Clarithromycin Resistance Prevalence and Icea Gene Status in Helicobacter Pylori Clinical Isolates in Turkish Patients with Duodenal Ulcer and Functional Dyspepsia

  • Clarithromycin resistance in Helicobacter pylori is a principal cause of failure of eradication therapies, and its prevalence varies geographically. The IceA gene is a virulence factor associated with clinical outcomes. The objective of this study was to determine the current state of clarithromycin resistance prevalence, and to investigate the role of iceA genotypes in 87 Turkish adult patients (65 with functional dyspepsia and 22 with duodenal ulcer). A2143G and A2144G point mutations were tested by PCR-RFLP for clarithromycin resistance. Among the patients in the study, 28 patients were tested by agar dilution as well. Allelic variants of the iceA gene were identified by PCR. A total of 24 (27.6%) strains evidenced one of the mutations, either A2143G or A2144G. IceA1 was found to be positive in 28 of the strains (32.2 %), iceA2 was positive in 12 (13.8 %) and, both iceA1 and iceA2 were positive in 22 (25.3 %) strains. In conclusion, we discovered no relationships between iceA genotypes and functional dyspepsia or duodenal ulcer, nor between clarithromycin resistance and iceA genotypes. clarithromycin resistance appears to be more prevalent in Turkish patients.

  • 일련의 새로운 N-[1-(benzotriazol-1-yl)-X-치환-aryl]-Y-치환-arylamine 유도체를 합성하고 Pyricularia oryzae, Fusarium oxysporum f. sp. sesami, Valsa ceratosperma 및 Botrytis cinerea에 대한 항균활성$(pI_{50})$을 한천 희석법으로 측정하여 정량적인 구조-활성 관계$(QSAR_S)$를 분석한 바, X 및 Y-치환기들의 ${\pi}$${\sigma}$$M_R$ 파라미터가 항균 활성을 결정하는 중요한 요소이었다. 4-bromo 치환체(1d 및 2b)가 항균활성이 제일 큰 화합물이었으며, 중성 pH에서 1의 가수분해 반응에 대한 반감기$(T_{1/2})$는 약 1.5일 이었다. 기질 화합물의 분자궤도(MO) 함수와 항균 반응에 대한 자유에너지 관계$(LFER_S)$ 그리고 분자 설계의 결과들이 검토되었다.

In vitro and In vivo Anti-Helicobacter pylori Activities of Centella asiatica Leaf Extract

  • Helicobacter pylori infection is associated with an increased risk of developing upper gastrointestinal tract diseases. However, treatment failure is a major cause of concern mainly due to possible recurrence of infection, the side effects, and resistance to antibiotics. The aim of this study was to investigate the activities of Centella asiatica leaf extract (CAE) against H. pylori both in vitro and in vivo. The minimum inhibitory concentrations (MICs) against 55 clinically isolated strains of H. pylori were tested using an agar dilution method. The MICs of CAE ranged from 0.125 mg/mL to 8 mg/mL, effectiveness in inhibiting H. pylori growth was 2 mg/mL. The anti-H. pylori effects of CAE in vivo were also examined in H. pylori-infected C57BL/6 mice. CAE was orally administrated once daily for 3 weeks at doses of 50 mg/kg and 250 mg/kg. CAE at the 50 mg/kg dose significantly reduced H. pylori colonization in mice gastric mucosa. Our study provides novel insights into the therapeutic effects of CAE against H. pylori infection, and it suggests that CAE may be useful as an alternative therapy.

Leptospermum Scoparium 수증기 추출물인 마누카 기름의 항균효과 (Antimicrobial Activities of a Steam Distillate of Leptospermum Scoparium)

  • The antimicrobial activity of Manuka oil, a steam distillate from Leptospermum scoparium, was investigated, and it's MIC against ten kinds of microorganisms was determin ed. MICs against bacteria and fungi were measured by means of both two-fold dliution method and agar plate two-fold dilution method, respectively. MICs of Manuka oil against Staphylococcus aureus KCTC 1916 and Micrococcus luteus KCTC 1915, gram-positive microrganisms, were identical as 3.05 ${\mu}$g/ml, while it's antibacterial activity against gram-negative microrganisms such as Pseudomonas aeruginosa KCTC 2513, Escherichia coli KCFC 1682, Klebsiella pneunioniae KCTC 2001 or Proteus vulgaris KCTC 2433 was negligible(MIC: ${\geq}$ 1000 ${\mu}$g/ml), suggesting a high susceptibility of gram-positive bacteria to Manuka oil. In addition, MIC against Aspergillus niger KCTC 6077 was 24 ${\mu}$g/ml. and that against the other fungi, Tricophyton mentagrophytes KCTC 1374 and Candida albicans KCTC 1940 was ${\geq}$ 1000 ${\mu}$g/ml. When Manuka oil ointment was used in combination with other drugs. i.e.. gentamycin sulfate, chlotrimazol and hydrocortisone acetate, and diphenhydramine HCl and hydrocortisone acetate. it's antibacterial activity against Staphylococcus aureus KCTC 1916 was higher than Manuka oil ointment or other drugs alone. In conclusion, Manuka oil possesses a selective antibacterial activity against Staphylococcus aureus KCTC 1916, and can be used as a potent antibacterial agent against it.

삼나무 흑심재 추출물의 피부사상균에 대한 항진균 활성 (Antifungal Activity of Extracts from Black Heartwood of Cryptomeria japonica against Dermatophytes)

  • 본 연구는 삼나무 부위별 추출물들의 피부사상균 Trichophyton mentagrophytes, Trichophyton rubrum, Microsporum gypseum에 대한 항진균 활성을 평가하였다. 항진균 활성의 평가는 디스크확산법과 한천희석법을 사용하였고 활성 성분의 주성분을 확인하기 위해 GC/MS 분석을 실시하였다. 항진균 활성은 삼나무의 정상재와 흑심재 중 흑심재의 심재가 가장 높은 활성을 보였다. 활성 성분은 n-hexane과 chloroform를 전개용매로 이용하여 TLC를 실시하였다. TLC를 통해 분리된 활성 성분들에 대해 다시 항진균 활성을 측정한 결과 7번째 분획물이 가장 높은 활성을 나타내었다. 분획물에 대하여 GC/MS 분석을 실시한 결과 탄소개수 13개를 가지는 damascone에 탄소 2개가 더해진 sesquiterpene으로 예상되었다. 이밖에 elemol, eudesmol, hinokione 등도 검출되었다.

Contamination of Chicken Meat with Salmonella enterica Serovar Haardt with Nalidixic Acid Resistance and Reduced Fluoroquinolone Susceptibility

  • Salmonella contamination in chicken meat was studied with 100 chicken meat samples purchased from 55 shops located in various regions. A total of 21 isolates of Salmonella enterica were isolated from 21 chicken meat samples from four shops located at open markets, whereas there were none from supermarkets with well-equipped cold systems. Among these, 18 isolates were identified as Salmonella enterica serotype Haardt (S. Haardt) and three isolates were S. enterica serotype Muenchen. When the minimal inhibitory concentrations of the S. Haardt isolates were assayed with the agar dilution method to determine susceptibility to ampicillin, chloramphenicol, sulfisoxazole, tetracycline, and nalidixic acid, all 18 isolates were resistant to tetracycline and nalidixic acid and nine of these were resistant to ampicillin. These isolates showed reduced susceptibility to eight fluoroquinolones including ciprofloxacin, enrofloxacin, levofloxacin, gatifloxacin, gemifloxacin, moxifloxacin, norfloxacin, and ofloxacin. When quinolone resistance determining regions of gyrA and gyrB were sequenced, every isolate had the same missense mutation Ser83$\rightarrow$Tyr (TCC$\rightarrow$+TAC) in gyrA, whereas no mutation was found in gyrB. Pulsed-field gel electrophoresis with XbaI revealed a close relationship among these isolates, suggesting a contamination of raw chicken meat with clonal spread of nalidixic acid-resistant and quinolone-reduced susceptibility S. Haardt in chickens. Results in this study show the importance of a well-equipped cold system and the prudent use of fluoroquinolone in chickens to prevent the occurrence of quinolone-resistant isolates.

Antimicrobial and Anti-Biofilm Activities of the Methanol Extracts of Medicinal Plants against Dental Pathogens Streptococcus mutans and Candida albicans

  • Several medicinal plants are ethnomedically used in Korea as agents for treating infection, anti-inflammation, and pain relief. However, beyond typical inhibitory effects on cell growth, little is known about the potential anti-biofilm activity of these herbs, which may help to prevent cavities and maintain good oral health. This study aimed to investigate the antimicrobial and anti-biofilm activities of the methanol extracts of 37 Korean medicinal plants against dental pathogens Streptococcus mutans and Candida albicans, which synergize their virulence so as to induce the formation of plaque biofilms in the oral cavity. The antimicrobial activities were investigated by broth dilution and disk diffusion assay. The anti-biofilm and antioxidant activities were evaluated based on the inhibitory effect against glucosyltransferase (GTase) and the DPPH assay, respectively. Among 37 herbs, eight plant extracts presented growth and biofilm inhibitory activities against both etiologic bacteria. Among them, the methanol extracts (1.0 mg/ml) from Camellia japonica and Thuja orientalis significantly inhibited the growth of both bacteria by over 76% and over 83% in liquid media, respectively. Minimum inhibitory concentration (MIC) values of these methanol extracts were determined to be 0.5 mg/ml using a disk diffusion assay on solid agar media. Biofilm formation was inhibited by more than 92.4% and 98.0%, respectively, using the same concentration of each extract. The present results demonstrate that the medicinal plants C. japonica and T. orientalis are potentially useful as antimicrobial and anti-biofilm agents in preventing dental diseases.

3 세대 세파계 항생제에 내성인 임상균주의 분포와 PCR 법을 이용한 TEM type $\beta$-lactamase 생산균주의 동정 (Prevalence of Strains Resistant to the Third Generation Cephalosporins among Clinical Isolates and Identification of TEM Type $\beta$-lactamase from Resistant Strains by PCR Method)

  • Compared to the first and second-generation cephalosporins, the third-generation cephalosporins are remarkably stable against hydrolysis by the $\beta$-lactamases produced by aerobic gram-negative bacilli, such as Enterobacteriaceae. Among these bacteria, the most prevalent plasmid-encoded $\beta$-lactamase is TEM-1 $\beta$-lactamase belonging to class A or group 2b. This enzyme is produced constitutively and is principally active against peniciflins and old cephalosporins rather than third-generafion cephalosporins, carbapenems and mmobactams. However, new TEM type $\beta$-lactamases including TEM-9 and TEM-12 evolved through point mutations in a gene encoding $\beta$-lactamase have been discovered from patients during chemotherapy. These $\beta$-lactamases are known to be capable of hydrolyzing most of the third-generatim cephalosporins. To study the prevalence of $\beta$-lactamases from clinical isolates collected in Korea. the minimal inhibitory concentratims(MICs) of several third-generation cephalosporins against 628 clinical isolates were determined by agar dilution methods, and $\beta$-lactamas-producing bacteria were isolated by use of cefinase disc. By polymerase chain reaction (PCR) method, clinical isolates harboring a gene for TEM type $\beta$-lactamase were identified among the $\beta$-lactamase producing strains. Twentiy three percent of the clinical isolates was resistant to the thirdgeneration cephalosporins, and more than 90% of resistant cells produced various $\beta$-lactamases. TFM type $\beta$-lactamases were dominant in gram-negative bacilli, such as Escherichia coli, Klebsiella pneumoniae, Enterobacter species. These results suggest the necessity of the development of new cephalosporins which are stable against $\beta$-lactamases like TEM.

Comparison of Fluoroquinolone Resistance Determinants in Uropathogenic Escherichia coli between 2 Time Periods of 1989 and 2010-2014 at Gangwon Province in Korea

  • Fluoroquinolone (FQ) resistant uropathogenic Escherichia coli (UPEC) have become a major problem in urinary tract infections (UTIs). The purpose of this study was to compare the quinolone resistance-determining region (QRDR) and plasmid mediated quinolone resistance (PMQR) determinants of FQ resistant UPEC between 1989 and 2010-2014. A total of 681 strains of UPEC clinical isolates was collected from Korean healthcare facility in 1989 (123 strains) and in 2010-2014 (558 strains). The minimum inhibitory concentrations (MICs) of FQs were determined by agar dilution method. QRDRs (gyrA, gyrB, parC and parE) and PMQR determinants (qnrA, qnrB, qnrS, aac(6')-Ib-cr and qepA) were analyzed polymerase chain reaction and sequencing method. Among 681 isolates, FQ resistant UPEC were 3 strains (2.4%) in 1989 isolates and 220 strains (39.4%) in 2010-2014 isolates. The rate of the FQ resistant UPEC strains in 2010-2014 isolates was increased than that of in 1989 isolates. UPEC isolates from 1989 and 2010-2014 were shown to carry mutations in gyrA (Ser83 and Asp87), gyrB (Ser464 and Thr469), parC (Ser80 and Glu84) and parE (Glu460, Ser458, Ile464 and Leu445). The most common mutations of QRDRs in 1989 isolates were Ser83Leu and Asp87Gly in gyrA and Ser80Ile in parC (2 strains: 66.7%) while those in 2010-2014 isolates were Ser83Leu and Asp87Asn in gyrA and Ser80Il2 and Glu84Val in parC (88 strains: 40.0%). PMQR determinants were detected only in 2010-2014 UPEC strains (47 strains: 21.4%).

임상에서 분리된 Acinetobacter baumannii의 항생제 내성 패턴과 유전학적 특징 (Antibiotic Resistant Patterns and DNA Fingerprint Analysis of Acinetobacter baumannii from Clinical Isolates)

  • Acinetobacter baumannii is gram-negative bacilli that can be widely found in environments. Recently, A. baumannii emerged as a serious nosocomial infection. A total of 92 A. baumannii were isolated from hospitalized patients in Seoul, Korea, between December 2010 and April 2011. Antimicrobial susceptibility testing was investigated using CLSI agar dilution methods. Tigecycline non-susceptible A. baumannii isolates were investigated by repetitive extragenic palindromic sequence-based PCR (rep-PCR). Pulsed-field gel electrophoresis was performed to determine the epidemiological relationships. All clinical isolates showed high-level resistance to the most commonly used antibiotics: Ciprofloxacin (87.0%), Ampicillin/sulbactam (82.6%), Cefotaxime (81.5%), Ceftazidime (80.4%). Moreover, 50.0% of these isolates were non-susceptible to tigecycline. When evaluated by RAPD analysis, generated distinct band ranging in size from 1kb to 8k band varying from 4 to 10 bands. Stricter surveillance and more rapid detection are essential to prevent the spread of multi drug resistant A. baumannii.
