50대 만성허리통증 환자들을 대상으로 다리들기와 다리내리기 운동이 배 근육의 활성도, 허리통증, 그리고 유연성에 미치는 영향 (Effects of Straight Leg Lifts and Double Leg Lowering Exercise on Abdominal Muscle Activity, Back Pain, and Flexibility in Patients with Chronic Low Back Pain in their 50s)

  • Purpose : The purpose of this study was to investigate the effects of Straight leg lifts (SLL) and double leg lowering (DLL) exercise on abdominal muscle activity, visual analog scale (VAS), and flexibility in patients with chronic low back pain (LBP). Methods : A total of 30 LBP patients were divided into two groups: those with SLL exercise group 15 (male=8, female=7) and those with DLL exercise group 15 (male=7, female=8). Before the intervention, the abdominal muscle activity, VAS, and flexibility were measured. After 4 weeks of intervention, the above variables were measured in the same way. The SLL exercise bends the leg $45^{\circ}$ in the supine position, and the DLL exercise was performed as opposed to SLL. At this time, the pressure biofeedback unit (PBU) was placed behind the lumbar to reduce the instability of the pelvis and muscles. The subjects were instructed to use the PBU to maintain the target pressure determined (40 mmHg) during the exercise. Results : The external oblique (EO), internal oblique (IO), and transverse abdominis (TrA) were significantly different in the SLL and DLL group, and EO, IO, and TrA activity improved more significantly increased in the DLL than SLL group (p<.05). The results on the VAS and flexibility were significantly different both group (p<.05). However, there was no significant difference between the groups (p>.05). Conclusion : SLL and DLL exercises in patients with LBP were able to confirm the increased activity of the abdominal muscles, decreased pain, and increased flexibility of the waist. In addition, DLL exercise is more effective in patients with LBP in terms of muscle activity.

  • Purpose: This study was to examine lateral abdominal muscle activation during maximum expiration exercise between healthy and chronic low back pain(CLBP) patients. Methods: The subjects were 16 CLBP patients and 16 healthy people between the ages of 22 and 53. The thickness of the abdominal muscles was measured using ultrasonography(LOGIQ Book XP, GE, USA). We instructed the subjects how to perform the exercises and measured changes in thickness of the transversus abdominis(TrA) and internal oblique(IO) muscles during the maximum expiration. The main outcome variables were the ratios of the TrA and IO thickness during the exercise versus in the relaxed position(TrA and IO activation ratios). Results: There were significant differences between CLBP patients and healthy subjects for TrA in the relaxed position. However there was no difference in the ratio of change in the muscle activity(TrA, IO). Conclusion: These findings, CLBP patients exhibited atrophy of the TrA muscle, but voluntary TrA muscle activation was similar to that of the normal subjects. Therefore, this exercise could be used during core strengthening in CLBP patients.

Effects of 4-week PNF Exercise Program on Activity, Posture, and Muscle Strength in a Patient with Abdominalis Weakness after Cesarean Section: A Single-Subject Study

  • Purpose: The purpose of this study was to determine the effect of proprioceptive neuromuscular facilitation (PNF) abdominal muscle strengthening exercise on the activity, posture, and body function of clients with abdominal muscle weakness after cesarean section. Methods: PNF abdominal muscle strengthening exercise was applied to one patient with abdominal muscle weakness after cesarean section. PNF abdominal muscle strengthening exercise was applied five times a week for four weeks. The 5-time supine-to-long sitting test (5-TSLST) and supine-to-stand test (SST) were used to measure activity, and the pelvis tilt angle test (PTAT) and anterior head translation test (AHTT) were used to measure posture. Body function was assessed using the transverse abdominis muscle strength test (TAMST), the internal oblique and external oblique muscle strength test (IOEOMST), and the rectus abdominis muscle strength test (RAMST). Results: Activity, posture, and body function were improved post-intervention. Conclusion: The study verified that PNF abdominal muscle strengthening exercises are effective when applied to patients with abdominal muscle weakness after cesarean section. The findings of this study provide useful data for future interventions in patients with abdominal muscle weakness after cesarean section.

만성 요통환자의 복부 섬부근과 표재근을 중심으로 환 운동 효과 비교 (A Comparison of the Improvement of Symptoms between Deep Abdominal Muscle Exercises Group and Superficial Abdominal Muscle Exercises Group in Patients with Chronic Low Back Pain)

  • Objectives: To compare the efficacy of two types specific exercise intervention (deep and superficial abdominal muscle exercises) in the treatment of patients with chronic low back pain (CLBP). Design: A randomized, comparative, repeated-measures. Subjects: Twenty-four patients with CLBP (at least for 3 months), mean $age{\pm}SD=51.58{\pm}16.21$ (yrs). Methods: Twenty-four patients with this condition were assigned randomly to two treatment groups. Both first and second group underwent 3 weeks specific exercise treatment program. The first group specific trained for the deep abdominal muscles. The second group specific trained for the superficial abdominal muscles. Results: After intervention, the first group showed a statistically significant reduction in pain intensity and functional disability levels (p<.05). The second group showed no significant change in these parameters after intervention (p>.05). Conclusions: According to results, a "deep abdominal muscle exercises" treatment approach appears more effective than a "superficial abdominal muscle exercises" treatment in patients with CLBP.

테이핑 또는 시각적 피드백을 병행한 드로우-인 교각운동이 복부 근활성도에 미치는 영향 (Effects of Draw-In Bridge Exercise with Taping or Visual Feedback on Abdominal Muscle Activity)

  • Purpose : The purpose of this study was to use oral instruction and taping to induce abdominal draw-in in the previous study, but there is a lack of research on the comparison of effects. Therefore, in this study, we would like to study how the effect of oral instruction, taping, and no feedback can affect muscle activity when performing abdominal draw-in pier exercises. Methods : Before the experiment, group A trained piers and applied only draw-in piers, group B applied taping feedback, and group C were divided into three groups per week for six weeks, and three sets of contractions for five seconds per group were performed five times, and a minute break was provided between each exercise. As a measurement tool, a wireless surface electromyography (EMG) was used to measure surface electromyography (Telemyo-DTS, NORAXON, USA) that derives a synthetic unit of muscle using a surface electrode. For statistical processing of data, the analysis is performed using the SPSS/PC Version 25.0 statistical program for Windows, and the statistical significance level is a=.05 Results : First, the duration was somewhat shorter in order to more accurately compare the muscle activity of the core muscles. Therefore, future research needs to be research that can be exercises with precise biofeedback and taping applied over a longer period of time. Secondly, it is believed that there is no significant difference, since the subjects were studied in healthy young adult men and women, not patients. Thirdly, they could not control the daily life of the subjects. Through future research, it is necessary to analyze the correlation between muscle activity and abdominal muscle thickness by adding ultrasound in addition to muscle activity in various age groups. Conclusion : During draw-in bridge exercise according to feedback, the muscle activity of the back muscle increased, and there was no significant difference in the muscle activity of the back muscle according to each feedback. Therefore, both the feedback applied during the draw-in pier exercise and the control group are effective in changing the thickness of the abdominal muscles.

Changes of abdominal muscle activity according to trunk stabilization exercises using a Swiss ball

  • Objective: The purpose of this study was to compare the activities of the abdominal muscles according to trunk stabilization exercises using Swiss ball in normal individuals. Design: Cross-sectional study. Methods: Ten healthy university students participated in this study. The subjects were required to complete the following three exercise positions: Exercise position 1, sitting on a Swiss ball and lifting the legs; Exercise position 2, pushing to a plank position from an ending position; and Exercise position 3, push-up posture with the legs on a Swiss ball. Changes in the trunk muscle activities were estimated using Biometric electromyography (EMG). Activities of the dominant side internal oblique muscle (IOM) and external oblique muscle (EOM) were estimated in all participants. The maximal voluntary isometric contraction (MVIC %) was measured to standardize the EMG signals for the IOM and EOM during maximum resistance when sitting up with each shoulder pointing towards the contralateral pelvis with knees bent and both arms crossed on the chest for 5 seconds. Results: There was a significant difference in the activity of the internal and external oblique muscles between Exercises 1 and 2 and Exercises 1 and 3 (p<0.05). Furthermore, the IOM/EOM activity ratio was the greatest during Exercise 3 and the smallest during Exercise 1. IOM and EOM activities were the greatest during Exercise 2 with greater EOM activity. Conclusions: In future studies, it will be necessary to investigate muscle activities by supplementing the above-mentioned limitations during the stabilization exercise. The results of this study may be used as a basis for controlling the intensity and frequency of exercise while prescribing trunk stabilization exercises.

  • PURPOSE: This study was carried out to examine the changes in the trunk control ability and hand grip when the abdominal draw-in maneuver using breathing was applied to stroke patients, and provide basic data regarding the trunk stabilization exercise. METHOD: After randomly placing patients in group I-applying the existing abdominal draw-in exercise and group II -applying the abdominal draw-in exercise using breathing, the intervention program was performed ten times per set (five sets per session), once a day, four times a week, for a total of four weeks. For the pre-tests before the intervention, trunk damage and hand grip were measured. After the four weeks of intervention, post-tests were conducted in the same way as the pre-tests for analyzing the study results. RESULTS: Both groups had significant differences in trunk control ability and hand grip (p < .05). The comparison between the two groups showed, significant differences only in the trunk control a ability(p < .05). CONCLUSION: The abdominal draw-in exercise using breathing led to the efficient contraction of abdominal muscles, which implies this is a more effective intervention to improve the trunk control ability.

플랭크 운동 동안에 머리와 다리 위치가 몸통근육과 위등세모근의 근활성도에 미치는 영향 (Effect of Head and Leg Positions on Trunk and Upper Trapezius Muscle Activities during Plank Exercise)

  • Purpose: This study investigated the muscular activity of abdominal muscles during a variety of plank exercises following changes in the leg and head positions. Methods: Thirty healthy individuals participated in this study. They performed six variations of plank exercises, including three changes in head position and two changes in leg position. Each plank was defined as head neutral-leg neutral, head up-leg neutral, head down-leg neutral, head neutral-leg wide, head up-leg wide, and head down-leg wide. During the plank excises, the muscle activities of the rectus abdominis, internal oblique, erector spinae, and upper trapezius were measured. Results: The head down position significantly increased the rectus abdominis activity compared to other head positions (p<0.05). On the other hand, the upper trapezius muscle activity was significantly higher with the head up position compared to other head positions (p<0.05). Regardless of head positions, both the rectus abdominis and internal oblique muscles were significantly activated with leg wide position compared to the leg neutral position (p<0.05). Conclusion: Head and leg positions could change the muscular activities of abdominal muscles during plank exercises. For example, the head down position is effective for activating the rectus abdominis while the leg wide position could be advantageous for enhancing the internal oblique and rectus abdominis.

Comparison of Abdominal Muscle Thickness Using Ultrasound Imaging During Bridging Exercises With a Sling and Ball in Healthy Young Adults

  • Background: Bridging exercises are used to enhance the functional stability of the lumbopelvic region in clinical settings. Although most of the studies on bridging exercises have compared the complete activation of the trunk muscles, some recent studies have examined the functional stability of the trunk and the lumbopelvic region and assessed the appropriate recruitment of the local and global muscles during different task levels. Objects: The purpose of this study was to investigate the changes in muscle thickness in the transverse abdominis (TrA), internal oblique (IO), and external oblique (EO) muscles during a common bridging exercise on an unstable surface and to determine whether these changes differ based on the surface used. Methods: Twenty-five healthy young adults (8 males, 17 females) were recruited. The subjects were randomly assigned to either the exercise progression with a sling bridge group or the ball bridging exercise progression group, each with three stages of increasing difficulty. Each position was measured three times with an ultrasonic diagnostic imaging system, and the mean values were recorded for analysis. Results: No significant differences were observed between the TrA, IO, or EO muscle thickness ratios between the sling and ball exercise groups (p > 0.05). There were also no significant differences in the EO muscle thickness ratios between the tasks irrespective of whether the sling or ball was used. However, the TrA and IO thickness ratios in both groups were significantly greater during stages 2 and 3 compared to stage 1. Conclusion: The results suggest that the use of slings and balls during bridging exercises is effective in activating the deep abdominal muscles.

Hook-lying 자세에서 한쪽 다리 들기시 지지면의 안정성에 따른 복사근의 근활성도 비교 (Comparison of Abdominal Oblique Muscle Activity During Leg Raising in Hook-lying Position According to Surface Conditions)

  • To improve trunk stability, various exercise protocols were introduced into the clinical field. Trunk and lumbar stability exercises on unstable surfaces are especially recommended to improve lumbar stability. The purpose of this study was to compare abdominal oblique muscle activity during leg raising in hook-lying position among 3 different type of surface conditions (on floor (F), vestibular board (VB), and foam roll (FR)). Sixteen able-bodied volunteers, who had no medical history of lower extremity or lumbar spine disease, were recruited for this study. Surface electromyography (EMG) activity was recorded from the internal and external oblique muscles of both sides. The normalized EMG activity was compared using a one-way repeated ANOVA. The results showed that the EMG activities of the internal oblique and external oblique of the lifted leg side during straight leg raising significantly increased under the FR condition when compared to the F condition. There was no significant difference of the EMG activity in abdominal oblique muscles between the VB and the FR conditions. The EMG activity of the internal oblique of supported leg side during the straight leg raising was significantly greater under the FR condition than the VB and F conditions (p<.05). The composition ratio of EMG activity of internal oblique muscles during straight leg raising was significantly increased under the FR condition. Therefore, straight leg raising exercise on foam roll in hook lying position could be beneficial to improve trunk and lumbar stability.
