacotherapy in the management of diabetes, many patients in our country do not achieve satisfied therapeutic outcomes. Pharmaceutical care service can be defined as a patient-centered clinical service provided by pharmacists to improve therapeutic outcomes and quality of life of patients, by identifying, and preventing or resolving drug-related problems (DRPs). Pharmaceutical care service is interdisciplinary team-based practice, and is provided through collaborative practice agreement (CPA) between one or more physicians and pharmacists. This article describes a model of pharmaceutical care service which can be adopted in our country for patients with diabetes in the ambulatory care settings. With the successful implementation of this service, clinical, economic, and humanistic outcomes of patients will be improved. Therefore, by actively implementing pharmaceutical care service, pharmacist should contribute to the promotion of patients' health and to the advancement of health care delivery system.

Noise Reduction and Estimating the Similarity of Ambulatory ECG Signals (이동형 심전도 신호의 잡음 제거 및 유사도 평가)

  • In this study, we develope an ambulatory ECG acquisition system by implementing a patch-style and wireless electrode. To alleviate the inherent noisy characteristics of the mobile signal, we apply a matched filter and concurrently detect R-peak values. Moreover, the measure for resolving shape distance is computed to estimate the relative similarity between two ECG signals and to decide whether the abnormal characteristics in ECG exist or not.

  • In this study, we present an efficient ambulatory speech audiometric system to detect one's hearing problems at an earlier stage as possible without his or her visit to the audiometric testing facility such in a hospital or a clinic. To estimate a person's hearing threshold level in terms of speech sound response in his or her local environment, a digital assistant(PDA) device is used to generate the speech sound with implementing audiometric Graphic User Interface(GUI) system. Furthermore, a supra-aural earphone is used to measure a subject's hearing threshold level in terms of speech sound by the compensating the transducer's gain by adopting speech sound calibration system.

  • In this paper, we designed a low power and small-sized, light weighted intelligent ambulatory monitoring system using a flash memory card. The system's hardware specifications are as follows: 2 channels, 8bit/250Hz sampling rate, 20M byte storage capacity, a single-chip microcontroller (68HC11E9). To easily interface with PC based system, FFS(Flash File System) was used. We obtained the QRS detection rate of 99.14 through the evaluation with MIT/BIH database.

  • This study describes the ambulatory ECG monitoring system for the remote autom atic diagnosis. System: tlardware is based on one chip microcomputer(80c31) and its peripherals which consists of A/D, EPROM, RAM, LCD display and two preamplifiers, Power circuits, control logic circuits. A/D converted data were differentiated and low pass filtered. The detection of QRS complex and R point were accomplished by software algorithm based on adaptive threshold computed on low pass fi:leered signal. Rhythm analysis is performed by RR interval and average RR interval. The performance of QRS detection algorithm is evaluated by using MIT/BIH data base. Using this system, the trends of the arrythmia during the long term could be saved and displayed.

  • The purpose of this study was to analyze the ambulatory care facilities according to the change of medical circumstance and to find the proper ones in Korea. A logical model was used with the data which were obtained from the National Statistical Office, the Ministry of Health and Welfare in Korea and OECD Health Data. A proper ambulatory care facility in Korea is suggested as like as MOB (Medical Office Building). It should be considered for outpatient in distance within 1km long. It's proper size may be designed for 10,000persons to 50,000persons by regions.

Sensor fusion based ambulatory system for indoor localization

  • Indoor localization for pedestrian is the key technology for caring the elderly, the visually impaired and the handicapped in health care districts. It also becomes essential for the emergency responders where the GPS signal is not available. This paper presents newly developed pedestrian localization system using the gyro sensors, the magnetic compass and pressure sensors. Instead of using the accelerometer, the pedestrian gait is estimated from the gyro sensor measurements and the travel distance is estimated based on the gait kinematics. Fusing the gyro information and the magnetic compass information for heading angle estimation is presented with the error covariance analysis. A pressure sensor is used to identify the floor the pedestrian is walking on. A complete ambulatory system is implemented which estimates the pedestrian's 3D position and the heading.

Exposure to Formaldehyde of Ambulatory Care Nurses in University Hospital (대학병원 내 외래간호사의 포름알데히드 노출 평가)

  • Objective: This study aimed to evaluate the exposure status of formaldehyde(FA) among the nurses in ambulatory care departments of university hospital. Methods: Two university hospitals were surveyed. The FA concentration in air were measured to target 62 nurses in 8 ambulatory care departments(89 samples). Air sampling and analysis of FA were carried out according to the OSHA Method ID-205. The survey was conducted with questionnaire asking about how to control FA and whether the nurses use the formaldehyde protectors or not. Results: FA was detected in all samples. The maximum concentration of FA was 0.258 ppm and the geometric mean was 0.023 ppm. There was no sample that exceeded any of exposure standards by OSHA-TWA whereas there were 54 samples(60.7%) that exceeded the standards by NOISH-TWA. Among 62 nurses handling FA, 13 nurses(21.0%) used the protective gloves while nobody used a gas mask. It was assessed that any of 8 common ambulatory care departments did not use a safety cabinet for FA in which local exhaust ventilation was fixed. Conclusions: Nurses in ambulatory care departments were exposed to FA. Therefore, the environment management of a workplace, the health management of a nurses, FA handling education and installing a FA cabinet with a local exhaust ventilation were needed because FA as a carcinogen was able to cause any cancer to a human body if it was emitted in air for long time.

  • The effects of regional health insurance on access to ambulatory care are examined in this paper. Access is measured as use-disability ratios. The data are collected in a household interview survey at Hwachon county before and after the introduction of regional health insurance. Before the introduction of regional health insurance, low-income class has less contacts with physicians than high-income class. This disparity in accessibility among economic classes is reduced with the health insurance coverage, but not removed, even after adjusting for health need.
