Exploring the Direction of Improvement in Consideration of the Talent Image of Each School in the Science High School Admissions (과학고등학교 입학전형에서 학교별 인재상을 고려하는 개선 방향 탐색)

  • This study aims to explore the direction of improvement that considers the talent image of science high schools in the admissions process. To this end, first, the talent image and competencies of science high schools were divided into 'publicity' and 'efficiency,' 'execution' and 'achievement' types, and 'publicity & execution' was predominant in most cases. Next, a total of 28 science high school teachers, education experts, and middle school teachers were surveyed on the degree of reflection of talent image and the direction of improvement in the admissions process of three science high schools. The results of the survey are as follows: first, the evaluation of 'publicity' was very limited in the first stage of admissions process, and selection based on 'achievement' was high. Second, there was a strong tendency to select based on 'efficiency & achievement' in the second stage of interview evaluation. Third, we need to improve the selection of students who fit the 'publicity & execution' type of talent image by introducing 'publicity & execution' type of talent image by introducing 'write a competency-oriented self-introduction letter, reflect other subjects' grades and comparative studies in the school life record, and experts' interviews' to the first-stage of admissions process. Fourth, convergence and open-ended questions that enable divergent thinking should be jointly submitted to reflect the 'publicity & execution' type of talent image in the second stage of interview evaluation. In conclusion, the direction of improvement that considers the talent image of science high schools in the admissions process should be changed to competency-oriented in the first-stage of admissions process and in the second stage of interview evaluation consisting of open-ended questions.

  • The purpose of this study supplies the basic data to plan the optimum mental health service that is reflected needs of the elderly people with mental illness by checking how their demographic-sociological distinctive feature and function level have an effect on their needs of mental health service by being absent situation of a study of the elderly people with mental illness. As results of analysis, the residental type, the disease period, and the decision person of admission-discharge control of demographic-sociological distinctive feature is much effect on needs of programs and also effect commonly on the needs of the health promotion program and the discharge support program. Generally, the function level of the elderly people with mental illness is favorable and the order of function level is the group activity program, the counseling program, and the resource network program.

Clinical outcome of acute myocarditis in children according to treatment modalities

  • Purpose: There is currently little evidence to support intravenous immune globulin (IVIG) therapy for pediatric myocarditis. The purpose of our retrospective study was to assess the effects of IVIG therapy in patients with presumed myocarditis on survival and recovery of ventricular function and to determine the factors associated with its poor outcome. Methods: We reviewed all consecutive cases of patients with myocarditis with left ventricular dysfunction verified by echocardiogram who had visited 3 university hospitals between January 2000 and September 2009. These patients were divided into 2 groups. Group 1 consisted of 23 patients (69.6%) who received IVIG alone or IVIG in combination with steroids, and group 2 consisted of 10 patients (30.3%) who received neither IVIG nor other immunosuppressive agents. Clinical manifestations, laboratory results, echocardiographic findings, and outcomes were compared between these 2 groups. Results: One year after the initial presentation, the difference in the probability of survival did not show statistical significance in IVIGtreated patients ($P$=0.607). Of the echocardiographic parameters on admission, a shortening fraction of less than 15% was associated with unremitting cardiac failure. Furthermore, anemic patients were more likely to have elevated N-terminal fragment levels of the B-type natriuretic peptide (NT-proBNP) in the progressed group ($P$=0.036). Conclusion: There was no difference between the IVIG-treated patients and the control patients in the degree of recovery of left ventricular function and survival. Prospective, randomized, clinical studies are needed to elucidate the effects of IVIG treatment during the acute stage of myocarditis on ultimate outcomes.

Medical Service Factors Associated with Patient Satisfaction: The Difference between Inpatient and Outpatient of A Medium-sized Hospital (환자만족도에 영향을 미치는 의료서비스 품질 요인: A 중소병원 입원.외래환자 간 차이)

  • This study attempts to analyze medical service factors associated with patients' satisfaction, and its difference between inpatients and outpatients in A medium-sized hospital located in rural area. Questionaries have been collected from 411 patients of 96 inpatients and 315 outpatients. The survey was carried out for 4 weeks, from August 20, 2011. The study found that hospital awareness was only factor related to patients' satisfaction in inpatients. In the case of outpatients, several factors were related to patients' satisfaction. They are included qualified health care providers, infirmary accommodations, staff kindness, hospital awareness, and convenient procedure of treatment and admission. Based on the findings, it is desirable to differentiate the strategy in improving patients' satisfaction between patient type; either inpatient or outpatient to being competitive.

Factors Affecting the Downward Mobility of Psychiatric Patients: A Korean Study of National Health Insurance Beneficiaries

  • Objectives: The purpose of this study is to examine the magnitude of and the factors associated with the downward mobility of first-episode psychiatric patients. Methods: This study used the claims data from the Korean Health Insurance Review and Assessment Service. The study population included 19 293 first-episode psychiatric inpatients diagnosed with alcohol use disorder (International Classification of Diseases, 10th revision [ICD-10] code F10), schizophrenia and related disorders (ICD-10 codes F20-F29), and mood disorders (ICD-10 codes F30-F33) in the first half of 2005. This study included only National Health Insurance beneficiaries in 2005. The dependent variable was the occurrence of downward mobility, which was defined as a health insurance status change from National Health Insurance to Medical Aid. Logistic regression analysis was used to assess factors associated with downward drift of first-episode psychiatric patients. Results: About 10% of the study population who were National Health Insurance beneficiaries in 2005 became Medical Aid recipients in 2007. The logistic regression analysis showed that age, gender, primary diagnosis, type of hospital at first admission, regular use of outpatient clinic, and long-term hospitalization are significant predictors in determining downward drift in newly diagnosed psychiatric patients. Conclusions: This research showed that the downward mobility of psychiatric patients is affected by long-term hospitalization and medical care utilization. The findings suggest that early intensive intervention might reduce long-term hospitalization and the downward mobility of psychiatric patients.

Factors Affecting the Profitability of Private Hospitals in Korea (민간병원의 수익성 관련요인)

  • This study was performed to identify factors affecting the profitability of private hospitals in Korea different and to make informations that could be very helpful to hospitals in improving profitability. Data used in this study were collected from 112 hospitals with complete general data of present conditions as well as financial statements(balance sheets, income statements). They were chosen from hospitals that passed the standardization audit undertaken by the Korean Hospital Association from 1998 to 2001 for the purpose of accrediting training hospitals. The dependent variables were used operating margin to total assets and operating margin to gross revenues as proxy indicators for profitability. The independent variables were ownership type, location, bed size, period of establishment, debt to total assets, current ratio, fixed ratio, total asset turnover, average length of stay, bed occupancy rate, admission ratio of outpatients, personnel costs per adjusted inpatient, and fiscal years. The factors had significantly positive effect on operating margin to total assets and operating margin to gross revenues were bed size, total asset turnover. And the factors had significantly negative effect on operating margin to total assets and operating margin to gross revenues were period of establishment, debt to total assets, average length of stay, personnel costs per adjusted inpatient. The adjusted $R^2$ of multiple regression equation was 25.2%, 21.4% respectively. It is very important for private hospitals to improve profitability by raising total asset turnover, and reducing debt to total assets, average length of stay, and personnel costs per adjusted inpatient using the rational and efficient business strategy.

A rare type of single coronary artery with right coronary artery originating from the left circumflex artery in a child

  • The presence of a single coronary artery is a rare congenital anomaly; such patients often present with severe myocardial ischemia. We experienced the case of a 13-year-old girl with the right coronary artery originating from the left circumflex artery. She visited our Emergency Department owing to severe chest pain; her cardiac enzyme levels were elevated, but her initial electrocardiogram (ECG) was normal. Echocardiography showed normal anatomy and normal regional wall motion. When she presented with recurrent chest pain on admission, the ECG showed significant ST-segment elevation in the left precordial leads and inferior leads with ST-segment depression in aVR lead, suggesting myocardial ischemia, and her cardiac enzyme levels were also elevated. We performed coronary angiography that showed a single right coronary artery originating from the left circumflex artery without stenosis. We confirmed the presence of a single coronary artery using coronary computed tomography. In addition, the treadmill test that was performed showed normal results. She was discharged from the hospital without any medications but with a recommendation of a regular followup.

The Continuum of Rehabilitation Care and the Rehabilitation Services that are Needed by People with Severe Disabilities (재가 중증 장애인의 지속적 재활 관리 실태와 재활 서비스 요구)

  • Objectives: As the number of people with disabilities is increasing and their needs for care are varied, the continuum and comprehensiveness of their rehabilitative care are getting more important. This study was performed to understand the utilization of rehabilitation services and requirements of care among the people with severe disabilities in Korea. Methods: We interviewed 578 disabled persons who had severe extremity and cerebral impairment in the urban and rural areas of Korea. The questionnaire included questions on their general characteristics, the type of disability, their physical function (ADL, IADL), their use of rehabilitation services after discharge and their requirements for rehabilitation care Results: Only 12.6% of people with disability in the community continuously used the available medical rehabilitation care. The associated factors for utilization of rehabilitation services were pain and admission for rehabilitative treatment in the acute phase. There was a great need for rehabilitation services in community and this varied according to gender, the socio-economic status, the functional status and the geographic region. The gap between utilization and need for rehabilitation services was largest in the economic support. The gap of primary health care was larger in the rural area than in the urban area. Conclusions: The needs for rehabilitation service were diverse according to the individual functional status, the regional characteristics and other general characteristics of people with disability. Strategies should be considered to eliminate the barriers to obtain rehabilitation services for the people with disability in the community.

A Retrospective Clinical Study of Patients who visited the Emergency Room of Korean Medicine Hospital affiliated with the Korean Medical School (한의과대학 한방병원 한방응급실에 내원한 환자에 대한 후향적 임상 분석)

  • Objectives: The purpose of this study was to identify the characteristics of patients who visited the emergency room (ER) of a Korean medicine (KM) hospital and to provide basic data for further research. Methods: We conducted a retrospective analysis using electronic medical records of 263 patients who visited the ER of OO University Oriental Hospital from January 1 to December 31, 2019. Results: Of the 263 patients' medical records, 245 were included in this analysis. The male-to-female ratio was 1.09:1. The average age was 52 years, with patients in their 40s having the largest distribution. The district where the hospital was located had the largest number of patients. The distribution by visit time was the lowest in the early morning. Most patients visited on Sunday, in September, and on the day of onset. The admission rate was 39.18%, and largest number of patients was admitted to the Department of Acupuncture and Moxibustion. The percentage of patients who revisited the KM hospital as an outpatient were 26.17%. The diagnosis for most patients was musculoskeletal disease. Acupuncture was the most common treatment, accounting for 62.45% of the cases, followed by herbal medicines, accounting for 54.69% of the cases. National health insurance was the most common type of insurance registered. Most patients visited the ER of the KM hospital via the ER of the hospital. Conclusions: This clinical analysis helped us identify the characteristics of patients visiting the ER of a KM hospital. Continuous data accumulation is required in relation to this for further studies.

The Influence of Job Engagement on Customer-Orientation: Mediating Effect of Organization Trust (조직신뢰가 고객지향성에 미치는 영향 -직무열의의 매개영향을 중심으로-)

  • The purpose of this research is to investigate the influence of institution trust on customer-orientation of hotel practician. Comparison between two types of governance structure demonstrated that the relationship between institution trust and customer-orientation were significantly different each other. As hypothesized, influence of organization trust on customer-orientation was much stronger in privately owned organizations than that of public ownership organizations. In the test of mediating effect of job engagement between two variables were statistically significant Based on the results from SEM model, we can suggest that improvement of institution trust need to be emphasized through the rational introduction of customer-oriented system, establishing priority task system according to individual business criticality, admission and enhance of fault. Empirical evidences from structural equation model support the governance type positively influence the job attitude in service organizations.
