Root Cause Analysis of Construction Accident Using 5 Why (5Why를 통한 건설사고 원인분석)

  • Purpose: Looking for the root cause of a construction accident leads to the problem of unreasonable construction costs and air setting by the person placing an order. In addition, low-cost bidding by a contractor with insufficient capacity is combined, creating an inappropriate construction structure that can cause an accident before the start of construction. Method: Analysis of the problem that the lack of original contractor resources in the construction environment is passed on to suppliers, and the phenomenon that partners and their workers are forced to push ahead with excessive work to secure a minimum margin. Result: Going back to the root of construction accidents, there are several dimensions of causes from physical phenomena to root causes, but the reason why accident prevention measures so far remain almost at the one-dimensional level of responding to the phenomenon is the lack of fundamental cause analysis. Conclusion: It is necessary to shift the paradigm to safety accident measures led by the client (the client) and the government, and construction accidents are reduced only when root cause of construction accidents is found through fundamental cause analysis techniques such as 5Why.

Development of Accident Classification Model and Ontology for Effective Industrial Accident Analysis based on Textmining (효과적인 산업재해 분석을 위한 텍스트마이닝 기반의 사고 분류 모형과 온톨로지 개발)

  • Accident analysis is an essential process to make basic data for accident prevention. Most researches depend on survey data and accident statistics to analyze accidents, but these kinds of data are not sufficient for systematic and detailed analysis. We, in this paper, propose an accident classification model that extracts task type, original cause materials, accident type, and the number of deaths from accident reports. The classification model is a support vector machine (SVM) with word occurrence features, and these features are selected based on mutual information. Experiment shows that the proposed model can extract task type, original cause materials, accident type, and the number of deaths with almost 100% accuracy. We also develop an accident ontology to express the information extracted by the classification model. Finally, we illustrate how the proposed classification model and ontology effectively works for the accident analysis. The classification model and ontology are expected to effectively analyze various accidents.

Text Analytics for Classifying Types of Accident Occurrence Using Accident Report Documents (사고보고문서를 이용한 텍스트 기반 사고발생 유형 및 관계 분석)

  • Recently, a lot of accident report documents have accumulated in almost all of industries, including critical information of accidents. Accordingly, text data contained in accident report documents are considered useful information for understanding accident processes. However, there has been a lack of systematic approaches to analyzing accident report documents. In this respect, this paper aims at proposing text analytics approach to extracting critical information on accident processes. To be specific, major causes of the accident occurrence are classified based on text information contained in accident report documents by using both textmining and latent Dirichlet allocation (LDA) algorithms. The textmining algorithm is used to structure the document-term matrix and the LDA algorithm is applied to extract latent topics included in a lot of accident report documents. We extract ten topics of accidents as accident types and related keywords of accidents with respect to each accident type. The cause-and-effect diagram is then depicted as a tool for navigating processes of the accident occurrence by structuring causes extracted from LDA. Further, the trends of accidents are identified to explore patterns of accident occurrence in each of types. Three patterns of increasing to decreasing, decreasing to increasing, or only increasing are presented in the case of a chemical plant. The proposed approach helps safety managers systematically supervise the causes and processes of accidents through analysis of text information contained in accident report documents.

  • Universal Design involves designing products and spaces so that they can be used by the widest range of people possible. Universal Design evolved from Accessible Design, a design process that addresses the needs of people with disabilities. This paper introduces the universal design concept through analysis of causes of railway accident within the urban railway station. Railway accident analysis was performed by the internal facilities. We analyzed the standard of accident type, accident cause and accident subject such as elevator, escalator, stairs etc.

  • In Korean cases, the occupation of building construction is below 10% of the total industry workers but from a accident rate point of view, it is a risk serious industry and it's hazard is very considerable which accident rate is up to 30% of the total industry workers. The reason of high accident rate of the building construction is its own characteristic, hazard of the work in which is implicated and the shortage of safety counter plan and information, so it is most required to the building construction that educating, training, short and long period approaching to prevent the hazard. The safety information of building construction which is related to safety accident is divided into human and material such as hazard, work situation, cause and counter plan. The existing hazard cases imply all of the information about the work type and work progress of the building construction, so if the research of the every fact is well accomplished, we can obtain the useful information and prevent the hazard of building construction. From this point of view, the emphasis of this study is based on analyzing the cause of hazard first, suggesting the safety information each part of work type and work progress second, and finally making the all existing hazard cases to the Data Base and developing the safety information system.

  • The purpose of this study was to identify the current situation of safety accident received by Korea Consumer Agency and to provide a basis for preparing measures to prevent safety accident. This study was a descriptive study that secondary analyzes 4934 safety accidents accident information received from Korea Consumer Agency from 2013 to 2017. In this study, safety accident occurred more frequently woman, between 50-59 years old, the cause of safety accident were adverse effect after medical procedure(42.4%), the place where safety accident occurred private hospital(28.6%), integumentary system(41.3%) where safety accident occurred, symptoms of safety accident caused other organ damage and pain(13.1%) occurred most frequently and characteristics of safety accident differed depending on place of safety accident. Based on the results of this study, it is suggested to established a surveillance system that can prevent safety accident in which medical problems occur.

A Study on the Systematic Cause Analysis of Shipboard Fire Accident Case using STAMP Methodology

  • The ship system is complex and advanced, and the operation relationship between each element is very high. So it is necessary to approach it in terms of an overall and integrated system in addition to the traditional sequential approach of finding and removing the direct cause of the accident when analyzing the accident. In this study, it is analyzed the recent fire accidents on ships occurred the Korean terrestrial water using a STAMP methodology that is different from conventional accident analysis techniques. This analysis reviews a range of factors, including safety requirements to prevent fires in ships, inappropriate decisions and actions, situations, equipment defects, and recommendations derived from accident analysis results. Through a comprehensive approach to accident prevention using STAMP, alternative evaluations are presented at the component level within the entire system of ships, and they are systematically used for accident prevention and risk evaluation as well as simple accident analysis.

National Police Office which controls a traffic accident does not execute an up-to-date technique. And, it is working yet by the hand, There is to traffic accident analysis and the research regarding the analysis against the research which it follows in geography element and composition element and an accident cause is weak. Consequently, effectively establishment and it enforces a traffic safety policy and from the hazard which it evaluates traffic accident data the system and scientific analysis against a traffic accident occurrence cause and a feature in basic must become accomplished. The research which it sees constructs a traffic accident data in GIS base. It is like that, it uses the PDA where is not the collection of data of text form in existing and at real-time it converts store and an accident data rightly in standard traffic accident data form and it will be able to manage. It was related with a space data peculiarity and the research regarding the system development with the geography analysis data about an accident cause under manifesting it accomplished.

  • Objectives: Six cases of non-oral occupational methanol poisoning were found in mobile phone parts manufacturers in South Korea during the period from 22 January 2016 to 5 October 2016. We conducted this study to determine the cause of the accident including social environment factor. Methods: We interviewed the cases. Based on that, we described this poisoning accident narratively and arranged the evolution of the accident in chronological order. We used the Haddon matrix method to investigate the cause of the accident. Results: It was found that six cases were all dispatched workers who were under poor working condition including working time exceeded 12 hours a day, not given basic occupational health and safety information etc. Occupational disease associated with dispatched work was difficult to be reported. 2 cases were not able to identify the cause of the disease within one year. Conclusions: In short, the direct cause of the poisoning accident was that workers were exposed to high concentration of methanol(over 1,000 ppm). One of the main fundamental cause is that dispatched worker is rampant in the supply chain of mobile phone production. To protect dispatched workers, appropriate occupational safety health measures are needed urgently.

Characteristics Analysis of Accident Factors of UK Civil Unmanned Aircraft Using SHELL Model and HFACS (SHELL 모델과 HFACS를 활용한 영국 민간 무인 항공기 사고 요인 특징 분석)

  • The unmanned aerial vehicle industry has developed a lot, but the possibility of accidents is increasing due to potential risks. In this study, SHELL models and HFACS were used to analyze unmanned aerial vehicle accidents in the UK and to identify the main causes and characteristics of accidents. The main cause analyzed by the SHELL model was identified as an abnormality in the alarm system. The main cause of the accident analyzed by HFACS was identified as the technical environment. The common cause identified by the SHELL model and HFACS was identified as a mechanical problem of unmanned aerial vehicles. This is due to the lack of accurate information or functionality of the alarm system in the operator interface, which often prevents the operator from responding to sensitive information. Therefore, in order to prevent civil UAV accidents, the stability and reliability of the system must be secured through regular inspections of the UAV system and continuous software updates. In addition, an ergonomic approach considering human interfaces is needed when developing technologies.
