치조열에서 재 시행한 골이식의 분석 (Analysis of Repeated Bone Graft after Secondary Bone Graft in Children with Alveolar Cleft)

  • Purpose: The most widely accepted protocol for alveolar cleft reconstruction is to repair it during the mixed dentition stage. There were lower resorption rate (about 88%) at this stage. However we found some cases that need repeated bone grafting. Therefore we sought to analyze the cause of repeated alveolar bone grafting in connection with other factors. Methods: From January 2000 to January 2006, thirty-nine secondary alveolar bone grafts with iliac crest spongiosa were carried out. In 39 patients, 5 patients who had significant bone graft resorption received repeated alveolar bone graft. In all the cases, the causes of repeated bone grafts were dental root exposure(angulation), and the deficiency of the bony support for lateral incisor or canine eruption. In 3 cases, there was deficiency of the alveolar bone at the cleft side. There was the need of repeated bone grafts for orthodontic treatment in 2 cases and for application of dental implants in 1 case. Results: During the follow-up period, the clinical and radiologic examinations showed that repeated alveolar bone grafts were maintained successfully without any complications. The volume of the repeated bone graft was sufficient for orthodontic treatment and implantation. Conclusion: The essential conditions for successful alveolar bone grafting includes the status of cleft sided teeth, further treatment and planed schedule, as well as canine eruption. Alveolar bone grafting has to be performed with difference of each case in mind.

Quantification of Microstructures in Mice Alveolar Bone using Micro-computed tomography (${\mu}CT$)

  • Periodontal inflammation increases the risk of tooth loss, particularly in cases where there is an associated loss of alveolar bone and periodontal ligament (PDL). Histological and morphometric evaluation of periodontal inflammation is difficult. Especially, the lengths of the periodontal ligament and interdental alveolar bone space have not been quantified. A quantitative imaging procedure applicable to an animal model would be an important clinical study. The purpose of this study was to quantify the loss of alveolar bone and periodontal ligament by evaluation with micro-computed tomography (micro-CT). Another purpose was to investigate differences in infections with systemic E. coli LPS and TNF-${\alpha}$ on E. coli lipopolysaccharide (LPS) in loss of alveolar bone and periodontal ligament model on mice. This study showed that linear measurements of alveolar bone loss were represented with an increasing trend of the periodontal ligament length and interdental alveolar process space. The effects of systemic E. coli LPS and TNF-${\alpha}$ on an E. coli LPS-induced periodontitis mice model were investigated in this research. Loss of periodontal ligament and alveolar bone were evaluated by micro-computed tomography (micro-CT) and calculated by the two- and three dimensional microstructure morphometric parameters. Also, there was a significantly increasing trend of the interdental alveolar process space in E. coli LPS and TNF-${\alpha}$ on E. coli LPS compared to PBS. And E. coli LPS and TNF-${\alpha}$ on E. coli LPS had a slightly increasing trend of the periodontal ligament length. The increasing trend of TNF-${\alpha}$ on the LPS-induced mice model in this experiment supports the previous studies on the contribution of periodontal diseases in the pathogenesis of systemic diseases. Also, our findings offer a unique model for the study of the role of LPS-induced TNF-${\alpha}$ in systemic and chronic local inflammatory processes and inflammatory diseases. In this study, we performed rapidly quantification of the periodontal inflammatory processes and periodontal bone loss using micro-computed tomography (micro-CT) in mice.

교정치료와 관련된 치조골 높이 변화에 대한 연구 (A study on the change of alveolar crest height following orthodontic treatment)

  • 교정치료에 의해 빚어지는 치조골 소실의 정도를 좌, 우측 제1대구치의 근, 원심면에서 치료 전, 후의 파노라마 필름을 이용하여 비교평가하고 치조골 소실에 미치는 영향 요소들을 알아보고자 하였다. 고정식 교정장치 치료를 받은 216명의 환자들은 104명의 성장군과 112명의 성인군으로 나뉘어 졌으며 4개 소구치의 발거가 이루어진 경우는 각각 50명씩이었다. 치료 전, 후 파노라마 사진에서 상하 좌우 제1대구치의 근심 및 원심 부위의 치조골 높이를 백악법랑질경계에서 치조골의 최첨점까지 치아 장축에 평행하게 계측하여 다음과 같은 결론을 얻었다. 1. 치료 시작시, 성인군은 성장군보다 유의하게 치조골 수준이 낮았다. 2. 치료 후 치조골 수준은 치료 전 수준보다 유의하게 낮았다. 3. 성장군과 성인군 간에 치조골 변화량의 유의한 차가 없었다. 4. 성인발치군은 비발치군보다 유의하게 더 많은 치조골 소실을 모든 원심면에서 보였다. 5. 상악은 하악보다 통계적으로 유의하게 더 많은 치조골 소실을 근심면에서 보였다. 6. 성인군은 원심면에서 근심면보다 유의하게 더 많은 치조골 소실을 보였다.

하치조신경전달마취의 최신지견 (Updates on the Inferior Alveolar Nerve Block Anesthesia)

  • The inferior alveolar nerve (IAN) block is the most frequently used mandibular injection technique for achieving local anesthesia for restorative and surgical procedures. However, The IAN block does not always result in successful anesthesia, especially pulpal anesthesia. Lidocaine is used as a "standard" local anesthetic for the inferior alveolar nerve. Articaine recently joined Korean market as a form of dental cartridge. It has an advantage of superior diffusion through bony tissue. A variety of trial was performed to improve the success rate of inferior alveolar nerve block. In this review, the recent update related with inferior alveolar nerve block anesthesia will be discussed on the anatomical consideration, anesthetic agent, technique, and complications.

증령에 따른 치고졸의 변화에 관한 방사선학적 연구(I) (Radiographic Study of the Alveolar Bone Changes with Aging)

  • The author measured the degree of alveolar bone resorption around mandibular central incisors in Korean female(254). Mandibulr central incisors were selected from the females who do not have periodontal disease or malocclusion. The selected radiograms were enlarged in the $5"\times7"$ printing papers for the precise measuring. The obtained results were as follows : 1. The average alveolar bone resorption around mandibular central incisors in Korean women were 1.91mm in 3rd decade, 2, 16mm in 4th decade, 2.51mm in 5th deacade, 2.70mm in 6th decade, 2.94mm in older age group. 2. Alveolar bone resorption and age were in positive correlation; there is a tendency that the alveolar bone resorption increase with aging. 3. The regression equation is as follows. 계측용 Grid를 이용한 치조골 흡수상에 관한 연구 (A ROENTGENOGRAPHIC STUDY OF ALVEOLAR BONE RESORPTION USING MEASURABLE GRID)

  • The author had performed the measurement of the various teeth regions on 200 cases of Full-mouth roentgenogram taken with the measurable Grid to detect the degree of alveolar bone resorption, aged from 20 to 60 years of Korean, and to verify the effective result of measurable Grid as a new device which enables the observers to determine the correct length of images on the periapical standard film. The results were obtained as follows. 1. It was found that the degrees of alveolar bone resorption were different in the various teeth regions. 2. As a whole, alvolar bone resorption of anterior teeth regions was more severe than that of posterior teeth regions. 3. Alveolar bone resorption of mandibular region was more severe than that of maxillary region. 4. In sex difference, alveolar bone resorption of male is more severe than that of female, and it was increased with aging. 5. Measurable grid can be used as an adjunct of evaluation of alveolar bone resorption and of calculating the tooth length.

Tooth hypersensitivity associated with paresthesia after inferior alveolar nerve injury: case report and related neurophysiology

  • Inferior alveolar nerve (IAN) injury is usually caused by stretching or crushing of the neurovascular structures and postoperative intra-alveolar hematoma or edema after dental procedures. This results in paresthesia in the ipsilateral chin, lip (vermilion border, skin, and mucosa), and labial or buccal alveolar mucosa of the mandibular anterior teeth. However, there are no reports of sensory alterations in the teeth, especially tooth hypersensitivity, after IAN injury. I report a case in which paresthesia of the lower lip and hypersensitivity of the lower anterior teeth occurred simultaneously after the removal of the third molar that was located close to the IAN. In addition, I discuss the reasons for the different sensory changes between the tooth and chin (skin) after nerve injury from a neurophysiological point of view. Since the dental pulp and periodontal apparatus are highly innervated by the inferior alveolar sensory neurons, it seems necessary to pay attention to the changes in tooth sensitivity if IAN injury occurs during dental procedures.

Inducible Nitric Oxide Synthase mRNA Expression and Nitric Oxide Production in Silica-Induced Acute Inflammatory Lung Injury

  • Stimulated alveolar macrophages and neutrophils produce nitric oxide, a free radical by an inducible nitric oxide synthase(iNOS), which reacts with superoxide anion to form peroxynitrite, a more highly reactive toxic species. The objectives of the present study were to evaluate acute inflammatory lung injury and to determine iNOS mRNA induction and nitric oxide production by rat broncho-alveolar lavage cells following intratracheal treatment of silica. After 4 h exposure to silica, differential counts of broncho-alveolar lavage cells and lactate dehydrogenase(LDH) activity as well as total protein in the broncho-alveolar lavage fluid were determined. Broncho-alveolar lavage cells were also assayed for iNOS mRNA and the productions of nitrite and nitrate measured in the cells cultured. Differential analysis of broncho-alveolar lavage cells showed that the number of alveolar macrophages slightly decreased following silica treatment; however, red blood cells, lymphocytes, and neutrophils significantly were increased by 9-, 14-, and 119-fold following silica treatment, respectively, compared with the saline control. It was also found significant increases in the LDH activity and total protein in the lavage fluid obtained from silica-treated rats, indicating silica-induced acute lung injury. Northern blot analysis demonstrated that the steady state levels of iNOS mRNA in broncho-alveolar lavage cells were increased following silica treatment. The productions of nitrite and nitrate in the cultured cells were significantly increased by 2-fold following silica treatment, respectively, which were attenuated by the NOS inhibitor $N{\omega}-nitro-L-arginine-methyl$ ester(L-NAME) and partially reversed by L-arginine. These findings suggest that nitric oxide production in alveolar macrophages and recruited neutrophils is increased in response to silica. Nitric oxide may contribute in part to acute inflammatory lung injury.

Use of mandibular chin bone for alveolar bone grafting in cleft patients

  • Background: We evaluated and compared the outcomes of different ossification processes in patients with alveolar cleft in whom correction was performed using endochondral bone graft or intramembranous bone graft. Methods: The patients were divided into two groups: the endochondral bone (iliac bone or rib bone) graft group and the intramembranous bone (mandibular bone) graft group. Medical records and radiologic images of patients who underwent alveolar bone grafting due to alveolar cleft were analyzed retrospectively. Through postoperative and follow-up radiologic images, the height of the interdental bone septum was classified into four types based on the highest point of alveolar ridge. Then, the height of the interdental bone septum and the area of the bone graft were evaluated according to the type of bone graft. In addition, the occurrence of complications and the need for an additional bone graft, the result of postoperative orthodontic treatment, and the eruption of impacted teeth were investigated. Results: Thirty patients were included in this study. There was no significant difference in the change of the interdental bone height and the area of the bone graft according to the type of bone. There was no significant difference in the success rate of the surgery according to the type of bone. One patient underwent an additional bone graft surgery during the follow-up period. Conclusions: The outcomes of alveolar bone grafting were not significantly different according to the type of bone graft. If appropriate to the size of the recipient site, the chin bone is a useful graft material in alveolar cleft, as is the iliac bone.

수직두개계측법에 의한 과개교합 및 개교에 관한 연구 (A STUDY 01 OEEP ()VER81TE AND OPEN BITE BY VERTICAL CEPHALOMETRIC ANALYSIS)

  • The author studied and analyzed statistically 112 adults female ranging in age from 18 to 20 years with normal occlusion, 56 adults female ranging in age from 18 to 24 years with deep overbite and 53 adults female ranging in age from 18 to 28 years with open bite by vertical cephalometric analysis. The results were as follows; 1. In comparing normal occlusion with deep overbite and open bite, skeletal linear measurements were more significant than dentoalveolar linear measurements. SN-MP angle, SN-OP angle, PP-OP angle and Xi angle (ANS-Xi-Pog) were significant in anglular measurements. 2. Upper posterior facial height (SE-PNS), upper anterior alveolar height, lower posterior alveolar height, lower posterior alveolar height/lower anterior alveolar height and SN-PP angle were non significant between deep overbite and open bite. 3 The most significant items between deep overbite and open bite were lower anterior facial height (ANS-ME) and SN-MP angle. 4. Correlation coefficients of angular measurements were higher in deep overbite, while that of linear measurement total anterior facial height (N-ME) was higher in open bite. 5. In the multiple regression equation, significant variables were total anterior facial height (N-ME), lower anterior alveolar height, upper anterior alveolar height, upper posterior alveolar height, Xi angle (ANS-Xi-Pog) and ramus height (AR-Go) in deep overbite, and total anterior facial height (N-Me), lower anterior alveolar height, ramus height (AR-Go), lower posterior alveolar height, PP-MP angle and upper posterior facial height (SE-PNS) in open bite.
