지방의료원의 재무적 효율성과 공공성 향상을 위한 관련 요인 분석 (Financial Integrity Strategies for Sustainable Development of Local Public Medical Centers: Focused on Financial Efficiency and Publicness)

  • The objective of this study is to investigate financial integrity strategies for sustainable development of local public medical centers, and particularly focus on seeking ways to enhance its financial efficiency and publicness. The data which was collected from 33 local public medical centers was analyzed by Data Envelopment Analysis to measure its financial efficiency. Then, Matrix Analysis was used to examine the association of financial efficiency and publicness of local public medical centers with related factors. In the aspects of facilities and location, according to the results, the local public medical centers which have larger number of available hospital beds or located in bigger cities were examined to have higher degree of publicness. In the aspect of human resources, greater number of doctors made both financial efficiency and the degree of publicness decreased, whereas higher participation rate of educational program for doctors affects increasing its financial efficiency and publicness. Lastly, in the aspect of costs, higher labor, material, and administrative cost diminished financial efficiency, but enhanced the degree of publicness. Based on these results, this study concluded that enhancing the publicness of local public medical centers should be pursued by increasing the accessibility with better facilities and location, and also concurrently organizing rational expenditure structure with appropriate cost investment to the resources of local public medical centers. Also, it is necessary to enhance both financial efficiency and publicness simultaneously by improving the quality of health care services through the educational programs for medical staffs.

국립대학교병원의 DEA 효율성과 경영성과 간의 관계 (Relationship between DEA Efficiency and Management Performance of National University Hospitals)

  • This study, an examination of the plan for effective resource management for national university hospitals, was performed to determine the relationship between the effectiveness of patient output by considering input personnel using the DEA model and Management Performance, in which 4 years of data from 12 national university hospitals were analyzed. Among the components of the effectiveness, utilizing properly imput components from the application of VRS(Variable Returns to Scale), the Management Performance of national university hospitals was found to be affirmatively influenced by PTE(Pure Technical Efficiency). Regarding the Management Performance of national university hospitals in their attempts to establish a plan for effective human resource management, the management of patient numbers on the basis of calculation level with input is more recommendable than the controlling of personnel input using patient numbers. Thus, the establishment of a strategy plan for the proper allocation of administrative staff and sickbeds, considering the variation of patient numbers, is suggested.

친환경양식어업육성 민간보조사업비 집행효율 증대 방안 (A Study on the Efficiency of Execution of the Private Subsidy for Environmentally Friendly Aquaculture)

  • The Ministry of Maritime Affairs and Fisheries is supporting the "Environmentally Friendly Fisheries Promotion and Private Assistance Project" to increase production of eco-friendly, high-value-added foods. Private-backed operators selected for the project will have an economic opportunity to build, expand or refurbish eco-friendly fishing grounds. To facilitate this, the scope of project support was expanded after 2015 and an opportunity was prepared for private subsidy projects to grow to a new level in terms of quality and quantity by switching to a public offering project. However, starting in 2017, demand from the private sector will plunge and selected projects will be delayed or the project implementation rate will be lowered. It was a time for subsidy-related laws and systems to be reinforced but private subsidy projects do not temporarily shrink simply due to the increased administrative burden on private subsidy operators on subsidy management. It plans to review the correlation between laws and systems related to the construction of fish farms and suggest ways to enhance the efficiency of implementation so that economic advantages, the biggest advantage of the private subsidy project, can be recovered within the current legal system.

수도권 대학병원 행정 직원의 직무만족에 미치는 영향 (The Influence on Metropolitan University Hospital Administrative Staff Job Satisfaction)

  • 본 연구는 의료 환경이 공급자 중심에서 소비자 중심으로 변화함에 따라 행정직원에게 동기를 부여하여 직무만족도를 향상시킴으로써 경영의 효율성을 높이고자 하였다. 연구대상은 수도권 대학병원 행정직원 총 305명으로 설문된 데이터를 이용하였다. 분석방법은 AMOS 21 Ver.을 사용하여 구조모형방정식(SEM)으로 통계적 검증을 실시하였다. 첫째, 모형의 적합도 검증결과 $X^2=206.776$, df=77, Q=2.685, P<.001, GFI=.915, TLI=.927, CFI=.946, RMSEA=.074로 분석 되었으며, 본 연구를 검증하는데 타당한 것으로 나타났다. 둘째, 성과보상과 근무환경은 동기부여에 통계적으로 정(+)의 방향으로 영향을 미친다는 점이 나타났다. 셋째, 동기부여는 직무만족에 통계적으로 정(+)방향으로 유의하게 나타났다. 넷째, 의사소통이 직무만족에 정(+) 방향으로 영향을 미친다는 점이 통계적으로 유의하게 나타났다. 다섯째, 매개효과 검증 결과, 성과보상과 직무만족의 관계에 있어 동기부여는 완전매개로 나타났고, 근무환경과 직무만족의 관계에 있어 동기부여는 부분매개로 나타났다. 공정하고 합리적인 성과보상과 직장 안정성, 복리후생 등의 근무환경, 조직내외의 원활한 의사소통은 직무만족도를 높인다. 이를 위해서는 경영자의 관심과 의지가 필요하며, 다양한 교육훈련과 프로그램개발이 필요하다. 본 연구를 통하여 병원 행정 직원의 만족도 향상을 위한 제도적 개선 방안을 모색하는 기초적인 자료를 제공하고자 한다.

Railway Governance and Power Structure in China

  • Over the last $15{\sim}20$years, many countries have adopted policies of railway privatization to keep up with increasing competition from road and air transport. Although each country and case has its own history, market characteristics, political context as well as administrative process, railway privatizations (including railway restructure, concession etc.) in the west usually are accompanied with the establishment of new regulatory regimes. Therefore, railway governance has been innovating towards an interaction of government, regulator, industry bodies, user groups, trade unions and other interested groups within the regulatory framework. However, it is not the case in China. Although China had seen a partial privatization in some branch lines and is experiencing a much larger-scale privatization by establishing joint-ventures to build and operate high-speed passenger lines and implementing an asset-based securitization program, administrative control still occupies absolutely dominant position in the railway governance in China. Ministry of Railway (MOR) acts as the administrator, operator as well as regulator. There is no national policy that clearly positions railway in the transportation network and clarifies the role of government in railway development. There is also little participation from interested groups in the railway policy making, pricing, service standard or safety matter. Railway in China is solely governed by the mere executive agency. Efficiency-focused economic perspective explanation is far from satisfaction. A wider research perspective from political and social regime is of great potential to better explain and solve the problem. In the west, separation and constrains of power had long been established as a fundamental rule. In addition to internal separation of political power(legislation, execution and jurisdiction), rise of corporation in the 19th century and association revolution in the 20th century greatly fostered the growth of economic and social power. Therefore, political, social and economic organizations cooperate and compete with each other, which leads to a balanced and resonable power structure. While in China, political power, mainly party-controlled administrative power has been keeping a dominated position since the time of plan economy. Although the economic reform promoted the growth of economic power of enterprises, it is still not strong enough to compete with political power. Furthermore, under rigid political control, social organizations usually are affiliated to government, independent social power is still too weak to function. So, duo to the limited and slow reform in political and social regime in China, there is an unbalanced power structure within which political power is dominant, economic power expanding while social power still absent. Totally different power structure in China determines the fundamental institutional environment of her railway privatization and governance. It is expected that the exploration of who act behind railway governance and their acting strength (a power theory) will present us a better picture of railway governance as a relevant transportation mode. The paper first examines the railway governance in China and preliminarily establishes a linkage between railway governance and its fundamental institutional environment, i.e. power structure in a specific country. Secondly, the reason why there is no national policy in China is explored in the view of political power. In China, legislative power is more symbolic while party-controlled administrative power dominates political process and plays a fundamental role in Chinese railway governance. And then, in the part three railway finance reform is analyzed in the view of economic power, esp. the relationship of political power and economic power.

Super-SBM 및 Tobit 모델을 기반으로 한 중국지역 환경효율성 평가 및 영향요인 연구 (Evaluation of regional Eco-Efficiency and its influencing factors in China: Based on Super-SBM and Tobit model)

  • 본 연구에서는 2011년부터 2021년까지 중국의 31개 성(省)급 행정구역을 연구 대상으로 삼았으며, 자본, 노동, 토지 및 자원 투입을 투입 변수로, GDP 및 녹색 범위를 예상 산출량으로, 폐수, 폐가스 및 고형 폐기물 등의 배출을 바람직하지 않은 산출물로 하고, 지역 환경효율성을 측정하기 위해 super-SBM 모형을 이용하였다. 외부 환경요인이 환경효율성에 미치는 영향을 분석하기 위해 토빗 회귀분석을 이용하였다. 그 결과 중국의 평균 환경효율성 수준은 낮았고 동부지역의 환경효율성은 다른 지역보다 높았으며 서부, 북동부 및 중부 지역에서 큰 차이가 있었다.

행정서비스 활용을 위한 디지털 트윈 정책 연구 (Research on the Digital Twin Policy for the Utilization of Administrative Services)

  • 본 연구의 목적은 행정서비스 활용을 위한 디지털 트윈 정책 연구이다. 본 연구는 1,000명의 대상을 기반으로한 모바일 설문조사를 통해 진행되었으며 그 연구 결과는 다음과 같다. 첫쨰, 디지털 트윈을 활용하기 위해 경기도 측면에서 적용할 수 있는 적절한 서비스 발굴이 선행되어야 한다. 경기도의 현장 업무에 적절한 디지털 트윈 서비스 발굴 시도가 우선적으로 선행되고 이를 통한 업무의 효율성 증진이 필요하다. 둘쨰, 경기도 디지털 트윈 행정서비스는 중앙정부 사업과의 중복성은 방지하고 연계 활용 가능한 모델을 구축해야 하며, 도와 시군의 현안 연관성이 높고 수요자 즉 도민이 원하는 행정서비스를 중심으로 추진되어야 한다. 셋쨰, 행정서비스 운영방식은 경기도의 디지털 트윈 행정서비스 개발을위해서 시범사업 참여를 통한 표준모델 구축 방안을 검토해야 한다. 사업 추진 방식으로 경기도가 주관기관이 되어, 협약사업 방식으로 추진하고, 경기도 디지털 트윈 자문위원단을 통해 사업 추진 전반에 대한 지원 체계를 마련하라는 것을 제언하고자 한다. 넷째, 전담부서와 행정서비스 구축, 운영, 관리 등을 위한 관련 제도 마련이 되어야 한다. 경기도에서 디지털 트윈 실현을 위해서는 사업 추진 및 운영과 법·제도적 개선을 위한 다양한 역할을 수행 할 수 있는 전담 조직이 필요하며, 전담 조직 지정을 위해서 기존 부서의 확대 개편 방안과 신설 부서 운영에 대한 검토가 필요하다. 본 연구의 한계 요인은 경기도 중심의 참여자들에 대한 조사로서 향후에는 전국을 기반으로 연구하는 것을 제언한다. 본 연구의 기대효과는 디지털 트윈 서비스를 공적인 업무를 적용시 활용할 수 있는 기초 자료로 이용할 수 있다.

맘퀴스트 생산성지수를 활용한 강원도 보건소의 생산성 변화 분석(2006-2013) (Analysis of the Productivity Trend of Public Health Centers in Gangwon-do Using the Malmquist Productivity Index(2006-2013))

  • Objectives : The purpose of this study was to evaluate the productivity changes of 18 public health centers in Gangwon-do from 2006 to 2013 using the Malmquist Productivity Index(MPI). Methods : Data were collected from Statistics Korea from 2006 to 2013. The input variables were the numbers of medical, nursing and administrative personnels. The output variables were the performances of health promotion programs. Along with the traditional input-oriented DEA analysis, the MPI was calculated. Results : First, among the 18 public health centers, the productivity index of 14 public health centers was increased. Second, the annual productivity showed a 6% increase. Third, the productivity improvements were mainly caused by Scale Efficiency Change. Conclusions : Improving the productivity of public health centers requires the support and external policies of the national and local government. Internally, public health centers need to maintain scale optimization of the center. Additionally, efforts should be made to effectively use limited resources.

소지역 추정방법을 이용한 실업자 수 추정 사례연구 (Estimation of the Number of the Unemployed Using Small Area Estimation Methods)

  • 정보화 사회에서는 목표지향적이고 세분화된 통계의 필요성이 높아지고 있으나 현재 사용되는 조사체계를 이용하면 추정 분산이 커져 생산된 통계의 정확도가 낮아진다. 표본크기를 늘리면 추정분산을 줄일 수 있으나 비용이나 시간 면에서는 비효율적이다. 현재와 비슷한 규모의 표본조사구 조사와 일반 행정통계를 이용하여 일정 신뢰수준을 갖춘 통계를 생산할 수 있는 소지역 추정법에 대한 연구가 진행되어 개발 적용되고 있다. 본 연구에서는 소지역 추정법을 활용하여 대전광역시의 5개 구별 실업자 수를 추정하고 추정치의 CV 값을 계산하여 추정방법의 효율성을 비교하는 사례분석을 실시하였다. 또한 합성추정량과 복합추정량의 MSE를 보다 정확하게 계산하는 방법으로 잭나이프 방법을 제안하고 계산방법을 보였다.

지역사회 임상시험시 개인별 무작위배정과 군집 무작위배정의 효율성 비교 (Comparison of Efficiency between Individual Randomization and Cluster Randomization in the Field Trial)

  • Objectives . In large-scale field trials, randomization by cluster is frequently used because of the administrative convenience, a desire to reduce the effect of treatment contamination, and the need to avoid ethical issues that might of otherwise arise. Cluster randomization trials are experiments in which intact social unit, e.g., families, schools, cities, rather than independent individuals are randomly allocated to intervention groups. The positive correlation among responses of subjects from the same cluster is in matter in cluster randomization. This thesis is to compare the results of three randomization methods by standard error of estimator of treatment effect. Methods : We simulated cholesterol data varing the size of the cluster and the level of the correlation in clusters and analyzed the effect of cholesterol-lowering agent. Results : In intra-cluster randomization the standard error of the estimator of treatment effect is smallest relative to that in inter-cluster randomization and that in individual randomization. Conclusions : Infra-cluster randomization is the most efficient in its standard error of estimator of treatment effect but other factor should be considered when selecting a specific randomization method.
