집단운동치료 프로그램이 치매노인의 인지기능과 일상생활동작 및 균형 수행능력에 미치는 영향 (The Effect of the Dementia Elderly Persons on the Cognitive Function, ADL and Balance-Performing Ability the Group Therapeutic Exercise Program)

  • The aim of study was to assess the effectiveness of dementia elderly persons on the cognitive function, Activities of Daily Living(ADL) and balance-performing ability the group therapeutic exercise programs. Fifteen community-dwelling subjects participated in this study. An 4-week group therapeutic exercise program, including strengthening and balance training, breathing and gait exercise, and recreation, consisted of activities related to daily living. The Mini-Mental State Examination-korean(MMSE-K), Modified Barthel Index(MBI), Berg Balance Scale(BBS), and Timed Up & Go (TUG) test were measured during before and after exercise points The results of this study were as follows : 1) After eight weeks, the MMSE-K and MBI total scores were significantly increased to more after than before(p<.05, p<.01). 2) in the MBI contents, personal hygiene, dressing, ambulation and chair, bed transfers scores were significant increased to after. 3) after BBS, TUG scores were statistically significant higher than the before scores(p<.05, p<.05, p<.01). These findings suggest that group therapeutic exercise program can be used to improve the cognitive function, ADL and balance-performing ability in dementia elderly persons.

Translation and Validation of the Activities of Daily Living Scale with Iranian Elderly Cancer Patients Treated in an Oncology Unit

  • Background: This study evaluated the validity and reliability of applying the Katz's Activities of Daily Living (ADL) scale in an Iranian sample of elderly oncologic patients following initial cancer treatment. Materials and Methods: The scale was translated with the forward-backward procedure to give an Iranian version. The ADL scale was then applied in a random sample of 400 oncologic patients aged 60 and older following initial cancer treatment. Assessment of the scale stability was twice, with a 14-days (two weeks) interval, to 30 (of the 400) eligible elderly cancer patients in March 2012. To measure treatment effects, the index was run with 150 patients in a three month recall, following oncology processing. Exploratory and confirmatory factor analysis was performed for assessment of construct validity of the Katz's ADL. Reliability was measured with internal consistency (Cronbach's alpha co-efficient), and test/retest (Spearman's r value) of the instrument. Criterion validity was evaluated by comparing the Katz with Physical Function (PF) subscale of SF 36. Known-group validity was approved by comparing of Katz' ADL between quartile groups of PF subscale of SF 36. Results: In our study the ADL demonstrated a high degree of internal homogeneity (Cronbach's alpha 0.923). There was a high correlation between scores of two time measurement of Katz's ADL (p value of two- related- samples test was 0.3). Construct validity showed a correlation coefficient of 0.572 between the ADL and PF scores. In factor analysis, 2 factors were extracted. Evidence for the reliability of the questionnaire was good and known group validity was approved by significant differences of ADL score between quartiles of the PF subscale of SF36. Conclusions: The results suggest that the Iranian version of ADL applied for oncologic older adult patients following initial cancer treatment is a reliable and a valid clinical instrument and comparable to those reported in other studies.

u-라이프케어를 위한 HMM 기반의 분산 행위 인지 알고리즘 (A Distributed Activity Recognition Algorithm based on the Hidden Markov Model for u-Lifecare Applications)

  • 본 논문에서는 일상 공간에서 발생할 수 있는 인간의 일상생활 행위(ADL: Activities of Daily Living)들을 인지하는 분산 모델을 제시한다. 사용자의 환경, 위치 및 행위 정보를 간단한 센서들이 부착된 가정용 기기 혹은 식기들을 통해 무선 센서 네트워크로 수집하며 분석하고, 이 정보를 기반으로 사용자의 생환패턴, 건강상태 등을 파악하여 이에 요구되는 라이프케어 서비스를 제공한다. 하지만 서비스의 제공을 위해서는 높은 수준의 행위인지 데이터가 요구되나 충분히 분석되어지지 않은 센싱 데이터들은 고차원 상창 추론을 위한 일상생활 행위 인지 모델의 구축을 어렵게 한다. 그러나 수집 데이터의 순서를 통해 행위를 인지할 수 있다는 것에 착안하여 센서 데이터들의 순서를 특정 행위 패턴을 분석하는 데 활용하고, 이를 기반으로 한 분산 선형 시간추론 알고리즘을 제안한다. 이 알고리즘은 가정, 사무실 및 병원과 같은 소규모 환경에서 행위를 인지하는 데 적절하다. 제안한 알고리즘의 성능평가를 위해서 MIT Media Lab에서 제공하는 공개 데이터를 사용하였으며, 75% 이상의 평균 행위 인지 정확도를 보였다.

뇌졸중 환자의 낮병원 프로그램이 도구적·일상생활활동 수행 능력과 삶의 만족도에 미치는 영향 (Effect of K-ADL·K-IADL and Quality of life in Day Hospital Program for Stroke Patients)

  • 본 연구는 낮병원 프로그램을 받은 뇌졸중 환자들의 일반적인 특성과 삶의 만족도 중 어떤 항목이 도구적 일상생활활동과 삶의 만족도에 가장 영향력을 미치는지 인과 관계 분석을 하였다. 국립재활병원에 입원한 환자 중 실험군 41명과 대조군 41명을 선정하여 도구적 일상생활활동수행능력을 평가하고 삶의 만족도 검사를 실시하였다. 연구 결과 실험군과 대조군의 도구적 일상생활활동수행능력 비교에서 실험군이 대조군 보다 높은 기능을 보였으며, 삶의 만족도 비교에서도 모든 항목에 대해 실험군이 대조군보다 높게 나타났다. K-ADL과 K-IADL 및 삶의 만족도에 일반적인 특성 항목 중 어떤 항목이 영향을 주는지 알아 본 결과 K-ADL과 K-IADL 모두 연령, 경제상태 순으로 나타났으며, 삶의 만족도에서는 경제상태, 학력, 혼인상태, 직업유무, 일상생활시 조력자 유무 순으로 나타났다. K-ADL과 K-IADL에 삶의 만족도 항목 중 어떤 항목이 영향을 주는지 알아 본 결과 K-ADL은 통증, 사회기능, 정신건강, 신체적 역할 순으로 나타났으며 K-IADL에서는 신체적 역할제한이 가장 설명력이 높은 것으로 나타났다. 이 연구를 통해 뇌졸중 장애인들이 병원에서 재활치료가 끝난 이후에 신체적 기능을 유지하고 이차 장애를 예방하며 사회통합과 삶의 만족도 향상을 도모하기 위해서 지속적인 재활치료가 강조되어야함을 알 수 있었다.

Sensor Module for Detecting Postural Change and Falls

  • In this study, a postural change detection sensor module (PCDSM) was developed to detect postural changes in activities of daily living (ADL) and falls. The PCDSM consists of eight mercury sensors that measure angle variations in $360^{\circ}$ rotation and $90^{\circ}$ tilting. From the preliminary study, the output characteristics of the PCDSM were confirmed with the angle variations of rotational motion and a tilting table. Three experiments were conducted to test rotational motion, postural changes, and falling and lying. The results confirmed that the PCDSM could effectively detect postural changes, movement patterns, and falls or non-falls.

An investigation of factors influencing the participation of stroke survivors in social and leisure activities

  • Objective: Participation restrictions are serious problems that stroke survivors experience while reintegrating into family, work, community, and social situations after participating in rehabilitation programs. The purpose of this study was to explore the factors affecting participation in activities of daily living (ADL), as well as social and leisure activities of individuals with hemiparetic stroke. Design: Cross-sectional study. Methods: The study involved 96 participants who were diagnosed with a first stroke 6 months before the study (58 men, 38 women; $60.3{\pm}14.3years$). The Berg Balance Scale, Modified Barthel Index, Manual Function test, and Activity Card Sort were used to assess static and dynamic balance function, upper limb function, level of independence, and their level of participation within the community. A regression analysis was used to identify the influence of factors affecting participation in ADL, social and recreational activities. Results: The Activity Card Sort scores were significantly affected by the Manual Function test and Modified Barthel Index scores (p<0.05). Participation in leisure activities was affected by the level of independence. Participation in social activities was affected by the balance function and level of independence of the participants. Conclusions: The results of this study have shown that participation restrictions are affected by upper limb function, balance function, and the level of independence in individuals with hemiparetic stroke.

지역사회 거주 치매 노인의 신체활동 부족 영향요인 : 2014 노인실태조사 활용 (Factors Influencing Insufficient Physical Activity in Community-dwelling Elderly Individuals with Dementia: Using a 2014 Survey of Living Conditions of Elderly Individuals)

  • Objectives : This study aimed to identify the factors influencing insufficient physical activity in community-dwelling elderly individuals with dementia. Methods : Data were analyzed using a 2014 Survey of Living Conditions of Elderly Individuals. Metabolic Equivalent Task (MET) hours were calculated using self-reported weekly frequency, duration, and types of physical activities to measure the degree of physical activity. Results : Mean MET hours were $4.03{\pm}8.59$. Factors influencing insufficient physical activity included limitations in activities of daily living (ADL), lower frequency of social group participation and unsatisfactory relationships with friends or community. When demographic factors were adjusted, patients with ADL limitation had 11.2 times higher risks of insufficient physical activities than those without. Conclusions : Community-dwelling elderly with dementia performed low levels of physical activity. Further research is needed to develop strategies to encourage physical activity participation in this population.

Factors Impacting the Physical Function of Older Adults in Korean Long-Term Care Hospitals

  • Purpose: This study was conducted to examine activities of daily living (ADL) of older adults admitted to Korean long-term care hospitals (LTCHs), and to explore the patient and organizational factors that have an impact on the ADL of this population. Methods: A secondary analysis of the Korean minimum data set (K-MDS) of patients (N=14,369) and of the profiles of LTCHs (N=358) from the Health Insurance Review and Assessment Service was done between January and July 2008. The outcome variable was ADL score 6 months after baseline assessment. Multi-level linear regression was employed to explore the patient and organizational factors that affected ADL scores. Results: Of the patients, 45.4% had a baseline ADL score of between 31 and 40, with a score of 40 indicating that the patient was entirely dependent for all items. None of the organizational characteristics were significantly associated with effects on the ADLs of older adults who had been in a LTHC for at least 6 months. However, patient characteristics, such as age, baseline ADL, frequency of physical therapy, urinary incontinence, fecal incontinence, pressure ulcers, and having a tube or catheter, were significantly associated with ADL 6 months after baseline. Conclusion: In order to maintain and improve the ADL of older adults in LTCHs, we should develop strategies to prevent urinary and fecal incontinence, pressure ulcers, unnecessary tubes or catheters, providing adequate physical therapy. Additional studies should include more detailed information regarding nursing staff, including RN hours for direct care, education level and turnover.

자가 뜸 요법이 여대생의 생리통과 일상생활 수행에 미치는 영향 (Effect of Self Moxibustion on Dysmenorrhea and Activities of Daily Living in Female College Students)

  • Purpose: This study was to verify the effect of self moxibustion on dysmenorrhea and ADL. Method: This study was undertaken from October 25th to December 31st, 2004. The subjects of this study consisted of 31 female college students attending two colleges. Of those subjects, 16 and 15 subjects were assigned to the experimental and the control groups respectively. Self moxibustion was performed daily for 7 days before the expected menstrual date, and subjects performed moxibustion on both palms. Result: After self moxibustion, the scores of dysmenorrhea and difficulty of ADL by GRS decreased significantly in the experimental group. There was a significant difference between both groups in the changes of the scores of dysmennorhea and difficulties of ADL after self moxibustion. Conclusion: Self moxibustion had a practical effect on reducing dysmenorrhea and difficulties of ADL. Accordingly, we can use self moxibustion as a useful nursing intervention in the community.
