A Case Report of Unresectable Pancreatic Carcinoma Patient for Relieving Cancer Related Pain and Improving Quality of Life by Korean Medical Treatment (한방치료를 통해 수술이 불가능한 췌장암 환자에서 통증개선 및 삶의 질을 회복한 증례)

  • Objectives : The purpose of this study is to report the effect of Korean medicine on the pancreatic carcinoma patient. Method : A patient was diagnosed as unresectable pancreatic carcinoma. She complained insomnia, abdominal pain, dyspepsia and sleep disturbance. She was treated by Korean medicine composed of acupuncture, herbal medication and wild ginseng pharmacopuncture. Results : All symptoms took a favorable turn after Korean medicine treatment. As treatment was performed, as intensity of pain, insomnia, dyspepsia and fatigue were decreased. Conclusion : This study suggests that Korean medicine treatment has effectiveness for improvement of cancer related symptoms of a pancreatic carcinoma patient. Consequently, the treatment is helpful in improving quality of life.

Is Laparoscopy-assisted Radical Gastrectomy Safe in Patients with Child-Pugh Class A Cirrhosis?

  • Purpose: We investigated early postoperative morbidity and mortality in patients with liver cirrhosis who had undergone radical gastrectomy for gastric cancer. Materials and Methods: We retrospectively reviewed the medical records of 41 patients who underwent radical gastrectomy at the Chonnam National University Hwasun Hospital (Hwasun-gun, Korea) between August 2004 and June 2009. There were few patients with Child-Pugh class B or C; therefore, we restricted patient selection to those with Child-Pugh class A. Results: Postoperative complications were observed in 22 (53.7%) patients. The most common complications were ascites (46.3%), postoperative hemorrhage (22.0%) and wound infection (12.2%). Intra-abdominal abscess developed in one (2.4%) patient who had undergone open gastrectomy. Massive ascites occurred in 4 (9.8%) patients. Of the patients who underwent open gastrectomy, nine (21.9%) patients required blood transfusions as a result of postoperative hemorrhage. However, most of these patients had advanced gastric cancer. In contrast, most patients who underwent laparoscopic gastrectomy had early stage gastric cancer, and when the confounding effect from the different stages between the two groups was corrected statistically, no statistically significant difference was found. There was also no significant difference between open and laparoscopic gastrectomy in the occurrence rate of other postoperative complications such as ascites, wound infection, and intra-abdominal abscess. No postoperative mortality occurred. Conclusions: Laparoscopic gastrectomy is a feasible surgical procedure for patients with moderate hepatic dysfunction.

  • Since the reservoir of leptospires organism is consisted of a broad spectrum of animals, the best method of prevention is vaccination. The clinical trial of leptospires vaccine conducted on human volunteer for its immunogenicity and safety. Summarized results are as following : 1. The Oral temperature among vaccinated group ranged from $36.7{\pm}0.46^{\circ}C\;to\;37.0{\pm}0.34^{\circ}C$, while in placebo injected group it ranged from $36.4{\pm}0.46^{\circ}C\;to\;36.7{\pm}0.53^{\circ}C$. There was no association between vaccination and fever (p>0.05) 2. Mild local reactions revealed in vaccinees were swelling (50-75% ), Redness($75{\sim}90%$), and induration ($25{\sim}40%$). Placebo injected group revealed only redness in 12.5% in 1st injection and 37.5% in second injection. The duration local reactions on injection site for th vaccinees and place groups disappeared within 48 hours. 3. Generalized Symptoms complained by the vaccinees were myalgia (25%), back pain(15%), headache (15%), pruritus(15%), and abdominal pain(10%), whereas placebo group complained of headache (25%), myalgia(12.5%), back pain(12.5%), pain in eyes(12.5%), abdominal pain(12.5%) pruritus (12.5%) and nausea(12.5%). 4. The serological test(MAT) of vaccinees showed geometric mean antibody titer as follows : a. L. icterohemorrhagiae lai 1 week after 1st vaccination : 22.45 1 week after 2nd vaccination : 111.23 3 week after 2nd vaccination : 266.64 b. L. canicola canicola 1 week after 1st vaccination : 24.62 1 week after 2nd vaccination : 123.92 3 week after 2nd vaccination : 276.55 c. L. icterohemorrhagiae copenhageni 1 week after 1st vaccination : 28.28 1 week after 2nd vaccination : 128.55 3 week after 2nd vaccination : 247.88 Whereas all of the place injected group showed below 1:20 titers. The sero-conversion rate of vaccinees were 100 percent.

  • The function of the liver is so complicated and important that various Hepatic diseases are occured due to functional retardation of liver. Above all, the incidence of Hepatitis and liver cirrhosis among Koreans has shown an increasing tendency recently. 113 cases of Hepatitis and 125 cases of liver cirrhosis which had been admitted, more than 10 days, to KOREA Hospital between November, 1968 and July,1972, were studied through clinic charts. (A) Hepatitis 1) of the 113 cases; 54 cases (47.7%) were Infectious hepatitis; 40 cases (35.4%) were chronic hepatitis 2) of the 113 cases ; 80 cases (70.8%) were male and 33 cases (29.2%) were female; the sex ratio was 2.4 : 1 The ages of the onset of the disease was as follows; 34 cases (30.1%) were among $30{\sim}40$. 3) Patients had abdominal pain (77.9%) anorexia (66.4 %) and general weakness(82.3%) as symptom and jaundice (94.7%) hepatomegaly (76.1%) as sign 4) 57 cases of all had complication 26 cases (45.6%) were parasite, 12 cases (21.1%) were diabeties mellitus. 5) 99 casea (87.6%) of all were improved and recovered. (B) Liver Cirrhosis 1) Etiologic factors are hepatitis (56cases) and alcoholics (28 cases). 2) of the 125 cases, 84 cases (67.2%) were male and 41 cases (32.8%) were female; the sex ratio was 2 : 1 The age of the onset of the disease was as follows; 47 cases (37.6%) were among 41-50. 3) Patients had symptoms: indigestion (64.8%), Abdominal pain (60%), general wenkness (35.2%) and signs; Hepatomegaly (61.6%) Ascites (59.2%), Jaundice(56.8%). 4) 107 cases of all had complications; Hepatic coma was 20 cases (18.7 %), Ascites was 16 cases (15.0%). 5) 69 cases (55.2%) were improved. Comparing Bio-efficacy of Liquid DL-methionine Hydroxy Analogue Free Acid with DL-methionine in Broiler Chickens

  • The present experiment was conducted to assess the efficacy of methionine hydroxy analogue free fatty acid (MHAFA) in comparison to DL-methionine (DL-Met) utilizing day-old commercial broiler chicks (n=300). The chicks were randomly distributed into 30 groups of 10 chicks each. Three dietary treatments, viz. D1-maize-soybean meal based basal diet (Control), D2- control diet supplemented with DL- methionine to meet its requirements and D3-control supplemented with MHA-FA @ 1.54 times of DL-methionine used in D2, were formulated. Each dietary treatment was offered to 10 replicated groups of chicks following completely random design (CRD). The chicks fed on supplemental DL-Met had significantly higher (p<0.01) gain in body weight, followed by MHA-FA group and control during 0-3 weeks of age. During overall growth period (0-6 weeks), chicks in DL-Met and MHA-FA groups grew better (p<0.01) than those in control. Feed conversion ratio (FCR) improved (p<0.01) on supplementation of either DL- Met or MHA-FA in the basal (Control) diet during 0-3 weeks of age. The FCR for overall period, however, did not differ statistically (p>0.05) amongst the treatments. The eviscerated yields emanated from diets with DL-Met or MHA-FA were higher (p<0.01) than that in Control. Abdominal fat pad was also more (p<0.01) in broilers fed control diet than in DL-Met or MHA-FA supplemented group. Breast yield was higher (p<0.05) in MHA-FA fed broilers than those fed DL-methionine supplemented or un-supplemented diets. The efficacy of MHA-FA in comparison to DL-Met for growth was 62.11, 64.82 and 63.88% and for feed efficiency was 62.98, 67.73 and 64.01% at 0-3, 3-6 and 0-6 weeks of age, respectively, while it was 65.85, 71.40 and 67.49% for eviscerated yield, abdominal fat pad reduction and breast yield at 6 weeks of age, respectively.

Anti-obesity Effects of Misaengtang in Rats Fed on a High-Fat Diet or Normal Diet (고지방 식이와 일반사료를 섭취한 랫드에서 미생탕의 항비만 효과)

  • Misaengtang (MST), a formula of Korean herbal medicines, has been used as a weight-controlling recipe. We have investigated two experiment of body weight regulation by MST In rats. i) The anti-obesity effect of MTS on a high fat diet-induced obesity, male Sprague-Dawley rats were fed with a high-fat diet containing 30% in the absence or presence of MST (0.3, 1 or 3%) or a reference orlistat (0.05%) for 6 weeks. ii) weight-decrease effect of MST on normal diet, same animal were fed with a normal diet in the absence or presence of MST (3%) for 6 weeks. And the body weights, daily feed and water consumptions, organ weights, fat weights serum biochemistry were measured. In both experiments, MST and orlistat did not affect the body weight gain. But orlistat significantly increased the feed and water consumptions, leading to low-feed efficiency, and orlistat markedly reduced abdominal, paratesticular and perirenal fat weights, although increased the kidney weights. In MST, low dose (0.3%) of MST decreased the perirenal fat and increased the kidney weights in rats fed HFD, and MST 3% decreased the abdominal fat weights in rats fed normal diet. In addition, Orlistat caused changes in parameters of hepatotoxicity (AST and glucose), nephrotoxicity (BUN and B/C ratio) and lipid metabolism (HDL and triglycerides). In comparison, MST decreased AST, ALP and ALT, the hepatotoxicity markers, and somewhat improved the hepatic fatty degeneration. Taken together, it is suggested that MST does not exert anti-obesity activity as well as remarkably direct effects, but MST may be potentially benefit for dietary cure and exercise-cure of obesity.

Multicentric T cell lymphosarcoma in a Jeju native boar

  • A 7-year-old Jeju native boar was requested to the Veterinary Pathology Laboratory of Cheju National University with a clinical signs of anorexia, melena, lethargy and sudden death. At necropsy, four coalescing firm masses were occupied in the abdominal cavity between ventral surface of stomach and pancreas. Individual mass was a yellowish white in color and up to 6 cm in diameter. These masses were not encapsulated and bulged from the cut surface. Liver and spleen were enlarged and pale nodules were presented in these tissues. Some yellowish white nodules up to 5 mm in diameter were scattered in kidneys. Histopathologically, lymphoblastic tumor cells were occupied in the abdominal masses, multifocal areas of liver, kidneys, and spleen. Morphologically lymphoblastic tumor cells were round to oval in shape, and medium to large in size. They had round to oval nuclei, moderate amount of eosinophilic cytoplasm, and many mitotic figures. Immunohistochemistry revealed that tumor cells were CD3-positive and $CD79{\alpha}$-negative, consistent with T-cell lineage. Based on gross, microscopic findings and immunohistochemistry, this case was diagnosed as porcine multi-centric T cell lymphosarcoma. In animals, as in human, the T-cell lymphomas are generally more aggressive than B cell types and respond less well to therapy. In our best knowledge, this is the first report for porcine T cell lymphosarcoma in Korea.

Experimental Study Trends on the Acupuncture Moxibustion Treatment for Visceral Hypersensitivity: Based on the Data of PubMed (내장감각과민의 침구 치료에 대한 실험연구 현황: PubMed를 중심으로)

  • Objectives : The aim of this study is to review the current trends in experimental studies on the acupuncture moxibustion treatment for visceral hypersensitivity. Methods : PubMed was searched for experimental studies about visceral hypersensitivity and acupuncture/moxibustion. Data were extracted and tabulated from the selected articles about experimental method, intervention, result and mechanism. Results : Total 23 articles were reviewed. Chronic visceral hypersensitivity animal model was applied in 17 studies (74%). Visceral hypersensitivity was measured by abdominal withdrawal reflex scoring or/and abdominal electromyogram. Acupoints like ST25, ST36, ST37, BL25, LI11, BL32 and PC6 were treated by electroacupuncture or moxibustion. All articles reported that electroacupuncture or moxibustion treatment is significantly effective in reducing visceral hypersensitivity. Treatment mechanisms were studied, related to mast cell, serotonin (5-HT) and receptor (5-HT3R and 5-HT4R), substance P (SP), vasoactive intestinal polypeptide (VIP), c-fos positive cell, corticotropin-releasing hormone (CRH), purinergic 2X (P2X)2, P2X3, P2X4, P2X7, N-methyl-D-aspartate (NMDA) receptor (NR1 and NR2B), prokinectin (PK) 1 and PK2. Conclusions : Evidences on acupuncture/moxibustion treatment for visceral hypersensitivity in animal studies warrant more research on effective acupoins, electro-acupuncture methods and treatment durations.

The Micro-Current Stimulation Inhibits Adipogenesis by Activating Wnt/β-Catenin Signaling (Wnt/β-catenin 신호 활성화를 통한 미세전류 자극의 지방생성 억제 효과)

  • This study aimed to evaluate the inhibitory effect of micro-current stimulation(MCS) on adipogenesis regarding with Wnt/β-catenin pathway using the ob/ob mouse and 3T3-L1 cell line. 6-week old ob/ob male mice were equally assigned to four groups: obese group(ob), obese with MCS groups(50 μA, 200 μA, and 400 μA). 6-week old C57BL/6J male mice were assigned to the control group(CON). We analyzed abdominal adipose tissue volume by using in vivo micro-CT and measured the body weight, feed intake, liver weight and triglycerides in serum. All the MCS groups showed that significantly reduced body weight and triglycerides in serum. In the case of liver weight and abdominal adipose tissue volume, the inhibitory effect of adipogenesis was shown in the 200 μA and 400 μA groups. To elucidate the anti-obesity effect of MCS, β-catenin, C/EBPα and FAS protein expressions were analyzed by western blotting. β-catenin expression was upregulated, C/EBPα and FAS expression were down-regulated in the relatively high-intensity groups(200 μA and 400 μA). Thus, the 200 μA and 400 μA for the intensity of MCS were chosen for cell experiments. In the 3T3-L1 cell line, Wnt/β-catenin pathway including Wnt10b, Wnt3a, β-catenin and Cyclin D1 was activated in all MCS groups. Accordingly, the expression level of C/EBPα was decreased during the differentiation and lipid droplet was significantly reduced in Oil red O staining results. These results suggest that the Wnt/β-catenin signaling might be activated by MCS with current intensities between 200-400 μA and it may lead to anti-obesity effects.

Association between drinking behaviors and components of metabolic syndrome in subjects in their 20s and 30s: data obtained from the Korea National Health and Nutrition Examination Survey (2016-2018)

  • BACKGROUND/OBJECTIVES: Numerous studies have examined the relationship between drinking behaviors and metabolic syndrome (MetS) for adults, but these include very few studies for young adults. This study therefore undertook to investigate the association between drinking behaviors and components of MetS among adult drinkers aged 20-30 years. SUBJECTS/METHODS: Using the 2016-2018 Korea National Health and Nutrition Examination Survey data, drinking behaviors of adults in the age group 20-30 years were divided into 4 groups: 1) group A, good drinking habits; 2) group B, frequent binge drinking but not frequent drinking; 3) group C, frequent drinking but not frequent binge drinking; 4) group D, frequent drinking and binge drinking. The association between MetS components and drinking behaviors was analyzed by applying multiple logistic regression analysis. RESULTS: We determined the prevalence risk compared to group A. In men, the prevalence risk of high triglyceride (TG) increased 2.051-fold in group C and 1.965-fold in group D. Moreover, in group D, the prevalence risk of low high density lipoprotein cholesterol (HDL-C) increased 0.668-fold, high blood pressure (BP) increased 2.147-fold, and MetS increased 1.567-fold. In women, there was an increased prevalence risk of low HDL-C (0.353-fold) and MetS (3.438-fold) in group C, whereas group D showed increased prevalence risk of abdominal obesity (2.959-fold), high TG (1.824-fold, and low HDL-C (0.424-fold). CONCLUSIONS: Our study indicates that frequent drinking increases the risk of high TG, whereas frequent and binge drinking increases the risk of high TG, low HDL-C, high BP, and prevalence of MetS in men. In women, frequent drinking without binge drinking increases the risk of low HDL-C and MetS, whereas frequent and binge drinking increases the risk of abdominal obesity, high TG, and low HDL-C. We propose that improvements in the drinking behaviors can reduce the prevalence of MetS.
