A Study about HRV of the Patients with abnormality on EKG (심전도상 이상 소견환자의 심박변이도(HRV)에 관한 고찰)

  • Objectives : This study was designed to investigate heart rate variability(HRV) in patients with abnormality on EKG by power spectrum analysis of HRV. Methods : The patient group consisted of 147 patients diagnosed as abnormal on EKG at the Oriental Medical Hospital of Dong-eui University from November 2003 to September 2005. We divided the patient group into 9 subgroups (bradycardia, arrhythmia, PVC, AF, AV block, RBBB, LVH, cardiac ischemia, LAD ). The control group consisted of 117 patients who were diagnosed as normal on EKG at the same hospital during the same period. We checked HRV of the two groups over 5 minutes and compared the HRV index between groups. Results and Conclusions : In the time domain analysis, SDNN was significantly higher in the PVC and AF groups than control group and RMSSD was significantly higher in the all patient group and the bradycardia, PVC and AF groups than in the control group. In the frequency domain analysis, Ln(LF) was significantly higher in the all patient group and the PVC and AF groups than the control group but lower in the LAD group. Ln(HF) was significantly higher in The all patient group and bradycardia, PVE and AF groups than control group. LF/HF ratio was significantly lower in the all patient group and bradycardia, arrhythmia, AF, AV block and LAD groups than control group. The autonomic nerve system and parasympathetic nerve system were higher in the patient group with abnormal EKG compared with the control group.

A Clinical Analysis of 24 cases of Cardiac Contusion and Cardiac Concussion (둔상에 의한 심타박상과 심좌상의 임상적 고찰)

  • Background: In the patients with thoracic injury, we suspect simultaneous cardiac contusion or concussion. We analyzed the patients with possible cardiac injury by electrocardiography, serum creatine kinase (CK), creatine kinase isoenzyme fraction (CK-MB) screening, followed by two dimentional echocardiogram (2-DE) to access the severity of injury. Material and Method: From January 1997 to April 1998, 15-month retrospective study of suspicious myocardial injury was undertaken in including 24 patients admitted for suspected cardiac injury. All patients with history or signs of blunt chest injury were checked serially and the serial CK, CK-MB fraction, electrocardiography (EKG) analysis screening were followed by 2-DE. Result: The age range was between 20-40 years and were predominant male patients in(M:F=3:1). Most common causes of injury were traffic accidents, 15 patients(62.5%). Associated injuries involved multiple rib fractures, sternal fracture and such. EKG findings on the cardiac concussion were within normal limits, EKG findings on the cardiac contusion were nonspecific ST and T wave abnormality. In cardiac contusion patients, CK-MB fraction did not increase significantly on admission but on 2nd, 3rd, 4th hospital days, it increased significantly (p=0.0080, 0.0130, 0.0130). The average admission days were 9.22 in concussion and 26.18 in contusion patients(p=0.0075). Most common complication was the adult respiratory distress syndrome(7 cases), 5 out of the patients with ARDS were mechanically ventilated. There were no deaths. Conclusion: We believe the serial checks of CK-MB, EKG and subsquent two-dementional echocardiographic sector scanning are presently the most sensitive indicators available for structural and functional cardiac injury.

Primary Survey of Cardiac Troponin I Elevated Groups in Trauma Patients (외상 환자에서의 cardiac troponin I 상승군에서의 기초 조사)

  • Background: Cardiac troponin I (cTnI) is a sensitive cardiac marker of myocardial injury. In normal coronary angiogram, positive cTnI values may be detected in various events such as sepsis, stroke, trauma and so on. To investigate characteristics of cTnI positive group in trauma patients, we designed this study between cTnI positive group and cTnI negative group. Method: Trauma patients who visited emergency room within 24 hours after accidents were included. Patients who had renal failure, acute coronary syndrome, sepsis, spontaneous SAH were excluded. Retrospective study of 97 trauma patients was done. We investgated ISS (injury severity score), positive cTnI, EKG abnormality, shock class, ICU admission rate and mortality. Result: In comparing with non chest trauma group, chest trauma group, whose chest AIS (Abbreviated Injury Score) is more than 3 point, had significant values in ISS, positive cTnI, EKG abnormality, shock class and ICU admission rate. Also, in non chest trauma group, we found several patients whose cTnI level was positive. When non chest trauma group was divided into two subgroups, the mortality and shock class of positive cTnI group were higher than that of negative cTnI group. When all trauma patients were divided into two groups, a positive cTnI group had higher values in ISS, shock class, ICU admission rate and mortality than that in a negative cTnI group. Conclusion: We found that cTnI were positive in patients of cardiac contusion but also in various trauma cases. In non chest trauma patients, we assumed that hypotension caused cTnI elevating. The cTnI could play a role in predicting prognosis in trauma patients.

The Research on the Sasang Constitutional Characteristics of Stroke Inpatients (뇌졸중(腦卒中) 환자(患者)의 사상체질별(四象體質別) 특성(特性)에 관(關)한 연구(硏究))

  • 1. Objectives The purpose of this research is to find out the Constitutional characters of Stroke inpatients. 2. Methods Stroke inpatients admitted into KyungHee Oriental Medical Center from January 2000 to April 2004, were divided into 2 group, cerebral infarction 493 inpatients and cerebral hemorrhage 83 inpatients. This research investigated the distribution of Sasasng Constitution, the gender, the age decade, the period of admission, the elapse of the time after onset, the course of admission, the frequency of Sasang Constitutional Herb-medicine, the past history of hypertension and diabetes mellitus, the obesity, the prevalence rate of hypertension and diabetes mellitus, EKG abnormality and hyperlipidemia. 3. Results & Conclusions The Sasang Constitutional distribution of cerebral infarction 493 inpatients was 0.4% of Taeyangin, Soyangin 34.1%, Taeurnin 56.4%, Soeumin 9.1%. The Sasang Constitutional distribution of cerebral hemorrhage 83 inpatients was Taeyangin 1.2%, Soyangin 47.9%, Taeurnin 43.4%, Soeumin 9.6%. Stroke incidence rate of Taeumin and Soyangin inpatients was high. Sasang Constitution had significant relations with the prevalence of the obesity and hypertension in cerebral infarction inpatients, the prevalence of obesity, diabetes mellitus and ECG abnormality in cerebral hemorrhage inpatients.
