A Study on the Relationship of Hypertropied Palatine Tonsil with Maxillary Sinusitis and Adenoid Vegetation (소아에서 구개편도 비대와 상악동염, 아데노이드 비대의 상관성에 관한 연구)

  • Hypertropied palatine tonsil is a common disorder in pediatric otolaryngology. It could be easily observed and adenoid vegetation could induce maxillary sinusitis by the obstruction of nasopharynx and choanae. This is a study on the relationship of the hypertropied palatine tonsil, 42cases, with maxillary sinusitis and adenoid vegetation The results was as followings ; 1. The sex distribution was male 30cases($71.4\%$), female 12cases($28.6\%$) and age distribution was from 3 to 17year-old. 2. Hypertropied palatine tonsil distribution was the degree of both 3, 2cases($4.8\%$), one 3 the other 2, 3cases($7.l\%$), both 2, 22cases($52.4\%$), one 2 the other 1, 9cases($21.4\%$), both 1, 5 cases($11.9\%$). and only one 1, 1case($2.3\%$). 3. By the PNS X-ray, there were resulted 29 cases($69.0\%$) both maxillary sintis, 4cases($9.5\%$) only one maxillary sinusitis and 9cases($21.4\%$) norma] sinus. 4. By the Head latera] X-ray, there were resulted 13cases($31.0\%$) large adenoid, 24cases($57.l\%$) mediate adenoid, and 5cases($l1.9\%$) small adenoid.

PIV Measurement on Airflows in the Abnormal Nasal Cavity with the Adenoid Vegetation (아데노이드 비대증에 의한 비정상 비강 내 유동의 PIV해석)

  • Airflow in the nasal cavity of Korean adults is investigated experimentally by PIV measurement. Quantitative data for normal and abnormal nasal cavities with adenoid vegetation are obtained. The CBC PIV algorithm with window offset is used for PIV flow analysis. Average and RMS distributions are obtained for inspirational and expirational nasal airflows. Comparisons between western and Korean nasal airflows are appreciated. Due to the difference in geometry of the frontal part of nasal cavity, the flow near nares shows the difference. For the joint research on nasal deceases, PIV measurements of nasal airflow for nasal cavities with 50% and 70% adenoid vegetation are conducted for the first time. Comparisons in nasal airflows for both normal and abnormal cases are also appreciated.

  • Knowledge of airflow characteristics in nasal cavity is essential to understand the physiological and pathological aspects of nasal breathing. Several studies have utilized physical models of the healthy nasal cavity to investigate the relationship between nasal anatomy and airflow. Since the final goal of these works is their contribution to the diagnosisand treatment of nasal diseases, the next step on this topic is naturally studies for disordered nasal cavities. In this paper, as the first application, airflows in the normal and abnormal nasal cavities with adenoid vegetation are investigated experimentally by PIV, and comparisons of both cases are appreciated. Dense CT data and careful treatment of model surface under the ENT doctor's advice provide more sophisticatedcavity model. The CBC PIV algorithm with window offset is used for PIV flow analysis. Average and RMS distributions are obtained for inspirational and expirational nasal airflows. Airflow characteristics that are related with the abnormalities in nasal cavity are presented.

Semi-longitudinal study of adenoid and jaw growth of normal occlusal children aged 6 to 17 (6세에서 17세 사이의 정상 교합 아동의 아데노이드와 악골의 성장에 관한 준종단적 연구)

  • Reduced nasal breathing can influence the growth at)d development of facial structures. It nay have many causes, and enlarged adenoid is the most frequent one. To investigate the effects of adenoids to jaw growth, we must first understand the normal growth of adenoids and jaws, and the relationship between size of adenoids and the values lot the jaw variables. The purpose of this study is to present a more objective standard of nasopharyngeal size and jaw dimension at each bone age, by using Cervical Vertebrae Maturation Index(CYMI) of Hassel, from normal occlusion children aged 6 to 17. The results of this study suggests as follows : 1. At same bone age, female's chronologic age was about 2 year older than male. 2. There was a growth peak of nasopharyngeal(NP) height and depth between CVMI 1 to 2 in male, hut in female NP height and depth gradually increase through CVMI 1 to 6. 3. Relative airway of nasopharynx increases the most between CVMI 1 to 2 period in both gender 4. Among adenoid measurements, Ad2-related variables and upper pharynx, and among dentofacial measurements inter canine width in both arch, maxillary intermolar width and palatal depth had high correlation coefficient with adenoid percentage.

A Study on the Causes of Child Hearing Loss Under Age 5 in Korea (우리나라 유아난청의 원인에 관한 고찰)

  • This paper was designed to investigate the causes of child hearing loss which is essential to language development especialy in young childhood. Accurate history taking, precise otolaryngological examination and impedence audiometry were performed on 185 hard of hearing children under age 5 during last 8 months and investigated its causes. The results were as follows: 1. Infections diseases were the most common causes of sensorineural hearing loss (44/123cases, 30.1%), and mother's condition during pregnancy the second, and birth injury or head trauma the third. 2. Acute or chronic ear ear diseases were the most common causes of counductive hearing loss (32/62, 51.6%), frequent URI the second (19/62, .30.6%), and adenoid vegetation the third (7/62, 11.3%). 3. The majority of causes of child hearing loss under age 5 was exogenous (179/185 cases, 96.8%), and the minority was endogenous (6/185 cases, 3.2%).

The Clinical Considerations of Serous Otitis Media and Ventilation Tube (삼출성중이염과 중이내 통기관 유치술의 임상적 고찰)

  • Serous otitis media (SOM) is one of the most common otologic diseases which was first discribed by Politzer in 1869. Currentely, among many methods introduced to treat SOM, ventilation tube insertion is considered to be the most popular and standard method being used. However due to complication of it, there remains many disputable various problems. In order to review the clinical aspect of SOM and search for the effects and safety of ventilation tube, we studied 97 SOM patients, who had performed V-tube from Jan. 1981 to Feb. 1982 at ENT dept. of Severance Hospital. Analizing the clinical symptoms, age distribution, associated disease, otoscopic findings of ear drum, degree of hearing loss, hearing gain after insertion of V-tube and complications, following results were obtained. 1) As a subjective symptom, among 29 cases all patients had hearing loss, 19 cases (65.6%) had tinnitus, 12 cases (44.4%) had autophony and 10 cases (34.5%) of the patients had sensation of ear fullness. 2) In age distribution, 6 to 10 year old group was most numerous up to 42 cases (43.3%) and 20 years and over was 23 cases (23.5%) 3) In adult group SOM tend to be involved unilaterally whereas young child group had tendency involving bilaterally. 4) 49 cases (50.05%) were associated with tonsillitis and adenoid vegetation, 15 cases (15.5%) were associated with sinusitis, 4 cases (4.1%) had nasal allergy. 5) In preoperative otoscopic findings, 62 ears (47.7%) had retraction, 37 ears (23.6%) had bulging, 34 ears (21.7%) had color change, and 29 ears (19.7%) had no significant findings. 6) In characteristics of middle ear fluid, child group was tend to have mucinous content (84.2%) while adult group had serous content (62.5%). 7) Average preoperative air-bone gap of pure tone was 25.3 dB. 8) 24 ears (72.7%) had over 10 dB of postoperative hearing gain and average hearing gain was 17.2 dB. 9) There were 44 ears (28.1%) of complications. Among them 37 ears (23.6%) had infection, 3 ears had atelectasis, 2 ears had granulation tissue, 2 ears had permanent perforations. 10) Among 37 ears suffered from post-op. infection, 19 ears (51.4%) had initial infection just after insertion of ventilation tube, 18 ears (48.6%) were infected during the course of post-ventilation tube. Of 37 infected ears, 26 ears (70.3%) responded to conservative care, while 11 ears (29.7%) was cured after removal of ventilation tube.
