의사결정나무 분석을 이용한 성인 암경험자의 문제수면 위험군 예측: 2013-2016년도 국민건강영양조사 자료 분석 (Identification of Risky Subgroups with Sleep Problems Among Adult Cancer Survivors Using Decision-tree Analyses: Based on the Korean National Health and Nutrition Examination Survey from 2013 to 2016)

  • Purpose: This study was performed to assess problems associated with sleep (short and long sleep duration) and to identify risky subgroups with sleep problems among adult cancer survivors. The study is based on the Korea National Health and Nutrition Examination Survey (KNHANES VI and VII) from 2013 to 2016. Methods: The sociodemographic and clinical data of 504 Korean cancer survivors aged 20-64 years was extracted from the KNHANES VI and VII database. Descriptive statistics for complex samples was used, and decision-tree analyses were performed using the SPSS WIN 24.0 program. Results: The mean age for survivors was approximately 51 years. The mean sleep duration was 6.97 hours; 36.2% of participants had short (< 7 hours) and 9.9% had long (> 8 hours) sleep duration. From the decision-trees analyses, the characteristics of the adult cancer survivors related to sleep problems were presented with six different pathways. Sleep problems were analyzed according to the survivors' sociodemographic information (age, education, living status, and occupation), clinical characteristics (body mass index, hypercholesterolemia, and anemia) and health-related quality of life (HRQoL). The HRQoL (${\leq}0.5$ or > 0.5 cutoff point) was a significant predictor of the participants' sleep problems because all six pathways were started from this predictor in the model. Conclusion: Health care professionals could use the decision-tree model for screening adult cancer survivors with sleep problems in clinical or community settings. Nursing interventions considering these specific individual characteristics and HRQoL level should be developed to have adequate sleep duration for Korean adult cancer survivors.

  • Purpose: The purpose of this study was to determine core nursing intervention in nursing records and to compare perceived nursing intervention priority and nursing intervention frequency of general surgery department. Methods: Subjects were 70 nurses who work in the general surgery department. Data was collected using a nursing intervention classification and analyzed by frequency and mean. Results: The most frequent nursing interventions of nursing records were orderly risk management, coping assistance, tissue perfusion management, skin/wound management and nutrition support. Important nursing interventions were tissue perfusion management, respiratory management, electrolyte acid-base management, elimination, peri-operative care. The most frequent nursing interventions were drug management, peri-operative care, risk management, tissue perfusion management, patient education. Conclusion: This study found that nursing records were different from intervention priority and nursing frequency. So further study is needed for finding focused intervention of specific subjects and differences with priority of nursing and frequency of nursing.

중년기 성인이 인식하는 웰빙의 의미 (Meaning of Well-being for the Middle Aged Adults)

  • Purpose: This research was done to explore the meaning of 'well-being' as experienced by middle aged people. Method: The data were collected by individual in-depth interviews with 107 middle aged adults and it was analysed using qualitative content analysis. Results: 4 components of the meaning of well-being as perceived by the participants were identified as follows: the life of free without physical discomfort and psychosocial distress; the life of comfort with plenty of time, space, material, and mind; the life of purity with natural material and honest mind; the life of harmony with extended consciousness. Conclusion: Therefore the identified meanings of the well-being in this study should be reflected to the nursing education and the nursing practice.

  • Purpose: The purpose of this study was to explore the direction for development of the Korean Journal of Adult Nursing toward becoming an international journal through analysing the accepted and rejected papers during the last three years (2007-2009). Methods: Two hundred and ten accepted papers were analyzed focusing on research methodology and key words using descriptive statistics. In addition, rejected papers were reviewed to analyze their study designs and key words. Results: The proportion of quantitative research was 86.4% while the proportion of qualitative research was 9.5%. The majority of the qualitative research design was survey (71.8%). Sixty percent of the research had verbal consent and 32.7% had written consent from the participants. The prevailing data collection settings were hospitals (52.1%), and community (22.7%). The most frequently used research domain was health. It was noted that theoretical framework was rarely presented. The paper rejection rate was 31.5% and among the rejected paper, 75.3% was survey. Conclusion: The results of this analysis suggest that published studies have been improved and diversified compared with the papers published before the year 2007. However, translation research, clinical trials by nurses, and more detailed evaluation process for ethics in research need to be facilitated.

  • The purpose of this study was to develop a standardized Nursing Diagnosis/ Intervention Protocol through a literature review and validity testing. Seven nursing diagnoses were selected as major nursing diagnosis in the field of Nutritional and Eliminative problem. The nursing intervention list was made by an expert group's review of Nursing Intervention Classification(NIC) suggested nursing interventions. Nursing activities which were included in each nursing intervention were sorted to follow the nursing intervention process after review and revision. The expert group's validity testing was done twice using the Likert scale. As a result the Nursing Diagnosis/ Intervention Protocol for Nutritional and Eliminative Problems was made to include 7 Nursing Diagnoses, 51 Nursing Interventions and 631 Nursing Activities.

성인 선천성 심장질환자들이 지각한 부모 양육태도, 질병인식과 불안간의 관계 (The Relationships among Perceived Parental Bonding, Illness Perception, and Anxiety in Adult Patients with Congenital Heart Diseases)

  • Purpose: The purposes of this study were to identify the relationships among perceived parental bonding, illness perception, and anxiety and to determine the influences of perceived parental bonding and illness perception on anxiety in adult patients with congenital heart diseases. Methods: In this study a descriptive correlational design with survey method was utilized. The participants were 143 adult patients with congenital heart disease being cared for in the cardiology out-patient clinic of A medical center. Data were collected using the Parental Bonding Instrument, Illness Perception Questionnaire Revised Scale, and Cardiac Anxiety Questionnaire Scale. Data were analyzed using descriptive statistics, independent t-test, one-way ANOVA, Pearson correlation analysis, and hierarchial regression analyses. Results: There showed significant positive relationships of anxiety with maternal overprotection, consequences, and personal control respectively. Among predictors, maternal overprotection (${\beta}=.45$), consequence (${\beta}=.26$), and personal control (${\beta}=-.03$) had statistically significant influence on anxiety. Conclusion: Nursing interventions to decrease maternal overprotection and negative consequence, and to enhance personal control are essential to decrease the anxiety of adult patients with congenital heart diseases.

성인 발열환자의 발열관리 실태 조사 (A Descriptive Study for Fever Management in Adult Febrile Patients)

  • Purpose: The specific aims of this study were to define fever from the nurse's and physician's perspectives, describe fever management method and decision-making, and identify differences of definition of fever and fever managements between nurses and physicians. Methods: A cross-sectional survey using a 23-item questionnaire was used. A total of 305 nurses and physicians were surveyed, and 246 useful questionnaires were returned. Results: Body temperature of $36.5{\sim}38.3^{\circ}C$ were considered as adult febrile, and fever managements were initiated when the temperature was $37.0{\sim}38.0^{\circ}C$. More than half of the nurses and physicians, made a clinical decision regarding fever managements based on their own experiences. The most frequently used interventions in nursing practice were antipyretics, intravenous hydration, and external cooling. Nurses provided external cooling to lower body temperature than physicians (Z=-4.67, p<.001). Most participants reported that they needed a fever management protocol. Conclusion: Based on the information obtained, nurses respond with sensitivity to lower body temperature. We found significant differences among nurses, between nurses and physicians in approaching care of adult febrile patients.

간호이론 개발을 위한 일 연구 (The Research for the Nursing Theory Development)

  • Purpose: This study was to understand to the nursing theory frame contained in the traditional philosophy and western nursing theory for the theory development fit for Korean people and to identified the strategy for nursing theory development. Method: To perform this study various relevant texts-western, eastern, classical and contemporary were reviewed. Result: The metaparadigm of the nursing in western considers four major factors-Human being, Environment, Health and Nursing. In Korea, the metaparadigm of nursing considers three major factors-Universe, Human being (body, life, health, disease) and Nursing. The key strategy for establishing comprehensive nursing theory which combines both western and eastern is to study the nursing literature based on eastern and western philosophy. Theorists should study the life and culture of the specific nation in order to understand the essence of the thoughts and psychology of the patients and to investigate the phenomena, which will be the foundation of theory development. Conclusion: Western nursing theory must be utilized to provide practical framework for the development of nursing theory, and practical approach is necessary for combining western and eastern theory.

뇌졸중 환자의 간호성과지표 개발 (A Development of Nursing Outcome Indicators for Stroke Patients)

  • Purpose: This study was conducted to develop nursing outcome indicators based on nursing activities done for stroke patients. Method: focus group meeting and delphi technique, which consisted of clinical nurse experts, neurologist, and nursing professors. CVI(Index of Content Validity) and user validity test was performed. Result: 12 nursing diagnoses, 29 nursing interventions and nursing outcomes were identified. The former were from NIC and the latter were developed according to nursing interventions. They were verified by experts in focus group. 199 nursing activities were identified, 133 nursing outcome indicators were developed. In user validity, usefulness and usability were tested. Conclusion: This systemic approach of measuring nursing outcomes verified nurses' positive effects in changing patients health status and nursing contributions in the health care system as a profession.

국제간호실무분류체계(ICNP)를 이용한 간호기록 분석 - 심장내과 간호기록을 중심으로 - (Crossmapping of Nursing Problem and Action Statements in Nursing Records with International Classification for Nursing practice)

  • Purpose: this study is to explore how useful ICNP nursing phenomena and actions classification is to describe the nursing problem and nursing action statements of nursing records. Method: The number of nursing phenomena statements found in this research were 323. Out of these 323, 222 statements can be fully classified, 62 statements can be partially classified, and 39 statements can not be classified at all by terms from the ICNP phenomena classification axis. Result: The number of nursing practice statements were 318, 252 of which can be fully classified, 63 statements can be partially classified, 3 statements cannot be classified at all by terms from the ICNP nursing action classification axis. Conclusions: In order to describe all the statements found in nursing records, not only new terms but also new axis need to be added to the ICNP.
