Overview of Flow Diagnosis in a Shock Tunnel

  • In this work, an overview of flow diagnosis in a shock tunnel is made by means of using established techniques that are easy to setup, economical to arrange, and simple to measure. One flow condition was considered having Mach number of 6 at the nozzle-exit, regarded as freestream. Measured aerothermodynamic data such as shock wave speed, wall static and total pressures, surface heat flux, and shock stand-off distance ahead of test model showed good agreement with calculation. This study shows an overall procedure of flow diagnosis in a shock tunnel in a single manuscript. Outcomes are thought to be useful in the field of education and also in a preliminary stage of high-speed vehicle design and tests, that need to be performed within a short time with decent accuracy.

키넥트 센서를 이용한 실용적인 3차원 안면 진단기 연구 (Study on the Practical 3D Facial Diagnosis using Kinect Sensors)

  • Facial diagnosis based on quantitative facial features has been studied in many Korean medicine fields, especially in Sasang constitutional medicine. By the rapid growing of 3D measuring technology, generic and cheap 3D sensors, such as Microsoft Kinect, is popular in many research fields. In this study, the possibility of using Kinect in facial diagnosis is examined. We introduce the development of facial feature extraction system and verify its accuracy and repeatability of measurement. Furthermore, we compare Sasang constitution diagnosis results between DSLR-based system and the developed Kinect-based system. A Sasang constitution diagnosis algorithm applied in the experiment was previously developed by a huge database containing 2D facial images acquired by DSLR cameras. Interrater reliability analysis result shows almost perfect agreement (Kappa = 0.818) between the two systems. This means that Kinect can be utilized to the diagnosis algorithm, even though it was originally derived from 2D facial image data. We conclude that Kinect can be successfully applicable to practical facial diagnosis.

Pregnancy Diagnosis in Sows by Using an On-Farm Blood Progesterone Test

  • To improve animal production, a simple and accurate pregnancy diagnosis plays a very important role. Therefore, the purpose of this study was to develop an on-farm blood progesterone enzyme immunoassay (EIA) system for monitoring the early pregnancy in sows. Star tubes coated with mouse monoclonal anti-progesterone antibody were used for this proposed EIA system which was tested in field trials. The results could be obtained within 30 minutes either by spectrophotometry or the naked eye. Heparinized fresh blood samples collected from the ear vein of sows 17-22 days after breeding (day 0) were tested qualitatively to diagnose sows as pregnant or non-pregnant with high ( > 3 ng/ml) or low ($${{\leq_-}}3ng/ml$$) progesterone in the blood. To provided a double check data, plasma progesterone levels were also measured quantitatively by the same EIA system with some modification. Total agreement of diagnosis by the on-farm EIA kit and by farrowing or abortion from 128 tested sows was found to be 92.2% accuracy (93.1% on pregnant diagnosis and 83.3% on non-pregnant diagnosis). It was concluded that the on-farm EIA blood progesterone test is a very useful method for monitoring the early pregnancy status of sows.

구내방사선사진의 인접면 치아우식 진단에 대한 유용성 평가 (The Value of Periapical Radiograph in the Diagnosis of Interproximal Caries)

  • Purpose : To compare the diagnostic performance of clinical and radiologic examination for the interproximal caries on intraoral periapical radiographs and to evaluate the value of periapical radiographs. Methods: One hundred seven dental patients were examined clinically, with a mouth mirror and an explorer, by a dentist at the department of oral medicine, and the presence or absence of interproximal caries lesion was recorded. The patients were prescribed one or more dental periapical radiographs. Radiographs were assessed for the presence of interproximal caries by three oral and maxillofacial radiologists independantly. Two thousand sixty interproximal surfaces were included in this study. The diagnostic accuracies of clinical and radiologic examinations for interproximal caries were calculated. To assess the degree of agreement between clinical and radiologic examinations, Cohen's coefficient of agreement was computed. Results: The specificity of clinical and radiologic examination was 0.991, 0.997 and the sensitivity was 0.279, 0.985 respectively. The diagnostic accuracy of radiologic examination was statistically significantly higher than that of clinical examination (P<0.05). Cohen's kappa value of clinical and radiologic examination was 0.335, 0.942 respectively. These results suggested that clinical examination show only fair agreement, whereas radiologic examination show perfect agreement. Conclusion: The diagnositic performance of the dental periapical radiographs on interproximal caries were higher than that of clinical examination, thus this study showed the validity of periapical radiographs for detecting interproximal caries lesion without bitewing radiograph.

Accuracy of the 2008 Simplified Criteria for the Diagnosis of Autoimmune Hepatitis in Children

  • Purpose: Classical criteria for diagnosis of autoimmune hepatitis (AIH) are intended as research tool and are difficult to apply at patient's bedside. We aimed to study the accuracy of simplified criteria and the concordance with the expert diagnosis based on the original criteria. Methods: A cohort of children under study for liver disorder was selected through consecutive sampling to obtain the prevalence of AIH within the group of differential diagnoses. AIH was defined, based on classical criteria, through committee review of medical reports. Validity indicators of the simplified criteria were obtained in an intention to diagnose approach. Optimal cut-off and the area under the receiver operating characteristic (ROC) curve were calculated. Results: Out of 212 cases reviewed, 47.2% were AIH. For the optimal cut-off (6 points), the simplified criteria showed a sensitivity of 72.0% and a specificity of 96.4%, with a 94.7% positive and a 79.4% negative predictive value. The area under the ROC curve was 94.3%. There was a good agreement in the pre-treatment concordance between the classical and the simplified criteria (kappa index, 0.775). Conclusion: Simplified criteria provide a moderate sensitivity for the diagnosis of AIH, but may help in indicating treatment in cases under suspicion with 6 or more points.

디지털 유방촬영술에서 BI-RADS의 구분에 따른 알고리즘별 영상의 융복합적 평가 (Convergence of the Image Evaluation by BI-RADS Classification in Accordance with Algorithms in DR Mammography)

  • BI-RADS의 구분에 따라 Algorithm의 변화로 영상의 질을 개선하는 기법을 이용하여 시각적 가시화의 차이를 일치도와 민감도로 평가하였다. 유방촬영을 시행한 172명을 대상으로 유방촬영의 판독소견과 자료 체계의 신뢰도를 평가하였다. Category 5단계(C0,C1,C2,C3,C4), 유방 실질 함유량 4단계(Fatty, Fibroglandular, Heterogeneous nodular, Diffuse dense), 병변 3종류(석회화, 결절, 종괴)로 분류하여 TE와 PV의 신뢰도를 평가하고, 민감도와 진단의 일치도, 정확도를 평가함으로 다방면의 융복합적 분석을 하였다. TE보다 PV가 병변의 신뢰도와 민감도와 정확도가 높았다. 유방 실질 함유량에 따른 평가에서 정확도는 PV가 높았다. TE에서는 Fatty는 모두에서, Fibroglandular는 종괴와 석회화가, Diffuse dense는 결절과 석회화가 구별이 용이하였다. PV에서는 Fatty는 모두에서, Fibroglandular는 결절, Heterogeneous nodular은 결절과 종괴, Diffuse dense는 결절과 석회화가 구별이 용이하였다. 민감도는 결절은 Fatty, Fibroglandular, Heterogeneous nodular에서 TE가 더 민감했고, 종괴에서는 Heterogeneous nodular, Diffuse dense에서 TE가 더 민감했으며, 석회화에서는 모두에서 TE가 더 민감하였다. 이에 Algorithm 기법을 적절히 변화시켜 활용한다면 진단과 판독에 정확성을 높일 것이라 사료된다.

구강 악안면 영역의 암종 진단에 있어서 $[^{18}F]$-Fluorodeoxyglucose를 이용한 양전자방출 단층촬영의 임상적 연구 (CLINICAL STUDY OF POSITRON EMISSION TOMOGRAPHY WITH $[^{18}F]$-FLUORODEOXYGLUCOSE IN MAXILLOFACIAL TUMOR DIAGNOSIS)

  • Positron Emission Tomography(PET) is a new diagnostic method that can create functional images of the distribution of positron emitting radionuclides, which when administered intravenously in the body, makes possible anatomical and functional analysis by quantity of biochemical and physiological process. After genetic and biochemical changes in initial stage, malignant tumor undergoes functional changes before undergoing anatomical changes. So, early diagnosis of malignant tumors by functional analysis with PET can be achieved, replacing traditional anatomical analysis, such as computed tomography(CT) and magnetic resonance image(MRI), etc. Similarly, PET can identify malignant tumor without confusion with scar and fibrosis in follow up check. In the Korea Cancer Center Hospital(KCCH) from October 1997 to September 1999, clinical study was performed in 79 cases that underwent 89 times PET evaluation with [18F]-Fluorodeoxyglucose for diagnosis of oral and maxillofacial tumors, and the data was analysed by Bayesian $2{\times}2$ Classification Table. The results were as follows : Evaluation for initial diagnosis with FDG-PET (P<0.005) 1. Agreement rate or accuracy rate is 88.9%. 2. Sensitivity is 95.2%, and specificity 66.7%. 3. Positive predictive rate is 90.9%, and negative predictive rate 80.0%. 4. In consideration of tumor stage, diagnostic rate in less than stage II was 90% and in greater than stage III 100%. 5. In consideration of tumor size, diagnostic rate in less than T2 was 92.3% and in greater than T3 100%. After primary treatment, evaluation for follow up check with FDG-PET (P < 0.001) 1. Agreement rate or accuracy rate is 85.4%. 2. Sensitivity is 87.5%, and specificity 82.4%. 3. Positive predictive rate is 87.5%, and negative predictive rate 82.4%. 4. In 24 recurred cases, 6 had distant metastasis, and 5 of them were diagnosed with FDG-PET, resulting in diagnostic rate of FDG-PET of 83.3%. From the above results, Positron Emission Tomography with [18F]- Fluorodeoxyglucose appears to be more sensitive and accurate for detecting the presence of oral and maxillofacial tumors, and has various clinical applications such as early diagnosis of tumor in initial and follow up check and detection of distant metastasis.

The effect of radiographic imaging modalities and the observer's experience on postoperative maxillary cyst assessment

  • Purpose: The purpose of this study was to compare the accuracy of postoperative maxillary cyst (POMC) diagnosis by panoramic radiographs versus computed tomography (CT) and by oral and maxillofacial radiologists versus non-specialists. Materials and Methods: Sixty-five maxillary sinuses with POMCs and 63 without any lesion were assessed using panoramic radiographs and CT images by five oral and maxillofacial radiologists and five non-specialists on a five-point scale. The areas under receiver operating characteristic (ROC) curves were analyzed to determine the differences in diagnostic accuracy between the two imaging modalities and between the two groups of observers. The intra-observer agreement was determined, too. Results: The diagnostic accuracy of CT images was higher than that of panoramic radiographs in both groups of observers (p<0.05). The diagnostic accuracy of oral and maxillofacial radiologists for each method was higher than that of non-specialists (p<0.05). Conclusion: The use of CT improves the diagnosis of POMC, and radiological training and experience leads to more accurate evaluation.

Dental students' ability to detect maxillary sinus abnormalities: A comparison between panoramic radiography and cone-beam computed tomography

  • Purpose: To compare the diagnostic ability of undergraduate dental students to detect maxillary sinus abnormalities in panoramic radiographs(PR) and cone-beam computed tomography (CBCT). Materials and Methods: This was a retrospective study based on the evaluation of PR and CBCT images. A pilot study was conducted to determine the number of students eligible to participate in the study. The images were evaluated by 2 students, and 280 maxillary sinuses were assessed using the following categories: normal, mucosal thickening, sinus polyp, antral pseudocyst, nonspecific opacification, periostitis, antrolith, and antrolith associated with mucosal thickening. The reference standard was established by the consensus of 2 oral radiologists based on the CBCT images. The kappa test, receiver operating characteristic curves, and 1-way analysis of variance with the Tukey-Kramer post-hoc test were employed. Results: Intraobserver and interobserver reliability showed agreement ranging from substantial (0.809) to almost perfect (0.922). The agreement between the students' evaluations and the reference standard was reasonable (0.258) for PR and substantial(0.692) for CBCT. Comparisons of values of sensitivity, specificity, and accuracy showed that CBCT was significantly better(P<0.05). Conclusion: CBCT was better than PR for the detection of maxillary sinus abnormalities by dental students. However, CBCT should only be requested after a careful analysis of PR by students and more experienced professionals.

딥러닝을 기반으로 한 CAD 시스템의 갑상샘 질환의 진단 유용성 (Evaluation of Diagnostic Usefulness of Thyroid Lesions of Deep Learning-based CAD System)

  • 본 연구는 인공지능 기반 컴퓨터 진단 보조 시스템(CAD)인 S-DetectTM를 통해 진단된 갑상샘 병변과 세침흡인 검사(FNAB) 결과를 비교하여 분석하고, 진단의 일치도와 정확도를 평가하고자 한다. 2023년 5월부터 2023년 9월까지 경남 소재 N 병원 내과에서 60명의 환자를 대상으로 후향적 연구를 수행하였다. S-DetectTM를 사용하여 갑상샘 결절의 초음파 소견과 악성 위험도를 분석하고, 이를 세침흡인 검사 결과와 비교하여 정확도를 확인하였다. S-Detect 방법과 세침흡인 검사 방법 간의 민감도, 특이도, 정확도, 양성예측도 및 음성 예측도를 분석하였으며, 두 방법 간의 진단 일치도를 Kappa 분석을 통해 확인하였다. S-Detect 분석결과는 민감도 90.5%, 특이도 83.2%, 정확도 88.3%, 양성 예측도 80.7%, 음성 예측도 92.7%로 나타났다. 또한, S-Detect 방법과 세침흡인검사 방법 간의 진단 일치도 분석 결과, Kappa 값이 0.719(p<0.05)로 높게 나타났으며, 두 방법 간에 유사한 일치도를 보였다. 따라서, 인공지능 기반 컴퓨터 진단 보조 시스템(CAD)인 S-Detect는 갑상샘 병변에서 악성 결절과 양성 결절을 구별하는데 유용하며, 갑상선 세침흡인 검사 전에 적절히 활용하면 불필요한 조직 검사를 줄일 수 있을 것으로 생각한다.
