대구치의 자가이식을 동반한 골격성 II급 부정교합의 악교정수술 치험례 (Autotransplantation of a Third Molar as a Lower Second Molar Combined with Orthognathic Surgery)

  • We report the treatment of an adult Class II malocclusion with severe crowding and a hopeless lower second molar. According to the treatment plan, 4 premolars and 4 third molars were to be extracted for 2-jaw surgery. To replace the hopeless lower second molar, one upper third molar was successfully autotransplanted during the pre-surgical orthodontic treatment. Multiple teeth are frequently extracted for treatment purposes in adult surgical cases. Under precise diagnosis, the reuse of extracted teeth to replace missing teeth can be a successful alternative even in adult surgical patients.

부정교합의 유형에 따른 두개저와 하악골의 형태에 환한 연구 (THE VARIATION OF MANDIBULAR PATTERN AND CRANIAL BASE ANGLE IN CRANIOFACIAL MALOCCLUSION)

  • This study was designed to investigate the variation of mandibular pattern and cranial base and their association in the craniofacial malocclusion. The material was the 165 cephalometric radiographs taken from craniofacial malocclusion. The sample was devided into two groups by age child group aged from 10 to 13 years and adult group aged over 18 years, and each group was devided into 3 types of malocclusion; class I, Class II div. 1 and Class III malocclusion. Child group consist of 35 samples and adult group consist of 20 samples in each malocclusion type. Various angular and linear measurements on the cephalometric radiographs were recorded and statistically analyzed. The results of the study were as follows; 1. The cranial base angle was largest in Class II div. 1 malocclusion and smallest in Class III malocclusion 2. The anterior cranial base length was largest in Class II div. 1 malocclusion but posterior cranial base length did not show statistical difference. 3. The mandibular body length of Class III malocclusion was larger than those of Class I and Class II div. 1 malocclusion in the adult group but did not shown difference in the child group. The ramus height of Class III malocclusion was larger than those of Class I and Class II div. 1 malocclusion in the child and adult group, but there were no difference between Class I and Class II div. 1 malocclusion. 4. The mandibular position was showed low correlation with the cranial base angele.

Three-dimensional changes in lip vermilion morphology of adult female patients after extraction and non-extraction orthodontic treatment

  • Objective: To investigate the three-dimensional lip vermilion changes after extraction and non-extraction orthodontic treatment in female adult patients and explore the correlation between lip vermilion changes and incisor changes. Methods: Forty-seven young female adult patients were enrolled in this study (skeletal Class III patients were excluded), including 34 lip-protruding patients treated by extraction of four first premolars (18 patients requiring mini-implants for maximum anchorage control and 16 patients without mini-implants) and 13 patients requiring non-extraction treatment. Nine angles, seven distances, and the surface area of the lip vermilion were measured by using pre- and post-treatment three-dimensional facial scans. Linear and angular measurements of incisors were performed on lateral cephalograms. Results: There were no significant changes in the vermilion measurements in the non-extraction group. The vermilion angle, vermilion height, central bow angle, height/width ratio, and vermilion surface area decreased significantly after the orthodontic treatment in the extraction groups, but the upper/lower vermilion proportion remained unchanged. Significant correlations were found between the changes in incisor position and those in vermilion angles, vermilion height, and surface area. Conclusions: Extraction of the four first premolars probably produced an aesthetic improvement in lip vermilion morphology. However, the upper/lower vermilion proportion remained unchanged. The variations in the vermilion were closely related to incisor changes, especially the upper incisor inclination changes.

성인 두개안면골의 성장변화(24세에서 31세까지) (THE LONGITUDINAL GROWTH CHANGES OF CRANIOFACIAL STRUCTURE IN KOREAN ADULT (DURING THE AGE OF 24 TO 31))

  • 최근 성인환자의 증가에 따라 왕성한 사춘기성장이 완료된후 성인에서 일어나는 성장변화를 이해하는 것이 임상교정학에 있어서 매우 중요하게 되었으나 이에 대하여 알려진 바가 많지 않다. 따라서 저자등은 한국인 성인의 두개안면부의 성장변화를 관찰하기 위하여 성인 남자 25명과 여자 15명의 두부방사선규격사진을 촬영한 후 7년후에 추적하여 다시 촬영한 연속 두부방사선규격 사진을 분석하여 다음과 같은 결과를 얻었다. $\cdot$ 여자에서 하악골은 시계방향으로 회전하였으나 남자에 있어서는 변화가 없었으며 하악골의 길이 변화는 없었다. $\cdot$ 여자에서 상악전치는 설측경사 하였다. $\cdot$ 남녀 모두에서 전안면고와 전하안면고의 증가가 있었고 여자에서 전하안면고의 증가가 후안면고의 증가보다 많았다. $\cdot$ 두개저는 관찰기간동안 변화가 없었다. $\cdot$ 성인 두개안면부의 성장은 개개인에 따라서 매우 큰 차이를 보이는 다양성을 가지고 있었다.

성인에서 Hyrax 장치로 완속상악확장하여 폭경부조화를 개선한 증례 (Correction of Transverse Discrepancy with Slowly Maxillary Expansion by Hyrax type expander in Adult Patient)

  • Approximately 30% of adult patients who want orthodontic treatment have transverse discrepancy with insufficient width of the maxilla. Particularly, in Class III patients requiring orthognathic surgery, the frequency of insufficient width of the maxillary arch related to respiratory problems is high. We report a case of non-surgical maxillary expansion using a Hyrax type expander with an orthognathic surgery, based on the reports that the ratio of non-fused midpalatal suture is not high in adults. A 30 years and 2 months old woman with a long face showed an Angle Class III with a vertical growth pattern. Class III molar and canine relation, anterior edge bite, and mandibular incisor compensatory lingual inclination were observed. The posterior buccal overjet seemed to be appropriate, but I diagnosed that there was a transverse discrepancy, for the following reasons. The inter-canine and inter-molar widths were sufficient but excessive lingual inclination of the mandibular molars was observed when assessing the bucco-lingual inclination based on the center of resistance of the maxillary and mandibular first molar. For this reason, it was expected that intercuspal interference would occur during orthodontic decompensation. Therefore, slow maxillary expansion using Hyrax type expander was performed and 2-jaw rotation surgery was performed to improve aesthetic and occlusion. Adults can also improve width discrepancy by non-surgical methods, which can avoid SARPE requiring additional surgery or segmental surgery lacking stability and predictability.

자석의 견인력을 이용한 상악골 확대 장치의 효과 (EFFECT OF MAXILLARY EXPANSION APPLIANCE USING MAGNETIC ATTRACTION FORCE)

  • To study the possibility of attraction magnetic forces to expand maxillary arch, we used 2 big adult dogs, 2 small puppies, 1 small adult dog as experiments, and 1 small adult dog as a control. We measured the intercanine width and intermolar width and histologically observed in the suture and cervical and apex region of teeth and took occlusal X-rays to observe separation of suture line in the maxilla. The results were as follows: 1. Expansion velocities of intercanine (0.25mm/day) and intermolar widths (0.23mm/day) in puppies were faster than those (0.135mm/day, 0.09mm/day) in adults. 2. In all experiments in adults (0.135mm/day) and puppies (0.25mm/day), expansion velocity of intercanine widths were faster than those (0.09mm/day, 0.23mm/day) of intermolar width. 3. In all experiments ectatic changes were observed and cellularities of fibroblast increased in the suture line. Only in adults dogs the separations of palatal suture were observed in the occlusal X-ray view. 4. In the puppies bony deposition was particularly observed in the suture line and micro-bony fragments were often observed. 5. In the all experiments no root resorption was observed in the cervical and root area, but normal root resorption due to eruption of permanent teeth was observed in the puppies.

두부X-선 규격사진의 계측에 의한 한국인 성인 정상교합자의 악안면두개골격의 분석 (A Cephalometric Analysis of Korean Adult Normal Occlusion)

  • Upon analyzing the literature, it becomes apparent that norms for orthodontic diagnosis were from normal occlusion with pleasing face, and that the form and size of craniofacial skeleton were different between racial or ethnic groups. Therefore, this study was intended to establish Korean adult norms be useful for diagnosing the cases and to compare those to other ethnic or racial groups. Lateral cephalometric radiographs were taken of ninety-one Korean adults with normal occlusion showing pleasing face consisted in forty-three males and forty-eight females. The subjects ranged in age from seventeen to twenty-six years. The tracings involved measurements that were analyzed with a digitizer. The findings of this study can be summarized as follows, 1. Norms of Korean adult males, females, and combined sexes were established. 2 It was found that in the form and shape of craniofacial skeleton, there were no significant differences between males and females, but in the size males were larger than females 3. There were a great variation between ethnic or racial groups in the measurements of craniofacial skeleton 4 The size of Korean cranial base was smaller than the American Caucasian. 5 The maxilla and mandible of Korean were less protruded than those of the other ethnic groups 6 The dental pattern of Korean were more proclined than that of the American Caucasian and less protruded than that of the American Negro and the North Indian.

Moyers 분석법에 의한 한국인 정상교합 성인의 악안면 두개 골격 형태에 관한 연구 (ROENTGENOCEPHALOMETRIC STUDY OF CRANIOFAIAL FORM ON KOREAN ADULT OF NORMAL OCCLUSION BY MOYERS' ANALYSIS)

  • This study intended to calculate the cephalometric norms of Korean adult and to compare those with norms of the North American Caucasian by Moyers. Cephalometric headplates of 41 males and 31 females ranged in age from eighteen to twenty-six with normal occlusion and pleasing face were employed for this investigation. The tracings of the standard lateral cephalograms were analyzed by Moyers' method. As a result of this study, the following conclusion can be made. 1. Norms of Korean adult male and female were calculated. 2. There was no significant sexual difference in the basic craniofacial morphology. 3. The size of craniofacial skeleton was larger in male than in female. 4. The Korean male showed lower value of mandibular plane angle to craial base than that of the Caucasian male. 5. There was no significant difference in the anterior total facial height, however, in the posterior facial height the Korean male was larger than the Caucasian male, which manifested that the Korean male held more square-shaped profile. 6. The anteroposterior length of anterior and posterior cranial base, maxillary and mandibular skeletal and dentoalveolar effective length of the Korean male were shorter than those of the Caucasian male, and this suggested that the craniofacial profile depth of the Korean male was shorter than that of the Caucasian male.

Angle씨 제III급 부정교합의 두개 및 악안면 경조직에 관한 연구 (A COMPARATIVE STUDY ON CRANIOFACIAL SKELETON BETWEEN ANGLE'S CLASS III MALOCCLUSION AND NORMAL OCCLUSION)

  • This is to investigate the difference between craniofacial correlation of Angle's class III malocclusion and that of normal occlusion. For this investigation, 28 adult males and famales, who have class III malocclusion, and 35 adult males and 46 adult famals., who have normal occlusion were selected to measure actual dimension of cranial base and to investigate ratio of various parts of maxillofacial skeleton against cranial base. Class III malocclusion were, also , classified based on SNA and SNB angle in normal range. Results were as follows. 1. In class III malocclusion, actual dimension of cranial base were appeared to be less than normal occlusion in both sex. 2. In class III malocclusion, values were less than normal occlusion in BN/ptm-A in both sex, but had no significance between two. 3. In class III malocclusion, ramal inclination, mandible inclination, BN/Go-Pog, lower genial angle were appeared to be greater. Thus, characteristic mandibular protrusive appearance of class III malocclusion was due to relative ratio and morphologic characteristic of mandibular body dimension against cranial base. 4. In class III malocclusion, upper incisors were labially tilted and lower incisors were lingually tilted compared to normal occlusion. 5. In typing of class III malocclusion, the most common type was found to be one which maxillas were in normal range, while mandibles were in protrusive tendency.
