Effects of Providing Information on Stress and Maternal Role Confidence of Women with Preterm Labor (정보제공이 조기진통임부의 스트레스와 모성역할자신감에 미치는 효과)

  • Purpose: The purpose of this study was to examine the effects of providing information on Stress and maternal role confidence of pregnant women with preterm labor. Methods: The sample were 66 pregnant women with preterm labor admitted at 1 hospital in Busan. In the subjects, 33 were the experimental group and 33 were the control. Data was collected from March 1st to October 31st, 2007. A hand-out including the knowledge of preterm labor, fetus state and curing process. The tool for the stress had 14 questionnaires including the levels of anxiety and depress. And the tool for the maternal role confidence had 23 questionnaires. The data analyzed by $X^2$-test, t-test and pearson correlation coefficient with SPSS 12.0/pc program. Results: After providing information, the stress of the experimental group decreased significantly than the control(t=1.95, p=.048). And the maternal role confidence of the experimental group increased significantly than the control(t=-2.40, p=.016). There is a significant correlation between the stress and maternal role confidence(p<.01). Conclusion: The special program or educational nursing intervention should be created for each pregnant women with preterm labor and ante-natal nursing service may assessed and consulted on anxiety-depression and maternal role confidence.

Relationship between the Discomfort of Tinnitus and the Quality of Life in Tinnitus Patients (이명장애와 삶의 질의 관계)

  • Purpose: To investigate audiometery and characteristics related to tinnitus in tinnitus patients, and to understand the relationship between the discomfort of tinnitus and the quality of life. Methods: The subjects were 110 patients with tinnitus. The data were collected using tinnitus handicap questionnaire, quality of life, and pure tone audiometry, which measured hearing level in dB and frequency in Hz matching of tinnitus. Results: Majority of tinnitus patients (75.5%~81.8%) had normal auditory sensation. but 82.7% of patients indicated that they have heard tinnitus "frequently" or "always", and the major sounds of tinnitus were "wing, wung, wang". The matching sounds of tinnitus had various range in hearing level(dB) and frequency(Hz). Tinnitus patients reported moderate levels of the discomfort of tinnitus and the quality of life, and there were negative correlation between the discomfort of tinnitus and the quality of life(r=-.314, p<.001). Conclusion: These findings suggest that special attention is needed for tinnitus patients because the physical, psychological, and social impacts of tinnitus could be decrease their quality of life. Specialized nursing intervention considering specific conditions related to tinnitus is required to solve various health problems of tinnitus patients.

Evaluation of Nutritional Status of Inpatients with Medical Health Problems (내과 입원환자의 초기 영양상태 평가)

  • Purpose: To investigate nutritional status of inpatients by using subjective, and objective evaluation methods and to find the relationship between them. Method: The subjects were 101 inpatients with medical health problems at a university hospital. Nutritional status was evaluated by the Subjective Global Assessment(SGA) and physical assessment including percentage of weight loss, serum albumin, hemogloin, and hematocrit. Data were analyzed using frequency, percentage, mean, Kendall's tau. Results: Grouping by the SGA, 61.4% were classified as severe malnutrition group. When applying the objective methods(physical assessment), 1.9~42.6% were diagnosed as malnutrition each item. The percentage of weight loss during previous 1~6month(${\tau}=.43{\sim}.54$, P=.0001), serum albumin(${\tau}=-.26$, P=.0003), hemoglobin of male(${\tau}=-.38$, P=.0001), and hematocrit of male(${\tau}=-.34$, P=.0001) were significantly correlated with SGA score. The coincidence rate of nutrition evaluation between the objective methods and SGA were 27.7~35.6%, 20.8%, 47.5%, 58.4% in percentage of weight loss, albumin, hemoglobin, and hematocrit. Conclusion: These findings showed a majority of inpatients were exposed to the risk of malnutrition. We recommend to evaluate inpatients' nutritional status periodically and to develop nursing intervention to solve their nutritional problems.

The Effects of Foot Reflexology on Depression and Anxiety in Coal Workers' Pneumoconiosis (발반사 요법이 탄광부 진폐증 환자의 우울과 불안에 미치는 영향)

  • Purpose: This study was to evaluate the effects of foot reflexology on depression and anxiety in coal workers' pneumoconiosis. Method: The research design used was a non-equivalent control group pretest-posttest design. The foot reflexology was performed for 60 minutes twice a week through five weeks in the experimental group, not in the control group. The subjects of this study consisted of both the experimental group of twenty-nine and the control group of thirty coal workers' pneumoconiosis. The data were collected from December 10, 2002 to February 15, 2003. To evaluate the effects of foot reflexology the scores of depression and anxiety were measured before and after the experiment in both groups. Data of this experiment were analyzed by Chi-square test, t-test, unpaired t-test, ANCOVA and Repeated Measures ANOVA with the SAS Program. Results: The score of depression decreased in the experimental group, not in the control group. There was a significant difference of depression between the two groups. The score of anxiety decreased in the experimental group, not in the control group. There was a significant difference of anxiety between the two groups. Conclusion: As a summary of the study results, foot reflexology was regarded that it attributed to the emotional promotion of coal workers' pneumoconiosis. Therefore the foot reflexology is considered that might be used as an effective skill of nursing intervention program for coal workers' pneumoconiosis.

The Effects of Group Exercise on Fat Distribution, Lipid Profiles in Elderly Women (집단체조프로그램이 여성노인의 신체조성, 혈청지질에 미치는 효과)

  • Purpose: This study was aimed to identify the effects of group exercise on physiological functions in Elderly. Method: The design of research was one group pretest-posttest design. The subjects were sixty two elderly women persons over sixty years old to live in C-GUN, The exercise period was 12-weeks and frequency was three times per week. the exercise duration was sixty minites. In order to evaluate the effect of exercise, we measured %body fat, fat mass(FM), fat free mass(FFM), body mass index(BMI), body metabolic rate(BMR), Total Cholesterol(TC), Triglyceride(TG), high density lipoprotein-c(HDL-C), low density lipoprotein-c(LDL-C) before and after the 12 week's group exercise program. The data analyzed by the paired t-test using SPSS package. Result: There was significant difference in FFM(p=.000), BMR(p=.000), HDL-C(p=.010). There was no significant difference in weight(p=.122), %body fat(p=.062),, FM(p=.704), BMI(p=.136), TC(p=.125), TG(p=.064), LDL-C(p=.575) Conclusion: This group exercise for elderly women can be recommended as an effective nursing intervention for elderly women in a community.

Stress and Health Status of Primary Caregivers Caring for Demented Elderly (재가 치매노인을 돌보는 주 간호자의 스트레스와 건강상태)

  • Purpose: The purpose of this study was to investigate the stress level and health status of primary caregivers living with demented elderly. Method: The subject was 92 primary caregivers who had been taking care of demented elderly at home. The research tools were a stress questionnaire and health status questionnaire. The data were collected through direct interview with a questionnaire in the B city, from February to March 2004. The data were analyzed by t-test, ANOVA, Pearson's correlation coefficients and Scheffe test using SPSS/PC 11.0 program. Results: The primary caregivers living with demented elderly showed slightly high level of stress and moderate level of health status. Moreover, there was a significant correlation between the stress level and the mental and the physical health status. Conclusion: Based on the results, we recommend the development of an intervention program to decrease the stress level of primary caregivers living with demented elderly for improving their health status.

Reliability and Validity of the Clinical Judgment Rubric on Simulation Practice with a Post-operative Rehabilitation Case (수술 후 재활 사례에 적용한 임상 판단력 평가도구의 신뢰도 및 타당도)

  • Purpose: The purpose of this study was to evaluate the reliability and validity of the clinical judgement rubric on simulation practice with a post-operative rehabilitation case. Methods: Methodological study design was used to evaluate the reliability and validity of the clinical judgement rubric on simulation practice. The participants were 35 students in a college of nursing. Results: There were showed reliability and validity of the clinical judgement rubric on simulation practice with a post-operative rehabilitation case. In terms of internal consistency, the Cronbach's ${\alpha}$ for all columns was .821. Factor analysis showed that planning, intervention and evaluation domains with Eigen values of 69.87% were bound, unlike the original tool where four factors including noticing, interpretation, responding and reflecting domains were bound. Convergent validity was established by the correlation between the total clinical judgement score and critical thinking disposition before and after simulation practice. Discriminative validity was established by the correlation between the total clinical judgement score and simulation effectiveness score. Conclusion: The findings of this study suggested that clinical judgement rubric on simulation practice is reliable. Repeated studies are needed to objectively evaluate the clinical judgement rubric of simulation practice for applying various clinical situations.

Effect of Self-monitoring Rehabilitation Program after Stroke on Physical Function, Self-efficacy and Quality of Life (뇌졸중 환자의 자가감시 재활 프로그램이 신체적 기능, 자기효능감 및 삶의 질에 미치는 효과)

  • Purpose: This study was conducted to identify the effects of a self-monitoring rehabilitation program based on the Bandura's self-efficacy theory on the activities of daily living (ADL), 6-minute walking distances, self-efficacy and quality of life (QoL) among stroke patients after three to six months. Methods: The participants consisted of 29 patients in the experiment group and 28 patients in the control group who admitted at rehabilitation specific hospital. Self-monitoring program developed by the researcher lasted twice a week for 8 weeks from August to September, 2013. Results: ANCOVA showed that all of dependent variables of this study, ADL and 6-minute walking distances as a physical function, self-efficacy and QoL for intervention group were higher than those for control group(p<.001). Conclusion: The self-monitoring rehabilitation program based on the self-efficacy theory was found to be effective in improving physical function, self-efficacy and QoL for early post-stroke patients. Early rehabilitation program for stroke patients was recommended to consider the self-monitoring of current physical and psychosocial status as a strategy of self-management.

Effects of Meridian Acupressure on Constipation in the Institutionalized Elderly (경혈지압이 시설노인의 변비완화에 미치는 효과)

  • Purpose: In this study, we attempted to provide an evidence for the effects of meridian acupressure on relieving and preventing constipation in the institutionalized elderly. Methods: The research design was a non-equivalent control group, non-synchronized design. The subjects consisted of 31 institutionalized elderly (experimental group: 16, control group: 15). The experimental group was given meridian acupressure for 10 minutes daily for 2 weeks. The data was analyzed by the ${\chi}^2$-test, Fisher's exact test, t-test and repeated measures ANOVA. Results: The number of bowel movements per week of the experimental and control group performed meridian acupressure verified by repeated-measures analysis of variance revealed that interaction existed between the meridian acupressure availability and the measurement point (F=98.183, p<.001). Repeated measures analysis of variance to compare the changes in Bristol stool form scale scores of the experimental and control group performed meridian acupressure revealed that interaction existed between the meridian acupressure availability and the measurement point (F=48.896, p<.001). Conclusion: The results of this study show the meridian acupressure is a useful nursing intervention on constipation in the institutionalized elderly.

Effects of Tai Chi Exercise on Physical Fitness and Quality of Life in Elderly Women (타이치 운동이 여성노인의 체력과 삶의 질에 미치는 효과)

  • Purpose: This study was performed to investigate the effects of Tai Chi exercise on physical fitness and quality of life (QOL) in elderly women. Methods: This study was designed as a quasi-experimental, nonequivalent control group pre-post test design. The subjects were elderly women over 60 years old who enrolled senior citizen centers and elderly welfare center. Thirty-three people were conveniently assigned to an experimental group (n=15) or a control group (n=18). The data were collected from September 2009 to May 2010. The experimental group was trained Tai Chi exercise for 8 weeks (one hour Tai Chi classes twice a week and once a week by themselves at home). Data were analyzed by Fisher's exact test, ${\chi}^2$-test and independent t-test. Results: The experimental group experienced significant improvements in flexibility. The experimental group experienced significant improvements in physical component summary (p=.017) and mental component summary (p=.027) of QOL. Conclusion: Tai Chi exercise can be used as a nursing intervention for improvement of physical fitness and QOL in elderly women.
