상악전치부 결손부에서 골유도재생술식을 동반한 임플란트 수복의 증례보고 (Ridge augmentation and implant placement on maxillary anterior area with deficient alveolar ridge : case report)

  • Atrophic alveolar ridge of maxillary anterior area is commonly observed after the extraction of teeth in patients with severely compromised periodontal disease, causing difficulties with implant placement. Successful esthetics and functional implant rehabilitation rely on sufficient bone volume, adequate bone contours, and ideal implant positioning and angulation. The present case report categorized the ridge augmentation techniques using guided bone regeneration (GBR) on the maxillary anterior site by Seibert classification. Case I patient presented for implant placement in the position of tooth #11. The alveolar ridge was considered a Seibert classification I ridge defect. Simultaneous implant placement and GBR were performed. Eight months after implantation, clinical and radiological examinations were performed. Case III patient presented with discomfort due to mobility of the upper maxillary anterior site. Due to severe destruction of alveolar bone, teeth #11 and #12 were extracted. After three months, the alveolar ridge was considered a Seibert classification III ridge defect. A GBR procedure was performed; implantation was performed 6 months later. Approximately 1-year after implantation, clinical and radiological examinations were performed. During the whole treatment period, healing was uneventful without membrane exposure, severe swelling, or infection in all cases. Radiographic and clinical examinations revealed that atrophic hard tissues and buccal bone contour were restored to the acceptable levels for implant placement and esthetic restoration. In conclusion, severely resorbed alveolar ridge of the maxillary anterior area can be reconstructed with ridge augmentation using the GBR procedure so that dental implants could be successfully placed.

Influence of the anterior arch shape and root position on root angulation in the maxillary esthetic area

  • Purpose: This study was conducted to characterize the relationship of the angulation between the tooth root axis and alveolar bone axis with anterior alveolar(AA) arch forms and sagittal root position (SRP) in the anterior esthetic region using cone-beam computed tomography (CBCT) images. Materials and Methods: CBCT images that met the inclusion and exclusion criteria were categorized using a recent classification of AA arch forms and a SRP classification. Then, the angulation of the root axis and the alveolar bone axis was measured using mid-sagittal CBCT images of each tooth. The relationships of the angulation with each AA arch form and SRP classification were evaluated using 1-way analysis of variance and a linear regression model. Results: Ninety-eight CBCT images were included in this study. SRP had a greater influence than the AA arch form on the angulation of the root axis and the alveolar bone axis(P<0.05). However, the combination of AA arch form and SRP was more predictive of the angulation of the root axis and the alveolar bone axis than either parameter individually. Conclusion: The angulation of the root axis and alveolar bone axis demonstrated a relationship with the AA arch form and SRP in teeth in the anterior esthetic region. The influence of SRP was greater, but the combination of both parameters was more predictive of root-to-bone angulation than either parameter individually, implying that clinicians should account for both the AA arch form and SRP when planning implant placement procedures in this region.

Comparison of anterior maxillary and mandibular alveolar parameters in African American and Caucasian women: A retrospective pilot study

  • Purpose: The primary objective of this study was to analyze the thickness and height of alveolar bone around the maxillary and mandibular incisors. Additionally, this study aimed to compare bone parameters between Caucasian (CC) and African American (AA) female patients. Materials and Methods: In this retrospective pilot study, 50 female subjects(25 CC and 25 AA) were included. The inclusion criteria were AA or CC women between the ages of 18 and 50 with a normo-divergent facial pattern and Angle's class I, end-on class II, or mild class III malocclusion. The distance from the cementoenamel junction (CEJ) to the buccal and lingual alveolar crest; the alveolar ridge thickness at the mid-root and apex; and the buccal and lingual bone thickness at 3, 6, and 9mm from the CEJ were measured. Results: No significant difference was found (P>0.05) in the cortical bone thickness at 3mm, 6mm, or 9mm from the alveolar crest between CC and AA populations for most measurements. A significant difference in bone thickness was found (P<0.05) for the lingual surface of the central incisor, with maxillary bone thickness found to be higher than mandibular bone thickness. The measurements of lingual thickness were larger than those of buccal thickness for both races. Conclusion: There were no differences in maxillomandibular anterior alveolar bone measurements between normo-divergent adult AA and CC women, except for a few parameters at varying locations. However, future studies can be planned based the current pilot study data, which may provide valuable information.

Cone-beam computed tomographic comparison of chin symphysis bone particles and allograft versus iliac crest bone graft alone for reconstruction of alveolar bone defects in cleft patients

  • Objectives: The aim of this study was to compare morbidities and duration of surgery, as well as bone formation in alveolar defects reconstructed with symphysis bone combined with allograft and iliac crest bone graft in patients with cleft palate. Patients and Methods: This randomized clinical trial was performed with 22 patients with unilateral alveolar cleft with a follow-up period of 12 months. In 12 patients, alveolar defects were reconstructed with chin bone graft plus allograft (Group A), while for the other 10 patients, iliac bone crest was used as donor site (Group B). Duration of surgery as well as occurrence of morbidities and complications were recorded. In addition, cone-beam computed tomographic (CBCT) scans were performed before surgery and 12 months after surgical procedures in order to compare bone formation between the two groups. Results: Postoperative CBCT demonstrated a mean bone fill percentage of 76.9% of the alveolar defect in Group A, compared with 77.0% in Group B. Paresthesia in the lower lip or chin did not occur in any patients of Group A. The mean duration of the surgical process was significantly shorter for Group A (40 minutes vs 76 minutes, P<0.001). In addition, patients in Group A regained normal gait faster than patients in Group B (1 day vs 9.5 days). Conclusion: Mandibular symphysis bone graft in combination with allograft results in favorable outcomes in patients with unilateral alveolar clefts.

Anatomic evaluation of the posterior superior alveolar artery using cone-beam computed tomography: A systematic review and meta-analysis

  • Purpose: This systematic review examined the detection of the posterior superior alveolar artery, along with various anatomic characteristics, on cone-beam computed tomography images. Materials and Methods: Studies were identified electronically through the Web of Science, MEDLINE, Scopus, and Embase databases. The quality of the included studies was evaluated using a 5-item binary scale. The detection rate, location, and classified diameter of the posterior superior alveolar artery were estimated as prevalence values. The diameter of this artery, as well as the distances from the artery to the alveolar crest and sinus floor, were estimated as means with associated 95% confidence intervals. Results: Thirty-seven studies were enrolled, with 34 of these included in the meta-analysis. The mean detection rate was 79% (range: 72%-84%), and the mean diameter was 1.06±0.05 mm (range: 0.96-1.16 mm). The posterior superior alveolar artery was located intraosseously in 64% of cases. The mean distance of the artery from the alveolar crest was 16.71±0.49 mm (range: 15.75-17.68 mm), while the mean distance from the artery to the sinus floor was 8.85±0.4 mm (range: 8.05-9.64 mm). Conclusion: According to the findings of this meta-analysis regarding various anatomic characteristics of the posterior superior alveolar artery, severe hemorrhage after damage to this artery during sinus augmentation procedures is not a substantial clinical problem.

Assessment of alveolar bone changes in response to minimally invasive periodontal surgery: A cone-beam computed tomographic evaluation

  • Purpose: The aim of this study was to evaluate 3-dimensional cone-beam computed tomography (CBCT) images of alveolar bone changes in patients who underwent minimally invasive periodontal surgery-namely, the pinhole surgical technique (PST). Materials and Methods: Alveolar bone height was measured and compared on CBCT images of 254 teeth from 23 consecutive patients with Miller class I, II, or III recession who had undergone PST. No patient with active periodontal disease was selected for surgery. Two different methods were used to assess the alveolar bone changes postoperatively. In both methods, the distance between the apex of the tooth and the mid-buccal alveolar crestal bone on pre- and post-surgical CBCT studies was measured. Results: An average alveolar bone gain >0.5 mm following PST was identified using CBCT(P=0.05). None of the demographic variables, including sex, age, and time since surgery, had any significant effect on bone gain during follow-up, which ranged from 8 months to 3 years. Conclusion: PST appears to be a promising treatment modality for recession that results in stable clinical outcomes and may lead to some level of resolution on the bone level. More long-term studies must be done to evaluate the impact of this novel technique on bone remodeling and to assess sustained bone levels within a larger study population.

Implant placement with inferior alveolar nerve repositioning in the posterior mandible

  • This case report presents inferior alveolar nerve (IAN) repositioning as a viable approach for implant placement in the mandibular molar region, where challenges of severe alveolar bone width and height deficiencies can exist. Two patients requiring implant placement in the right mandibular molar region underwent nerve transposition and lateralization. In both cases, inadequate alveolar bone height above the IAN precluded the use of short implants. The first patient exhibited an overall low alveolar ridge from the anterior to posterior regions, with a complex relationship with adjacent implant bone level and the mental nerve, complicating vertical augmentation. In the second case, although vertical bone resorption was not severe, the high positioning of the IAN within the alveolar bone due to orthognathic surgery raised concerns regarding adequate height of the implant prosthesis. Therefore, instead of onlay bone grafting, nerve transposition and lateralization were employed for implant placement. In both cases, the follow-up results demonstrated successful osseointegration of all implants and complete recovery of postoperative numbness in the lower lip and mentum area. IAN repositioning is a valuable surgical technique that allows implant placement in severely compromised posterior mandibular regions, promoting patient comfort and successful implant placement without permanent IAN damage.

스트렙토조토신 유발 당뇨병 백서에서 실험적 치아이동중의 치조골 교체 (Alveolar bone turnover during experimental tooth movement in Streptozotocin-induced diabetic rat)

  • 당뇨병 흰쥐를 대상으로 치조골의 교체(turnover)와 치아 이동 중 치조골의 흡수와 형성의 활성을 비교 관찰하기 위하여 8주령의 웅성 Sprague-Dawley계 흰쥐를 정상대조군(N), 당뇨군(D), 정상-치아이동군(N-tm), 당뇨-치아이동군(D-tm)으로 나누고, 각 실험군은 치아이동 실험 1일, 3일, 7일, 14일의 소군으로 나누었다. 당뇨병은 치아이동 실험 18일전에 스트렙토조토신(50mg/체중 kg)을 꼬리정맥에 단 회 주사하여 유발하였다. 치아이동은 상악 제1대구치를 초기 교정력 40그램으로 근심 이동시켰다. 상악 제1대구치 주위 치조골을 대상으로 골형성 징후표지로서 치조골 alkaline phosphatase(ALP)와 osteocalcin(OC)을, 골흡수 징후표지로서 acid phosphatase (ACP)와 tartrate-resistant acid phophatase(TRAP)을 정량 분석하였으며, D군과 N군에서는 이들의 혈중 농도를 정량 분석하였다. 1. D군의 혈청 TRAP농도와 혈청 OC농도는 모든 실험 경과군에서 N군보다 감소하였고(p<0.05), 당뇨기간이 길어질수록 더 많이 감소한 것으로 나타났다. 또한 치조골 흡수 징후표지는 감소하는 경향이었으며 골형성 징후표지도 실험기간이 경과함에 따라 더 많이 감소하는 것으로 나타났다. 2. N-tm군의 치조골 ACP농토와 TRAP농도는 N군과 비교하여 1일과 3일군에서 가장 높았으며(p<0.01), 그 이후 감소경향을 나타냈고, 치조골 OC함량은 3일, 7일 14일군에서 N 군보다 높았다(p<0.001). 그러나 D-tm군에서 치조골 ACP와 TRAP농도는 정상 N군에 비하여 1일에는 낮으나 7일, 14일 경과군에서 가장 높았고(p<0.001), 실험 경과에 따라 계속 증가 하였으며(p<0.001), 치조골 ALP와 OC 함량은 3일 경과 후부터 증가하지만 정상 N군과 유의차가 없었다. 3. 당뇨-치아이동군의 치아이동량은 정상-치아이동군보다 많은 것으로 나타났다. 이러한 실험 결과는 당뇨병의 전신 골격과 치조골은 골 흡수 활성이 억제되어 있으며 골형성 활성은 당뇨 지속시간이 길어질수록 더 많이 억제되는 느린 골교체를 하며, 교정력에 의한 치조골 흡수활성이 처음 높아지는 시기는 정상치아이동시보다 늦고 더 오랬 동안 지속되며 치조골 형성활성은 낮고, 치아 이동량은 증가하며 이동중 치아동요도가 증가함을 시사한다.

백서에서 실험적 치아이동시 임신이 치조골 교체(Turnover)에 미치는 영향에 대한 연구 (The effects of pregnancy on alveolar bone turnover during experimental tooth movement in rats)

  • 임신중 전신의 호르몬 변화는 골대사과정에 영향을 주고 임신 중에는 골교체가 증가하며, 교정력에 의한 치아이동에는 치조골 교체가 수반된다고 알려져 있다. 이 연구는 임신으로 인한 전신의 호르몬 변화가 치아이동 중 치조골 교체에 주는 영향을 알아보기 위하여 시행되었다. 생후 10주령의 암컷 백서 60마리를 대상으로 정상군과 정상-치아이동군, 임신-치아이동군의 3군으로 나누고 치아이동일수에 따라 1,3,7,14일군으로 세분한 후 상악 제1대구치 에 40g의 교정력을 가하여 근심측으로 치아이동을 시행하였다. 치아이동 전후에 촬영한 X-ray사진을 계측하여 치아이동 거리를 비교하고 치조골에서의 alkaline phosphatase, tartrate-resistant acid phosphatase 농도를 측정하여 치아이동에 따른 골형성과 흡수 활성을 비교하여 다음과 같은 결과를 얻었다. 1. 치아이동량은 임신-치아이동군이 정상-치아이동군에 비하여 더 크게 나타났다(p<0.01). 2. 정상-치아이동군의 치아이동에 따른 골형성은 대조군과 차이가 없었지만 골흡수 경향은 증가되었고(p<0.01), 정상-치아이동군에서는 대조군에 비하여 치조골의 형성과 흡수 활성이 모두 치아이동 7일에 가장 크게 증가하였고 (p<0.01) 이후 골형성 활성은 감소하였지만(p<0.05) 골흡수 활성은 대조군보다 높게 유지되었다(p<0.01). 3. 임신군과 정상군의 치아이동에 따른 골형성과 흡수 경향에는 차이가 없었지만, 임신-치아이동군의 치조골 형성 (p<0.01)과 흡수(p<0.05) 활성의 증가는 3일경에 가장 높게 나타났으며, 이후 골형성 활성은 감소하였으나 골흡수 활성은 높게 유지되었고, 정상-치아이동군은 7일경에 치조골 형성과 흡수 활성의 증가가 나타났다. 임신중의 치아이동량이 정상 치아이동에 비하여 크게 나타났으며, 치조골 흡수와 형성 활성의 증가가 빠르게 나타났고 치조골 흡수가 더 오랬동안 지속되어 빠른 치조골 교체를 보이므로 임신상태에서 치 아이동은 정상상태보다 촉진될 가능성이 있음을 시사한다.

결핵균이 폐포대식세포의 기능에 미치는 영향에 관한 연구 -H37Ra 결핵균종에 의한 사람 몇 백서 폐포대식세포의 Superoxide 생성의 변화- (The Effects of Mycobacterium Tuberculosis on Alveolar Macrophages -The Alterations of Superoxide Production in both Human and Rat Alveolar Macrophages Exposed to Mycobacterium Tuberculosis H37Ra Strain-)

  • 연구배경 : 산소반응성 대사물에 의한 결핵균 살해능은 대식세포의 활성화 상태에 의해 좌우되는데 체외에서의 이러한 효과를 priming 이라 한다. priming 효과를 유도하는 물질에는 IFN-$\gamma$와 LPS가 대표적인 물질인데 이들 자체가 superoxide와 같은 산소반응성 물질의 생성을 증가시키지는 않지만 식균작용이나 PMA와 같은 화학적 물질에 반응하여 증강된 산소반응성 물질의 생성을 유도한다. 혈액단구세포와는 달리 폐포대식세포는 일상적인 외부환경에 노출되어 있기 때운에 priming 효과에 대한 논란이 있고 결핵균 세포벽의 각 성분이 대식세포의 활성화에 상반되는 결과를 보인다는 보고들이 있어 본 연구에서는 사람의 폐포대식세포와 혈액단구세포, 그리고 백서의 폐포대식세포에서 IFN-$\gamma$에 의한 priming 효과를 비교 관찰하였고 결핵균증 H37Ra 균증이 폐포대식세포에 노출되었을 때 나타나는 superoxide의 생성능의 변화를 비교하여 보았다. 방법 : 사람과 백서에서 얻은 기관지폐포세척액을 Petri dish에 부착시키고 cold shock 방법으로 부착된 대식세포를 분리하여 24시간 IFN-$\gamma$로 전처치한 후의 priming 효과와 H37Ra 결핵균종을 노출시켰을 시의 superoxide의 생성능을 ferricytochrome reduction 방법으로 측정하여 비교하였다. 결과 : 1) 사람 폐포대식세포에서는 고농도의 IFN-$\gamma$ 전처치로도 priming 효과가 관찰되지 않은 반면 혈액단구세포와 백서 폐포대식세포에서는 priming 효과를 관찰할 수 있었다. 2) 폐포대식세포를 비독력 결핵균종인 H37Ra 생균에 노출시킨 결과 사람과 백서 공히 triggering 효과를 나타내었고 그 분해물에 의한 노출 역시 유사한 결과를 나타내었다. 결론 : 사람의 폐포대식세포는 다른 대식세포와는 달리 일상적인 외부환경에 노출되어 있으으로 priming 효과가 관찰되지 않았으며 비독력 결핵균종인 H37Ra에 의해서는 폐포대식세포의 활성화를 억제하는 효과를 관찰할 수 없었다.
