Fracture of the clavicle and dislocation of the acromioclavicular joint occur commonly as separate injuries. However, complete acromioclavicular dislocation with an ipsilateral clavicle fracture is quite rare. We experienced a case of acromioclavicular joint dislocation associated with fracture of clavicle and brachial plexus injury treated by open reduction and internal fixation.
We experienced acromial erosion and subsequent fracture after the treatment of Rockwood type V acromioclavicular dislocation with hook plate and coracoclavicular ligament augmentation. It was treated by using a surgical technique to address an acromial fracture and subsequent losses of reduction in acromioclavicular joint with two trans-acromial cortical screws (crossbar technique). The reduction state of acromioclavicular joint could be maintained by these two screws. Our crossbar technique could be considered as a good salvage procedure for the reduction loss caused by cutout or significant erosion of acromion after insertion of clavicular hook plate.

Clavicular hook plate is known to be an effective treatment on acromioclavicular (AC) joint injury, but there have been some reports of complications, like osteolysis and bony erosion of the undersurface of acromion. Fifty-five year old male underwent open reduction and hook plate insertion on Rockwood type 5 acromioclavicular joint dislocation. He complained of protrusion of posterior acromion at 1 month after the surgery, and acromial fracture was noted in simple radiographs. The hook plate was removed and any other treatment for osteosynthesis was refused by the patient. At the 18 months after the surgery, the patient had no pain and a full range of motion with no tenderness around the shoulder joint. After two years, plain radiographs revealed complete bony union of the acromion fracture.
Fractures at the lateral end of the clavicle inevitably require surgical treatment as there is high potential for delayed union or nonunion. Acromioclavicular dislocation also requires stable and solid fixation for healing, and surgical treatment is recommended for the maintenance of joint function. The hook plate maintains the biomechanics of the acromioclavicular joint, enabling early range of motion. Therefore, for the past 10 years, the hook plate has been widely used in distal clavicle fractures and acromioclavicular joint injuries. However, the hook plate is associated with several complications, such as proximal clavicle fractures, widening of the hook hole, rotator cuff tear, subacromial impingement, and often acromial fractures. We report on two unusual cases of acromion fracture after hook plate fixation in patients with distal clavicle fracture and acromioclavicular dislocation alongside a literature review.
목적: 본 논문은 오구 쇄골 인대가 온전하면서 견봉-쇄골 관절 탈구를 동반한 오구 돌기 골절에서 봉합나사를 이용한 고정술의 소개와 임상적 결과를 알아보고자 하였다. 대상 및 방법: 26세 남자 환자로 제3형 견봉-쇄골 탈구와 동반된 오구돌기 골절을 최초로 봉합 나사를 이용한 간접적 정복술을 시행하였다. 결과: 술 후 3개월에 골절의 완전한 유합 및 정상 견관절 운동 범위 및 기능의 회복을 얻었으며, 합병증의 소견은 없었다. 결론: 봉합나사를 이용한 간접적 정복 및 고정술은 견봉-쇄골 관절 탈구를 동반한 오구돌기 골절의 치료에 있어서 안전하고 유용한 수술 방법으로 사료된다.
Background: Neer type II distal clavicle fractures have the drawback of coracoclavicular instability and insufficient distal bony fragment, thereby making it difficult to achieve adequate fixation. Although various surgical treatments have been described for Neer type II fracture, the optimal treatment remains controversial. This study reports the clinical results and usefulness of anatomical locking plate with additional K-wire fixation. Methods: A totally of 21 patients with type II distal clavicle fracture were included in the study. The surgical procedure reduced the fracture temporarily; it included insertion of one or two K-wire from the lateral margin of the distal fragment to the proximal fragment through the fracture site, followed by application and fixation of the locking plate. The bony union and migration of K-wire was evaluated in the follow-up radiography. The coracoclavicular distance and acromioclavicular joint arthrosis were assessed at the final follow-up. The Constant Score (CS) and Korean Shoulder Score (KSS) were evaluated for clinical scoring. Results: Bone union was achieved in all cases. At the final follow-up, coracoclavicular distance of the injured shoulder was increased, as compared to the intact shoulder (p=0.002), with no accompanying clinical symptoms. No K-wire migration was observed. At the final follow-up, K-wire irritation was observed in two cases and acromioclavicular arthrosis in one case, with no other adverse effects. Pain visual analogue scale, CS, and KSS were improved in all cases. Conclusions: The method of anatomical locking plate with additional K-wire fixation could be useful in achieving beneficial clinical results.
목적: 견쇄관절의 손상이나 원위 쇄골 골절의 치료에 대해서는 많은 연구가 있으나, 견쇄관절 손상을 동반한 원위 쇄골 골절의 치료에 대해서는 알려진 바가 적다. 저자들은 견쇄관절의 손상을 동반한 원위 쇄골 골절의 수술적 치료 및 결과, 예후를 평가하고자 한다. 대상 및 방법: 견쇄관절의 손상을 동반한 쇄골 외측단에서 2.5 cm 내를 침범한 원위 쇄골 골절로 수술적 치료를 받은 21명 (21예)의 환자를 대상으로 후향적 연구를 시행하였다. 단순 방사선 사진과 CT상 원위 쇄골의 골절선이 견쇄관절로 연장 (5예)되어 있거나, 7 mm 이상의 견쇄관절의 이개 (separation)가 있는 관절내 골절 (9예), 또는 건측과 비교해서 50% 이상 원위 쇄골의 상방전위가 있는 관절내 골절 (7예)을 견쇄관절의 손상이 있는 경우로 분류하였다. Craig 분류상 III형이 16예, V형이 5예였다. 금속판 (9예), 소형나사 (1예), K 강선 및 긴장대 강선 (10예) 및 경 견쇄관절 K 강선 (1예)을 사용하여 골절을 고정하였다. 건측에 비해 100%이상 원위 쇄골의 상방전위가 있었던 1예 및 골편이 작아 고정력이 약하다고 판단된 1예에서 경 견쇄관절(transarticular AC joint) K 강선 고정을 추가적으로 실시하였다. 견봉 쇄골 인대 또는 오구 쇄골 인대의 봉합이나 재건술은 시행하지 않았다. 결과: 21예 중 20예에서 골유합을 얻었으며 골유합까지의 기간은 평균 8.4주이었다. 최종 추시시 5예에서 견쇄관절의 외상성 관절염이 관찰되었고, 4예에서 견쇄관절의 이개가, 2예에서 건측과 비교하여 50% 이하의 상방전위가 남아 있었으나 견쇄관절의 이개 및 상방전위가 술전보다 증가된 예는 없었다. Kona 등의 기준에 따른 기능적 평가상 19예에서 우수, I예에서 양호, 1예에서 보통의 결과를 얻었다. 견쇄관절 손상의 분류에 따른 UCLA 점수는 I형에서 32.6점, II형에서 34점, III형에서 34.1점이었다. 술전 견쇄관절 손상 종류, 술후 잔여 관절의 이개 및 전위, 외상성 관절염의 유무와 임상 결과간에 유의한 연관성은 관찰되지 않았다 (p>0.05). 합병증으로는 K 강선 및 긴장대 강선 고정술로 고정한 1예에서 불유합이 관찰되어 술후 3개월 금속판 고정술 및 골이식술을 시행하여 골유합을 얻었으며, 금속판 고정술을 시행한 1예에서 추시 관찰 중 내고 정물의 이완이 있었으나 추가적인 치료 없이 골절이 유합되었다. 결론: 견쇄관절로 연장된 골절이 있거나 관절내 골절이 있으면서 견쇄관절 간격이 7 mm 이상 벌어진 경우 또는 정상측과 비교하여 50% 이상 원위 쇄골의 상방전위가 있는 견쇄관절의 손상이 동반된 경우라도 원위 쇄골의 정확한 정복과 견고한 내고정을 통해 만족할 만한 결과를 얻었다. 수술 전 견쇄관절의 손상 종류 및 수술 후 경도의 외상성 관절염이나 이개, 상방전위와 임상적 기능 사이에 유의한 연관성은 없었다.
Background: The purpose of this study was to evaluate the clinical outcomes and complications of hook plate fixation in acromioclavicular (AC) joint dislocations and distal clavicle fractures. Methods: We retrospectively reviewed a series of 60 consecutive patients with hook plate fixation for AC joint dislocation (group I) and distal clavicle fracture (group II). Groups I and II had 39 and 21 patients, respectively. Clinical results were evaluated using the pain visual analogue scale (VAS), simple shoulder test, and Constant-Murley scores. In addition, subacromial erosion and stiffness were evaluated as complications. Results: At the removal, the pain VAS was $2.69{\pm}1.30$ and $4.10{\pm}2.14$ in groups I and II, respectively, which were significantly different (p=0.003). The simple shoulder test score was $9.59{\pm}1.60$ and $7.81{\pm}2.67$ in groups I and II, respectively, which were also significantly different (p=0.002). Subacromial erosion was significantly more frequent in group II (14/21 patients, 66.7%) than in group I (15/39 patients, 38.5%) (p=0.037), and stiffness was also higher in group II (17/21 patients, 81.0%) than in group I (22/39 patients, 56.4%), but it was not significant. Conclusions: Hook plate fixation showed good clinical and functional results for the treatment of acute unstable AC joint dislocation and distal clavicle fracture. But, in distal clavicle fractures, there are more subacromial erosion and stiffness compare with acute unstable AC joint dislocation.
Background: The purpose of this study was to examine the clinical and radiological results of the all-arthroscopic, suture-button fixation technique to treat acute acromioclavicular (AC) joint separations. Methods: All patients with acute AC joint separations received all-arthroscopic, single suture-button (TightRope) procedure without a special guide. Postoperative Constant score (CS), pain level according to visual analogue scale, and range of motion (ROM) were evaluated. For radiological evaluation, coracoclavicular distances were measured bilaterally. Results: Between December 2010 and June 2012, 18 consecutive patients (4 women and 14 men; mean age, 29.3 years) with acute AC joint separations underwent surgical treatment after 6.4 days (range, 2-20 days) following the initial trauma. The average postoperative follow-up was 16.9 months. The mean CS was 92.4 (range, 84-96). The mean external rotation, forward flexion, and abduction were $75.8^{\circ}$ (range, $50^{\circ}-90^{\circ}$), $170^{\circ}$ (range, $150^{\circ}-180^{\circ}$), and $163.8^{\circ}$ (range, $140^{\circ}-180^{\circ}$), respectively. Five patients exhibited coracoclavicular ossifications. In two patients, superficial wound infections were successfully treated with antibiotic therapy. In one patient, a coracoid fracture was observed. No significant differences were found regarding pain, ROM, or strength parameters between both sides. The coracoclavicular distance was discovered to be approximately 2.8 mm greater on the affected side; however, this minimal reduction loss did not affect the functional results. Conclusions: The findings of this study suggests that all-arthroscopic treatment of AC joint separations using the single suture-button technique without a drill guide is safe, yielding good to excellent clinical results.
