Engraftment of Human Mesenchymal Stem Cells in a Rat Photothrombotic Cerebral Infarction Model : Comparison of Intra-Arterial and Intravenous Infusion Using MRI and Histological Analysis

  • Objective : This study aimed to evaluate the hypotheses that administration routes [intra-arterial (IA) vs. intravenous (IV)] affect the early stage migration of transplanted human bone marrow-derived mesenchymal stem cells (hBM-MSCs) in acute brain infarction. Methods : Male Sprague-Dawley rats (n=40) were subjected to photothrombotic infarction. Three days after photothrombotic infarction, rats were randomly allocated to one of four experimental groups [IA group : n=12, IV group : n=12, superparamagnetic iron oxide (SPIO) group : n=8, control group : n=8]. All groups were subdivided into 1, 6, 24, and 48 hours groups according to time point of sacrifice. Magnetic resonance imaging (MRI) consisting of T2 weighted image (T2WI), $T2^*$ weighted image ($T2^*WI$), susceptibility weighted image (SWI), and diffusion weighted image of rat brain were obtained prior to and at 1, 6, 24, and 48 hours post-implantation. After final MRI, rats were sacrificed and grafted cells were analyzed in brain and lung specimen using Prussian blue and immunohistochemical staining. Results : Grafted cells appeared as dark signal intensity regions at the peri-lesional zone. In IA group, dark signals in peri-lesional zone were more prominent compared with IV group. SWI showed largest dark signal followed by $T2^*WI$ and T2WI in both IA and IV groups. On Prussian blue staining, IA administration showed substantially increased migration and a large number of transplanted hBM-MSCs in the target brain than IV administration. The Prussian blue-positive cells were not detected in SPIO and control groups. Conclusion : In a rat photothrombotic model of ischemic stroke, selective IA administration of human mesenchymal stem cells is more effective than IV administration. MRI and histological analyses revealed the time course of cell migration, and the numbers and distribution of hBM-MSCs delivered into the brain.

성향정기산가미방을 중심으로 한 한의약 치료를 통해 단기간에 호전을 보인 전순환계 뇌경색 후 발생한 지남력장애 및 현훈을 주소로 한 환자 1례에 대한 보고 (A Case Report of a Disorientation and Dizziness with Acute Anterior Circulation Region Cerebral Infraction Improved by Korean Medical Treatment: Focusing on Sunghyuangjunggi-san-gamibang (Xingxiang zhengqi san))

  • Introduction: The aim of this study was to report the effect of Korean medicine treatments on a stroke patient with acute cerebral infarction. Method: A 63-year-old male with anterior and middle cerebral artery infarction had symptoms of dizziness and disorientation. The patient was treated with Korean medicine therapy, including the herbal medicine "Sunghyuangjunggi-san-gamibang." Treatment progress was assessed using the Korean version of the Modified Barthel Index (K-MBI) and the Korean dizziness handicap inventory (K-DHI). Results: After 14 days of treatment, the K-DHI score decreased from 78 to 7 and the K-MBI score increased from 18 to 24. The patient's main symptoms were improved after the treatment, and no side effects were observed. Conclusion: Korean medicine treatment, including Sunghyuangjunggi-san, might be a recommended therapeutic option for dizziness and disorientation in stroke patients.

시상경색으로 유발된 주시장애 1예에 관한 임상적 고찰 (A Clinical Case Report of Gaze Palsy due to Thalamic Infarction)

  • The purpose of this study is to present a case of gaze palsy due to thalamic infarction improved by acupuncture and herb medicine. Monocular gaze palsies which result from a supranuclear cerebral lesion are rare clinical manifestations. The pre-frontal cortico-oculomotor pathways travel across the thalamus and mesodienphalic junction terminating directly in oculomotor complexes. The acute disinhibition of these neurons by a posterior thalamic lesion results in a sustained but transient discharge of the medial rectus and tonic activation. Results of this study suggest a role for conservative therapy with herb medicine and acupuncture to treat gaze palsy due to thalamic infarction. Further research into oriental medical treatment for such disorders will be forthcoming.

뇌경색(腦硬塞)에 대(對)한 He-Ne laser 정맥혈관내(靜脈血管內) 조사(照射)의 효과(效果) (Effect of He-Ne laser intravascular irradiation in treatment of cerebral infarction)

  • 연구배경(硏究背景): He-Ne laser 정맥혈관내(靜脈血管內) 조사(照射) (Intravascular Laser Irradiation of Blood : ILIB)가 말초(末梢) 및 뇌혈관순환장애(腦血管循環障碍), 뇌기능장애(腦機能障碍), 동맥경화(動脈硬化) 등의 예방(豫防)과 치료(治療) 목적(目的)으로 사용(使用)되고 있으나 이에 대한 한의학(韓醫學)에서의 연구(硏究)는 찾아보기 힘들다. 이에 저자(著者)는 발병(發病) 48시간(時間) 이내(以內)에 내원(來院)하여 뇌전산화단층촬영(腦電算化斷層撮影)으로 진단(診斷)된 초기(初期) 뇌경색(腦梗塞) 환자(患者)에서 ILIB의 효과(效果)를 평가(評價)하고자 본(本) 연구(硏究)를 시행(施行)하였다. 아울러 흰쥐의 국소(局所) 뇌허혈(腦虛血)에 미치는 영향(影響)을 측정(測定)하였다. 방법(方法) : 대상(對象) 환자(患者) 40명(名)을 UC-SHJGS(우황청심환(牛黃淸心丸)-성향정기산(星香正氣散))만을 투여(投與)한 대조군(對照群)과 UC-SHJGS 투여(投與)와 ILIB를 병용(倂用)한 치료군(治療群)으로 나누어 시행(施行)하였다. 대상환자(對象患者)는 모두 변증(辨證)과 무관(無關)하게 UC, SHJGS만을 투여(投與)하고 침치료(鍼治療)는 중풍칠처혈(中風七處穴)로 한정(限定)하였으며 5일간(日間)의 치료기간(治療期間) 동안 치료군(治療群)에서는 출력(出力) 1.8-2.5mW의 He-Ne laser를 1일(日) 1회(回) 50분(分) 동안 조사(照射)하였다. 동물실험(動物實驗)은 흰쥐를 대상(對象)으로 중대뇌동맥(中大腦動脈)을 결찰(結紮)하여 국소(局所) 뇌허혈(腦虛血)을 유발(誘發)하고 대조군(對照群)과 ILIB를 실시(實施)한 실험군(實驗群)으로 나누어 그 허혈면적(虛血面積)과 부종율(浮腫率)을 측정(測定)하였다. 결과(結果) : 1. 증상호전지수(症狀好轉指數)는 대조군(對照群)과 치료군(治療群) 사이에 유의(有意)한 차이(差異)가 없었다. 2. 치료전후(治療前後) 측정(測定)한 내경동맥(內頸動脈) siphon부(部)의 혈관반응성(血管反應性)은 대조군(對照群)과 치료군(治療群)에서 모두 유의성(有意性)있는 변화(變化)가 없었다. 3. 치료전후(治療前後) 측정(測定)한 요골동맥(橈骨動脈)의 혈관반응성(血管反應性)은 대조군(對照群)과 치료군(治療群)에서 모두 유의성(有意性) 있는 변화(變化)가 없었다. 4. 치료전후(治療前後) 측정(測定)한 PT, a-PTT은 대조군(對照群)과 치료군(治療群)에서 모두 유의성(有意性)있는 변화(變化)가 없었고, fibrinogen은 치료군(治療群)에서 치료전(治療前)보다 유의성(有意性) 있게 증가(增加)하였으나 평균값이 정상(正常) 범위(範圍)에서 벗어나지 않았다. 5. 국소(局所) 뇌허혈(腦虛血) 동물(動物) 실험(實驗)에서는 실험군(實驗群)에서 신경학적(神經學的) 결손(缺損), 허혈(虛血) 면적율(面積率), 부종율(浮腫率)이 유의성(有意性)있게 감소(減少)하였다 결론(結論) : 이상(以上)의 연구(硏究) 결과(結果) 뇌경한(腦硬寒)의 초기(初期) 치료(治療)에서 기존(旣存)의 한의학(韓醫學) 치료(治療)에 ILIB를 추가(追加)하는 것이 치료(治療) 효과(效果)를 증가(增加)시킨다는 증거(證據)는 없었으며 동물(動物) 실험(實驗)에서는 뇌경색(腦梗塞)의 치료(治療)에 응용(應用)할 수 있는 가능성(可能性)을 보여주고 있으나 그 효과(效果)와 적응증(適應症)을 구체적(具體的)으로 증명(證明)하기 위한 체계적(體系的)이고 다양(多樣)한 임상실험(臨床實驗)이 지적(持績)되어야할 것으로 생각된다.

급성기 뇌경색 환자의 생활습관과 혈장 homocysteine 농도에 관한 단면적 연구 (A Cross-Sectional Study on Lifestyle and Plasma Homocysteine Concentrations in Patients with Acute Cerebral Infarction)

  • Objectives: We tried to clarify the relationship between lifestyle and plasma homocysteine concentration. We analyzed the data of patients with acute ischemic stroke from the Hanbang stroke registry supported by the Ministry of Science and Technology of Korea. Methods: 458 subjects were recruited from the patients admitted to the Internal Medical Department of Kyunghee University oriental medical center, Dongguk University Ilsan oriental medical center, Kyungwon University Songpa oriental medical center and Kyungwon University Incheon oriental medical center from April 2007 to August 2008. We divided the subjects into four groups by quartiles of plasma homocysteine concentrations and compared the first and last quartile groups. Results: 1. In univariate analysis, the high homocysteine concentration group showed higher rate of older age, male gender, smoking, drinking and lower values in vitamin B12, folic acid, creatinine than the control group with statistical significance. 2. In multivariate analysis, older age, male gender smoking and folic acid showed a close relationship with statistical significance (each OR=3.620, OR=4.218, OR=3.558, OR=0.789). Conclusions: According to the analysis, the relationship between high homocysteine concentration and smoking in patients with acute ischemic stroke were clarified. We hope to standardize and establish criteria on homocysteine in Korea by large epidemiological survey. These study results could be utilized in the future as a basis material.

초저체온 및 순환정지하에서 Aprotinin의 안전성 (Safety of Aprotinin Under Hypothermic Circulatory Arrest)

  • 초 저체온 및 순환정지하 개심수술시 Aprotinin을 사용하는 경우 혈관내 응고와 관련된 신기능장애 등의 합병증이 증가된다는 보고가 있다. 저자들은 1992년 11월부터 95년 8월까지 초저체온 및 순환정지하에서 대 동맥 수술을 한 44례 환자 중 고농도 Aprotinin을 사용한 20명의 환자를 대상으로 수술 후 주요장기에 미치 는 aprotinin의 영향을 조사하였다. 초저체온하 순환정지 시에는 좌측대퇴동맥을 통하려 저혈류로 순환시켜 흉부대동맥내로 공기유입이 되지 않도록 하였으며 순환정지시간이 길어질 것으로 예상되거나 대동맥궁을 치 환하는 경우에는 선택적뇌관류를 하였다. 순환정지시 활성응고시간은 639초에서 1531초로 1례를 제외한 모 든 환자에서 활성응고시간을 750초 이상 유지하였다. 대상환자 20례중 수술사망은 4례에서 발생하였다. 사망원인은 출혈 1례, 폐출혈 1례, 좌 관상동맥 박리에 따른 좌심실 기능부전이 1례, 근리고 다발성 뇌경색이 1례 있었다. 수술 후 뇌손상이 2례에서 발생하였으나 1례는 수술전 부터 자측 총경동맥 박리가 원인이었고 1례에서는 그 원인을 알 수 얼었다. 생존한 환자중 수 술 후 신 기능이나 간기능의 이상은 얼\ulcorner다. 결론적으로 초저체온 및 순환정지를 이용한 대동맥수술시 avotinin을 사용한 결과 ACT를 750초이상 충분 히 유지하고, low flow retrograde perfusion을 유지하는 경우 신기능의 장애를 포함한 혈관내 응고와 관련된 합 병증이 증가되지 않고, 안전하게 사용할 수 있는 것으로 나타났다.

중풍환자에서 양격산화탕에 의한 세포활성 물질 생성의 조절 효과 (The Effect of Yangkyuk-Sanhwa-Tang on Cytokine Production in the Patients with Cerebral Infarction)

  • Yangkyuk-Sanhwa-Tang(YST) has been widely used as a formula for the Soyangin cerebral infarction (CI) patients according to Sasang constitutional philosophy. Brain cells produce cytokines and chemokines during the inflammatory process after stroke both in animal models and in patients. Previously, regulation of serum cytokine levels by YSThas been observed in individuals at the acute stage of CI disease, but there have not been other scientific investigations on YST. The author investigated the effect of YST on theproduction of various cytokines using peripheral blood mononuclear cells (PBMCs)from the Soyangin (CI) patients, and Soyangin normal group. The cytokine production was analyzed using enzyme-linked immunosorbent assay (ELISA). The amount of interleukin (IL)-1, IL-1, IL-6, IL-8, and tumor necrosis factor (TNF)- in culture supernatant significantly increased in the LPS-treated cells compared with unstimulated-cells (P < 0.05). However, in LPS-stimulated PBMCs, cytokines level in CI patients group was higher than that of normal group. YST (1 mg/ml) significantly inhibited IL-1, IL-1, and IL-8 production in PBMCs stimulated with LPS (about 85% for IL-1, 87% for IL-1, and 53% for IL-8, P < 0.05), but did not significantly inhibit IL-6 and TNF- production in the CI patients group. We also show that YST significantly increased LPS-induced IL-1, IL-6, and TNF- production in the normal group. Thesedata suggest that YST has a regulatoryeffect on the cytokine production, which might explain its beneficial effect in the treatment of CI.

A case of tacrolimus-induced encephalopathy after kidney transplantation

  • We present a case of tacrolimus-induced encephalopathy after successful kidney transplantation. An 11-year-old girl presented with sudden onset of neurologic symptoms, hypertension, and psychiatric symptoms, with normal kidney function, after kidney transplantation. The symptoms improved after cessation of tacrolimus. Magnetic resonance imaging (MRI) showed acute infarction of the middle cerebral artery (MCA) territory in the right frontal lobe. Three days later, she had normal mental function and maintained normal blood pressure with left hemiparesis. Follow-up MRI was performed on D19, showing new infarct lesions at both cerebral hemispheres. Ten days later, MRI showed further improvement, but brain single photon emission computed tomography (SPECT) showed mild reduction of uptake in both the anterior cingulate gyrus and the left thalamus. One month after onset of symptoms, angiography showed complete resolution of stenosis. However, presenting as a mild fine motor disability of both hands and mild dysarthria, what had been atrophy at both centrum semiovale at 4 months now showed progression to encephalomalacia. There are two points of interest in this case. First, encephalopathy occurred after administration of tacrolimus and improved after discontinuation of the drug. Second, the development of right-side hemiplegia could not be explained by conventional MRI; but through diffusion tensor imaging (DTI) and diffusion tensor tractography (DTT) of white matter tract, visualization was possible.

뇌졸증 후 급성기 운동이 뇌허혈 유발쥐의 뒷다리근 질량에 미치는 영향 (Effect of exercise during acute stage of stroke on affected and unaffected hindlimb muscle mass of cerebral ischemic rat)

  • The purpose of this study was to identify the effect of cerebral ischemia on affected(Lt) and unaffected(Rt) side of soleus, plantaris and gastrocnemius muscle mass and determine the effect of exercise on affected and unaffected side on soleus, plantaris and gastrocnemius muscle mass during acute stage of stroke. Sixteen male Sprague-Dawley rate with 200-270g body weight were randomly divided into three groups: control, stroke, and exercise after stroke(St+Ex) group. The control group received sham operation and the stroke group and St+Ex group received transient right MCA(middle cerebral artery) occlusion operation. The St+Ex groups ran on a treadmill for 20min/day at 10m/min and $10^{\circ}$ grade for 6days. During the experimental period body weight and diet intake was measured every morning. On the 7th day after operation, muscles were dissected from both affected and unaffected side of hindlimb. Cerebral infarction of stroke and St+Ex groups were identified by staining with TCC for 30 minutes. The data were analyzed by Kruskal-Wallis test and Mann-Whitney U test using the SPSSWIN 9.0 program. Significance was accepted at the level of p<0.05. The results were summarized follows : 1) There were no significant difference of the body weight on the first day of experiment among 3 groups. Whereas on the 7th day, the body weight of both stroke group and St+Ex group were significantly smaller than that of control group. Body weight of St+Ex group on the 7th day tended to be larger than that of stroke group. 2) Total diet intake of both stroke group and St+Ex group were also significantly smaller than that of control group. While total amount of diet intake in St+Ex group tended to be larger than that of stroke group. 3) The weight of gastrocnemius muscle of affected side in stroke group significantly decreased compared to that of control group and the weight of soleus and plantaris muscle of affected side in stroke group tended to decrease compared to that of control group. 4) The weight of plantaris muscle of unaffected side in stroke group significantly decreased compared to that of control group and the weight of soleus and gastrocnemius muscle of unaffected side in stroke group tended to decrease compared to those of control group. 5) The weight of gastrocnemius muscle of affected side in stroke group significantly decreased compared to that of unaffected side and there was no significant difference of the weight of soleus and plantaris muscle in stroke group between affected side and unaffected side. 6) The weight of soleus, plantaris and gastrocnemius muscle of both affected side and unaffected side in St+Ex group had a tendency of increase compared to those of stroke group. The relative weight of soleus and gastrocnemius muscle of affected side and soleus muscle of unaffected side in St+Ex group had a tendency to increase compared to those of stroke group. Based on these results, exercise during acute stage of stroke might attenuate muscle atrophy of both affected and unaffected side of hindlimb muscles.

소음인(少陰人) 울광말증(鬱狂末證)으로 진단된 혈관성치매 환자 치험 2례 (Two Case Reports on Soeumin Woolgwang-incurable Disease with Vascular Dementia)

  • 1. Objectives The purpose of this case is to evaluate the effects of Sasang constitutional medical diagnosis and treatment on Soeumin Woolgwang-incurable disease with Vascular dementia. 2. Methods We treated two patients who attacked an acute cerebral infarction with dementia. We prescripted Doksampalmul-tang(獨蔘八物湯) for their physical symptoms. The improvement of their diseases were checked through their sleep, irritability, stool and vital sign. 3. Results After the Sasang constitutional medication of Doksampalmul-tang(獨蔘八物湯) were given, their physical symptoms were improved over all. 4. Conclusions This case-studies show an efficient result of using Sasang constitutional medication in the treatment of vascular dementia.
