Statement of problems: Long-term success of esthetic restorations, depends on various factors, especially on color stability of the restorations. The color stability is affected by resin cements beneath the porcelain laminate veneers. Purpose: The purpose of this study was to comprehend the color stability of resin cements by comparing with 4 different kinds of resin cements (Choice, Variolink II, Nexus 2, Rely-X). Material and methods: Porcelain laminate disk samples were luted with resin cements. After thermocycling of each 1000, 2000 and 3000 cycle, specimens were measured by spectrophotometers and the color differences were. Results: 1. After thermocycling of 1000, 2000 and 3000cycle, color difference, $({\Delta}E^*)$ of all specimens were incresed, but were not statiscally significant differences. 2. In the Choice of A1 shade and the Nexus 2 of A3 shade, color difference$({\Delta}E^*)$ after thermocycling of 3000cycle was $incre^*ased$ significantly than after thermocycling of 1000cycle. However, other groups did not show any statistically significant differences (P<0.05). 3. After thermocycling of 3000cycle, color difference$({\Delta}E^*)$ between A1 shade and A3 shade of Nexus 2 showed statistically significant difference. However no statistically significant differences were observed in other groups (P<0.05). 4. The mean of color difference$({\Delta}E^*)$ is 1.0-2.06 after 3000 thermocycling and the data were ranked in the ascending order of Variolink II, Nexus 2, Choice and Rely-X in A1 shade group and Choice, Rely-X, Variolink II and Nexus 2 in A3 shade group. Conclusion: Above results revealed that, after 3000 thermocycing, the color difference $({\Delta}E^*)$ of experimental resin cements was within clinically acceptable ranges. Color stability of 4 different resin cements was comparatively excellent.

This study aimed to devise the method to construct safe and convenient bicycle road by considering the peripheral environmental factor related to using bicycle. Analyzing the existing design of bicycle road and construction case, this study established the method to design bicycle road that reflects site condition and presented the optimal design method for each type of bicycle road to construct safe and convenient bicycle road by analyzing the type of traffic accident for each type of bicycle road, surveying present situation and local survey. It was found that the optimum design of bicycle road for specification, width, curve radius, ascending slope, etc in consideration of peripheral environment and separating traffic between users of traffic means should be done by installing safety sign, safe facilities and separation facilities to design safe bicycle road. Further, the minimum traffic space of bicycle users and connection between bicycle roads should be ensured to design safe bicycle road. It is judged that information related to safety and convenience of bicycle road such as slope, route information, location of convenience facilities, information to the public traffic should be provided so as to activate the users of bicycle.
Levels of platelets and other hematological values were monitored in 21 Saimiri and 12 Aotus monkeys over a period of three weeks post·infection with monkey·adapted Indochina CDC-1 strain of Plasmedium falciparum. In both Snlinoiri sciureus boliviensis and Aetus nancymai karyotype-1 monkeys the severest thrombocytopenia was observed at 14 days post-infection coinciding with peak parasitemia, neutropenia, Iynlphocytosis, and anemia associated with severe hemoglobinemia and elevated fibrinogen degeneration products(FDP's), MCH and MCV profiles in Aotus monkeys decreased with ascending parasitemia. In contrast, these parameters in Saimiri were characterized by a significant compensatory increase correlating with parasitemia. In general, thrombocytopenia was one of the earliest clinical manifestations of the infection with the platelets returning to normal levels shortly after peak parasitenlia at 14 days. Platelet kinetics had a strong correlation with hematologic and parasitologic values in the Aotus nlodel. No consistent associations were observed between platelet kinetics and other parameters in the Saimiri model. These data indicate that the Aotus model for malaria is more predictable than the Saimiri. Further, platelet turnover rates and recovery provide a useful prognostic parameter during malaria infection. The results are discussed in relation to the value of the two species of monkeys as models for the pathogenesis of human malaria.
Primary and metastatic tumors involving the heart are relatively uncommon in dogs. In this study, we provide the echocardiographic diagnosis of intracardiac masses in 2 Yorksire terrier dogs. In the first case, the mass was attached between ascending aorta and pulmonary artery and caused moderate aortic regurgitation and moderate left ventricular dilation. The case was graded into ISACHC II heart failure. The dog was treated with common cardiac medications (i.e. furosemide, enalapril, pimobendan) and oral chemotherapeutic agent (i.e. lomustine). In the second case, the mass was occupied 2/3 of the left atrium and caused marked dilation of left atrium and severe mitral regurgitation (~5 m/s), but not severe congestive heart failure (ISACHC Ib). Although the nature of progression of the mass was likely to cardiac myxoma, the biopsy was not performed due to the owner's refusal. The dog was currently treated with cardiac medications (i.e. ramipril, clopidogrel) and bronchodilator (i.e. aminophylline). Those two dogs are still survived and are currently regularly checked.
Six cases of congenital heart disease were operated on by means of cardiopulmonary bypass between December, 1975 and April, 1976. Two cases of ventricular septal defects (VSD), two cases of VSD, associated with ruptured aneurysm of sinus Valsalva, two cases of atrial septal defects (ASD) and one case of pulmonic stenosis with patent ductus arteriosus were operated. Sarns roller pumps and Bentley Temptrol oxygenators were used for extracorporeal circulation. Pump oxygenator was primed with Ringer's lactate solution, 5% dextrose in water, mannitol, and ACD blood. Flow rate ranged from 2.0 to $2.4L/M^2/min$. Bicarbonate was added to the oxygenator with estimated amount as 15 mEq/L/hr. Venous catheters were introduced into superior and inferior vena cava, and oxygenated blood was returned to the body through aortic cannula inserted into ascending aorta. Moderate hypothermia ($30^{\circ}C$) was induced by core cooling. Aorta was cross clamped for 15 minutes and released for 3 minutes, and repeated clamping when necessary. Atrial and ventricular septal efects were closed by direct sutures. Aneurysms of sinus Valsalva ruptured into the right ventricle were repaired through right ventriculotomy by d:rect closure with Dacron patch reinforcement. Cardiopulmonary bypass time varied from 66 to 209 minutes, and aorta cross clamping time ranged from 13 to 56 minutes. Postoperative bleeding was minimal except one case who needed for evacuation of substernal hematoma. Intra- and postoperative urinary output was satisfactory. Acid-base balance, partial pressure of $O_2$, electrolytes, and hematological changes during intra- and post-perfusion period remained at the acceptable ranges. No mortality was experienced.
There is tendency of increasing number and decreasing age of patients who are indicated for Rastelli operation for their cyanotic congenita heart disease. So there is the need to find the creiterion which saves the patients from early postoperative hemodynamic disturbances. We reviewed the 26 patients who had been performed Rastelli operation at Seoul national University hOipital from January 1981 to June 1988. mean age of the patients was 7.8 $\pm$ 3.4 years (range 2.5-1.5 years), mean body surface area(BSA) 0.79 $\pm$ 0.25 $m^{2}$(range 0.49-1.51 $m^{2}$) and mean hematocrit 57.95 $\pm$ 12%(range 48-80%). We diveded these patients into survived group and died group before postoperative 72 hours, and analysed preoperative arterial oxygen saturation($SaO_{2}$), the ratio of diameter of fight pulmonary artery to ascending aorta(RA:/AA), the ratio of both right and left pulmonary artery diameter to descending thoracic aorta(RPA+LPA/DTA), pulmonary artery index(PA index), cardiopulmonary bypass time, aorta cross-clamping time, postoperative perfusion state and total amount of dopamine infused postoperatively. The results showed that RPA+LPA/DTA and PA index were statistically significatn factors to influence early postoperative cardiac death rate (P<0.05). Especially there were good linear correlations between PA index(X) and perpheral perfusion index(Y)(Y = -1.15 + 0.02 X, r = 0.86, P<0.01) and between PA index(X) and total amount of dopamine infused before postoperative 72 hours(mg/kg, Y)(Y = 61.94 - 0.15 X, r = - 0.80, P<0.01). Also there were tendencies that the higher RPA + LPA/DTA(Y), the betterperipheral perfusion (X) and the lower need of dopamine(X), but no statistical significance. (Y = 0.78 + 1.60 X, r = 0.49, P>0.05) And the discrimant analysis showed that patients with PA index over 221 $mm^{2}$/BSA could undergo correction with 25 per cent of error rate. In conclusion, early postoperative hemodynamic states could be predicted by preoperatively measured PA index, and which can be used as a criterion for Rastelli operation performed on cyanotic congenital heart disease.
Acute aortic dissection associated with high mortality rate has an extremely poor prognosis if early diagnosis and treatment are not received. Recently, with advanced computed tomography and echocardiography, diagnostic rate is higher and early operation is possible. Therefore preoperative medical therapy at ER(emergency room) lowered the mortality rate. This study was done to analyze the results with preoperative management at ER and operations, retrospectively. Material and Method: A series of 42 patients treated surgically for acute aortic dissections from 1991 to 2001 were included in this study. There were 18 males and 24 females. Mean age was 51.1 years. The admission course through emergency and outpatient department(OPD) was 34 and 8 respectively. Result: 26 patients underwent ascending aorta replacement-7 combined aortic valve replacements, 7 patients underwent descending aorta replacements and 9 patients received Bentall's operation. At emergency department, 20 patients received antihypertensive drugs and $\beta$-receptor blockers and 6 patients died. 22 patients did not receive antihypertensive and $\beta$-receptor block drugs and 10 patients died. There were 16(38%) overall deaths. Conclusion: Early diagnosis at ER or OPD is essential for acute aortic dissection, and it is important to select the most appropriate noninvasive interventions as possible. Therefore, preoperative drug therapy at ER is suggested according the patient conditions.
To understand the clinical results of aortic root replacement with either inclusion or open technique, we analysed 53 patients who underwent replacement of the aortic root with composite graft between October,1980, and May, 1995. Annuloaortic ectasia was the most common indication for operation(29 patients), follwed by aortic dissection(22 patients). Among 53 patients, 19(35%) had Marfan syndrome. Three patients died during hospitalization (Mortality: 5.5%). The follow up was possible in 48 patients(Follow-up rate; 94%,mean duration;37 months). The actuarial survival rate at 24 months was 95% in open technique group, and 87% in inclusion technique group. Late complications developed in 10 patients. Dissecting aneurysm in the remaining aorta was noted in 3 patients with inclu ion 1,schnique, and a pseudoaneurysm from coronary artery anastomosis site developed in a patient with inclusion technique. In conclusion, there was no statistical differences in survival for 24 months between inclusion technique and open technique group. But late problems in the remaining aorta or death from unknown cause occurred with moderate frequency : careful follow-up after aortic root replacement thought to be important for long term survival.
Forty nine patients [M: 31, F: 18], age from 2 months to 17 years [mean= 4.9 years], underwent operations, from April 1986 to December 1992, for the relief of subvalvular aortic stenosis in normal atrioventricular and ventriculoarterial connections.There were 4 anatomic types of subaortic stenosis : membranous in 29 cases [59.2%], fibromuscular in 11 [22.4%], diffuse tunnel type in 7 [14.3%], and miscellaneous in 2 cases. Thirty four patients [69.4%] had associated cardiac anomalies, of which ventricular septal defect was the most common [27 cases]. Other anomalies were patent ductus arteriosus, coarctation of the aorta, valvular aortic stenosis, double chambered right ventricle [DCRV], infundibular pulmonic stenosis, persistent left superior vena cava, and rigt aortic arch. Mean systolic pressure gradient between the left ventricle and ascending aorta was 26.4$\pm$17.6 mmHg : 13.1$\pm$17.6mmHg in the membranous type, 22.0$\pm$18.4mmHg in the fibromucular type, and 56.1$\pm$38.4mmHg in the diffuse tunnel type. Operative procedures were determined according to the type of subvalvular aortic stenosis : simple excision of subaortic membrane in the membranous type [29 cases], left ventricular myectomy with or without myotomy or fibrous tissue excision in the fibromuscular type [11 cases]. Among the 7 of diffuse tunnel type cases, ventricular myectomy was performed in 2 and a modified Konno operation was performed in 5 . Postoperative follow up was made with periodic echocardiography. The Mean postoperative follow up period was 33.8 months. There were 2 hospital mortalities [4.1%] and 2 late deaths. Residual stenosis remained in 3 cases and recurrence developed in 2 cases during the follow up period. 5 years actuarial survival rate was 91.8$\pm$3.9% and 5 year complication free rate was 72.3$\pm$10.4%. Conclusions : 1. Subvalvular aortic stenosis should be relieved completely as soon as possible when diagnosed, regardless of left ventricular outflow tract pressure gradient. 2. Good results were obtained using only simple excision of subaortic membrane in the membranous type of subaortic stenosis. However, aortoventriculoplasty [modified Konno prodedure] was necessary for good results in the diffuse tunnel type. 3. Periodic postoperative echocardiography was helpful in detecting the progression of residual stenosis and development of new stenosis.
The arterial blood pressure response elicited by stimulating the peripheral afferent fibers of different groups and origins was studied in cats. Experimental animals were anesthetized with a-chloralose [60mg/kg] and artificially ventilated with a respirator. The lumbosacral spinal cord was exposed through a laminectomy and L7 ventral root was isolated. The sural, medial gastrocnemius and common peroneal nerves were also exposed in the hindlimb. The arterial blood pressure was monitored continuously while the exposed peripheral nerves and L7 ventral root were being stimulated. Then, spinal lesions were made on the dorsolateral sulcus area, dorsolateral funiculus and other areas at the thoracolumbar junction. The arterial blood pressure responses were compared before and after making spinal lesions. The following results were obtained. 1. The mean arterial blood pressure was elevated from 103*7.3 to 129*8, 1 [mean*S.E.] mmHg [p<0.001] during stimulation of the sural nerve with C-strength [1000T], 20Hz. Stimulation with Ad-strength, 1Hz resulted in the depression of the arterial pressure by 8 mmHg [p<0.01]. 2. Stimulation of the medial gastrocnemius nerve with Ad-strength did not elicit any significant change in arterial blood pressure. Stimulation with C-strength, 20 Hz induced a pressor response from 102*6.2 to 117*6.4 mmHg [p<0.01] while that with C-strength, 1Hz induced a depressor response from 104*6.1 to 93*4.9 mmHg [p<0.001]. 3. A pressor response by 56 [from 107*7 5 to 163*9.4] mmHg [p<0.001] was induced during stimulation of the common peroneal nerve with C-strength, 20Hz stimuli. Stimulation with A4-strength, 1Hz depressed the arterial blood pressure from 111~9.3 to 94*7.8 mmHg [P<0.005]. The activation of the ventral root afferent fibers with C-strength, 20 Hz stimuli induced a pressor response by 22 mmHg [from 115*9.4 to 137*8.6 mmHg] [p<0.001]. 4. The pressor response elicited during stimulation of the sural nerve was abolished by making lesions on the dorsolateral sulcus area bilaterally. With the medial gastrocnemius nerve, the pressor response had not been abolished completely by the dorsolateral sulcus lesions. The pressor response disappeared completely with addition of the bilateral dorsolateral funiculus lesions. 5. The depressor response induced by stimulation of the sciatic nerve with Ad-strength, 1Hz was decreased by making lesions on the dorsolateral funiculus. 6. From the above results it is concluded that the difference in the blood pressure responses to the activation of the muscular afferent and the cutaneous afferent fibers is responsible for the groups of afferent fibers and the spinal ascending pathways.
