Long-term stability after multidisciplinary treatment involving maxillary distraction osteogenesis, and sagittal split ramus osteotomy for unilateral cleft lip and palate with severe occlusal collapse and gingival recession: A case report

  • In this report, we describe a case involving a 34-year-old woman who showed good treatment outcomes with long-term stability after multidisciplinary treatment for unilateral cleft lip and palate (CLP), maxillary hypoplasia, severe maxillary arch constriction, severe occlusal collapse, and gingival recession. A comprehensive treatment approach was developed with maximum consideration of strong scar constriction and gingival recession; it included minimum maxillary arch expansion, maxillary advancement by distraction osteogenesis using an internal distraction device, and mandibular setback using sagittal split ramus osteotomy. Her post-treatment records demonstrated a balanced facial profile and occlusion with improved facial symmetry. The patient's profile was dramatically improved, with reduced upper lip retrusion and lower lip protrusion as a result of the maxillary advancement and mandibular setback, respectively. Although gingival recession showed a slight increase, tooth mobility was within the normal physiological range. No tooth hyperesthesia was observed after treatment. There was negligible osseous relapse, and the occlusion remained stable after 5 years of post-treatment retention. Our findings suggest that such multidisciplinary approaches for the treatment of CLP with gingival recession and occlusal collapse help in improving occlusion and facial esthetics without the need for prostheses such as dental implants or bridges; in addition, the results show long-term post-treatment stability.

A Case Report on the Treatment of A TMJ Osteoarthritis Patient with Anterior Open Bite Using An Intermaxillary Traction Device (전치부 개교합을 동반한 골관절염 환자에 대한 악간견인장치의 응용)

  • A patient with TMJ osteoarthritis and anterior open bite was treated with an intermaxillary traction device. Pretreatment examination revelaed a pain in both TMJ during mouth opening, moderate tendernesso f left sternocleidomastoid and right trapezius muscles. Anterior open Bite was aobserved with interincisal distance of 2mm. Tomograms and MRI showed anterior disc displacement withouit reductoin of both temporomandibular joints, and the condyles were flattened and slightly eroded. A pair of full-coverage occlusal appliances was made on both maxillary and mandibular dentition, with pivoting fulcrum on the site of the second moalr. Traction force was gained by the intermaxillary orthodontic elastics which were hooked by orthodontic brackets on the labial surfaces of the upper and lower anterior and premolar teeth. After 8 weeks of traction treatment, the joint pain was subsided completely and the anterior open bite was closed to get an edge to edge relationship of anterior teeth.

  • Plasma etching was studied to obtain high-efficiency Si solar cells. SiN nanoparticles were observed upon the plasma treatment using SF6 gas. The mechanism of the nanoparticles formation has been studied. A net increase in the current density (Jsc) of the cells of $1.7mA/cm^2$ and in the conversion efficiency ($\eta$) of 2.1% is obtained after the plasma treatment for 10s, thanks to the significant decrease of reflection in the shorter wavelength range.

  • After studying several methods of the application a device of a treated sewage in anticipation of the future shortage of the duty of water, we could have some conclusion as the following : Advanced treatment systems arc essential prerequisites in reusing a treated sewage. And in a short term, the application of reusing a treated sewage should go first to new building areas near the sewage but for the long run, it should cover the whole area of Taegu, it is desirable that the pipe line networks which include dual water systems as well as water supply should be spread throughout the whole city. The city authorities have to make every effort to step up publicity activities on this plan to all the citizens and building owners to steadily carry out this project and encourage private constructors to participate with the help of SOC. And for the long run, it is desirable that the application of reusing a treated sewage should be obligatory.

Treatment Efficacy on Oral Malodor according to Pre-treatment Volatile Sulfur Compound Level (구취의 심도에 따른 치료 효과에 대한 비교 연구)

  • Considering various factors contributing oral malodor, the accurate prediction of prognosis is very important to both clinician and patients. The present study has been performed to invetigate the relationship between treatment effeicacy and pre-treatment volatile sulfur compounds (VSC) level. Ninety patients were divided into three groups, A(<150ppb), B(150< <200ppb), and C(>200ppb) groups, according to pre-treatment VSC level detected by Halimeter, and each group included 30 patients. Routine therapeutic measures for oral were provided to each patient which consisted of oral prophylaxis, tooth brushing and flossing instruction, tongue scraping by proper device, and gargling of 0.25% ZnCl2 Solution. The group with high pre-treatment VSC level (>150ppb) showed significant reduction of VSC level at 1 and 3 weeks after. However, the group with low pre-treatment VSC level (<150ppb) did not show any significant reduction during the experimental periods. Collectively, the results suggested that patients with high pre-treatment VSC level show better prognosis.

  • The video based electronic portal imaging device (EPID), which could display the portal image in near real time, was implemented to verify treatment position error in FSRT(Fractionated Stereotatic Radiation Therapy) instead of a portal film. Also, Developed FSRT system was composed of the stereotactic frame, frame mounting system and collimator cones. The verification of treatment position is very crucial in special therapies like FSRT. In general, the FSRT uses high dpse rate at small field size for treating small intracranial lesions. To evaluate quantitative positioning errors in FSRT, we used the first FSRT image as reference image and obtained the second FSRT image that was moved 2mm intentionally and detected intracranial contours after image processing. The generated 2mm error could be verified by overlapping only contours of two images. Through this study, the radiation treatment efficiency could be improved by performing precise radiation therapy with a developed video based EPID and FSRT.

The Effects of Partial Vibration on Tibia of Osteoporosis Induced Rat (골다공증이 유발된 쥐 정강이뼈에 적용한 부분 진동자극 효과)

  • The pharmacological therapies and whole body vibration as non-pharmacological therapies were known to have adverse side effects. Therefore, partial stimulation was suggested and its effects were evaluated. This study aimed to evaluate the site-specific effects of partial stimulator for treatment of osteoporosis induced by estrogen deficiency. Sixteen virginal Sprague-Dawley rats (12 weeks old) were divided into 2 groups(no stimulation, stimulation groups). All rats were ovariectomised to induce osteoporosis. After 3 weeks of operation, the right tibiae in rats of stimulation group (frequency: 10Hz, cycle: 1500, strain on bone surface: $2000{\mu}{\varepsilon}$) were stimulated perpendicularly at right tibia by using partial stimulator for 6 weeks (3days/week). The right tibiae in rats were scanned, before stimulation (0 week) and at 6 weeks after stimulation by using in-vivo micro computed tomography. For investigation of changes in morphological characteristics, structural parameters were measured and calculated. At 6 weeks the morphological characteristics (relative value) in stimulation group were significantly enhanced than those in no stimulation group (p<0.05). In this study, we find that after 6 weeks of partial stimulation, the morphological characteristics of tibia trabecular bone were enhanced. Thus, we concluded that partial stimulation could be used to treat osteoporosis.

  • In this paper, I describe a system for remote monitoring of devices Android-based sensor data recorded during the operation of the ballast water treatment system. The proposed remote monitoring system is Designed and implemented when a problem occurs in the ballast water treatment system installed in the vessel, provide information if you are unable to determine the cause in the field or engineer dispatch request of external. System developed is composed of a server holding the sensor data information collected from the vessel and a mobile device that is not bound to time and place. The transmission of sensor data between mobile devices and server, and is implemented by TCP/IP to transmit information securely. System is composed of request to the server from the mobile device after the user inputs the input values, to transmit the sensor data. The device provides the information of the sensor with a high level of importance to the user. Through the remote monitoring sensor information of the ballast water treatment system, the system is made to predict the failure of the sensor.

  • ZnO thin films on (001) sapphire substrates have been deposited by pulsed laser deposition(PLD) technique at the oxygen pressure of 350 mTorr. In order to investigate the effect of post-annealing treatment with oxygenn pressure of 350 mTorr on the optical property of ZnO thin films, films have been annealed at various substrate temperatures after deposition. After post-annealing treatment in the oxygen ambient, the optical properties of the ZnO thin films were characterized by PL(Photoluminescence) and structural properties of the ZnO were characterized by XRD, and have investigated structural property and optical property for application of light emission device.

  • Pulmonary thromboembolism (PTE) increases the pressure of the right ventricle and leads to symptoms and signs, such as dyspnea and hypoxia. If PTE causes hemodynamic instability, thrombolytic therapy should be considered. A mechanical thrombectomy is an alternative treatment to thrombolytic therapy and should be considered when thrombolytic therapy is contraindicated. Various devices are used in mechanical maceration and catheter-directed thrombolysis, but there is no standard mechanical device for PTE as yet. We report here on 2 clinical experiences of mechanical thrombectomy using the Arrow-Trerotola percutaneous thrombolytic device to remove residual clots after systemic thrombolysis in patients with massive PTE.
