We experienced one case of surgical treatment of abdominal aortic aneurysm using retroperitoneal approach. The transperitoneal route has been the standard surgical approach for elective and emergency treatment of aneurysmal or occlusive disease. Among its cited advantages for aortic reconstruction are familarity with the exposure, easy access to the infrarenal aorta and iliac vessels, possibility of simultaneous inspection of the intra-abdominal viscera, and speed of opening and closure.Despite the proved versatility of the transperitoneal approach, it is commonly associated with prolonged ileus, increased third space fluid loss, and significant pulmonary complications. The retroperitoneal approach, on the other hand, has many advantages; excellent exposure for the repair of juxtarenal or suprarenal abdominal aortic aneurysms and visceral vessel occlusive disease, fewer postoperative complications, decreased postoperative third-space fluid losses from intraoperative evaporation and ileus, and improved postoperative pulmonary function. Atherosclerosis was most common cause of abdominal aortic aneurysm. Dacron graft[18mm] was replaced successfully.Postoperative course was uneventful and he was discharged in good condition.

Noninfectious aortitis, inflammatory abdominal periaortitis, and idiopathic retroperitoneal fibrosis are chronic inflammatory diseases with unclear causes. Recent studies have shown that some cases of aortitis are associated with immunoglobulin G4 (IgG4)-related systemic disease. Herein, we report a case of IgG4-related aortitis (IgG4-RA) that was diagnosed after surgery. Our patient was a 46-year-old man who had experienced abdominal pain for several weeks. Preoperative evaluations revealed an area of aortitis on the infrarenal aorta. He underwent surgery, and histological examination resulted in a diagnosis of IgG4-RA.
저자들은 복부 대동맥에 발생한 동맥-요관루를 혈관 내 접근을 통해 성공적으로 치료할 수 있었던 드문 증례를 보고하는 바이다. 동맥-요관루는 극히 드물지만, 사망률이 7~23%에 이르는 치명적인 질환이다. 저혈량쇼크와 같은 치명적 합병증을 예방하기 위해서는 조기 진단과 함께 즉각적인 치료가 필수적이다. 하지만 질환 자체가 희귀하고 민감도가 높은 검사 방법이 없기 때문에, 진단을 위해서는 높은 수준의 임상적 의심이 반드시 필요하다. 복강 내 수술, 방사선 치료 및 요관 스텐트의 장기 설치 등의 특징적인 과거력을 가진 환자에서 외상적 사건이 동맥-요관루 발생의 촉발 요인이 될 수 있다. 복부 대동맥에 발생한 동맥-요관루 환자에서도 혈관 내 인조혈관 스텐트의 삽입은 효과적인 치료 방법이다.
Between January 1984 to June 1994, fourteen patients from 37 to 80 years of age [mean 66.42 11.71 years of age have undergone surgical treatment of abdominal aortic aneurysm in Kyung Hee Univ. Hospital. There were 11 males and 3 female patients. All but one were infra-renal type. The etiology of the aneurysm consisted of twelve atherosclerotic, one inflammatory and one traumatic abdominal aortic aneurysm.Two patients were operated on for ruptured abdominal aortic aneurysm. We performed dacron graft interposition in all patients and one patient was also performed aorto-renal end to side anastomosis. Two patients died of postoperative complications which was a pulmonary insufficiency in one, acute renal failure in another patient.Remaining twelve patients were discharged with good condition and followed up from 2 months to 87 months.[mean $34.58{\pm}29.79$ months.
복부 대동맥류의 수술에 있어서 만성 폐쇄성 폐질환은 수술 사망에 유의한 영향을 미치는 위험 인자로 알려져 있다. 따라서 수술 후 호흡기 합병증을 줄이기 위해서는 강제적 기계 호흡을 줄이고 가능한 환자의 자발 호흡을 유지하는 것이 수술 결과를 향상시킬 수 있다. 본 증례에서는 복부 통증을 주소로 내원한 77세의 남자 환자로 수술 전 검사에서 약 9 cm크기의 복부 대동맥류가 발견되었으나, 심한 만성 폐쇄성 폐질환이 동반되어 수술 후 사망률이 높을 것으로 예상된 경우에서 경막외 마취등을 통해 환자의 자발 호흡을 유지한 상태로 복부 대동맥류 절제 및 인조혈관 삽입술을 시행하여 좋은 결과를 얻을 수 있었다.
Aneurysms of the descending thoracic aorta can be caused by various etiologies. So, its abrupt rupture leads life-threatening state, it must be operated as soon as possible. Surgical treatment of the descending thoracic aortic aneurysm requires temporary cross-clamping of major artery. The obligatory occlusion of the descending thoracic aorta during management causes proximal arterial hypertension and distal arterial hypotension. The former may leads to left ventricular failure, or cerebrovascular accident, whereas the latter may leads to spinal cord ischemia or renal injury. Some have recommended insertion of temporary shunt around the occluded descending aorta to prevent above problems. Still others would favor expeditious operation employing simple aortic occlusion during the repair of the descending aorta. Recently we had experienced two cases of dissecting aneurysms of descending thoracic aorta which performed aortoplasty with Gore-Tex conduit under simple aortic occlusion. The one was 34-year-old female patient with traumatic dissecting aortic aneurysm [5 em X 5 cm] on the descending thoracic aorta distal to the origin of the left subclavian artery and the other was 58-year-old female patient with atherosclerotic dissecting descending thoracic aortic aneurysm [6 cmX7 cm] and diffuse abdominal aortic aneurysms [3X5 cm]. Both patients performed standard left posterolateral thoracotomy. After the aneurysmal sac was mobilized, occluding vascular clamps were placed on the transverse aorta proximal to the origin of the left subclavian artery, and on the distal descending aorta without adjuvant bypass procedures for 31 and 32 minutes, respectively, and the aneurysmal sac was repaired with 18 mm ringed Gore-Tex conduit graft. Both patients postoperative courses were uneventful.
Abdominal organs are the most vulnerable body parts under vehicle trauma, and there is high mortality from acute injuries in accidents. There are various ways to reduce this high mortality; one method is Resuscitative Endovascular Balloon Occlusion of the Aorta, which has recently become very popular as a minimally invasive alternative in the emergent management of patients with non-compressible hemorrhages below the diaphragm. However, high safety factor for patients is applied in actual clinical practice because there is no exact standard for the operating time. Therefore, in this study, the effects of the mechanical behavior of organ tissues for the duodenum, kidney, and liver on the operating time of Resuscitative Endovascular Balloon Occlusion of the Aorta is investigated in order to obtain data needed to establish standards of operating time. In characteristic analysis of organ tissues, uniaxial tensile test and compression test are conducted according to the operating time.
Fish oil and shortening have been suggested to have opposite effects on cardiovascular disease (CVD). This study investigated the effect of shortening and fish oil on CVD risk factors and aorta histopathology, and the association between risk factors and aorta histopathology. Male Wister rats (n=30) were fed an AIN-93G diet containing 20% fat in the form of fish oil, shortening, or soybean oil for 4 weeks. Total cholesterol (TC), triacylglyceride (TG), and C-reactive protein levels were significantly (P<0.001) lower in the fish oil than in soybean oil and shortening groups. HDL-cholesterol concentrations were significantly different (P<0.001) between groups. In addition, LDL-cholesterol levels were significantly (P<0.001) lower in the fish oil and shortening groups than in the soybean oil group. Insulin and glucose concentrations did not differ among groups. Effect of dietary fat on tissue fatty acid composition significantly differed in abdominal fat and brain compared with RBC, heart, kidney and liver. The aortic wall was significantly (P=0.02) thinner in the fish oil group than in the soybean oil and shortening groups. The aortic wall thickness was positively correlated with TG and TC, but negatively with EPA + DHA levels of all tissues. These results suggested that fish oil had protective effects on aorta histopathology by hypolipidemic action in this rat model.
A 15 years Old girl was admitted with chief complaints of intermittent claudication of lower extremity, dizziness, and headache for 5 years. On admission, malignant hypertension was noted in the upper part of body [190-150/120-110] but femoral & dorsalis pedis pulse could not palpate. Once she had experienced C. V. A. due to hypertension of upper part, about years ago. On auscultation, systolic murmur was audible along the left sternal border. E.C.G. Showed left ventricular hypertrophy pattern, and others within normal limit. Retrograde aortography demonstrated diffuse narrowing of entire thoracic aorta with underdeveloped lower abdominal aorta [below the renal artery] & both common lilac artery, and rich collaterals, but normally visualized greater arteries in the aortic arch. On left posterolasteral thoracotomy, entire descending thoracic aorta revealed marked narrowing with mild perivascular adhesion, but no mediastinal pleura adhesion. These findings suggest as congenital type of atypical coarctation in the entire thoracic aorta with mild secondary change. But histopathology was showed the findings of chronic non-specific aortitis, later. Dacron by pass graft was performed with end to side anastomosis between graft and aortic wall. After operation, all her preoperative symptoms & signs were disappeared, and discharged with good general condition.
A 3 day old Holstein-Friesian calf which showed the clinical signs such as suckling inability, abdominal distension and defication insufficiency was diagnosed as congenital atresia coli with abnormality of abdominal aorta.
