  • Soft tissue characterization and modeling based on living tissues has been investigated in order to provide a more realistic behavior in a virtual reality based surgical simulation. In this paper, we characterize the nonlinear viscoelastic properties of intra-abdominal organs using the data from in vivo animal experiments and inverse FE parameter estimation algorithm. In the assumptions of quasi-linear-viscoelastic theory, we estimated the nonlinear material parameters to provide a physically based simulation of tissue deformations. To calibrate the parameters to the experimental results, we developed a three dimensional FE model to simulate the forces at the indenter and an optimization program that updates new parameters and runs the simulation iteratively. The comparison between simulation and experimental behavior of pig intra abdominal soft tissue are presented to provide a validness of the tissue model using our approach.

  • The three cases of cryptorchid are reported here in order to demonstrate the different surgical techniques that were carried out to remove crytochid testes in dogs and to introduce clinical cases on crytorchidism. Three dogs were presented in the clinic of Ecole Veterinaire de Lyon in May, 1998 with two in the left-sided unilateral abdominal cryptorchid and with one in bilateral inguinal mobile cryptorchid. There were two poodle at the age of more than 4 years old and 8 years old respectively with one boxer at 4 years old. In two abdominal cryptotchids, the skin was incised in caudal paramedian approach. Then, the linea alba were incised and cryptorchidectomy was peformed accompaning one with castration and the other with vasectomy by the request of the owners. In inguinal testicles, prescrotal skin incision was made to remove both testicles. The surgical outcome was good without any Their behaviors became agreeable as the proprietors reported. In the dog with vasectomy, the behavior changes were not noticeable.

  • Intussusception after colonoscopy is an unusual complication. A MEDLINE search revealed only 7 reported cases. We present a report of a 28-year-old man who developed abdominal pain several hours after routine colonoscopy and in whom computed tomography (CT) revealed colocolic intussusception. We postulate that this condition is iatrogenic and induced by suctioning of gas on withdrawal of the colonoscope. A common observation among the reported cases was abdominal pain several hours after colonoscopy and right-sided intussusception. All cases had colonoscopy reaching the right side of the colon. Treatment for adult intussusception remains controversial with regard to reduction versus resection, especially given the high association with a pathological cause and malignancy. Among the 8 reported cases, only the current case did not require surgery. A combination of benign colonoscopy, CT, and the clinical picture should provide sufficient information to initially choose a more conservative treatment approach.

  • The purpose of study was to evaluate effects of a trunk muscle strength training on sitting balance of children with spastic cerebral palsy. Five individuals with spastic cerebral palsy(three females. two males; mean aged 6.6) participated three times a week over 6 weeks. Five individuals with spastic cerebral palsy(three females. two males; mean aged 7.0) who did not receive strength training served as control. BPM(Balance Performance Moniter) was used to measured sitting balance and EMG(electromyograph, NORAXON, USA) was used to measured activities of abdominal and erector spine muscle. The obtain result are as follows. 1. The result of this study were following that maximum perturbation area and perturbation velocity were significantly reduced strength training group compared with control group(p<.05). 2. The result of this study were following that maximum perturbation area and perturbation velocity were significantly reduced after strength training compared with pre strength training in strength training group(p<.05). 3. The result of this study were following that maximum perturbation area and perturbation velocity were not significantly reduced at interval of six week in control group(p<.05). 4. The result of this study were following that abdominal muscle activity was significantly increased strength training group compared with control group(p<.05). 5. The result of this study were following that erector spine muscle activity was significantly increased strength training group compared with control group(p<.05). 6. The result of this study were following that abdominal muscle activity and erector spine muscle were significantly increased after strength training compared with pre strength training in strength training group(p<.05). 7. The result of this study were following that abdominal muscle activity and erector spine muscle were not significantly increased at interval of six week in control group(p<.05). In conclusion, the result of this study suggest that improved sitting balance and increased activities of trunk muscle by trunk strength training in spastic cerebral palsy. The result was proposed that therapeutic approach of spastic cerebral palsy with impaired sitting balance should use trunk strength training.

  • Obesity is considered as a primary health problem over the past century in line with life environmental changes. It is mainly associated with increased risk of numerous chronic diseases which may significantly reduce health-related quality of life. Therefore, efforts to reduce weight should be performed. This study suggested a novel approach to reduce body fat by applying external stimulation which is micro-current stimulation (MCS). In this clinical study, we evaluated the potential effects of MCS for reduction of abdominal fat and weight loss. Prior to the clinical test, computational simulation was conducted to find the proper MCS conditions that allow externally applied stimulation to reach the internal fat section from the external skin. Particularly, the clinical study evaluated the unilateral effects of MCS for body fat loss and lipolysis without any additional limitations such as physical exercise and dietary therapy. The results showed that whole body fat, waist circumferences, and abdominal fat are gradually decreased after intervention in proportion to the time. From the results, we can estimate that MCS can be effective on the body fat loss by activation of lipolysis in human adipose.

  • We evaluated forty cases of traumatic diaphragmatic ruptures that we have experienced from Mar. 1976 to Mar. 1992. Thirty patients were male and 10 were female[M:F=3:1]. The age distribution was ranged from 2 to 76 years with the mean age of 35 years. The traumatic diaphragmatic ruptures were due to blunt trauma in 26 cases[traffic accident 20, fall down 4, others 2] and penetrating trauma in 14 cases[stab wound 13, gun shot 1]. In the blunt trauma, 21 of 26 cases were diagnosed within 24 hours after injury and all cases except one in penetrating trauma were diagnosed within 24 hours. In the blunt trauma, the rupture site was located in the left in 20 cases and in the right in 6 cases. In the penetrating trauma, the rupture site was located in the left in 10 cases and in the right in 4 cases. The repair of 40 cases were performed with thoracic approach in 19 cases, thoracoabdominal approach in 17 cases and abdominal approach in 4 cases. The postoperative mortality was 7.5 %[3/40]. The causes of death were septic shock[1], acute renal failure[1] and hypovolemic shock[1].

  • We evaluated sixteen patients of traumatic diaphragmatic injuries that we have experienced from Jan. 1987 to Aug 1993. Age was ranged from 6 to 71 years, predominantly in the fourth and fifth decades. 13 were male and 3 were female, a ratio of 4.3: 1. Blunt trauma was develped in 11 [Lt 7, Rt 4], penetrating trauma in 5 [Lt 2, Rt 3]. Preoperative diagnosis of diaphragmatic injury was possible in 8 patients [72.2 %] in blunt trauma, and 1 patient [20 %] in penetrating trauma. 8 cases[54.5%] in blunt trauma, and 4 cases in penetrating trauma were treated within 24 hours, meanwhile, patients treated after 10 days were 3, all by blunt trauma. The repair of 16 cases were performed with thoracic approach in 4 cases, thoracoabdominal approach in 3 cases, and abdominal approach in 9 cases. The herniated organs in thorax were stomach [5], colon [3], liver [2], and pancreas [1]. Postoperative complication were developed in 9cases[56.3%] significantly related with delayed operation time [p < 0.01 ]. Hospital mortality was 12.5 % [2/16], and the causes of death were hypovolemic shock in one and hepatic failure due to portal vein rupture in another.

  • Activity of the trunk muscles is essential for maintaining stability of the lumbar spine because of the unstable structure of that portion of the spine. The central nervous system deals with stabilization of the spine by contraction of the abdominal and multifidus muscles in anticipations of reactive forces produced by limb movement. Recent evidence indicates that the lumbar multifidus muscle and transversus abdominis muscle may be involved in controlling spinal stability. Stabilization training in neutral spine is an integrated approach of education in proper posture and body mechanics along with exercise to improve strength, flexibility, muscular and cardiovascular endurance, and coordination of movement.
