  • This paper covers the design and implementation process of RF hypertermia system for cancer therapy. Among many hyperthermic methods, RF capacitive heating method is discussed because it can heat the deep-seated tumors selectively. The RF power oscillator and its applicators were designed and implemented. And the experiments were performed with agar phantom and dog to prove that the system can heat any depth selectively. And the electrical safety and appropriateness of clinical application was proved through the human living-body test.

  • The biological effects for the use of hypertherinla to treat malignant tumors has been well studied and encouraging clinical results have been reported. However, the engineering and technical aspects of hyperthermia for the deepseated tumors has not been satisfactory. We have developed the FF capacitive hyperthermia device(GHT RF8)by cooporation with Yonsei Cancer Center and Green Cross Medical Equipment Corporation. It was composed with 8.10 MHz RF generator, capacitive electrode, matching system, cooling system, temperature measuring thermocouples and control PC computer. We have measured the temperature and thermal distribution in agar phantom, animals and human tumors.

  • Since March 1985, we have treated cancer patients with local hyperthermia using a 2,450-MHz microwave thermogenerator. Prior to clinical trial, a 2,450-MHz microwave generator remodeled from a household electric range was tested and evaluated to test its clinical applicability. We studied the thermoprofile and tried to find out suitable electric power ranges to produce optimal temperature of $42-44^{\circ}C$ in a lump of meat, agar phantom and animal tumor models. The present study confirmed the intratumoral temperature to be $1-3^{\circ}C$ higher than in surrounding normal tissue.

  • Spiral microwave antennas have been developed and measured the thermal distribution in agar phantom. The design has been configured in three types, 3 cm $\phi$ applicator with 24.5 cm length (A type),4 cm $\phi$ with 12.2 cm (B type) and 6 cm $\phi$ with 24.5 cm length (C type). The relative specific absorption rate (SAR) measured in phantom have been used to estimate the depth and profile of effective heating. The applicator of copper antenna with 4 cm $\phi$ diameter and 12.2 cm length (B type) has the most homogeneous (FWHM=3.5 cm) and heating into deep site ($D_{eff}=4\;cm$).

  • Although ultrasonic CT is one of the useful techniques for tissue characterization, the reconstructed images, such as the velocity distribution and attenuation constant distribution, are degraded by reflection and refraction of ultrasonic beam. This paper studied the degradation effects on attenuation images using agar gel phantoms which were developed to evaluate ultrasonic CT. We found that the reconstructed attenuation constants at the center of the phantoms were less than the actual values by 0.6 dB/cm when phantom velocity differs by 25 m/s from surrounding saline. We also studied a correction method for refraction and phase cancellation effects, where the correction was made using the maximum value in the received subdata, as obtained by sub-arraying microprobes located at each sampling point. Using this method, we could obtain an improvement in the reconstructed image by the correction on the attenuation effect.

  • Hyperthermia for the treatment of cancer has been introduced for a long time and the biological effect for the use of hyperthermia to malignant tumors has been well established and encouraging clinical results has been observed. Unfortunately, the engineering or technical aspects of hyperthermia for the deep seated tumors has not been satisfactory. We have researched and developed the radiofrequency capacitive hyperthermia system (GHT- RF8). It was composed with 8-9 MHZ RF generator, capacitive electrode, matching system, cooling system, temperature measuring system and control computer. The thermal profile was investigated in agar phantom, animals and in human tumors, which was heated with capacitive RF device.

A Study on the Accuracy of Measurement of Residual Urine Amount depending on the Bladder form using Ultrasonic (초음파를 이용한 Bladder형태에 따른 잔뇨량 측정의 정확도 연구)

  • This study is to increase the accuracy of the diagnosis of benign prostatic hyperplasia by presenting a method that can accurately measure the residual urine amount of the bladder by using an ultrasound image. Agar powder, Propanol and distilled water were used as materials for making a phantom. In order to measure the volume, a $10m{\ell}$ cylinder, syringe and beaker were used. The image was obtained by scanning phantoms produced into six shapes. Each constant value was obtained by using the expression designed to measure the residual urine amount of the bladder and was compared and analyzed. The measuring method of Bladder volume was presented and a constant value for each shape was obtained and five observers measured it five times. According to the results of clinical application, the errors of Ellipse-beanbag, Shield-shield were 11.0%, 18.2%, respectively. Constant values depending on the shape of each phantom were presented in order to accurately measure the volume of the bladder in measuring the amount of residual urine for the diagnosis of benign prostatic hyperplasia. The accuracy of the volume using this was verified statistically(p > 0.05). Therefore, it is considered to be useful in diagnosing benign prostatic hyperplasia by using the ultrasound imaging measuring method presented.

  • Hypertermia for the treatment of cancer has been introduced for a long time and the biological effect for the use of hyperthermia to treat malignant tumors has been well established and encouraging clinical results have been obserbed. Unfortunately, however, the engineering or technical aspects of hyperthermia for the deep seated tumors has not been satisfactory. We developed the radiofrequency capactive hyperthermia device (Greenytherm-GY8) in cooperation with Yonsei Cancer Center and Green Cross Medical Corporation. It was composed with $8{\sim}10MHz$ RF generator, capacitive electrode, matching system, cooling system, temperature measuring system and control PC computer. The thermal profile was investigated in agar phantom, animals and in human tumors, heated with capactivie RF device. Deep and homogeneous heating could be achieved in a large phantom of 25cm diameter and 19cm thick when heated with a pair of 23cm diameter electrodes, coupled to both bases of the phantom, when the size of the two electrodes was not the same, the region near the smaller electrode was preferentially heated. It was, therefore, possible to control the depth of heating by choosing proper size of electrodes. Therapeutic temperature $(42^{\circ}C{\sim}43^{\circ}C)$ could be obtained in the living animal experiments. Indications are that deep heating of humn tumors might be achieved with the capacitive method, provided that subcutaenous fat layer is cooled by temperature controlled bolus and large size of electrodes.

An Experimental Study on the Effect of Combined X-ray and Microwave Hyperthermia on the Rectum and Urinary Bladder of Rats (흰쥐의 직장과 방광에 X-선 조사와 마이크로파 온열요법의 효과에 관한 실험적 연구)

  • Hyperthermia can enhance the radiation effect as a synergistic reaction in combined X-ray irradiation and hyperthermia; hyperthermia sensitize radioresistant S-phase cells and inhibit cellular recovery from sublethal damage. We fabricated 100 watts, 2450 MHz microwave applicator for hyperthermia and planned the method and condition of heating and measured the temperature by using Agar phantom as a preliminary test. For biological examination, 102 rats were divided into 4 groups as hyperthermia, X-ray irradiation (6Gy-15Gy), combined X-ray and hyperthermia, and normal control groups. Microscopic examination of the rectum and bladder was done and the results were as followings: 1. The microwave generator with 100 watts, 2450MHz magnetron could be heating up to $40^{\circ}{\sim}50^{\circ}C$ for one hour in living tissue. 2. The thermal distribution in tissue equivalent phantom with microwave can be maintained at $40^{\circ}{\sim}44^{\circ}C$ in area of 3cm in depth and 2-10cm in diameter. 3. In Hyperthermia alone group, there was submucosal edema of the rectum but no histologic change in the urinary bladder was seen. 4. The minimal necrosis of the mucosa was appeared in the rectum and bladder after 15 days of 6 Gy and 8 Gy irradiation respectively. The minimal necrosis of the muscle layer of rectum and bladder was appeared after 15 days of 8Gy and 60days of 10Gy irradiation respectively. 5. In combined group of radiation and hyperthermia, thermal enhancement ratio (calculated at necrosis of mucosa and muscle layer) of rectum and bladder was 1.0, and it suggest that there is no change of tolerance dose of normal rectum and bladder.

Signal Change of Normal Saline by Oxygen Injection in FLAIR Image (산소주입에 의한 FLAIR 영상에서 생리식염수의 신호 변화)

  • It was reported that there were some cases in which signal was not inhibited but high signal appeared in cerebrospinal fluid on FLAIR(fluid attenuated inversion recovery) of MRI(Magnetic Resonance Imaging) in case a person inhales high-concentration oxygen. This study was to prepare basic database. We produced a phantom fixed with agar gel and by using it, obtained the images of the signals of normal saline into which oxygen was injected and normal saline diluted with contrast media by changing the TI(Inversion Time) of FLAIR technique and analyzed them. In the result of FLAIR technique of MRI using Philips Achieva MR 3.0T in Busan P Hospital, the SNR(Signal to Noise Ratio) of normal saline into which oxygen was injected was higher than the SNR of normal saline into which oxygen was not injected. However, it was not higher than the SNR of normal saline diluted with contrast media. In the TI 1,800ms, we could obtain the images which do not have the rise of the signal due to oxygen. In the CNR(Contrast to Noise Ratio) of normal saline into which oxygen was injected and normal saline diluted with contrast media as well, it was higher in the TI 1,800ms than in the TI 2,800ms that is mainly used clinically. It is thought that the result of this study could be basic database for studies on change of signal of cerebrospinal fluid as a result of injection of oxygen in FLAIR technique of MRI.
