  • It is well known that multipoint and computerized intraoperative mapping systems improve the results of surgery for Wolff-Parkinson-White syndrome and show tremendous potential for opening an entirely new era of surgical intervention for the more common and lethal types of supraventricular tachyarrhythmias such as atrial flutter and atrial fibrillation. In addition, the ability to map and ablate the sometimes fleeting automatic atrial tachycardia is greatly enhanced by computerized mapping systems. In this study, we have developed 16 channel computerized data analysis system using microcomputer for basic research of electrophysiology and electrical propagation. This system is expected to enable us to study pathophysiology of cardiac arrhythmia and to improve the results of diagnosis and surgical treatment for cardiac arrhythmia.

Evaluation of mental and physical load using inverse regression on sinus arrhythmia scores

  • This paper develops a statistical mode which estimates mental and physical loads of light work from sinus arrhythmia (SA) scores. During experiments, various levels of mental and physical loads (respectively scored by information processing and finger tapping rates) were imposed on subjects and SA scores were measured from the subjects. Two methods were used in developing workload estimation model. One is an algebraic inverse function of a multivariate regression equation, where mental and physical loads are independent variables and SA scores are dependent variables. The other is a statistical multivariate inverse regression. Of the two methods, inverse function resulted in larger mean squqre error in predicting mental and physical loads. Hence, inverse regression model is recommended for precise workload estimation.

Amiodarone으로 치료한 postoperative automatic junctional tachycardia (Postoperative Automatic Junctional Tachycardia treated with Amiodarone)

  • Automatic junctional tachycardia is one of common atrial arrhythmia after open heart surgery which is often refractory to antiarrhythmic agents. We have experienced refractory automatic junctional tachycardia in two patients. In the first, it occured after cryosurgery for AV nodal reentry tachycardia and simultaneous dissection of a posterior septal bypass tract. In the second, it complicated the postoperative course of a patient who received intracardiac repair for double outlet right ventricle, ventricular septal defect, and pulmonary stenosis. Conventional therapy with atrial pacing, verapamil, digoxin, and electrical cardioversion were ineffective. Therefore, amiodarone was administered intravenously and it controlled automatic junctional tachycardia. The need for accurate and rapid diagnosis of this condition along with results of treatment are discussed.

  • In this paper, we proposed a ventricular fibrillation detection algorithm based on wavelet transform and artificial neural network. we selected RR intervals, the 6th and 7th wavelet coefficients(D6, D7) as features for classifying ventricular fibrillation. To evaluate the performance of the proposed algorithm, we compared the result of the proposed algorithm with that of fuzzy inference and fuzzy-neural network. MIT-BIH Arrhythmia database, Creighton University Ventricular Tachyarrhythmia database and MIH-BIH Malignant Ventricular Arrhythmia database were used as test and learning data. Among the algorithms, the proposed algorithm showed that the classification rate of normal and abnormal beat was sensitivity(%) of 96.10 and predictive positive value(%) of 99.07, and that of ventricular fibrillation was sensitivity(%) of 99.45. Finally. the proposed algorithm showed good performance compared to two other methods.

  • We estimated the changes of ECG and electrolytes in serum after intravenous administration of Succinylcholine Chloride (SCC), 0.15 mg/kg in ten normal mongrel dogs (mean 13 kg). Hyperkalemia was observed in the highest level by 6.46$\pm$0.8 mEq/L at ten minutes after the administration of SCC. The ECG appeared temporary the most severe changes as the increased T wave, the disappeared P wave, the prolonged of conduction times (PR, QTc intervals and QRS complex), and arrhythmia as ventricular premature contraction at 3 and 5 minutes after the administration SCC. Therefore, the changes of ECG after administration of SCC were suggested to specific and independent from hyperkalemic changes. Because these changes were observed to differ from ECG by hyperkalemia, and the highest period of $K^+$value in serum differ from the appearance period of severe changes of ECG and arrhythmia by SCC.

  • Detection methods for atrial tachycardia and fibrillation on the time axis have the advantages of light operational load and are easy to apply to various applications. Despite these advantages, arrhythmia detection algorithm on the time axis cannot stand much noise such as motion artifacts, moreover the peak detection algorithm has high complexity. In this paper, we use a spectrum analysis method for the detection of atrial tachycardia and fibrillation. By applying spectrum analysis and digital filtering on obtained electrogram signals, we can diagnose heart arrhythmia without using peak detection algorithm.

  • 심장의 활성 근육의 움직임에 의하여 발생되는 전기적 변화량을 나타내는 심전도는 부정맥 또는 허혈성 심장질환을 진단하는데 널리 활용되고 있다. 특히 심실빈맥(Ventricular Tachycardia) 또는 심실세동(Ventricular Fibrillation)과 같이 치명적인 심장리듬이 발생하기 이전에, 심실조기수축(Ventricular Premature Contraction)을 검출하여 생명을 위협할 수 있는 부정맥을 조기에 진단할 수 있는 연구들이 일부 진행되고 있다. 이에 따라서 본 연구에서는 심전도 신호의 R-R 간격 정보와 R-peak 정보의 진위성을 판단하여 PVC 부정맥 패턴뿐만 아니라 PVC 파형이 연속적으로 진행되는 PVC-RUNs을 효율적으로 검출할 수 있는 부정맥 진단 알고리즘을 제안하고자 하였다.

  • Resibufogenin is a single compound isolated from the skin venom gland of the toad (Bufo bufo gargarizans cantor). Formulations containing toad venom have been widely used as complementary and alternative medicines. However, like digitalis, resibufogenin possesses both pharmacological and toxicological activities. Our previous data indicated that resibufogenin induces electro-toxicity, including delayed afterdepolarization and triggered arrhythmias at high concentration, both in cardiac fiber in vitro and in beating heart in vivo.

  • This paper describes a design of hardware system for real time automatic diagnosis of ECG arrhythmia based on pipeline processor consisting of three microcomputer. ECG data is acquisited by 12 bit A/D converter with hardware QRS triggered detector. Four diagnostic parameters-heart rate, morpholigy, axis, and ST segment-are used for the classification and the diagnosis of arrhythmia. The functions of the main CPU were distributed and processed with three microcomputers. Therefore the effective data process and the real time process using microcomputer can be obtained. The interconnection structure consisting of two common memory unit is designed to decrease the delay time caused by data transfer between processors and be which the delay time can be taken 1% of one clock period.
