Primary astrocytic mitochondrial transplantation ameliorates ischemic stroke

  • Mitochondria are important organelles that regulate adenosine triphosphate production, intracellular calcium buffering, cell survival, and apoptosis. They play therapeutic roles in injured cells via transcellular transfer through extracellular vesicles, gap junctions, and tunneling nanotubes. Astrocytes can secrete numerous factors known to promote neuronal survival, synaptic formation, and plasticity. Recent studies have demonstrated that astrocytes can transfer mitochondria to damaged neurons to enhance their viability and recovery. In this study, we observed that treatment with mitochondria isolated from rat primary astrocytes enhanced cell viability and ameliorated hydrogen peroxide-damaged neurons. Interestingly, isolated astrocytic mitochondria increased the number of cells under damaged neuronal conditions, but not under normal conditions, although the mitochondrial transfer efficiency did not differ between the two conditions. This effect was also observed after transplanting astrocytic mitochondria in a rat middle cerebral artery occlusion model. These findings suggest that mitochondria transfer therapy can be used to treat acute ischemic stroke and other diseases.

  • Atrial fibrillation (AF) can occur in acute coronary syndrome (ACS), which is a serious medical condition and may require the use of antiplatelet agents in addition to anticoagulants for stroke prevention. Recently, novel or non-vitamin K antagonist oral anticoagulants (NOACs) have been increasingly used for stroke prevention in patients with AF instead of traditional OACs. The duration of treatment or treatment with a stepwise approach (e.g. triple, double, or monotherapy) is determined depending on the clinical setting and the balance between the risks of ischemic stroke and bleeding. However, some concerns and controversies in the use of NOACs in patients with AF and ACS need to be addressed. Here, the current management for NOAC therapy in patients with ACS and AF will be reviewed based on recently published guidelines.

한방 의료기관에 입원한 허혈성 뇌졸중 환자의 아형 분포 (TOAST Distribution of Ischemic Stroke Patients Admitted to Oriental Hospitals)

  • Objectives: The aim of this study was to determine distribution patterns of TOAST subtypes of ischemic stroke patients admitted to oriental hospitals and to get a better understanding of present conditions in oriental medicine by comparing with the Korea stroke registry (KSR), the largest and representative data. Methods: Clinical data were collected from acute ischemic stoke patients. MRI studies including vascular images were performed in all cases. TOAST criteria were used to determine subtypes of ischemic stroke patients. According to the duration from disease onset to hospital admission time, patients were assigned to 3 groups (Group I0 to 3 d, Group II4 to 7 d, Group III8 to 28 d) and the distribution of TOAST subtypes were compared among these three groups. Results: We collected 514 sets of clinical data from 10 oriental hospitals between May 2007 and September 2009. Small vessel occlusion (SVO) subtype was the most common (57.62%), followed by large artery atherosclerosis (LAA, 29.98%). Compared with TOAST distribution of KSR, the proportion of ischemic stroke patients with SVO subtype was higher than that of KSR. On the other hand the proportion of patients with stroke of undetermined etiology (SUE) was lower. Distributions of SVO, LAA and cardioembolism (CE) in group were I 66.4%, 23.8% and 8.9%, respectively; those in group IIIwere 51.03%, 34.71% and 11.57%, respectively. Conclusions: In oriental hospitals, the proportion of ischemic stroke patients diagnosed as SVO type was higher than that of KSR. At early stage (from onset to 2 d) proportion of SVO was very high, however after 7 days from onset it decreased with concomitant increases in proportions of LAA and CE. These phenomena may be due to the facts that 1) at early stage emergency treatments are limited in oriental hospitals, 2) after early stage many patients prefer oriental treatments, including rehabilitation.

초급성 허혈성 뇌졸중에서 관류 전산화단층촬영의 임상적 적용에 대한 연구 (Clinical Application of Acute Ischemic Stroke in Perfusion Computed Tomography)

  • 초급성 허혈성 뇌졸중 환자를 대상으로 관류 전산화단층촬영(CT)의 임상적 적용을 평가하였다. 뇌졸중 증상 발현 62명의 환자를 대상으로 하였고, 관류 CT는 소뇌 기저부위에서 8 cm 상방으로 스캔하여 후처리 과정을 거쳐 뇌혈용적(cerebral blood volume, CBV), 뇌혈류량(cerebral blood flow, CBF), 평균 조영제 통과시간(mean transit time, MTT) 및 조영제 최고 도달시간(time to peak, TTP) 등의 네 가지 지도의 영상을 얻었다. 관류 CT의 CBV, CBF, MTT, TTP 지도에서 병변을 평가하였으며, 병변 부위와 정상측 대칭부위에서 MTT와 TTP를 측정하여 차이를 비교하였다. 관류 CT의 네 가지 지도 모두에서 관류결손을 인지할 수 있었고, 관류 결손이 인지되는 부위에서 MTT와 TTP의 현저한 지연이 있었다. 관류 CT의 MTT와 TTP영상이 초급성 허혈성 경계부위의 페넘브라를 반영하였다. 관류 CT의 네 가지 지도를 이용하면 뇌졸중의 조기 진단, 허혈 중심부, 허혈 페넘브라를 알 수 있게 되어 관류결손 부위의 혈류역동학적 평가가 가능함으로써, 허혈성 뇌졸중 환자의 진단 및 효과적인 치료를 위해 관류 CT가 유용하여 임상적 적용이 가능하다.

Carotid Intraplaque Hemorrhage is Associated with Acute Cerebral Ischemic Events and Progression of Stenosis on Magnetic Resonance Imaging

  • Purpose: To investigate the association of carotid intraplaque hemorrhage (IPH) with acute cerebral ischemic events and progression of stenosis using magnetic resonance (MR) imaging. Materials and Methods: From April 2014 to December 2016, 53 patients underwent carotid plaque MR imaging, including magnetization-prepared rapid acquisition with gradient-echo (MPRAGE) sequence. A total of 66 carotid arteries in 53 patients had carotid stenosis, and they were included in this study. Carotid arteries were classified according to the presence of IPH, the age of hemorrhage, and degree of stenosis. We assessed ipsilateral cerebrovascular event rates and progression of stenosis between the IPH and no-IPH groups. Results: Of the 61 carotid arteries assessed, 34 (56%) had IPH, and 27 (44%) had no IPH. Acute cerebral ischemic events were more frequent in the IPH group (47% vs. 22%, P = 0.045), especially in the < 30%-stenosis group (100% vs. 0%, P = 0.028). However, there was no significant difference in the incidence of ischemic events according to the age of hemorrhage (50% vs. 44%, P = 0.492). Among the 61 carotid arteries, 20 carotid arteries had previously undergone carotid artery imaging and were evaluated for plaque progression. The trend for progression of stenosis favored the IPH group versus the no-IPH group, with a marginal P-value ($20%{\pm}12.7$ vs. $9.6%{\pm}5.7$, P = 0.063). Conclusion: IPH was associated with an increased incidence of acute ischemic events, especially in the mild-stenosis group and it was also associated with progression of stenosis. Evaluation of the carotid IPH by carotid plaque MR could improve discrimination of carotid plaques that cause ischemic events and progression of stenosis.

중풍의 기능평가에 영향을 미치는 요인에 관한 임상적 고찰 (A Clinical Study on the Factors that Influence Functional Evaluation of Stroke)

  • Objectives: This study investigated significant factors that influence functional evaluation of stroke so as to be a fundamental data for estimating prognosis of stroke patients. Methods: 204 patients were studied within 7 days of admission, after the diagnosis of stroke through brain CT scan, brain MRI scan and clinical observations. They were hospitalized in the oriental medical hospital of Dongeui University from February to July in 2001. They were examined at the early stage of onset, after 2 weeks, 4 weeks and 6 weeks, and measured for average mark and the degree of improvement by using the Activity Index. Results: Ischemic stroke, past history of stroke, hypertension, diabetes mellitus, risk factor of obesity, non-professional emergency treatment and hospitalizing time after 1 day from onset, high blood pressure, tachycardia pulse and high blood sugar in abnormal vital sign in acute stage, conscious, cognitive or communication disorder, motor aphasia, dysphagia, constipation for more than 3 days, urinary incontinence, visual field defect, insomnia, and chest discomfort in early stage of onset had a negative influence on functional evaluation. Conclusions: Type of stroke, past history, risk factors, emergency treatment and hospitalizing time after onset, abnormal vital sign and intercurrent symptoms in Acute stage were relevant factors in predicting functional evaluation of stroke.

A Clinical Study of Korean Medical Treatment and Korean and Western Medical Treatment on Acute Ischemic Stroke Patients

  • Objectives : This is a study of ischemic stroke patients designed for comparing Korean and Western medical treatments and Korean medical treatments alone. Methods : 44 patients were diagnosed by B-MRI scan as having suffered ischemic stroke. They had entered Dong-eui Korean Medicine hospital within seven days of attack, and remained over seven days, all between May 2005 and October 2007. Patients were divided into two groups; a group treated with Korean medical treatments, and another group treated with Korean and Western medical treatments (but in both groups examinations were done and Western medications were given for hypertension, diabetes mellitus and so on) Results : NIHss change one month later was not statistically significant for either groups but the NIHss gap between them was significant. Conclusions : NIHss change one week later was not statistically significant for either group. NIHss change one month later was not statistically significant for either groups but the Korean medical treatment group had a significant NIHss gap more than the Korean and Western medical treatment group.

  • This is a study of ischemic stroke patients designed for comparison Korean-Western treatments and Korean medical treatments alone. 91 patients were diagnosed by B-MRI scan as having suffered ischemic stroke. They had entered Dong-eui Korean Medicine hospital within seven days of attack, and remained over seven days, all between May 2005 and March 2006. Patients were divided into two groups; a group treated with Korean medical treatments, and other group treated with Korean-Western medical treatments(but examinations were done and medications were given in hypertension, diabetes mellitus and so no) The Korean medical treatment group showed significant changes in NIHss after 1month. The Korean medical treatment group had insignificant improvement that measure for NIHss by treated Korean-Western medical treatment group after 1week. The Korean medical treatment group had significant improvement that measure for NIHss by treated Korean-Western medical treatment group after 1month.

Impact of Off-Hour Hospital Presentation on Mortality in Different Subtypes of Acute Stroke in Korea : National Emergency Department Information System Data

  • Objective : Several studies have reported inconsistent findings among countries on whether off-hour hospital presentation is associated with worse outcome in patients with acute stroke. However, its association is yet not clear and has not been thoroughly studied in Korea. We assessed nationwide administrative data to verify off-hour effect in different subtypes of acute stroke in Korea. Methods : We respectively analyzed the nationwide administrative data of National Emergency Department Information System in Korea; 7144 of ischemic stroke (IS), 2424 of intracerebral hemorrhage (ICH), and 1482 of subarachnoid hemorrhage (SAH), respectively. "Off-hour hospital presentation" was defined as weekends, holidays, and any times except 8:00 AM to 6:00 PM on weekdays. The primary outcome measure was in-hospital mortality in different subtypes of acute stroke. We adjusted for covariates to influence the primary outcome using binary logistic regression model and Cox's proportional hazard model. Results : In subjects with IS, off-hour hospital presentation was associated with unfavorable outcome (24.6% off hours vs. 20.9% working hours, p<0.001) and in-hospital mortality (5.3% off hours vs. 3.9% working hours, p=0.004), even after adjustment for compounding variables (hazard ratio [HR], 1.244; 95% confidence interval [CI], 1.106-1.400; HR, 1.402; 95% CI, 1.124-1.747, respectively). Off-hours had significantly more elderly ≥65 years (35.4% off hours vs. 32.1% working hours, p=0.029) and significantly more frequent intensive care unit admission (32.5% off hours vs. 29.9% working hours, p=0.017) than working hours. However, off-hour hospital presentation was not related to poor short-term outcome in subjects with ICH and SAH. Conclusion : This study indicates that off-hour hospital presentation may lead to poor short-term morbidity and mortality in patients with IS, but not in patients with ICH and SAH in Korea. Excessive death seems to be ascribed to old age or the higher severity of medical conditions apart from that of stroke during off hours.

  • Objectives : This study was aimed to investigated the relationship between warning signs and stroke subtype in acute stroke patients. Methods : From Oct. in 2005 to Oct. in 2006, 409 acute stroke patients were included. Patients were hospitalized within 14 days after the onset of stroke in DongGuk University International Hospital, Kyungwon University In-cheon Oriental Medical Hospital, Department Cardiovascular and Neurologic Diseases(Stroke center) Kyung Hee University Oriental hospital. We investigated general characteristics, stroke subtype, and warning signs(facial spasm, neck stiffness). Results : Hemorrhagic stroke had more facial spasm than ischemic stroke. (odds ratio 3.60) Small vessel occlusion had more neck stiffness than others. (odds ratio 2.03) Conclusion : To acquire more concrete conclusions on warning signs, we need further and large scale of prospective researches.
