Sonographic Assessment of the Extent of Extrathyroidal Extension in Thyroid Cancer

  • Objective: This study aimed to determine the sonographic features suggestive of extrathyroidal extension (ETE) of thyroid cancers. Materials and Methods: We retrospectively reviewed the sonographic images of 1656 consecutive patients who had undergone thyroidectomy in 2017. The diagnostic performance of sonographic features suggestive of ETE was evaluated using operation and histopathologic reports. Sonographic features for gross ETE to the strap muscle and minor ETE were assessed for thyroid cancer abutting the anterolateral thyroid capsule. Sonographic features for tracheal invasion were assessed according to whether the angle between the tumor and the trachea was an acute, right, or obtuse angle. Sonographic features for recurrent laryngeal nerve (RLN) invasion were assessed based on the association between the tumor and tracheoesophageal groove (TEG) as preserved normal tissue, abutting or protruding into the TEG. Results: ETE was observed in 783 patients (47.3%), including 123 patients with gross ETE (7.4% [strap muscle, n = 97; RLN, n = 24; and trachea, n = 14]) and 660 patients with minor ETE (39.9%). Regarding the diagnosis of gross and minor ETE to the strap muscle, sonographic features of replacement of the strap muscle and capsular disruption showed the highest positive predictive value (75.9% and 58.5%, respectively). Thyroid cancer forming an obtuse angle with the trachea had the highest sensitivity for the diagnosis of tracheal invasion (85.7%), and thyroid cancer protrusion into the TEG showed the highest sensitivity for the diagnosis of RLN (83.3%). Conclusion: Sonography is considered beneficial in the diagnosis of ETE to the strap muscle, trachea, and RLN. Assessment of ETE is important for the accurate staging of thyroid cancer, which in turn determines the extent of surgery or whether active surveillance is appropriate or not.

  • Friction stir welding is a relatively new solid state joining process. A6061-T6 and A5052-H32 aluminium alloy has gathered wide acceptance in the fabrication of light weight structures requiring a high strength to weight ratio and good corrosion resistance. This friction stir process uses a non-consumable tool to generate frictional heat in the abutting surfaces. The welding parameters such as tool rotational speed, welding speed, etc., and tool pin profile play a major role in deciding FSP zone formation in A6061-T6 and A5052-H32 aluminium alloy. Three different tool pin profiles have been used to fabricate the dissimilar butt joints. The formation of friction stir processed zone has been analysed macroscopically. Tensile properties of the joints have been evaluated and correlated with the friction stir processed zone formation.

Cystic Lymphangioma of the Spleen: Report of a Case (비장에 생긴 낭성 림프관종 1예)

  • Splenic cystic lesion is uncommon in children, and cystic lymphangioma of the spleen has not been reported in Korean pediatric patients. Here we report a case of cystic lymphangioma arising from the spleen in a 16 year-old male. The patient presented with left flank pain for 5 days after blunt trauma to the same site. On physical examination, left abdominal tenderness and a palpable spleen were noted. Abdominal ultrasound and MRI revealed multiple septated macro-cystic mass abutting to the spleen medially, suggestive of cystic lymphangioma of the spleen. Laparotomy revealed a 20 cm sized cyst in the spleen, and 2,000mL of dark-brownish fluid was aspirated from the cyst. Splenectomy was performed. Pathological examination revealed the cystic lymphangioma. Post-operative recovery was uneventful, and the patient was discharged at 7 days after surgery.

  • Primary pericardial mesothelioma is extremely rare and the incidence is low among the mesotheliomas that originate from other parts of the body. The prognosis of the tumor is unfavorable due to its late presentation, difficulties in early diagnosis and complete resection, and the limited treatment options. Herein, we report a case of pericardial mesothelioma. The patient is a 55-year-old woman who presented with chronic cough and dyspnea. During the examination, pericardial effusion was found and pericardial window formation was followed. She visited our hospital because of persistent dyspnea, with right shoulder and chest pain. Four discrete masses were discovered in the chest CT. CT guided-fine needle aspiration biopsy was negative for malignancy. Right exploratory thoracotomy and partial resection of 3${\times}$3 cm mass abutting pericardium was performed and was histologically diagnosed as malignant mesothelioma, biphasic type. Pericardial mesothelioma is rare, but it should be remembered as an important differential diagnosis in patients with persistent pericardial effusion and symptoms of dyspnea and constrictive pericarditis.

  • The Composite waterproofing began to rise gradually 2000s in domestic. However, the sheet-membrane composite waterproofing method also acts as an abutment vulnerability that was a problem in a single method, and had the problem of damage to the formation of leakage paths. The existing non-overlap method, through vigorous research abutting sheet shall or could be developed to improve the seal performance of this method with the I-type joint coating material. Nevertheless, it has an integral top coat paint membrane and a sheet abutment limitation, damage to the upper membrane is damaged junction coating membranes has been generated. In this study, a method that has a layer of insulation on the joint bond breaker concept development, and to determine the physical properties with insulated joints to determine the breaking off of the upper joint is damaged coating membrane and tensile performance.

A Brief Review of κ-Carbide in Fe-Mn-Al-C Model Alloys

  • The multiple length scale analysis of previously designed Fe-Mn-Al-C based low-density model alloys reveals the difference in ordered ${\kappa}-carbide$, $(Fe,Mn)_3AlC_x$, between Fe-25Mn-16Al-5.2C (at%) alloy and Fe-3Mn-10Al-1.2C (at%) alloy. For the former alloy composition consisting of fully austenite grains, ${\kappa}-carbide$ showed majorly cuboidal and minorly pancake morphology and its chemical composition was not changed through aging for 24 h and 168 h at $600^{\circ}C$. Meanwhile, for the isothermally annealed ferritic alloy system for 1 hr at 500 and $600^{\circ}C$, the dramatic change in the chemical composition of needle-shape ${\kappa}-carbide$, $(Fe,Mn)_3(Fe,Al)C_x$, was found. Here we address that the compositional fluctuations in the vicinity of the carbides are significantly controlled by abutting phase, either austenite or ferrite. Namely, the cooperative ordering of carbon and Al is an important factor contributing to carbide formation in the high-Mn and high-Al alloyed austenitic steel, while the carbon and Mn for the low-Mn and high Al alloyed ferritic steel.

Critical heat flux in a CANDU end shield - Influence of shielding ball diameter

  • Experiments were performed to measure the critical heat flux (CHF) on a vertical surface abutting a coarse packed bed of spherical particles. This geometry is representative of a CANDU reactor calandria tubesheet facing the end shield cavity during the in-vessel retention (IVR) phase of a severe accident. Deionized light water was used as the working fluid. Low carbon steel shielding balls with diameters ranging from 6.4 to 12.7 mm were used, allowing for the development of an empirical correlation of CHF as a function of shielding ball diameter. Previously published data is used to develop a more comprehensive empirical correlation accounting for the impacts of both shielding ball diameter and heating surface height. Tests using borosilicate shielding balls demonstrated that the dependence of CHF on shielding ball thermal conductivity is insignificant. The deposition of iron oxide particles transported from shielding balls to the heating surface is verified to increase CHF non-trivially. The results presented in this paper improve the state of the knowledge base permitting quantitative prediction of CHF in the CANDU end shield, refining our ability to assess the feasibility of IVR. The findings clarify the mechanisms governing CHF in this scenario, permitting identification of potential future research directions.

Migrating Lobar Atelectasis of the Right Lung: Radiologic Findings in Six Patients

  • Objective: To describe the radiologic findings of migrating lobar atelectasis of the right lung. Materials and Methods: Chest radiographs (n = 6) and CT scans (n = 5) of six patients with migrating lobar atelectasis of the right lung were analyzed retrospectively. The underlying diseases associated with lobar atelectasis were bronchogenic carcinoma (n = 4), bronchial tuberculosis (n = 1), and tracheobronchial amyloidosis (n = 1). Results: Atelectasis involved the right upper lobe (RUL) (n = 3) and both the RUL and right middle lobe (RML) (n = 3). On supine anteroposterior radiographs (n = 5) and on an erect posteroanterior radiograph (n = 1), the atelectatic lobe(s) occupied the right upper lung zone, with a wedge shape abutting onto the right mediastinal border. On erect posteroanterior radiographs (n = 6), the heavy atelectatic lobe(s) migrated downward, forming a peri- or infrahilar area of increased opacity and obscuring the right cardiac margin. Erect lateral radiographs (n = 4) showed inferior shift of the anterosuperiorly located atelectatic lobe(s) to the anteroinferior portion of the hemithorax. Conclusion: Atelectatic lobe(s) can move within the hemithorax according to changes in a patient s position. This process involves the RUL or both the RUL and RML.

Bridge Park International Design Competition and Its Implications on Contemporary Landscape Design (브리지 파크 국제설계경기에 나타난 현대 조경설계의 경향)

  • A deserted town once vibrant with active commercial activities around a railroad station now tries to find a way to escape from depression and revive its life with a renewed civic pride. An open space adjacent to the Main Street, the commercial district of Buzzards Bay, Massachusetts, is waiting to be transformed and reconfigured to be a new ecological park to boost the economy of the community. Bridge Park is 26-acre land abutting the Cape Cod Canal with a railroad bridge as a backdrop. The existing condition of the site with a small salt marsh, woodland, lawn, and the vestige of old railroad easement along with the proximity to the commercial district poses an interesting question of how to make a medium scaled ecological park within an urban context. This paper examines the winning design proposals for the Bridge Park submitted to the International Design Competition held in April, 2005. Six winning proposals were introduced and discussed in terms of categories related to the trend of contemporary landscape design such as; 1) ecological ordinariness and geometric figures, 2) topography and spatial imagination, 3) minimal programs and open put 4) time and process oriented design, 5) park and economic effects and 6) diagrammatic plan and photo montage. Bridge Park Design Competition confirms the complex characteristics representing the contemporary landscape design overcoming the dichotomy between nature and culture and the 'pastoral ecological design' and 'landscape as an art'. The Park becomes the activating agent for the community rejecting the conventional and passive role as a romantic picturesque landscape. Bridge Park International Design Competition is a meaningful event to test the idea of new ecological urban park, and to fine-tune the trend of the contemporary urban park design.
