예후가 좋지 않은 갑상선암에 대한 최신 치료 방침 (Recent Improvements in the Treatment of High-Risk Thyroid Cancer)

  • Thyroid cancer is one of the slow-growing tumors with excellent oncological outcomes. However, a small set of patients with unexpectedly severe outcomes are usually ignored. Anaplastic thyroid cancer (ATC) remains one of the most aggressive and lethal solid tumors. Recently, dabrafenib and trametinib combination therapy or neoadjuvant BRAF induction therapy has shown promising results. In addition, a combination of targeted drugs, immunotherapy, surgery, and radiation therapy can improve overall survival in ATC patients. Another disease for which there is no breakthrough treatment is radioactive iodine-refractory differentiated thyroid cancer (DTC). To date, multikinase inhibitors (sorafenib, lenvatinib) targeting the growth factor signaling pathway have been developed and approved as anticancer agents for patients with advanced DTC. This review includes results from multikinase inhibitors to the emergence of new target molecules, including rearrangements during transformation (RET) and tropomyosin receptor kinase (TRK).

Unusual Intronic Variant in GSTP1 in Head and Neck Cancer in Pakistan

  • In the present case control study mRNA expression of the GSTP1 gene, encoding a phase II enzyme that detoxifies via glutathione conjugation, was investigated using semiquantitative PCR followed by SSCP for 49 confirmed head and neck (HN) cancer and 49 control samples. It was found that GSTP1 was upregulated in significantly higher number of cancers (OR 4.2, 95% CI 1.2-15.3). Grade wise correlation was also observed with more up regulation in patients with more advanced grades of HN carcinomas. We also found that 5 patients showed variation in mRNA with a larger product size than expected. Sequencing revealed insertion of an intronic segment between the $6^{th}$ and $7^{th}$ exon of the GSTP1 gene. Germline screening was performed showing mobility shifts which suggested mutation at the DNA level resulting in intronic portion retention. This study is of prime importance for drug design and treatment selection to overcome increased resistance of HN cancers to drugs due to alteration in the GSTP1 gene.

  • The combined induction chemotherapy is usually used to treat the advanced head and neck cancer. In particular, it is considered that the COB(Cisplatin, Oncovin, Bleomycin)regimen is very effective against the squamous cell carcinoma in head and neck region. Reirradiation of head and neck cancer in the era of intensity-modulated radiotherapy: patient selection, practical aspects, and current evidence

  • Locoregional failure is the most frequent pattern of failure in locally advanced head and neck cancer patients and it leads to death in most of the patients. Second primary tumors occurring in the other head and neck region reach up to almost 40% of long-term survivors. Recommended and preferred retreatment option in operable patients is salvage surgical resection, reporting a 5-year overall survival of up to 40%. However, because of tumor location, extent, and underlying comorbidities, salvage surgery is often limited and compromised by incomplete resection. Reirradiation with or without combined chemotherapy is an appropriate option for unresectable recurrence. Reirradiation is carefully considered with a case-by-case basis. Reirradiation protocol enrollment is highly encouraged prior to committing patient to an aggressive therapy. Radiation doses greater than 60 Gy are usually recommended for successful salvage. Despite recent technical improvement in intensity-modulated radiotherapy (IMRT), the use of concurrent chemotherapy, and the emergence of molecularly targeted agents, careful patient selection remain as the most paramount factor in reirradiation. Tumors that recur or persist despite aggressive prior chemoradiation therapy imply the presence of chemoradio-resistant clonogens. Treatment protocols that combine novel targeted radiosensitizing agents with conformal high precision radiation are required to overcome the resistance while minimizing toxicity. Recent large number of data showed that IMRT may provide better locoregional control with acceptable acute or chronic morbidities. However, additional prospective studies are required before a definitive conclusion can be drawn on safety and effectiveness of IMRT.

국소 진행된 두경부암 환자의 방사선치료 후 갑상샘기능저하증 (Hypothyroidism after Radiotherapy of Locally Advanced Head and Neck Cancer)

  • 목 적: 국소 진행된 두경부암 환자에서 방사선치료를 포함한 근치적 치료 후 갑상샘기능저하증의 발생률을 분석하고자 후향적 연구를 실시하였다. 대상 및 방법: 2000년 1월부터 2005년 12월까지 본원에서 국소 진행된 두경부암으로 진단받고 근치적 목적으로 방사선치료 단독 또는 수술 후 방사선치료를 받은 환자 중 수술로 갑상샘을 절제하지 않았고, 방사선치료 전 갑상샘기능이 정상이며, 하부 경부림프절이 방사선치료 범위에 포함된 115명의 환자를 대상으로 본 연구를 시행하였다. 환자의 중앙연령은 59세(28~85세)이고 남자 73명, 여자 42명이었다. 원발병소의 위치는 구강, 구인두, 하인두, 후두, 그 외가 각각 18, 40, 28, 22, 7명이었으며, 편측 경부절제술을 받은 환자가 19명(16.5%), 양측 경부절제술을 받은 환자가 18명(15.7%), 방사선치료만을 시행한 환자가 78명(67.8%)이었다. 방사선치료는 6-MV X-ray를 사용하였으며, 통상 분할 조사법에 의해 하루 1.8~2 Gy 씩, 주 5회 조사하였다. 원발병소에 조사된 방사선량은 중앙값 70.2 Gy (59.4~79.4 Gy)이었으며, 하부 경부림프절에 조사된 방사선량은 중앙값 50.0 Gy (44.0~66.0 Gy)이었다. 추적관찰기간은 2~91개월이었으며, 중앙값은 29개월이었다. 결 과: 전체 115명 환자 중에서 38명(33.0%)의 환자에서 갑상샘기능저하증이 관찰되었다. 전체 환자의 1년, 3년의 갑상샘기능저하증 발생률은 각각 28.7% (33명), 33.0% (38명)이었으며, 갑상샘기능저하증 발생시기의 중앙값은 8.5개월(0~36개월)이다. 갑상샘기능저하증 발생에 영향을 주는 인자로 단변량분석결과 경부절제술 여부가 위험인자로 분석되었으나, 다변량분석에서는 통계적으로 유의한 관련 인자는 없었다. 결 론: 두경부 종양환자의 방사선치료를 포함한 근치적 치료 후 갑상샘기능저하증이 발생할 수 있으며, 대부분이 1년 이내 발생하므로 치료 후 갑상샘기능검사를 정기적으로 실시할 필요가 있다. 특히 경부 림프절제술 후 방사선 치료를 시행하는 경우 더욱 주의하여 갑상샘기능저하증의 발생을 확인해야 한다.

End Stage Palliative Care of Head and Neck Cancer: a Case Study

  • Background: Locally advanced head and neck cancer is generally incurable and has a short survival rate. This study aimed to evaluate symptom relief, disease response, and acute toxicity after palliative hypo-fractionated radiotherapy and long-term survival in affected patients. Materials and Methods: Between January 2011 to December 2011, 80 patients who were histopathologically diagnosed as having stage III or stage IV head and neck squamous cell carcinoma based on Eastern Cooperative Oncology Group (ECOG) performance status 1-3, were offered palliative radiotherapy (20 Gy/5Fr/5 Days). Later these patients were evaluated on 30th day after completion of treatment for disease response based on World Health Organisation (WHO) criteria and palliation of symptoms using symptomatic response grading and acute toxicities by the Radiation Therapy Oncology Group (RTOG). Many patients were given post radiation therapy (RT) palliative chemotherapy for appropriate palliative care and a few patients were selected for further curative RT. The overall survival was also evaluated among this group of patients with last follow up date of 1st May, 2014. Results: The most common presenting complaint was pain followed by dysphagia. Most patients (60-70%) had appreciable relief in their presenting symptoms. A good response was observed in the majority following palliative RT; a few patients had progressive disease and some had stable and regressed disease. None of the patients experienced radiation toxicity that required hospital admission. Almost all showed grade one and two acute skin and mucosal toxicity one month after completion of treatment. The mean survival days for patients given only hypofractionated palliative RT was 307 days, those with post palliative RT and palliative chemotherapy was 390 days and patients who went on to receive further palliative RT and curative RT dose had significantly overall survival of 582 days. Conclusions: Advanced head and neck cancer should be identified for suitable palliative hypofractionated radiotherapy to achieve acceptable symptom relief in a great proportion of patients and should be followed by palliative chemotherapy or curative RT in suitable cases for long-term symptom-free survival.

Prognostic Value of Osteopontin in Patients Treated with Primary Radiotherapy for Head and Neck Cancer

  • Background: The prognostic value of tumor osteopontin (OPN) in patients with squamous-cell head and neck cancer (SCHNC) was investigated. Materials and Methods: OPN expression was assessed by immunohistochemical methods in 50 patients, who were treated with primary radiotherapy (RT) for locally advanced SCHNC. The effects of OPN on clinical parameters, local-regional control after RT and metastasis-free survival, was assessed. Results: The rate of OPN expression in tumor tissue was 76%. OPN positive cases had lower Hb levels (p=0.088). Mean time to local recurrence was 53.8 months (SE 3.9) in OPN-negative cases and 39.1 months (SE 4.7) in OPN-positive cases (p=0.047). OPN increased the risk of local recurrence 5.9 times (p=0.085). It had no effect on metastasis-free (p=0.116) or overall survival (p=0.123). OPN was positive in 12 of 19 cases that developed grade 3-4 acute radiation dermatitis (p=0.096). Conclusions: OPN expression is associated with an increase in local recurrence in patients who were treated with primary RT for locally advanced SCHNC.

Reconstructive methods to resolve intractable fistulas that develop after radiation therapy in patients with head and neck cancer

  • Background: Radiation therapy (RT) is frequently used for supportive treatment and management of advanced head and neck cancers. This study performed a retrospective review of the treatment methods that were used for intractable draining fistulas in seven patients who had received RT for head and neck cancers. Treatment methods used for two of the seven patients are presented in detail. Methods: From 2009 to 2020, seven patients underwent reconstructive surgery for intractable fistulas which occurred after RT for head and neck cancers. Patient characteristics, medical history, treatment method, and treatment outcome were reviewed for each case. The type of surgery performed, failure rate, and treatment period were also analyzed. Results: In this study, a total of seven patients received additional management for radiation-induced fistulas. Patients underwent a mean of 3.3±1.4 surgeries (maximum: six surgeries) to resolve their fistulas. The mean time interval from the first surgery to the last surgery for the patients to achieve resolution of the fistula was 8.7 months. Loco-regional flaps have performed an average of 1.9±1.5 times. However, all loco-regional flaps failed. Instead, the patients' intractable fistulas were resolved with the use of distant flaps or free tissue transfers. Conclusion: Fistulas that develop after head and neck cancer treatment following RT are difficult to treat with simple loco-regional flap procedures. Therefore, more aggressive treatment techniques, such as distant flap or free tissue transfer, may be needed to shorten patients' treatment periods and avoid unnecessary surgeries.

하인두암에서 후두인두 및 경부 식도 전 적출술 후 유리 공장 이식술의 합병증에 대한 후향적 분석 (Retrograde Analysis of Complications of Jejunal Free Flap after Total Pharyngo-Laryngo-Cervical Esophagectomy in Advanced Hypopharyngeal Cancer Treatment)

  • 진행된 하인두암의 수술 후 시행한 유리 공장 이식술을 시행한 본원의 경험에서 수술 후 누공 형성, 수술 부위 협착 등의 다양한 합병증과 그 치료방법을 경험하였다. 수술 방법에 있어서 한 개의 동맥과 정맥을 연결하였고 이식한 혈관의 혈전에 의한 이식 실패는 없었다. 수술 후 연하 기능에 있어서 영향을 주는 가장 중요한 요인은 이식 부위 협착이 었으며 협착의 치료 결과에 따라 술 후 연하 기능의 회복에 차이가 있음을 확인하였다.
