The Current Status and Acceptance of Traditional Medicine of East Asia in the UK

  • Objectives : Traditional medicine(TM) of East Asia has been taking its status as part of complementary and alternative medicine(CAM) in the UK. However, the efficacy and safety issues make it hard to be accepted in the healthcare system. The aim of the research is to find out the current status of TM of East Asia in the UK and to discuss some issues around its acceptance as formal healthcare method. Methods : Articles, books and regulations related to the acceptance of TM of East Asia were analysed and the internet websites were visited such as Westlaw UK for legal materials, government websites for formal documents, and some UK-based associations. Keyword searches were followed and the essential parts from the articles and documents were generalised for the analysis and discussion. Results : Issues over TM of East Asia include identity as medicine, efficacy and safety, and the statutory regulation can be a measure for the acceptance. Osteopathy and chiropractic therapies came under statutory regulation among the CAM in the UK. Conclusions : TM of East Asia could be under the statutory regulation in the near future. Efficacy and safety issues are the challenging barrier. However, the approach from the viewpoint of TM of East Asia is necessary for development and good practice. The simplified registration procedure for traditional herbal medicinal product in EU can be the model. Education, evaluation, assessment and managing quality of practice are essential for the standard care and formal regulation.

Effect of Balance Taping Therapy on Neck Pain in High School Students (경부통증을 호소하는 고등학생에게 적용한 밸런스 테이핑 요법의 효과)

  • Purpose: The purpose of this study was to examine the effect of balance taping therapy on neck pain in high school students. Methods: The study employed a randomized control group pretest-posttest design with four-time repeated measures. Data were collected from 62 high school students with neck pain. The experimental group (n=31) took balance taping therapy for six days with appropriate position and stretching education while the control group (n=31) applied patches including non-steroidal anti-inflammatory drugs (NSAIDS) for six days with appropriate position and stretching education. Neck pain, cervical range of motion (CROM) and neck disability were measured at pretest, day one, day three and day six which was the posttest day. Results: For the experimental group, the neck pain was significantly improved on all three days (F=16.82, p<.001), and extension and right lateral flexion of the CROMs were significantly improved over time compared to the control group (F=3.85, p =.011; F=2.71, p=.047, respectively). Neck disability was also improved in the experimental group compared to the control group (F=8.64, p<.001). Conclusion: The balance taping therapy was an efficient intervention for high school students with neck pain. Nurses could apply non-pharmacological interventions such as balance taping therapy without pharmacological side effects.

A Clinical Observation of Oriental Medicine-Based Long Care for Terminal Rectal Cancer Patient with Multiple Metastasis

  • Cancer is now the leading fatal disease in every developed country. Due to very low efficiency of conventional cancer therapeutics and important aspect of quality of life (QOL), complementary/alternative medicine (CAM) has been accepted worldwide, especially by late-staged cancer patients. Oriental medicine traditionally has stressed not only the tumor itself but the body having cancer, improving cancer-related immunity and extending survival period. Herein, we report one case of a terminal staged rectal cancer patient having spread-metastasis but showing a meaningful clinical course of around four years with Oriental medicine therapies. This study aimed to report the clinical capacity of Oriental medicine for patients with cancerous disease, to help development of Oriental medicine-derived anticancer treatments.

Immunopreventive Effects against Murine H22 Hepatocellular Carcinoma in vivo by a DNA Vaccine Targeting a Gastrin-Releasing Peptide

  • There is a continuing need for innovative alternative therapies for liver cancer. DNA vaccines for hormone/growth factor immune deprivation represent a feasible and attractive approach for cancer treatment. We reported a preventive effect of a DNA vaccine based on six copies of the B cell epitope GRP18-27 with optimized adjuvants against H22 hepatocarcinoma. Vaccination with pCR3.1-VS-HSP65-TP-GRP6-M2 (vaccine) elicited much higher level of anti-GRP antibodies and proved efficacious in preventing growth of transplanted hepatocarcinoma cells. The tumor size and weight were significantly lower (p<0.05) in the vaccine subgroup than in the control pCR3.1-VS-TP-HSP65-TP-GRP6, pCR3.1-VS-TP-HSP65-TP-M2 or saline subgroups. In addition, significant reduction of tumor-induced angiogenesis associated with intradermal tumors of H22 cells was observed. These potent effects may open ways towards the development of new immunotherapeutic approaches in the treatment of liver cancer.

Exploiting tumor cell senescence in anticancer therapy

  • Cellular senescence is a physiological process of irreversible cell-cycle arrest that contributes to various physiological and pathological processes of aging. Whereas replicative senescence is associated with telomere attrition after repeated cell division, stress-induced premature senescence occurs in response to aberrant oncogenic signaling, oxidative stress, and DNA damage which is independent of telomere dysfunction. Recent evidence indicates that cellular senescence provides a barrier to tumorigenesis and is a determinant of the outcome of cancer treatment. However, the senescence-associated secretory phenotype, which contributes to multiple facets of senescent cancer cells, may influence both cancer-inhibitory and cancer-promoting mechanisms of neighboring cells. Conventional treatments, such as chemo- and radiotherapies, preferentially induce premature senescence instead of apoptosis in the appropriate cellular context. In addition, treatment-induced premature senescence could compensate for resistance to apoptosis via alternative signaling pathways. Therefore, we believe that an intensive effort to understand cancer cell senescence could facilitate the development of novel therapeutic strategies for improving the efficacy of anticancer therapies. This review summarizes the current understanding of molecular mechanisms, functions, and clinical applications of cellular senescence for anticancer therapy.

Advising Patients Who Seek Complementary and Alternative Medical Therapies for Cancer (보완대체의학의 암치료법을 찾는 환자들에 대한 조언)

  • 많은 암 환자들이 보완대체의학(CAM)의 치료법을 이용한다. 의사들은 보완대체의학 치료법에 대한 권위 있는 정보들을 이를 찾는 환자들에게 책임 있는 조언을 줄 수 있기 위해 필요로 한다. 이 글은 암 환자에게 일반적으로 사용되는 선택된 보완대체의학 치료법의 효과와 안정성에 대한 최근의 증거를 요약했다. 식이요법과 보조식품, 한약 및 기타 생약제재, 침, 마사지, 운동, 심리 및 심신의학 등의 주요 카테고리가 포함되었다. 효과의 증거에 대한 '가능한 질병진행과 생존율에 대한 효과' 및 '가능한 완화효과'의 두 가지 범주가 고려되었다. 안전성의 증거를 평가하는데 있어 '직적접인 부작용의 위험'과 '통상적 치료법과의 상호작용에 의한 위험'의 두 가지 위험 요소가 고려되었다. 각각의 치료에 있어 효용성과 안정성에 대한 현재의 균형은 이 치료법이 적절히 권고가 되고 수용되어지거나(예를 들어 유방암과 전립선암을 가진 영양 섭취가 충분한 환자의 식이지방 감소) 억제되는 것을(예를 들어 과량의 비타민 A 보조제) 제시한다. 이러한 전략은 보완대체의학의 치료법을 찾는 암환자들에게 책임 있고 근거중심적이고 환자중심적인 조언을 제공하는 것을 발전시킬 수 있다.

Successful Treatment of a Symptomatic Discal Cyst by Percutaneous C-arm Guided Aspiration

  • Although discal cysts are a rare cause of low back pain and radiculopathy. Currently, surgical excision is usually the first-line treatment for discal cysts. However, alternative treatment methods have been suggested, as in some cases symptoms have improved with interventional therapies. A 27-year-old man presented with an acute onset of severe pain, and was found to have a discal cyst after an open discectomy. The patient underwent cyst aspiration and steroid injection through the facet joint under C-arm guidance. After the procedure, the patient's pain improved to NRS 0-1. On outpatient physical examination 1 week, and 1 and 3 months later, no abnormal neurological symptoms were present, and pain did not persist; thus, follow-up observation was terminated. When a discal cyst is diagnosed, it is more appropriate to consider interventional management instead of surgery as a first-line treatment, while planning for surgical resection if the symptoms do not improve or accompanying neurologic deficits progress.

Analysis of Nursing Needs of Home-dwelling Breast Cancer Patients Based on Counselling Contents (상담일지에 근거한 재가 유방암 환자의 간호요구도 내용분석)

  • Purpose: This study was performed to identify the nursing needs of home-dwelling breast cancer patients based on counseling contents. Methods: Descriptive research was conducted with content analysis. This study included 185 patients who underwent treatment for breast cancer in a tertiary hospital. The data were collected using personal counseling via telephone or face-to-face between March 2011 and July 2013. A total of 536 counseling contents were used in the analysis. Inductive content analysis was used to analyze the contents related to nursing counselling. Results: According to the results. the most frequently reported nursing needs was symptom management, followed by information needs and supportive intervention needs; symptom management needs included symptoms related to chemotherapy, daily living, surgery, and medication side effects; information needs included treatments, medication, clinical tests, and alternative therapies; and supportive intervention needs were related to emotional and social aspects. Conclusion: Our findings suggest that nursing interventions for home-dwelling breast cancer patients should be designed and provided with a consideration to these relevant nursing needs.

Therapeutic potential of traditional Korean music, Daegeum Sanjo in atopic dermatitis-like murine model

  • Atopic dermatitis (AD) is an allergic and inflammatory skin. Recently, the limitations and side effects of drug therapy, and possibility of alternative therapies, such as music therapy are emerging in the treatment of AD. Thus, the present study determined whether traditional Korean music, Daegeum Sanjo, regulates AD symptoms by comparing the rhythm, Jinyangjo-jangdan and Jungmori-jangdan in an AD-like murine model. Jinyangjo-jangdan and Jungmori-jangdan of Daegeum Sanjo reduced the duration of scratching behavior increased by DNFB challenge. Jinyangjo-jangdan and Jungmori-jangdan of Daegeum Sanjo attenuated clinical symptoms. However, Jinyangjo-jangdan and Jungmori-jangdan of Daegeum Sanjo did not inhibit IgE, histamine, interleukin (IL)-4, IL-6, or thymic stromal lymphopoietin levels in serum or AD-like skin lesions. In conclusion, the present study suggests that it is possible for Jinyangjo-jangdan and Jungmori-jangdan of Daegeum Sanjo to ameliorate AD symptoms. However, further study is needed to clarify significant mechanisms of Jinyangjo-jangdan and Jungmori-jangdan of Daegeum Sanjo therapy for AD symptoms.

Bioactive Compounds for the Treatment of Renal Disease

  • Kidney diseases including acute kidney injury and chronic kidney disease are among the largest health issues worldwide. Dialysis and kidney transplantation can replace a significant portion of renal function, however these treatments still have limitations. To overcome these shortcomings, a variety of innovative efforts have been introduced, including cell-based therapies. During the past decades, advances have been made in the stem cell and developmental biology, and tissue engineering. As part of such efforts, studies on renal cell therapy and artificial kidney developments have been conducted, and multiple therapeutic interventions have shown promise in the pre-clinical and clinical settings. More recently, therapeutic cell-secreting secretomes have emerged as a potential alternative to cell-based approaches. This approach involves the use of renotropic factors, such as growth factors and cytokines, that are produced by cells and these factors have shown effectiveness in facilitating kidney function recovery. This review focuses on the renotropic functions of bioactive compounds that provide protective and regenerative effects for kidney tissue repair, based on the available data in the literature.
