A Rare Case of Unilateral Pleural Effusion in a Pediatric Patient on Chronic Peritoneal Dialysis: Is it a Pleuroperitoneal Leakage?

  • Non-infectious complications of peritoneal dialysis (PD) are relatively less common than infectious complications but are a potentially serious problem in patients on chronic PD. Here, we present a case of a non-infectious complication of PD in a 13-year- old boy on chronic PD who presented with symptoms such as hypertension, edema, dyspnea, and decreased ultrafiltration. Chest and abdominal radiography showed pleural effusion and migration of the PD catheter tip. Laparoscopic PD catheter reposition was performed because PD catheter malfunction was suspected. However, pleural effusion relapsed whenever the dialysate volume increased. To identify peritoneal leakage, computed tomography (CT) peritoneography was performed, and a defect of the peritoneum in the left lower abdomen with contrast leakage to the left rectus and abdominis muscles was observed. He was treated conservatively by transiently decreasing the volume of night intermittent PD and gradually increasing the volume. At the 2-year follow-up visit, the patient had not experienced similar symptoms. Patients on PD who present with refractory or recurrent pleural effusion that does not respond to therapy should be assessed for the presence of infection, catheter malfunction, and pleuroperitoneal communication. Thoracentesis and CT peritoneography are useful for evaluating pleural effusion, and timely examination is important for identifying the defect or fistula.

Cecocolic Intussusception Caused by Ancylostoma caninum Infection in a Dog

  • An eight-month-old, outdoor, intact male English Pointer dog weighing 23.5 kg presented to the hospital with signs of hematochezia, soft stools, and weight-loss. There were no remarkable findings on physical examination, complete blood count, serum biochemistry, electrolyte and gas analysis, and radiography. The serologic and Polymerase Chain Reaction (PCR) tests for canine parvovirus were negative. A fecal smear examination showed rod-shaped, sporeforming bacteria. Additionally, a fecal flotation test showed ova of Ancylostoma spp. The size of ova was 60 × 40 ㎛, and it was identified as Ancylostoma caninum using light microscopy. The PCR test indicated a Clostridial perfringens infection and the presence of C. perfringens alpha toxin. The diagnosis given was C. perfringens enterotoxicosis with ancylostomiasis. Treatment included antibiotics (metronidazole, trimethoprim-sulfamethoxazole) and anthelmintics (afoxolaner, milbemycin oxime). After two weeks, the clostridial infection resolved, but ancylostomiasis persisted for six weeks. The anthelmintic was changed to Drontalâ plus (praziquantel/pyrantel pamoate/febantel). After four weeks, there were no remarkable findings in the fecal samples, but the patient still presented with watery stools and hematochezia. Survey of abdominal ultrasound had performed, and a target-like sign with multiple rings was seen in the cecocolic region. The patient was diagnosed with A. caninum-induced cecocolic intussusception from the history and clinical signs. After a surgery, he recovered fully. This is the first clinical case report of Ancylostoma caninum parasitizing from the small intestine and causing an intussusception in the large intestine.

개 신장 혈관육종의 진단 영상 1례 (Diagnostic Imaging of Renal Hemangiosarcoma in a Dog)

  • A 13-year-old male Yorkshire terrier was Presented with Persistent weight loss anorexia and dark brown urine of 3-month duration. On physical examination, a firm oval mass was palpated at left renal region. In hematology and blood chemistry, neutrophilia, anemia, thrombocytopenia and elevation of ALKP were found. Abdominal radiography, ultrasonography and ultrasound-guided percutaneous pyelogram revealed masses originated from left kidney, mildly dilated renal Pelvis and intact ureter. Urinalysis showed hematuria and proteinuria. Because the state of dog became deteriorated during transfusion and the frail renal tumor was suspected to be the cause of inflammation and anemia, nephrectomy was performed. Renal masses, approximately $2{\times}3cm\;and\;5{\times}4cm$ respectively in size, was surrounded by swollen and congested mesentery and ascites. Metastatic lesion was not found in other organs. During recovery, the dog showed cardiopulmonary arrest and did not respond to critical care. Histologically the kidney was affected by necrotic and hemorrhagic change. This hemangiosarcoma most likely arose from the renal parenchyma resulting In diffuse lesions in the kidneys thought to be the cause of chronic anorexia and weight loss.

고양이에서 수신증이 동반된 신선종 (Renal Adenoma with Hydronephrosis in a Cat)

  • 식욕부진 및 구토를 동반한 6세령 중성 수컷 고양이(domestic short hair)가 진료를 위해 지역 동물병원에 내원하였다. 복부방사선 및 초음파검사상 우측 신장의 종대 및 심한 수신증 소견을 나타내었다. 외과적으로 적출한 신장에 대하여 병리학적 검사를 실시하였다. 신장 단면절개 시, 2개의 유백색 종괴 및 심한 신우 종대가 확인되었다. 종괴의 대부분은 단일형태의 분화도가 높은 세관 구조로 이루어져 있었으며, 입방 내지 원주형 세포들이 한 층으로 배열되어 있고 내강에 유두상으로 돌출되어 있었다. 면역조직화학기법을 통해 종양세포는 cytokeratin (CK) MNF116에 대하여 강한 양성반응을 보였으나, CK 7에는 음성으로 나타내었다. 따라서 임상증상, 병리학적 소견 및 면역조직화학기법을 통하여 본 증례는 수신증이 동반된 유두상 신선종으로 확진되었다.

시추 개에서 발생한 T-cell 유래 림프육종 증례 (Spontaneous canine T-cell lymphosarcoma in a Shih Tzu dog)

  • This study describes a canine lymphosarcoma with a rapid proliferation and recurrence. A 4-year-old, male, Shih Tzu dog was examined for acute swelling mass. The mass had been identified since 3 months ago and enlarged $10{\times}7$ cm and located in the right axillary region. The surgical removal was recommended when patient visited veterinarian and the operation was conducted. The removed tumor was $11{\times}8{\times}7$ cm and firm, lobulated and white cut surface. Routine screening laboratory test was assessed with blood and radiological analysis. The metastasis sign was not detected on thoracic and abdominal radiography. Blood test revealed decreased lymphocytes. After surgical removal of the mass, microscopic histopathological examination was performed to determine the final diagnosis. Histopathologically, the tumors are characterized by the same histological features, including the presence of neoplastic cellular populations, and lymphocytes infiltration in varying proportions. Also, DNA was extracted and PCR analysis was employed to analyze the origin of tumor cells. T-cell specific nucleic acid fragments were specifically amplified by PCR. On the basis of the laboratory results, the tumor was diagnosed with canine T-cell lymphosarcoma. On the basis of our knowledge, this is the first report of canine T-cell lymphosarcoma in a Shih Tzu dog.

치료적 운동이 축구 특이적 골반 앞 기울임을 가진 유소년 축구선수 엉덩관절의 관절가동범위, 도수근력, 기능적 움직임 및 방사선학적 평가에 미치는 영향: 증례보고 (Effects of Therapeutic Exercise on Hip Joint Range of Motion, Manual Muscle Test, Functional Movement Screen and Radiological Evaluation in a Youth Football Player with Football-specific Anterior Pelvic Tilt: A Case Report)

  • PURPOSE: This study was conducted to determine the effects of therapeutic exercise on range of motion (ROM), the manual muscle test (MMT), functional movement screen (FMS) and radiological evaluation in a youth football player with football-specific anterior pelvic tilt (APT). METHODS: The subject of this case report was a 12-year-old youth football player, who presented with football-specific APT. Therapeutic exercise consisted of hamstring stretch, prone hip extension, abdominal crunch, bridging with isometric hip abduction, plank exercise with posterior tilt and posterior pelvic tilting exercise using a swiss-ball for 40 min/day, twice a week for 8-weeks. ROM, MMT, FMS (deep squat, hurdle step, inline lunge, shoulder mobility, active straight-leg raise, rotary stability and trunk stability push-up) and radiographs (lumbar lordotic and sacral horizontal angle) were analyzed before and after week 8 of therapeutic exercise. RESULTS: The ROM, MMT, and FMS increased and the lumbar lordotic angle and sacral horizontal angle improved after 8-weeks of therapeutic exercise. CONCLUSION: The results of this case report suggest that therapeutic exercise improves ROM, MMT and radiography associated parameters in youth football players with football-specific APT. These findings have clinical implications for therapeutic exercise in youth football players with football-specific APT.

저나트륨혈증으로 이뇨제를 중단한 간경변 환자 복수에 대한 한방치료 1례 (A Case Study of a Patient Who Discontinued Diuretics due to Hyponatremia and Was Treated with Korean Medicine for Ascites)

  • Objectives: This study examined a case of an 80-year-old woman with extensive ascites and hypervolemic hyponatremia due to decompensated liver cirrhosis. Methods: The patient was first treated with a diuretic to treat her ascites. However, during hospitalization, she developed hypervolemic hyponatremia due to liver cirrhosis. Therefore, we discontinued the diuretic and treated her with Korean herbal medicine (Insamyangyoung-tang with Oryeong-san). We measured her abdominal circumference, followed by radiography, to evaluate the effect of Korean herbal medicine. Result: We treated the patient only with Korean medicine and without diuretics, and her ascites was successfully reduced. No recurrence was observed. Conclusion: Korean herbal medicine can be used as an adjuvant therapy for patients with ascites and hyponatremia.

Adjunctive Therapy of Pimecrolimus for Treatment of Facial Discoid Lupus Erythematosus in a Dog

  • A two-year-old, spayed female, 22.5 kg Pungsan was referred with chronic crusts and erosion on the nose. A referring veterinarian prescribed an anti-inflammatory dose (0.5-1 mg/kg/day) of oral glucocorticoids for 5 months, but skin lesions showed no meaningful improvement. A dermatological evaluation revealed a crust, depigmentation, erosion, and erythematous lesion over the nasal planum with a loss of the normal cobblestone texture. Also, firm and multifocal plaques over the thigh, groin, axilla, and dorsum were detected. A cytology examination on the nose, thigh, groin, axilla, and dorsum revealed moderate neutrophilic inflammation and bacterial infection. Abdominal radiography and ultrasonography revealed subcutaneous calcified materials along the thigh, groin, axilla, and dorsum. Calcinosis cutis was suspected because of the adverse effect of previous prolonged corticosteroid therapy. A histopathology examination of the nose lesion revealed moderate to severe degenerative or apoptotic changes of the basal layer and lymphoplasmacytic interface dermatitis. Facial discoid lupus erythematosus (FDLE) was diagnosed based on the history and the clinical, cytological, and histopathological results. Minocycline (7 mg/kg PO q 12 h) and niacinamide (500 mg/dog PO q 12 h) were prescribed as initial treatment. Glucocorticoids were not administered due to the presence of calcinosis cutis induced by previous corticosteroid treatment. After 6 weeks of treatment, the clinical signs on the nose were mildly improved. At this time, topical 1% pimecrolimus cream (twice daily) was initiated, while minocycline and niacinamide were continued at the same dose. The nasal planum markedly improved after 6 weeks of additional treatment, hence minocycline and niacinamide were prescribed for an additional 2 weeks and stopped, and the patient was continued solely on topical pimecrolimus. The dog's skin lesion has been maintained in clinical remission with topical 1% pimecrolimus twice daily for more than 5 months.

복부 단순촬영의 이상적 영상구현을 위한 F. S system과 C.R system의 촬영조건 비교분석 (The Comparative Analysis of Exposure Conditions between F/S and C/R System for an Ideal Image in Simple Abdomen)

  • I. 목적 : 본 연구에서는 복부의 이상적 영상 구현을 위한 Film Screen (F.S) System과 Computed Radiography(C.R) System의 촬영조건의 차이를 비교 분석해 보고, C.R system에서 촬영조건 변화에 따른 영상의 특성을 분석함으로써, C.R 환경에서 촬영조건 설정의 중요성을 살펴보고자 한다. II. 대상 및 방법 : F.S system에서 촬영 조건을 70kVp로 고정시키고, mAs를 단계적으로 변화시켜 복부 단순겸사를 시행하여 영상을 획득한 후, 진단방사선과 전문의와 방사선사로 구성된 영상평가 팀의 정량적, 정성적 분석을 통하여 F.S system 최적의 영상을 선정하였다. 동일 환자를 대상으로 C.R system 에서 위와 동일한 방법으로 영상을 획득한 후, F.S system과 C.R system의 영상의 특성곡선(Characteristic Curves)을 비교분석하고, C.R system에서 촬영조건 변화에 따른 복부 장기의 농도변화 및 영상의 특정을 비교 평가하였다. 마지막으로, F.S system과 C.R system의 최적의 영상 획득을 위한 촬영조건의 차이를 비교 분석하고, C.R 환경에서 촬영조건의 중요성을 살펴보았다. III. 결과 : F.S system에서 영상평가팀의 평가를 통하여 선택된 최적 영상의 촬영 조건은 70kVp, 20mAs로 나타났다. C.R system에서는 촬영조건 12mAs, 70kVp에서 복벽과 직접노출구역의 농도 차이가 최대인, 진단적 가치가 높은 영상을 얻을 수 있었다. C.R system에서는 (70kVp로 고정)변화한 mAs량(3.2mAs, 8mAs, 12mAs, 16mAs, 20mAs)에 상관없이 복부 장기는 거의 통일한 농도를 나타냈으며 12mAs 기준으로 노출 조건의 감소 또는 증가에 따라서 복벽과 직접노출부분의 명확한 구분이 불가능하였으며, mAs량이 낮아질수록 양자반점(Quantum Mottle)으로 인한 artifact는 증가함을 볼 수 있었다. IV. 결론 : 위의 실험 결과 본원 system의 복부단순검사에서는 C.R system 사용 시 F.S system 보다 촬영 조건을 약 40% 감소시킬 수 있었다. C.R환경에서 x-ray장치 Image processing 장치, 자동 감도조절(EDR : exposure data recognition processing) 장치, PACS 등 사용하는 장비들의 특성을 분석하여 적정 노출조건을 재설정한다면, 최소의 피폭선량으로 최대의 영상정보를 지닌 진단적 가치가 높은 영상의 획득이 가능하다고 사료된다.

CT 검사 시 스캔 범위 내 상지 유무에 따른 영상의 질 평가(LUNG MAN 포함) (Evaluation of Image Quality According to Presence or Absence of Upper limbs in Scan Field of View During CT Examinations (Including LUNG MAN))

  • 본 연구의 목적은 흉 복부 다중검출기전산화단층촬영(multi-detector computed tomography; MDCT)에서 상지를 머리 위로 위치 잡이 할 수 없는 경우 흉 복부에서 거리에 따른 인공유무를 평가하고자 하였다. 128-채널 MDCT로 흉 복부 CT를 위한 인체대상과 흉부 팬텀을 현재 임상에서 검사하고 있는 조건(120 kVp, 110 mAs, standard algorithm)으로 검사하였다. 인체 검사 시 한번은 팔을 머리 쪽으로 올리고 검사하고, 팔을 내린 후 동일한 조건으로 한 번 더 검사하였다. 흉부 팬텀 실험은 환자와 동일한 조건으로 검사를 하고, 상지팬텀을 흉 복부에서 일정한 거리(0, 3, 7 cm)를 두고 검사하였다. 목적하는 부위에 관심영역을 설정하여 CT 값, 노이즈, 신호 대 잡음비, 대조도 대 잡음비를 측정하여 평가하였다. 인체를 대상으로 획득한 영상에서 노이즈는 팔을 올렸을 때와 비교하여 팔을 내렸을 때 지방, 갈비뼈, 근육 모두에서 증가하였다(0.79, 47.8, 27%). 팬텀 영상에서도 상지를 아래로 내렸을 경우 근육, 폐 실질에서 노이즈가 증가하였다(31.2, 9.4%). 또한 상지의 위치가 흉 복부에서 멀어질수록(0, 3, 7 cm) 노이즈가 감소하였다. 근육에서 노이즈는 상지가 흉부와 붙어있을 경우(0 cm)를 기준으로 3 cm, 7cm 떨어졌을 때 5, 25.12% 감소하였고, 폐실질에서 5.6, 15.35% 감소하였다. 흉 복부 CT 촬영 시 갠트리 내 검사 이외의 부위(상지 등)가 위치할 경우 흉 복부로부터 약 3 cm 이상 거리를 유지시킨 후 검사를 진행하면 발생할 수 있는 인공음영을 최소화시킬 수 있을 것으로 사료된다.
