Breast Reconstruction with Lower Abdominal Tissue Free Transfer (하복부 조직 유리피판에 의한 유방재건술)

  • Lower abdominal tissue is regarded as an ideal donor site for the breast reconstruction because it provides large skin territory and huge amount of soft tissues enough to the breast size. However it is not easy for the surgeon to reconstruct the really natural breast, and needs the learning curve with long time experience. Author represent the various reconstruction procedures for the breast using lower abdominal tissue such as muscle sparing free TRAM, DIEP, and SIEA free flaps to reinsure like breast. Indications, and selection of reconstructive methods, surgical timing, selection of donor sites and recipient vessel of these flaps were reviewed. In addition, detailed procedures, surgical tips and secondary adjuvant procedures are described for more symmetry of reconstructed breast. The muscle sparing free TRAM, DIEP, and SIEA free flaps would be enough to provide supple, huge amount of well vascularized tissue for the breast, if these flaps were selected for the appropriate indication according to patient's general condition, obesity, the opposite breast and abdominal tissue condition. Lower abdominal tissue was able to provide versatile designs with sufficient adipose tissue without compromising the integrity of abdominal wall.

  • Breast reconstruction provides dramatic improvement for patients with severe deformity. The reconstruction not only restores aesthetically acceptable breast for patients with mastectomy deformity but also recovers psychological trauma of 'losing feminity' after the cancer mastectomy. There are many options for breast reconstruction from simple prosthetic insertion to a flap operation using autologous abdominal tissue. The choice of operation method depends on the physical condition of the patient, smoking habits, and economic status. Among the many options, the method that uses the lower abdominal tissue is known as the TRAM (transverse rectus abdonimis myocutaneous) flap. Since the introduction of the TRAM flap in 1982 by Hartrampf, the art of breast reconstruction using lower abdominal tissue has been progressively refined to pedicle flap, muscle-sparinga TRAM flap, and recently there have been exciting and revolutionary changes associated with the adoption of the concept of perforator flap. This refined method of breast reconstruction utilizes lower abdominal tissue nourished by the deep inferior epigastric perforator (DIEP). With the DIEP free flap, almost all of the rectus muscle and anterior rectus sheath are preserved and the donor morbidity is minimized. Different from previous flap methods using lower abdominal tissue, DIEP free flap method preserves function of the rectus muscle completely. 1) Understanding the entire progression of breast reconstruction methods using lower abdominal tissue is necessary for plastic surgeons; the understanding of each step of the exciting progression and the review of the past history of the TRAM flap may provide insight for future development.

  • The purpose of this study is to compare the biocompatibility of commercial purity Ti, Ti-6AI-4V and Ti-6AI-7Nb alloy specimens with and without surface treatment in mouse abdominal connective tissue in vivo. Each metal was implanted into specific abdominal subdermal tissue site of female mouse. After 4 weeks, the implants were removed and abdominal tissues were fixed, dehydrated and embedded in glycol methacrylate resin. And the tissues were histologically prepared for microscopical evaluation. It was characterized by the presence of connective tissue with fibrous capsule surrounding the implant. The fibrous tissue surrounding the implant was studied to determine the biocompatibility of implanted metals. The average thickness of the fibrous capsule formed around the implant was much thinner for the hydrogen peroxide added hydrochloric acid solution-treated specimen than for the others. The results of this evaluation indicate that modification of the surface properties of titanium and titanium alloy implants changes the biological properties in the abdominal connective tissue. In conclusion, these observations suggest that the proper surface treatment performed in the study is effective for the improvement of biocompatibility.

Assessment of The Accuracy of The MR Abdominal Adipose Tissue Volumetry using 3D Gradient Dual Echo 2-Point DIXON Technique using CT as Reference

  • In this study, in order to determine the validity and accuracy of MR imaging of 3D gradient dual echo 2-point DIXON technique for measuring abdominal adipose tissue volume and distribution, the measurements obtained by CT were set as a reference for comparison and their correlations were evaluated. CT and MRI scans were performed on each subject (17 healthy male volunteers who were fully informed about this study) to measure abdominal adipose tissue volume. Two skilled investigators individually observed the images acquired by CT and MRI in an independent environment, and directly separated the total volume using region-based thresholding segmentation method, and based on this, the total adipose tissue volume, subcutaneous adipose tissue volume and visceral adipose tissue volume were respectively measured. The correlation of the adipose tissue volume measurements with respect to the observer was examined using the Spearman test and the inter-observer agreement was evaluated using the intra-class correlation test. The correlation of the adipose tissue volume measurements by CT and MRI imaging methods was examined by simple regression analysis. In addition, using the Bland-Altman plot, the degree of agreement between the two imaging methods was evaluated. All of the statistical analysis results showed highly statistically significant correlation (p<0.05) respectively from the results of each adipose tissue volume measurements. In conclusion, MR abdominal adipose volumetry using the technique of 3D gradient dual echo 2-point DIXON showed a very high level of concordance even when compared with the adipose tissue measuring method using CT as reference.

  • Objectives : The purpose of this study is to report the relationship between iridological constitution and interleukin 1 beta (IL-1 $\beta$) gene polymorphism. Methods : Iris constitution were diagnosed by automatic Iris analysis system, Bexel Irina(Korea). The blood was stored at - $20^{\circ}$... until it was ready to be extracted. The genomic DNA was extracted by inorganic procedure. The concentration of DNA was estimated by absorbance at 260 nm. The interleukin-1 beta (IL-1 $\beta$) gene polymorphism was detected by PCR amplification. Results & Conclusions : The author classified 166 individuals according to Iris constitution, and determined IL-1 $\beta$ genotype. The frequencies of Iris constitutions as follows : neurogenic type, 41 (24.7%); abdominal connective tissue weakness type, 53(31.9%); cardio-renal connective tissue weakness type, 50 (30.1%); the others type, 22 (13.3%). Especially, the frequency of abdominal connective tissue weakness type was significantly higher in err genotype than in the remaining constitutions. As a result, The author demonstrated the association among IL-1 $\beta$ genotype, IBD and Iris constitution.

Thickness of Rectus Abdominis Muscle and Abdominal Subcutaneous Fat Tissue in Adult Women: Correlation with Age, Pregnancy, Laparotomy, and Body Mass Index

  • Background Rectus abdominis muscle and abdominal subcutaneous fat tissue are useful for reconstruction of the chest wall, and abdominal, vaginal, and perianal defects. Thus, preoperative evaluation of rectus abdominis muscle and abdominal subcutaneous fat tissue is important. This is a retrospective study that measured the thickness of rectus abdominis muscle and abdominal subcutaneous fat tissue using computed tomography (CT) and analyzed the correlation with the patients' age, gestational history, history of laparotomy, and body mass index (BMI). Methods A total of 545 adult women were studied. Rectus abdominis muscle and abdominal subcutaneous fat thicknesses were measured with abdominopelvic CT. The results were analyzed to determine if the thickness of the rectus abdominis muscle or subcutaneous fat tissue was significantly correlated with age, number of pregnancies, history of laparotomy, and BMI. Results Rectus abdominis muscle thicknesses were 9.58 mm (right) and 9.73 mm (left) at the xiphoid level and 10.26 mm (right) and 10.26 mm (left) at the umbilicus level. Subcutaneous fat thicknesses were 24.31 mm (right) and 23.39 mm (left). Rectus abdominismuscle thickness decreased with age and pregnancy. History of laparotomy had a significant negative correlation with rectus abdominis muscle thickness at the xiphoid level. Abdominal subcutaneous fat thickness had no correlation with age, number of pregnancies, or history of laparotomy. Conclusions Age, gestational history, and history of laparotomy influenced rectus abdominis muscle thickness but did not influence abdominal subcutaneous fat thickness. These results are clinically valuable for planning a rectus abdominis muscle flap and safe elevation of muscle flap.

Breast Reconstruction with Superior Gluteal Artery Perforator Flap in Asian (동양인에서 위볼기동맥 천공지판을 이용한 유방재건)

  • Purpose: Breast reconstruction with lower abdominal tissue can produce the best outcome with acceptable rates of long-term complication. However, for cases in which sufficient abdominal tissue is not available, an superior gluteal artery perforator (SGAP) flap can be considered as the next option for autologous breast reconstruction. Materials and Methods: Among a total of 63 women who underwent breast reconstruction with free autologous tissue transfer from July 2010 to April 2011, SGAP flap was performed for four patients. In two cases, patients did not have enough abdominal tissue for sizable breast reconstruction. In another case, the patient had a long abdominal scar due to donor hepatectomy of liver transplantation. In the last case, which was a revisional case after radiation necrosis of a previous pedicled transverse rectus abdominis musculocutaneous (TRAM) flap, a large amount of healthy skin and soft tissue was needed. SGAP flap was elevated in lateral decubitus position. The internal mammary vessels were used for recipient vessels in all cases. Results: Breast reconstruction was performed successfully in all four cases without flap loss. Donor site complication was not observed, except for one case of seroma. The shape of the reconstructed breast was satisfactory in all patients. Conclusion: SGAP flap is an excellent alternative option for the TRAM or deep inferior epigastric artery perforator flap for breast reconstruction. In terms of narrower width, harder consistency of soft tissue, and shorter pedicle, it is clear that the SGAP flap is less competent than the TRAM flap. However, in cases where abdominal tissue is not available, SGAP flap is the only way of providing a large amount of healthy tissue.

Study on the Corelation between Iris Constitution and Primary Headache (원발성 두통과 홍채 체질과의 상관성 연구)

  • Objectives : The purpose of this study is to compare interrelations between primary headache (including migraine) and Iris Constitution through iris constitution examination. Methods : The subjects were 50 patients who admitted to Chonju Oriental Medicine Hospital attached to Wonkwang Univ. from February.1st.2004 to March.31th.2005. The subjects were constituted of primary headache(including migraine) patients who were examined by Iris Constitution analysis. Results & Conclusion : Among 50 cases of primary headache(including migraine) the distribution of Iris Constitution were as follows. 1. There were 14 cases of neurogenic type, 9 cases of abdominal connective tissue weakness type, 8 cases of cardio-renal connective tissue weakness type, 6 cases of neurogenic + abdominal connective tissue weakness type, 5 cases of neurogenic type+ cardio-renal connective tissue weakness type, 5 cases of abdominal connective tissue weakness type + cardio-renal connective tissue weakness type, 2 cases of hydrogenic type, 1 case of abdominal connective tissue weakness type + mesenchymal pathological connective tissue weakness constitution. 2. There were 25 cases of which included neurogenic constitution, 21 cases of which included abdominal connective tissue weakness constitution, 18 cases of which included cardio-renal connective tissue. Migraine have a meaningful interrelation with Iris constitution, especially with neurogenic type, abdominal connective tissue weakness type and cardio-renal connective tissue weakness type. 3. Interrelation between Region of migraine and Iris constitution is non-specific with the exception of interrelation between pain in all over head with neurogenic type. These results suggest that the further study in connection with these results should be followed. 4. The interrelation between the period of migraine and Iris constitution is not remarkable.

Effect of Dietary Conjugated Linoleic Acid (CLA) on Abdominal Fat Deposition in Yellow-feather Broiler Chickens and Its Possible Mechanism

  • A total of 60 one-day-old Yellow-feather broiler chickens were allotted into treatment and control groups. The treatment group was fed with the diet supplemented with 3% conjugated linoleic acid (CLA) for 48 d, while control group was fed with the diet supplemented with 3% rapeseed oil. Chickens were slaughtered in each group at the age of 49 d, and the blood and the abdominal adipose tissue were sampled. Serum cLeptin and serum cAdiponectin were measured by ELISA. The total RNA was extracted from adipose tissue to measure the abundance of the chicken growth hormone receptor (cGHR), insulin-like growth factor 1 (cIGF-1), insulin-like growth factor I receptor (cIGF-IR), peroxisome proliferator-activated receptor gamma ($cPPAR{\gamma}$), cAdiponectin and cAdipoIR mRNA by RT-PCR using ${\beta}$-actin as an internal standard. Results showed that the CLA decreased the abdominal fat index by 20.93% (p<0.05). The level of serum cLeptin but not serum cAdiponectin was significantly increased by CLA treatment (p<0.05). CLA down-regulated the relative abundance of cGH-R mRNA and $cPPAR{\gamma}$ mRNA in abdominal adipose tissue by 24.74% (p<0.05) and 66.52% (p<0.01) respectively. However, no differences were found between CLA treatment group and control group (p>0.05) in the relative abundance of cIGF-1, cIGF-IR, cAdiponectin, and cAdipoIR mRNA in abdominal adipose tissue. The data suggested that CLA inhibited abdominal fat deposition in broiler chicken may be determined by decreasing the GHR available for GH, and by inhibiting the differentiation of preadipocytes via down-regulation of $PPAR{\gamma}$, but independent of IGF and (or) GH-IGF pathway or adiponectin action.
